    Secondary prevention of stroke: Using the experiences of patients and carers to inform the development of an educational resource

Secondary prevention strategies that address medical risk factors and promote healthy lifestyles can reduce the risk. However, concordance with secondary prevention strategies is poor and there has been little research into patient and carer views. Objectives. To explore the experiences of patients and carers of receiving secondary prevention advice and use these to inform the development of an educational resource. Methods. A total of 38 participants (25 patients and 13 carers) took part in the study which used an action research approach. Focus groups and interviews were undertaken with patients and carers who had been discharged from hospital after stroke (between 3 and 24 months previously). Framework analysis was used to examine the data and elicit action points to develop an educational resource. Results. Participants’ main concern was their desire for early access to information. They commented on their priorities for what information or support they needed, the difficulty of absorbing complex information whilst still an in-patient and how health professionals’ use of language was often a barrier to understanding. They discussed the facilitators and barriers to making lifestyle changes. The educational resource was developed to include specific advice for medical and lifestyle risk factors and an individual action plan. Conclusion. An educational resource for secondary prevention of stroke was developed using a participatory methodology. Our findings suggest that this resource is best delivered in a one-to-one manner, but further work is needed to identify its potential utility.Peninsula Primary Care Research Networ

    Effects of hepatocyte nuclear factor-1A and -4A on pancreatic stone protein/regenerating protein and C-reactive protein gene expression: implications for maturity-onset diabetes of the young

    BACKGROUND: There is a significant clinical overlap between patients with hepatocyte nuclear factor (HNF)-1A and HNF4A maturity-onset diabetes of the young (MODY), two forms of monogenic diabetes. HNF1A and HNF4A are transcription factors that control common and partly overlapping sets of target genes. We have previously shown that elevated serum pancreatic stone protein / regenerating protein A (PSP/reg1A) levels can be detected in subjects with HNF1A-MODY. In this study, we investigated whether PSP/reg is differentially regulated by HNF1A and HNF4A. METHODS: Quantitative real-time PCR (qPCR) and Western blotting were used to validate gene and protein expression in cellular models of HNF1A- and HNF4A-MODY. Serum PSP/reg1A levels and high-sensitivity C-reactive protein (hsCRP) were measured by ELISA in 31 HNF1A- and 9 HNF4A-MODY subjects. The two groups were matched for age, body mass index, diabetes duration, blood pressure, lipid profile and aspirin and statin use. RESULTS: Inducible repression of HNF1A and HNF4A function in INS-1 cells suggested that PSP/reg induction required HNF4A, but not HNF1A. In contrast, crp gene expression was significantly reduced by repression of HNF1A, but not HNF4A function. PSP/reg levels were significantly lower in HNF4A subjects when compared to HNF1A subjects [9.25 (7.85-12.85) ng/ml vs. 12.5 (10.61-17.87) ng/ml, U-test P = 0.025]. hsCRP levels were significantly lower in HNF1A-MODY [0.22 (0.17-0.35) mg/L] compared to HNF4A-MODY group [0.81 (0.38-1.41) mg/L, U-test P = 0.002], Parallel measurements of serum PSP/reg1A and hsCRP levels were able to discriminate HNF1A- and HNF4A-MODY subjects. CONCLUSION: Our study demonstrates that two distinct target genes, PSP/reg and crp, are differentially regulated by HNF1A and HNF4A, and provides clinical proof-of-concept that serum PSP/reg1A and hsCRP levels may distinguish HNF1A-MODY from HNF4A-MODY subjects

    Drying dynamics of sessile-droplet arrays

    We analyze the diffusion-controlled evaporation of multiple droplets placed near each other on a planar substrate. Specifically, we calculate the change in the volume of sessile droplets with various initial contact angles that are arranged in different configurations. The calculations are supplemented by experimental measurements using a technique that interprets the variable magnification of a pattern placed beneath the droplet array, which is applied to the case of initially hemispherical droplets deposited in four distinct arrangements. We find excellent agreement between the predictions based on the theory of Masoud et al. [Evaporation of multiple droplets, J. Fluid Mech. 927, R4 (2021)] and the data gathered experimentally. Perhaps unexpectedly, we also find that when comparing different arrays, the droplets with the same order of disappearance within their respective array, i.e., fastest evaporating, second-fastest evaporating, etc., follow similar drying dynamics. Our study provides not only experimental validation of the theoretical framework introduced by Masoud et al., but also offers additional insights into the evolution of the volume of individual droplets when evaporating within closely-spaced arrays

    The North Sea Benthos Project: planning, management and objectives

    The ICES Benthos Ecology Working Group is integrating recent macrobenthic infaunal data (1999-2001) available from various sources, including national monitoring surveys, in North Sea soft bottom sediments. lt is expected to cover most of the North Sea. The main goal is an overall comparison with the North Sea Benthos Survey data of 1986, in order to determine whether there have been any significant changes and, if so, what may be the causal influences (e.g., climate change, fishing impacts). The work will contribute valuable information on several other topics such as habitat classification and the distribution of endangered species. Therefore, in addition to physico-chemical measurements of sediments samples alongside the benthic fauna, information on water depths, temperature, water quality and salinity will be incorporated in the analysis of species and community distributions. Also, we will use existing ecological and hydrographical models for currents, bottom shear stress and carbon input, along with information on the distribution of habitat types, to explain the observed distribution patterns. At the ASC, an overview of the data available will be presented as well as the anticipated outcomes, and the first steps taken to deal with taxonomic differences and other issues affecting the capability to integrate submitted information

    Radius and chirality dependent conformation of polymer molecule at nanotube interface

    Temperature dependent conformations of linear polymer molecules adsorbed at carbon nanotube (CNT) interfaces are investigated through molecule dynamics simulations. Model polyethylene (PE) molecules are shown to have selective conformations on CNT surface, controlled by atomic structures of CNT lattice and geometric coiling energy. PE molecules form entropy driven assembly domains, and their preferred wrapping angles around large radius CNT (40, 40) reflect the molecule configurations with energy minimums on a graphite plane. While PE molecules prefer wrapping on small radius armchair CNT (5, 5) predominantly at low temperatures, their configurations are shifted to larger wrapping angle ones on a similar radius zigzag CNT (10, 0).     Fractional exhaled nitric oxide in preterm-born subjects: A systematic review and meta-analysis

    Background Decreased lung function is common in preterm‐born survivors. Increased fractional exhaled nitric oxide (FeNO) appears to be a reliable test for eosinophillic airway inflammation especially in asthma. We, systematically, reviewed the literature to compare FeNO levels in preterm‐born children and adults who did or did not have chronic lung disease of prematurity (CLD) in infancy with term‐born controls. Methods We searched eight databases up to February 2018. Studies comparing FeNO levels in preterm‐born subjects (<37 weeks’ gestation) in childhood and adulthood with and without (CLD) with term‐born subjects were identified and extracted by two reviewers. Data were analysed using Review Manager v5.3. Results From 6042 article titles, 183 full articles were screened for inclusion. Nineteen studies met the inclusion criteria. Seventeen studies compared FeNO levels in preterm‐ and term‐born children and adults; 11 studies (preterm n = 640 and term n = 4005) were included in a meta‐analysis. The mean FeNO concentration difference between the preterm‐born and term‐born group was −0.74 (95% CI −1.88 to 0.41) ppb. For the six studies reporting data on CLD (preterm n = 204 and term n = 211) the mean difference for FeNO levels was −2.82 (95% CI −5.87 to 0.22) ppb between the preterm‐born CLD and term‐born groups. Conclusions Our data suggest that preterm born children with and without CLD have similar FeNO levels to term‐born children suggesting an alternative mechanism to eosinophilic inflammation for symptoms of wheezing and airway obstruction observed in preterm‐born subjects
