Forme atypique d’une maladie de Lyme
La maladie de Lyme est une zoonose qui se manifeste essentiellement par des signes cutanés, articulaires, neurologiques et cardiaques. Elle peut exceptionnellement mimer une dermatomyosite. Nous rapportons l'observation d'une patiente âgée de 37 ans qui a présenté des myalgies et un érythroedème de la face et en péri-orbitaire. Une dermatomyosite a été fortement suspectée devant une élévation des enzymes musculaires. Elle a été traitée par une corticothérapie à forte dose. L'évolution a été marquée par l'aggravation des myalgies et l'apparition d'un déficit musculaire. Elle a été alors hospitalisée dans notre service. Les enzymes musculaires étaient élevées. L'électromyogramme était sans anomalies. Le bilan immunologique était négatif. Une enquête infectieuse a été réalisée retrouvant une sérologie de Lyme positive. Après administration d'une antibiothérapie adaptée, l'évolution était bonne avec disparition du tableau clinico-biologique de dermatomyosite. La précocité du diagnostic et l'instauration d'un traitement efficace permet d'éviter des complications graves
SYNTHESIS, IN VITRO ANTIMICROBIAL, ANTI-LIVER CANCER EVALUATION OF SOME NOVEL BIS-CYANOACRYLAMIDE AND BIS-AZOLES DERIVATIVES
Objective: Synthesis of some novel bis [3-(aryl)-2-cyanoacrylamide], bis-pyrazole, bis-thiazole and bis-triazole derivatives starting from N,N'-ethane-1,2-diylbis(2-cyanoacetamide) (1) to evaluate for their in-vitro antibacterial, antifungal and anticancer activities.Methods: Reaction of N, N'-ethane-1,2-diylbis(2-cyanoacetamide) (1) with different aromatic and heteroaromatic aldehydes yielded the corresponding bis[3-(aryl)-2-cyanoacrylamide] derivatives, which reacted with hydrazine hydrate to give N,N'-ethane-1,2-diylbis[3-amino-5-(4-aryl)-1H-pyrazole-4-carboxamide] derivatives. Compound 1 reacted with each of thioglycolic acid, Phenyl isocyanate and elemental sulfur in presence triethylamine to give bis-thiazole derivatives. Diazotization of 1 with the desired diazonium chloride yielded the bis-hydrazone derivatives. The latter compounds refluxed with hydroxylamine hydrochloride and chloro- acetonitrile to give bis-triazole and bis-pyrazole derivatives respectively.Results: The structures of the newly synthesized compounds were confirmed by elemental analysis, IR, 1H NMR, [13]C NMR and mass spectral data. A total fourteen new synthesized compounds were evaluated for their in-vitro antibacterial, antifungal and anticancer activities against human liver cancer cell line (HEPG-2).Conclusion: The results obtained indicated that some of such compounds showed promising activities against Gram-positive, Gram-negative bacteria, fungi and anticancer activity in relation to the reference drugs ampicillin, gentamicin, Amphotericin B and vinblastine respectively.Â
Difficulté diagnostique d’une tuberculose laryngée isolée chez une femme diabétique
La tuberculose laryngée isolée est rare. Elle peut mimer une tumeur maligne retardant le diagnostic et aggravant le pronostic. Le terrain diabétique favorise la survenue des infections notamment la tuberculose dans sa forme sévère. Nous rapportons l'observation d'une patiente diabétique de type 2, âgée de 51 ans, hospitalisée pour une fièvre prolongée, une dysphonie et un diabète déséquilibré. Les hémocultures étaient négatives. L'examen laryngoscopique a confirmé un aspect érythémateux et épais des deux cordes, une lésion ulcérée peu profonde de l'hémilarynx gauche et une végétation érythémateuse de 5 mm de diamètre. L'examen histologique a révélé des granulomes épithéloïodes et giganto-cellulaires avec une nécrose caséeuse évocateurs de la tuberculose. La culture sur milieu de lobstein de tissus biopsiés à partir des cordes vocales a été positive. Aucun autre foyer tuberculeux n'a été retrouvé. Le traitement par une combinaison de l'isoniazide, la rifampicine, l'éthambutol, la pyrazinamide a permis une résolution de symptôme
Marginal bone loss around platform-switched and platform-matched implants following immediate dental implant placement – Systematic Review
Objective
This study aimed to examine marginal bone loss (MBL) around immediately placed platform-switched implants (PS) compared to platform-matched (PM) implants; and to critically appraise the available literature on this topic.
Materials and Methods
Randomized control trials (RCTs), non- randomized control trials (NRCT) and case series of immediate placement platform-switched and platform-matched implant, published in English were included in the study. Two databases, namely Medline and PubMed covering the period between July 1966 and July 2023 were searched. A total of five case series, five RCTs and one NRCT were included in this systematic review by using pre-defined study selection criteria and following the PRISMA protocol. A critical appraisal of the selected studies was completed using standardized appraisal checklists, including CASP tool for critical appraisal of RCTs, the Downs and Black checklist for NRCT, and the CEBMa checklist for case series studies.
Results
Five studies showed a statistically significant difference in MBL (PS: 0.18–0.78 mm, PM 0.51–1.19 mm). The studies featured a small sample size, and substantial methodological variability in patients’ selection criteria, implant and abutment designs, connection types and surgical protocols. A high risk of bias was identified, especially in case series studies.
Conclusion
The use of PS implants in immediate placement protocols can lead to a statistically significant reduction in MBL compared to PM implants. However, the results need to be interpreted with caution, given the numerous confounding variables and clinical heterogeneity existing between the studies
ALPHA EMITTERS RADIOACTIVITY CONCENTRATIONS IN SOME COSMETICS USED IN IRAQ USING LR-115 DETECTOR
Cosmetics play an important role of human external appearance, its products used to alter or enhance the facial appearance or the body and skincare, currently worldwide consumed with frequently use increasing the human body exposure to the various chemical elements including radioactive substances. This research aimed to measure the concentrations of alpha emitters, and the annual effective dose AED resulting from radon intake. LR-115 track detector used to measure radon, and radium concentrations in 20 cosmetics samples selected from markets. In this work the concentrations of radon in the samples found to vary from 9.876 to 30.97 Bq/m3 with 22.11 Bq/m3 a mean value, this mean value is a very small than 100 Bq/m3 the reference level limits of the World Health Organization (WHO). The annual radon effective dose varied between 0.249–0.781 mSv/y which is within the range of 0.2-10 mSv/y (UNSCEAR). Radium contents vary between 0.041–0.249 Bq/Kg with 0.115 Bq/Kg as a mean value. Radium and Uranium Concentrations of Powder Juice in the Iraq Markets Using CR-39 Detector
استخدمت مسحوق العصائر بصورة واسعة بسبب رخص ثمنها مقارنة بالعصائر الطازجة. هذه العصائر ربما تحتوي على بعض المواد المشعة. كان الهدف من الدراسة قياس تراكيز الراديوم واليورانيوم في بعض الانواع المختارة من الاسواق لمسحوق العصائر الاكثر استخداما. قيست تراكيز العناصر المشعة باستخدام تقنية كاشف CR-39 الموضوع في اناء مغلق. تركيز فعالية الرادون في هذه الدراسة كانت ضمن المدى 57.146 -151.129 Bq/m3 وبقيمة متوسطة 107.62 Bq/m3. وهي ضمن الحدود المقبولة 100 Bq/m3 لمنظمة الصحة العالمية (WHO), واقل من 200 Bq/m3 التي هي مستوى لوائح الوقاية الوطنية الاوربية. الجرعة السنوية الفعالة للرادون تغيرت بين 1.441-3.813 mSv/y وهي ضمن المدى 0.2-10 mSv/y المحدد من قبل (UNSCEAR). تركيز الراديوم وجد بانه يتغير بين القيم 0.188-0.498 Bq/kg وبقيمة متوسطة 0.354 Bq/kg. والجرعة السنوية الفعالة للراديوم كانت ضمن المدى 1.050-2.779 μSv/y. تركيز اليورانيوم تغيرت بين 0.150-0.398 ppm وبقيمة متوسطة 0.283 ppm. والجرعة السنوية الفعالة لليورانيوم كانت ضمن المدى 1.669-4.414 μSv/y. هذا البحث يساهم في تعريف وقياس الفعالية المحددة للعناصر المشعة الموجودة في مسحوق العصائر المتناولة بكثرة في العراق وبينت الدراسة ان هذه الانواع من العصائر امنة للاستهلاك.Powder juices are widely used by the people in the study area because its low price compared to fresh juices. These powder juices may contain some radioactive materials. The study aimed to measure radioactivity concentrations of Radium and Uranium in the most used kinds of powder juice samples, selected from Iraq markets. CR-39 detector with sealed can technique, used to measure the radioactive elements concentrations. The radionuclides observed in the present work are, the estimated radon activity concentration range is between 57.146-151.129 Bq/m3 with a mean value of 107.62 Bq/m3. The radon concentration mean value is within of 100 Bq/m3 the limits of (WHO) World Health Organization reference level, below of (NRPB) UK National Radiation Protection Board, and below of 200 Bq/m3 the European Commission Recommendation Level. The annual effective dose of radon varied between 1.441-3.813 mSv/y, are within the (UNSCEAR) range 0.2-10 mSv/y. Radium contents were found to vary between 0.188-0.498 Bq/kg and a mean value of 0.354 Bq/kg. The radium effective annual dose ranged between 1.050-2.779 μSv/y. The concentrations of uranium are in the range between 0.150-0.398 ppm with men value of 0.283 ppm. The effective annual dose of uranium varied between 1.669-4.414 μSv/y. This work helped in identifying and measuring the specified activity of radionuclide present in powdered fruit juices consuming in Iraq and showed that these kinds of powder juice are safe for consumption
Relaxant Effects of Essential Oils Of Eucalyptus Camaldulensis on Aortic Rings in Male Albino Rats
The essential oil (EO) produced a potent dilation in aortic rings with percentage of relaxation 95.659%.The significant relaxant effect on thoracic aortic rings was inhibited by GLIB (10-5) and 4-AP (1mM)with percentages of relaxation, 74.577% and 51.494%, respectively. On the other hand, vasorelaxation in aortic rings did not influenced by pretreatment with TEA and BaCl2. Pretreatment of aortic rings with nifedipine, significantly blocked the relaxation to 38.7%. On the other hand, the dose-response shifted to the right in endothelium denuded preparation. The percentage of relaxation in endothelium-denuded was 82.28%. Preincubation of aortic rings with L-NAME and Indomethacin altered the dilation from control significantly, and the percentage of relaxation found to be 60.18% and 77.077%, respectively. The present study suggest that the EO of E .camaldulensis depresses the aortic force development, probably acting as a Ca++ channel antagonism and partially via NO and cyclooxygenase pathways, the involvement of KATP and KV channels in the vasorelaxatoin induced by EO
The Role of K+ And Ca2+ Ion Channels in Α-Terpinyle Acetate-Induced Vasodilation in Rat’s Aortic Rings
The monoterpene, α-terpinyle acetate (TA) is a constituent of essential oils present in aromatic plants. Since the role of ion channels and endothelial hyperpolarizing factors in TA induced relaxation in rat’s aorta is unknown, the current study aimed to study the mechanism underlying the vasodilatory effect of TA in isolated aortic rings. Terpinyle acetate induced a potent vasodilation in rat aortic rings with a percentage of relaxation of 63.79 %. The results of the role of K+ channel subtypes in vasorelaxation revealed that both Kv and KATP played a major role since GLIB produced a maximum percent of inhibition in the relaxation produced by TA to 8.91 %; this was followed by 4-AP in which the percent of inhibition reduced to 14.95. On the other hand, Kir played no role in the TA induced vasorelaxation since BaCl2 did not produce any inhibition in aortic relaxation. Furthermore, also L-type Ca2+ channel played no role in TA induced relaxation since the L-type Ca2+ channel inhibitor Nifedipine did not reduce the percent of relaxation. Endothelium also played a considerable role in the induced vasorelaxation since, in denuded aorta, the percent of relaxation was reduced to 36%. Preincubation of the aortic ring with methylene blue, a soluble cGMP inhibitor also significantly reduced the TA induced relaxation to 16.39%. In contrast, preincubation with cyclooxygenase inhibitor Indomethacin did not produce any inhibitory effect on AT induced vasorelaxation. It can be concluded from these novel results that AT induced vasorelaxation involve the activation of KV, KATP channels and at least partly dependent on endothelium via the activation NO-cGMP signal transduction pathway
