    Penetration Pathways Induced by Low-Frequency Sonophoresis with Physical and Chemical Enhancers: Iron Oxide Nanoparticles versus Lanthanum Nitrates

    Low-frequency sonophoresis (LFS) has been shown to disrupt the structure of stratum corneum (SC) lipid bilayers and enhance SC permeability. In this study, we examined the penetration pathway of lanthanum nitrate (LaNO3) tracer in viable epidermis after combined treatment of LFS and tape stripping (TS), as a physical enhancer, or oleic acid (OA) application, as a chemical enhancer, using transmission electron microscopy (TEM). As a positive control, we visualized the passive diffusion pathway of LaNO3 and iron oxide (Fe3O4) nanoparticles after the incision of hairless mouse skin. Next, we applied LFS immediately after TS or OA application and visualized the penetration pathway of LaNO3. Each treatment showed restricted penetration to the SCā€“stratum granulosum (SG) interface or upper SG layer. However, the additional application of LFS induced diffuse intracellular distribution of LaNO3 throughout the viable epidermis. Quantitative analysis also revealed that combined treatment significantly increases LaNO3 penetration into viable epidermis when compared with each treatment. Our ultrastructural findings show the synergistic effect of LFS and TS or OA application on transdermal drug delivery. We also found that this combined treatment enhances the penetration of LaNO3 through the viable epidermis through an intracellular pathway

    Six-Month Comparison of Coronary Endothelial Dysfunction Associated With Sirolimus-Eluting Stent Versus Paclitaxel-Eluting Stent

    ObjectivesThis study was designed to investigate whether endothelial dysfunction is related to drug-eluting stent (DES) implantation at 6 months after stenting.BackgroundCurrent available DES could delay vessel healing and subsequently impair endothelial function.MethodsEndothelial function was estimated at 6-month follow-up in 75 patients (31 men, mean age 62.1 years) with a DES (39 sirolimus-eluting stents [SES], 36 paclitaxel-eluting stents [PES]), and 10 patients with a bare-metal stent (BMS) to the left anterior descending artery, by incremental acetylcholine (Ach) infusion (20 Ī¼g/min, 50 Ī¼g/min, 100 Ī¼g/min) and nitrate (200 Ī¼g/min) into the left coronary ostium. Vascular responses were quantitatively measured in arterial segments 5 mm proximal and distal to DES and compared with corresponding segments in the BMS group and midsegments in the left circumflex artery as a reference nonstented artery. All antianginal agents were withheld for at least 72 h before coronary angiography.ResultsGreater vasoconstriction to Ach was observed in both the SES and PES groups than in the BMS group or control segments of left circumflex artery. Vasoconstriction to Ach was more prominent in arterial segments distal to stents in both SES and PES groups compared with those in the BMS group (p < 0.001). The degree of vasoconstriction to Ach was similar between the SES and PES groups. Endothelium-independent vasodilatation to nitrate did not differ significantly between the study groups.ConclusionsAbnormal vasoconstriction to Ach was found in the SES and PES groups, especially in arterial segments distal to DES at 6 months after stenting, which suggests that DES has a potential long-term adverse effect on local coronary endothelial dysfunction

    Development and Validation of 3-Year Atrial Fibrillation Prediction Models Using Electronic Health Record With or Without Standardized Electrocardiogram Diagnosis and a Performance Comparison Among Models.

    Background Improved prediction of atrial fibrillation (AF) may allow for earlier interventions for stroke prevention, as well as mortality and morbidity from other AF-related complications. We developed a clinically feasible and accurate AF prediction model using electronic health records and computerized ECG interpretation. Methods and Results A total of 671ā€‰318 patients were screened from 3 tertiary hospitals. After careful exclusion of cases with missing values and a prior AF diagnosis, AF prediction models were developed from the derivation cohort of 25ā€‰584 patients without AF at baseline. In the internal/external validation cohort of 117ā€‰523 patients, the model using 6 clinical features and 5 ECG diagnoses showed the highest performance for 3-year new-onset AF prediction (C-statistic, 0.796 [95% CI, 0.785-0.806]). A more simplified model using age, sex, and 5 ECG diagnoses (atrioventricular block, fusion beats, marked sinus arrhythmia, supraventricular premature complex, and wide QRS complex) had comparable predictive power (C-statistic, 0.777 [95% CI, 0.766-0.788]). The simplified model showed a similar or better predictive performance than the previous models. In the subgroup analysis, the models performed relatively better in patients without risk factors. Specifically, the predictive power was lower in patients with heart failure or decreased renal function. Conclusions Although the 3-year AF prediction model using both clinical and ECG variables showed the highest performance, the simplified model using age, sex, and 5 ECG diagnoses also had a comparable prediction power with broad applicability for incident AF

    Neurocognitive impairments and neuropsychiatric risk in 22q11 deletion syndrome

    22q11 Deletion Syndrome (22q11DS) is associated with somatic anomalies, learning disability, and psychiatric disturbance; a high rate of schizophrenia (10-40%) has been reported in affected adults. The aim of this study was to acquire evidence for genetic disruption of specific developmental processes that may explain the later emergence of psychotic illness in 22q11DS. Neurobiological traits associated with risk for psychosis (endophenotypes) were selected for investigation following review of the existing literature on genetics, neurodevelopment and schizophrenia, and were investigated using cognitive and EEG event-related potential (ERP) methodologies. Adolescents and young adults with 22q11DS were compared to age- and IQ-matched controls on all measures. Between-groups analysis revealed abnormalities of auditory processing, working memory and expressive language in 22q11DS. ERPs provided strong evidence for schizophrenia-like disruption; mismatch negativity (MMN), an ERP elicited by any discriminable change in a repetitive auditory sequence, was reduced in amplitude at frontal electrodes but not at temporal sites. Anomalous context-dependence of speech MMN was also observed; individuals with 22q11DS displayed a specific deficit in eliciting MMN in response to voicing contrasts between speech sounds. No such deficits were found in children with Specific Language Impairment, although other abnormalities in auditory processing were apparent. Within-group analysis indicated that abnormal auditory ERPs were associated with psychiatric symptoms akin to schizotypal personality disorder in some 22q11DS individuals. The association between these impairments supports the view that MMN indexes neurophysio logical processes relevant to psychotic illness. The severity of neurocognitive abnormalities in 22q11DS was found to be modulated by the catechol-o- methyl transferase met 158 val polymorphism on the single non-deleted chromosome 22. Functional variants of the COMT gene have been associated with risk for schizophrenia and schizophrenia-like cognitive abnormalities in the general population. Thus developmental dysregulation of catecholamine systems may at least partially explain the association between 22q11DS and psychosis

    Overlapping Ablation Using a Coaxial Radiofrequency Electrode and Multiple Cannulae System: Experimental Study in ex-Vivo Bovine Liver

    Objective To assess the sizes and configurations of thermal zones after overlapping ablations using a coaxial radiofrequency (RF) electrode and multiple cannulae in ex-vivo bovine liver. Materials and Methods For ablation procedures, a coaxial RF electrode and introducer set was used. Employing real-time ultrasound guidance and overlapping techniques in explanted, fresh bovine liver, we created five kinds of thermal zones with one (n=10), two (n=8), four (n=3), and six ablation spheres (n=3). Following ablation, MR images were obtained and the dimensions of all thermal zones were measured on the longitudinal or transverse section of specimens. The shape of the composite ablation zones was evaluated using three-dimensional MR image reconstruction. Results At gross pathologic examination of ten single-ablation zones (spheres), the long-axis (transverse) and short-axis lengths of zones ranged from 3.7 to 4.4 (mean, 4.1) cm and from 3.5 to 4.0 (mean, 3.7) cm, respectively. The long-axis (transverse) and short-axis lengths of double-ablation zones (cylinders) with 23% overlap ranged from 7.0 to 7.7 (mean, 7.3) cm and from 3.0 to 3.9 (mean, 3.5) cm, respectively; those with 58% overlap ranged from 6.0 to 6.4 (mean, 6.2) cm and from 3.8 to 4.6 (mean, 4.3) cm, respectively. The long-axis (diagonal) and short-axis lengths on a transverse section of four-ablation zones (cakes) ranged from 8.5 to 9.7 (mean, 9.1) cm and from 3.0 to 4.1 (mean, 3.7) cm, respectively. Gross pathologic examination of three composite six-ablation zones (spheres) showed that the long-axis (diagonal) and short-axis lengths of zones ranged from 9.0 to 9.9 (mean, 9.4) cm and from 6.8 to 7.5 (mean, 7.2) cm, respectively. T2-weighted MR images depicted low-signal thermal zones containing multiple curvilinear and spotty regions of hyperintensity. Conclusion Using a coaxial RF electrode and multiple cannulae, together with ultrasound guidance and precise overlapping ablation techniques, we successfully created predictable thermal zones in ex-vivo bovine liver.ope

    Bare-metal stents versus drug-eluting stents in large (ā‰„3.5mm) single coronary artery: Angiographic and clinical outcomes at 6 months

    SummaryBackgroundAlthough drug-eluting stents (DES) have been shown to dramatically reduce restenosis and improve the rate of event-free survival in large randomized trials, the benefit of DES appears to be limited to restenosis. In large arteries, it is not clear which type of stent is more superior in angiographic and clinical outcomes between DES and bare-metal stents (BMS). We compared the angiographic and clinical outcomes of DES versus BMS in large arteries (ā‰„3.5mm).MethodTwo hundred and forty patients from March 2002 to March 2007 received stents; 196 patients were treated with DES (44.9% sirolimus-eluting stents; 43.9% paclitaxel-eluting stents; 11.2% zotarolimus-eluting stents) and 44 with cobaltā€“chromium BMS for single de novo lesions in a large vessel. All subjects received aspirin, clopidogrel, and/or cilostazol as the standard antiplatelet regimen. The angiographic and clinical outcomes were evaluated at 6 months.ResultsFor the baseline characteristics, there were no significant differences between the DES and BMS groups. In addition, for the initially implanted stent there was no difference in the length, stent diameter, and lesion site between the two groups. After 6 months, the follow-up angiogram showed that in-stent diameter restenosis and late loss was more common with BMS than DES (39Ā±21% vs. 19Ā±17%, p=0.007; 1.44Ā±0.83mm vs. 0.62Ā±0.58mm, p=0.009, respectively). However, the target-lesion revascularization/target-vessel revascularization, and total major adverse cardiac events showed no significant differences between the groups (5.3% vs. 3.6%, p=0.62; 5.3% vs. 4.6%, p=0.86, respectively).ConclusionThe DES and cobaltā€“chromium BMS placed in large coronary arteries showed equally favorable 6-month clinical outcomes, although the 6-month angiographic results appeared more favorable in the DES group than in the BMS group

    The Observation of Electrical Hysteric Behavior in Synthesized V 2

    The anomalous electrical conductance for the V2O5 foam synthesized via a foaming process was measured. In the annealing process, the synthesized V2O5 foam is recrystallized with the increase of annealing temperature. The recrystallization procedure was characterized by using physical analysis tools such as thermogravimetric analysis (TGA), differential scanning calorimetry (DSC), scanning electron microscopy (SEM), high-resolution transmission electron microscopy (HRTEM), and X-ray diffractometer. In the electrical analysis technique of current-voltage characteristics as a function of annealing temperature, an anomalous hysteric behavior appears at the annealing temperature of 400Ā°C. We conclude that the recrystallization of V2O5 nanoplates results in the anomalous behavior in voltage-dependent current characteristics
