Regionalmuseen in der Rhön â Kulturgut im Wandel
UnabhĂ€ngig davon, wie sie gefĂŒhrt werden und ob sie modernen useumspĂ€dagogischen AnsprĂŒchen genĂŒgen, sind Regionalmuseen ein unverzichtbarer Bestandteil des Kulturgutes des BiosphĂ€renreservats Rhön. Sie sind als Kleinode zu behandeln, denn ein jedes fĂŒr sich prĂ€sentiert nicht nur einen Teil der Kultur und Naturgeschichte der Region, sondern auch seine eigene Geschichte. Auch ein âverstaubtesâ Museum mit skurrilen AusstellungsstĂŒcken ist Teil der Rhön und hat seinen Charme. Nur nĂŒtzt der Charme nichts, wenn die Besucher fern bleiben und das Museum wirtschaftlich in seiner Existenz bedroht ist. In solchen FĂ€llen sind Engagement, Einfallsreichtum und die Bereitschaft zu Investitionen des TrĂ€gers erforderlich
Conflicts of interest in medicine and their management: current challenges and initiatives in Germany
Conflicts of interest (COI) in healthcare have increasingly gained attention in the lay press as well as among healthcare professionals. COIs increase the risk of undue influence on professional decision making and may have far-reaching consequences in healthcare. Therefore, it is essential to develop strategies to deal with such risk situations in order to prevent negative outcomes for patients and the health care system. This article describes recent research on COIs in Germany as well as initiatives aiming at more transparency and better management of COIs in Germany
A Randomised Controlled Trial
Liver surgery is still associated with a high rate of morbidity and mortality.
We aimed to compare different haemodynamic treatments in liver surgery. In a
prospective, blinded, randomised, controlled pilot trial patients undergoing
liver resection were randomised to receive haemodynamic management guided by
conventional haemodynamic parameters or by oesophageal Doppler monitor (ODM,
CardioQ-ODM) or by pulse power wave analysis (PPA, LiDCOrapid) within a goal-
directed algorithm adapted for liver surgery. The primary endpoint was stroke
volume index before intra-operative start of liver resection. Secondary
endpoints were the haemodynamic course during surgery and postoperative pain
levels. Due to an unbalance in the extension of the surgical procedures with a
high rate of only minor procedures the conventional group was dropped from the
analysis. Eleven patients in the ODM group and 10 patients in the PPA group
were eligible for statistical analysis. Stroke volume index before start of
liver resection was 49 (37; 53) ml/m2 and 48 (41; 56) ml/m2 in the ODM and PPA
group, respectively (p=0.397). The ODM guided group was haemodynamically
stable as shown by ODM and PPA measurements. However, the PPA guided group
showed a significant increase of pulse-pressure-variability (p=0.002) that was
not accompanied by a decline of stroke volume index displayed by the PPA
(p=0.556) but indicated by a decline of stroke volume index by the ODM
(p<0.001). The PPA group had significantly higher postoperative pain levels
than the ODM group (p=0.036). In conclusion, goal-directed optimization by ODM
and PPA showed differences in intraoperative cardiovascular parameters
indicating that haemodynamic optimization is not consistent between the two
monitors
Doped dialkylated phenazines : a novel series of highly conducting organic solids
Doping dimethylphenazine (M2P)-TCNQ and diethylphenazine (E2P)-TCNQ with phenazine gave rise to new conducting compounds with segregated stacks : (M2P)0.5 (P)0,5 TCNQ and (E2P)0,55 (P)0,45 TCNQ. The room temperature electrical conductivities are in the range 10-100 Ω-1 cm-1 for (1) and 1-10 Ω-1cm-1 for (2). The paramagnetism of both compounds could be separated into contributions from TCNQ and Phenazine stacks. The structural electrical and magnetic similarities of (1) with the well-known N-methylphenazinium (NMP)-TCNQ led to a reexamination of its magnetic properties. It was shown that an estimate of the charge transfer can be obtained from the temperature dependence of the magnetic susceptibility
Preoperative short-course radiotherapy versus combined radiochemotherapy in locally advanced rectal cancer: a multi-centre prospectively randomised study of the Berlin Cancer Society
BACKGROUND: The additional use of radiotherapy has changed the treatment of locally advanced rectal cancer (LARC) dramatically. But a major achievement has been the development of total mesorectal excision (TME) as a surgical standard and the recognition that the surgeon is the predominant prognostic factor. The benefit of preoperative hypofractionated radiotherapy (SCRT; five fractions each of 5 Gy), initially established by the Swedish Rectal Cancer Trial, has been demonstrated in conjunction with TME by the Dutch Colorectal Cancer Group. The concept of combined neoadjuvant radiochemotherapy (conventional radiation of about 50 Gy with chemotherapy) has not been compared over surgery alone with TME. However, the German Rectal Cancer Study Group recently demonstrated that preoperative radiochemotherapy (RCT) was better than postoperative radiochemotherapy in terms of local control. METHODS: Patients with histological proven rectal cancer staged T2N+ or T3 are randomized to receive either SCRT (25 Gy in five fractions of 5 Gy) plus TME-surgery within 5 days or RCT (50.4 Gy in 28 fractions of 1.8 Gy, continuous infusion 5-fluorouracil) plus TME-surgery 4-6 weeks later. All patients receive adjuvant chemotherapy (12 weeks continuous infusional 5-FU) and are followed up for 5 years. TME-quality is independently documented by the surgeon and the pathologist. Hypothesis of the study is that RCT is superior to SCRT in terms of local recurrence after five years. Secondary endpoints are overall survival, disease-free survival, complete resection rate (R0 resection), rate of sphincter saving resection, acute and late toxicity (radiation related side effects), and quality of life (including long term bowel function). DISCUSSION: Similar long-term survival, local control and late morbidity have been reported for both concepts of preoperative therapy in non-comparative studies. In addition to other ongoing (and recently published) comparative trials we include a larger number of patients for adequate power, apply quality-controlled TME and try to avoid the adjuvant treatment bias by mandatory adjuvant chemotherapy in both groups. Further more, stratification of the initially planned surgical procedure and sphincter-preservation will generate valid evidence whether RCT will allow a less aggressive (sphincter saving) surgical approach

The longitudinal magnetoresistance of a two- dimensional electron gas is shown to be strongly dependent on the presence of ohmic contacts between the voltage probes. By connecting or disconnecting contacts with additional gates across the potential probes it is possible to drastically change the amplitude of Shubnikov-de Haas oscillations
Current Carrying Edge Channels and the Role of Contacts in the Quantum Hall Regime
By selectively populating spin resolved edge channels using Schottky gates we experimentally investigate the transition from adiabatic to equilibrated transport in the Quantum Hall regime. The scattering processes within a contact e. g. provide an excellent possibility to equilibrate nonequally populated edge
channels. Here we demonstrate that not only with metallic reservoirs but also with increasing temperature one can study the crossover from adiabatic to equilibrated transport. Increasing the current results in a transition from edge channel to bulk transport
