In the present study, the authors applied the Cultural Values and Script Questionnaire, together with language measures (bidirectional translation, listening comprehension tasks), to explore the relationship between Polish-French bilinguality and bicultural identity among Polish migrants in France and Belgium and students learning French at a Polish University. We hypothesized that the Francophone acculturative context will lead to (i) integrated bicultural identity, as well as (ii) a balanced bilingual profile. Thirdly, we assumed there is a link between an individual’s bicultural identity and his or her bilinguality. The data partially confirm the two first hypotheses. An unexpected contrast effect revealed that students in Poland identify more strongly with French cultural values than with Polish values

    Anatomic variability of groin innervation

Inguinal hernia repairs are very common yet fairly complex surgical procedures.Variations in the anatomical course of the inguinal nerves require that diligenceis taken in their proper recognition. Inadvertent surgical injury to these nerves isassociated with long term postoperative pain and complications. The aim of thepresent study was to highlight the complexity and variation in the innervation ofthe inguinal region in order to increase proper nerve identification during surgicalinterventions. Bilateral dissection of the inguinal and posterior abdominal regionsin one human male cadaver revealed an atypical anatomic topography of thegroin innervation. This unusual case was observed at the Jagiellonian UniversityAnatomy Department during routine cadaveric preparations. The left ilioinguinalnerve was absent. The left genital branch of the genitofemoral nerve arose higherthan expected from the lumbar plexus and supplied the groin region, which istypically innervated by the ilioinguinal nerve. Furthermore, the left lateral cutaneousfemoral nerve and the right genital branch of the genitofemoral nerve alsofollowed uncharacteristic courses. Awareness of topographical nerve variationsduring inguinal hernia repair will help surgeons identify and preserve importantnerves, thus decreasing the incidence of chronic postoperative pain

Effects of hospital facilities on patient outcomes after cancer surgery: an international, prospective, observational study

    Get PDF
Background Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods A multimethods analysis was performed as part of the GlobalSurg 3 study-a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital.Findings Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. After adjustment for case-mix and country income group, hospitals with three or fewer of these facilities (62 hospitals, 1294 patients) had higher mortality compared with those with four or five (adjusted odds ratio [OR] 3.85 [95% CI 2.58-5.75]; p<0.0001), with excess mortality predominantly explained by a limited capacity to rescue following the development of major complications (63.0% vs 82.7%; OR 0.35 [0.23-0.53]; p<0.0001). Across LMICs, improvements in hospital facilities would prevent one to three deaths for every 100 patients undergoing surgery for cancer.Interpretation Hospitals with higher levels of infrastructure and resources have better outcomes after cancer surgery, independent of country income. Without urgent strengthening of hospital infrastructure and resources, the reductions in cancer-associated mortality associated with improved access will not be realised

Contemporary challanges to human resource management in terms of quality management in public administration

In contemporary public administration there are various trends similar to business organizations. One of them is a change towards flexible and responsive human resources. Such a change is necessary to adjust to the changeable requirements of clients. Nowadays it is not enough to apply traditional management models – they need to be adjusted to the trends. More and more often public administration implements quality systems. In this application public offices en-counter numerous problems with their employees: lack of motivation, communication problems, lack of effective communication and unwillingness to take responsibility

Influence of slim obstacle geometry on the flow and heat transfer in microchannels

    No full text
This paper presents a computational study on hydrodynamic and heat transfer characteristics of the laminar flow inside a rectangular 2D microchannel of height H, which includes a slim micro obstacle of height h and width w placed on the lower wall of the channel. The Reynolds number varies between 20 and 200. Three different values of height h and two different shapes of the slim obstacles: triangular and rectangular one, are considered. Thus, a total of 24 geometrical configurations of fluid flow are analyzed. Fluid flow equations are solved using the commercial CFD package of ADINA R&D, Inc. 9.1. Detailed analysis of the fluid velocity field and streamlines is carried out to investigate the flows in recirculation zone behind the obstacle. Results obtained show that the rectangular obstacle caused larger vortex formation in fluid flow. For flows with larger value of the (h/H) ratio, an increase in the value of loss coefficient factors is observed. Meanwhile, the increased Reynolds number causes the vortex zone behind the rectangular obstacle to be larger than behind the triangular one

Wulkanizaty kauczuku naturalnego napełnionego sadzą lub kredą i usieciowanego nadtlenkiem dikumylu, modyfikowano dodatkiem różnych olejków eterycznych o potwierdzonym w literaturze działaniu biobójczym. Natural rubber compounds filled with carbon black or calcium carbonate and crosslinked using dicumyl peroxide, were modified by addition of various essential oils with biocidal action confirmed in the literature. It was found that the type of essential oil and filler used determines the degree of crosslinking and mechanical properties of the vulcanizates. The resistance against mould fungi was shown by vulcanizates modified with thyme, clove or geranium oil, whereas bactericidal action was observed in cases of tea tree, mandarin or lemongrass oil used as modifying agents
