    Sinus Barotraumas in Commercial Aircrew

    BACKGROUND: Sinus barotraumas are a common condition in aviation medicine, sometimes compromising flight safety and even permanently grounding aircrew. Considering this and the ever-increasing amount of commercial aviation, a thorough examination is required. METHODS: In this survey study, an anonymous, electronic questionnaire was distributed to commercial aircrew of the three major commercial airlines operating in Finland (N = 3799), covering 93% of the target population (i.e., all commercial aircrew operating in Finland, N = 4083). Primary outcomes were self-reported prevalence, clinical characteristics, and health and occupational effects of sinus barotraumas in flight. Secondary outcomes were adjusted odds ratios (OR) for frequency of sinus barotraumas with respect to possible risk factors. RESULTS: Response rate was 47% (N = 1789/3799), with 61% (N = 1088) of the respondents having experienced sinus barotraumas in flight. Of those affected, 59% had used medications, 18% had undergone surgical procedures, and 53% had been on sick leave due to sinus barotraumas (38% during the last year) in flight.     BACKGROUND: Middle ear (ME) barotraumas are the most common condition in aviation medicine, sometimes seriously compromising flight safety. Considering this and the ever-increasing amount of commercial aviation, a detailed overview is warranted. METHODS: In this survey study, an anonymous, electronic questionnaire was distributed to commercial aircrew of the three major commercial airlines operating in Finland (N = 3799), covering 93% of the target population (i.e., all commercial aircrew operating in Finland, N = 4083). Primary outcomes were self-reported prevalence, clinical characteristics, and health and occupational effects of ME barotraumas in flight. Secondary outcomes were adjusted odds ratios (OR) for frequency of ME barotraumas with respect to possible risk factors. RESULTS: Response rate was 47% (N = 1789/3799), with 85% (N 1516) having experienced ME barotraumas in flight. Of those affected, 60% had used medications, 5% had undergone surgical procedures, and 48% had been on sick leave due to ME barotraumas (40% during the last year). Factors associated with ME barotraumas included a high number of upper respiratory tract infections [>= 3 URTIs/yr vs. 0 LIRTIs/yr: OR, 9.02; 95% confidence interval (CI) 3.99-20.39] and poor subjective performance in Valsalva ("occasionally" vs."a lways" successful: OR, 7.84; 95% CI 3.97-15.51) and Toynbee ("occasionally" vs."always" successful: OR, 9.06; 95% CI 2.67-30.78) maneuvers. CONCLUSION: ME barotraumas were reported by 85% of commercial aircrew.They lead to an increased need for medications, otorhinolaryngology-related surgical procedures, and sickness absence from flight duty.     Self-reported reasons for on-duty sleepiness among commercial airline pilots

    Experimental and epidemiological research has shown that human sleepiness is determined especially by the circadian and homeostatic processes. The present field study examined which work-related factors airline pilots perceive as causing on-duty sleepiness during short-haul and long-haul flights. In addition, the association between the perceived reasons for sleepiness and actual sleepiness levels was examined, as well as the association between reporting inadequate sleep causing sleepiness and actual sleep-wake history. The study sample consisted of 29 long-haul (LH) pilots, 28 short-haul (SH) pilots, and 29 mixed fleet pilots (flying both SH and LH flights), each of whom participated in a 2-month field measurement period, yielding a total of 765 SH and 494 LH flight duty periods (FDPs) for analyses (FDP, a period between the start of a duty and the end of the last flight of that duty). The self-reports of sleepiness inducers were collected at the end of each FDP by an electronic select menu. On-duty sleepiness was rated at each flight phase by the Karolinska Sleepiness Scale (KSS). The sleep-wake data was collected by a diary and actigraph. The results showed that "FDP timing" and "inadequate sleep" were the most frequently reported reasons for on-duty sleepiness out of the seven options provided, regardless of FDP type (SH, LH). Reporting these reasons significantly increased the odds of increased on-duty sleepiness (KSS >= 7), except for reporting "inadequate sleep" during LH FDPs.     Bound states of 3He at the edge of a 4He drop on a cesium surface

    We show that small amounts of 3He atoms, added to a 4He drop deposited on a flat cesium surface at zero temperature, populate bound states localized at the contact line. These edge states show up for drops large enough to develop well defined surface and bulk regions together with a contact line, and they are structurally different from the well-known Andreev states that appear at the free surface and at the liquid-solid interface of films. We illustrate the one-body density of 3He in a drop with 1000 4He atoms, and show that for sufficiently large number of impurities, the density profiles spread beyond the edge, coating both the curved drop surface and its flat base and eventually isolating it from the substrate.Comment: 10 pages and 7 figures.     Measuring functional outcome in upper extremity soft-tissue sarcoma : Validation of the Toronto extremity salvage score and the QuickDASH patient-reported outcome instruments

    Interest in functional outcome (FO) and health-related quality of life (HRQL) in extremity soft-tissue sarcoma (STS) patients has increased. The aim of this study was to validate two FO questionnaires for upper extremity STS patients: the Toronto Extremity Salvage Score (TESS) and short version of the Disability of Arm, Shoulder and Hand (QuickDASH), based on Finnish population data. A multi-center study was conducted at two academic sarcoma centers. Surgically treated upper extremity STS patients were invited to participate. Patients completed the TESS and the QuickDASH with HRQL questionnaires the 15D and the QLQ-C30. The scores were analyzed and compared. Fifty-five patients with a mean follow-up period of 4.7 years were included. Mean age was 63 years (standard deviation [SD] 14.6). The mean score for TESS was 88.5 (SD 15.1) and for QuickDASH 17.8 (SD 19.6). The QuickDASH had a statistically significantly better score coverage. A ceiling effect was noted, 27% and 20% for TESS and QuickDASH, respectively. The TESS and QuickDASH scores were strongly correlated ( r =-0.89). The TESS score strongly correlated with the QLQ-C30 ( r = 0.79) and the 15D score ( r = 0.70). The QuickDASH score correlated strongly with the QLQ-C30 score ( r =-0.71) and moderately with the 15D score ( r =-0.56). The TESS score had a statistically significantly stronger correlation with the 15D score than QuickDASH ( p < 0.005). Both the TESS and the QuickDASH provide reliable scores for assessing FO in upper extremity STS patients.     To wet or not to wet: that is the question

    Wetting transitions have been predicted and observed to occur for various combinations of fluids and surfaces. This paper describes the origin of such transitions, for liquid films on solid surfaces, in terms of the gas-surface interaction potentials V(r), which depend on the specific adsorption system. The transitions of light inert gases and H2 molecules on alkali metal surfaces have been explored extensively and are relatively well understood in terms of the least attractive adsorption interactions in nature. Much less thoroughly investigated are wetting transitions of Hg, water, heavy inert gases and other molecular films. The basic idea is that nonwetting occurs, for energetic reasons, if the adsorption potential's well-depth D is smaller than, or comparable to, the well-depth of the adsorbate-adsorbate mutual interaction. At the wetting temperature, Tw, the transition to wetting occurs, for entropic reasons, when the liquid's surface tension is sufficiently small that the free energy cost in forming a thick film is sufficiently compensated by the fluid- surface interaction energy. Guidelines useful for exploring wetting transitions of other systems are analyzed, in terms of generic criteria involving the "simple model", which yields results in terms of gas-surface interaction parameters and thermodynamic properties of the bulk adsorbate.Comment: Article accepted for publication in J.     Changing the structural and mechanical anisotropy of foam-formed cellulose materials by affecting bubble-fiber interaction with surfactant

    Cellulose fiber materials suitable for filtering, insulation, protective, and hygiene applications can be formed using aqueous foam as a carrier phase. The subtle fiber–bubble interaction provides a tool which can be utilized to alter both structural and mechanical material properties. Earlier model surface studies have only indicated clear surface-bubble adhesion when both the surface hydrophobicity and surface tension of the solution are high enough. In this work, we first show that for silica model surfaces these basic mechanisms are similar for both nonionic polyethylene glycol sorbitan monolaurate (Tween 20) and anionic sodium dodecyl sulfate (SDS) surfactants. In the second step, thick nonwoven materials were foam formed from hydrophilic or hydrophobic viscose fibers using small amounts of cellulose microfibers (CMFs) to form a bonding agent. There was a clear variation in structure and strength properties between the samples made using different fibers and surfactants. The partial alignment and layering of fibers in the wet foam led to anisotropy in the mechanical properties of the formed samples. Using SDS, the fiber alignment was very strong for hydrophilic fibers but was reduced for hydrophobic fibers because of stronger coupling to bubbles during structure forming, impacting the microscale fiber network. For nonionic Tween 20, in addition to surfactant adsorption on the fibers, the ethoxylated surfactant headgroups are suggested to form bridges between CMFs and other fibers, restricting fiber movements during formation. For hydrophilic fibers, this showed up as a lower in-plane compression modulus but higher transverse strength for Tween 20 compared with SDS surfactant.     Prewetting transitions of Ar and Ne on alkali metal surfaces

    We have studied by means of Density-Functional calculations the wetting properties of Ar and Ne adsorbed on a plane whose adsorption properties simulate the Li and Na surfaces. We use reliable ab-initio potentials to model the gas-substrate interactions.
