    Lung resection and application of latissimus dorsi muscle flap for pulmonary aspergilloma: Case report

    Aspergilloma, is the most common form of aspergillus infectionthat presents as parenchymal cavitary lesion withsaprophytic colonization. This disease is associated withvariety of lung diseases such as tuberculosis, sarcoidosisand bronchiectasis. The patient in this study with tuberculosiswas diagnosed 12 years ago and treated for sixmonths. Most frequently referred symptom is hemoptysis.Aspergilloma get a diagnosis with chest x-ray and thoraxcomputerized tomoghraphy. In treatment usually anatomicresection is performed, bronchial stump is supportedand the remaining space is filled with a flap. The pateintwas admitted to our hospital with hemoptysis and left upperlobectomy was performed due to cavitary lesion andbronchial stump was supported with latissimus dorsi muscleflap. Especially when it comes to epidemics, the influence of the media increases remarkably. Individuals now use digital media as the initial reference source, especially on issues related to their health, and tend to see social media as a reference platform. However, uncontrolled information, conspiracy theories, and information pollution spread through social media make the subject difficult to understand; online shared manipulative news, excessive and unfounded information cause fear and panic. These posts about vaccines affect the perception and attitude towards vaccines. In this study, the comments and social media posts will be analysed using content analysis and discourse analysis methods in order to reveal the effect of social media in vaccine rejection and hesitation

    A case report of twin reversed arterial perfusion sequence with expectant management

    Acardiac twin or TRAP (twin reversed arterial perfusion) sequence is a rare complication of monochorionic pregnancies. In these cases, the heart is either absent or non-functional. It’s controversial whether conservative management or therapeutic treatment is better in TRAP-sequence. In this case, we present a 19-years old, primigravida diagnosed with spontaneous monochorionic monoamniotic twin pregnancy at 7th week of gestation. One of the fetuses had a crown-rump length of 8 mm and fetal heart rate 122/minute while the other one had a crown-rump of 7 mm with no detected fetal heartbeat. At the 11th week of gestation, the acardiac twin continued growing despite the absence of the fetal heart beat and fetal extremities. The cranium could not be evaluated clearly. The diagnosis of TRAP sequence was confirmed by the reversed direction of flow observed in the umbilical artery. Since the patient and her husband did not want any intervention, no interventional diagnostic and treatment modalities were applied. Preterm labor started at the 32th week. Because the presentations were transverse and breech, pump and acardiac fetus, respectively, a cesarean delivery was performed. A healthy female baby, weighing 1650 gr with APGAR scores of 9-10, first and fifth minutes, respectively, was delivered along with the acardiac fetus which was 1550 gram in weight, fetal heart beat negative, with upper and lower extremity deformities. The uniqueness of the present case is that there was no significant difference in the weight of both twins. The acardiac twin was as large as the pump twin. Except for twenty days hospitalization because of neonatal respiratory distress syndrome, which was a consequence of preterm labor, there were no problems with the pump twin even though managed conservatively. In monochorionic twin pregnancies, when one of the twins is found to be fetal heartbeat negative and it continues to grow with concomitant structural abnormalities, the TRAP sequence should come to mind. If the acardiac fetus is small and the pump fetus has no complications, the conservative approach can be considered. Our goal should be to deliver the pump twin with minimal complications. To achieve this, follow-ups and treatment should be individualized for each patient, the pump fetus should be monitored closely

    The Effect of 12-Week Passive Aquatic Bodywork on Sympathovagal Balance of Obese Youth

    Background:  Obesity has been identified as a global epidemic that is associated with numerous comorbidities such as type 2 diabetes, hypertension, cancer, cardiovascular disease. We have investigated the effects of Watsu® therapy and Immersion on HRV parameters of obese male subjects. Methods and Results: Volunteer 34 obese subjects with BMI above 30 without any other chronic health issues were included (age, 18.30±.31, height, 166.02±29.60, BMI, 36.54±5.96). Randomized controlled trial was conducted with Watsu and Immersion protocols whereas control group received no therapy in water or on land for 12 weeks. A baseline HRV was recorded 5 minutes in four different circumstances. Polar H7 heart rate sensor and digital standardized HRV signal processing software were used to record the R-R intervals in millisecond. The repeated measures were used to compare the conditions between the therapies. The HRV analyses were performed using three-way Mixed ANOVA. Multiple comparisons were done with a Bonferroni adjustment of the alpha level (0.05). The collective results of HF showed significant negative effect (13.01±1.36, 9.99±1.11) and HF laying supine value (20.62±2.22) was the highest (p<0.05). Conclusion: A significant inverse correlation was found between HRV values and both therapeutic interventions. Counterintuitively, 12-week of watsu and immersion therapies decreased LF and HF

    Gebelikte Tütün Dumanı Maruziyetinin Anne Sıçan Akciğer Dokusunda Meydana Getirdiği Değişiklikler Üzerine Alfa Lipoik Asitin Etkilerinin İncelenmesi

    Amaç: Çalışmamızda gebelikte tütün dumanına maruz kalan anne sıçanların akciğer dokusunda meydana gelen değişiklikler üzerine alfa lipoik asitin etkilerinin deneysel sıçan modeli üzerinde araştırılması amaçlandı. Yöntemler: Çalışmada 28 adet dişi Sprague-Dawley cinsi sıçanlar kullanıldı. Gebe sıçanlar; Kontrol grubu, Tütün dumanı (TD) grubu, Tütün dumanı + Alfa lipoik asit (TD+ALA) grubu ve Alfa lipoik asit (ALA) grubu olmak üzere rastgele dört eşit gruba ayrıldı. TD ve TD+ALA grubundaki sıçanlar çiftleşmeden önce sekiz hafta ve gebelik süresince günde iki saat tütün dumanına maruz bırakıldı. TD+ALA ve ALA grubundaki sıçanlara ise çiftleşmeden önce sekiz hafta ve gebelik süresince gün aşırı oral gavaj yolu ile 20 mg/kg dozunda alfa lipoik asit verildi. Deneyin sonunda sıçanlar dekapite edilerek akciğer dokuları çıkarıldı ve histolojik, biyokimyasal ve immünohistokimyasal metotlar uygulandı. Bulgular: TD grubuna ait akciğer kesitlerinde inflamatuar hücre artışı, konjesyon, ödem, hemoraji gibi histopatolojik bulgular gözlendi. ALA uygulamasıyla bu histopatalojik bulgularda istatistiksel olarak anlamlı oranda düzelmeler izlendi. TD grubunda VEGF immünreaktivitesinin kontrol grubuna göre anlamlı artış gösterdiği, TD+ALA grubunda ise TD grubuna göre VEGF immünreaktivitesinin anlamlı derecede azaldığı belirlendi. TD grubunda MDA değerlerinin kontrole göre anlamlı derecede arttığı, TD+ALA grubunda ise TD grubuna göre anlamlı derecede azaldığı gözlendi. Sonuç: Tütün dumanının gebe sıçan akciğerinde yol açtığı oksidatif hasarın, alfa lipoik asit tedavisinin antioksidan etkileri ile kısmen engellendiği belirlendi

    Effect of Caffeic Acid Phenethyl Ester on Vascular Damage Caused by Consumption of High Fructose Corn Syrup in Rats

    Fructose corn syrup is cheap sweetener and prolongs the shelf life of products, but fructose intake causes hyperinsulinemia, hypertriglyceridemia, and hypertension. All of them are referred to as metabolic syndrome and they are risk factors for cardiovascular diseases. Hence, the harmful effects of increased fructose intake on health and their prevention should take greater consideration. Caffeic Acid Phenethyl Ester (CAPE) has beneficial effects on metabolic syndrome and vascular function which is important in the prevention of cardiovascular disease. However, there are no known studies about the effect of CAPE on fructoseinduced vascular dysfunction. In this study, we examined the effect of CAPE on vascular dysfunction due to high fructose corn syrup (HFCS). HFCS (6 weeks, 30% fed with drinking water) caused vascular dysfunction, but treatment with CAPE (50 micromol/kg i.p. for the last two weeks) effectively restored this problem. Additionally, hypertension in HFCS-fed rats was also decreased in CAPE supplemented rats. CAPE supplements lowered HFCS consumption-induced raise in blood glucose, homocysteine, and cholesterol levels. The aorta tissue endothelial nitric oxide synthase (eNOS) production was decreased in rats given HFCS and in contrast CAPE supplementation efficiently increased its production. The presented results showed that HFCS-induced cardiovascular abnormalities could be prevented by CAPE treatment

    This paper discusses the expansion of Oceania through a Marshallese indigenous lens as a focal point. It explains that decolonizing methodologies allows reclaiming of space for mental liberation and reassurement of constitutional rights. It highlights similar occurrences of decolonization practices meeting resistance in the 21st century all while strengthening the human right argument that no human deserves any less than their fellow human brothers and sisters. It argues that an indigenous imagery can only be viewed through an indigenous lens where the researches’ level of purity is retained and unfiltered. It nevertheless argues that Marshallese ethnolinguistics reveal the same cultural practices in America, Judeo-Christianity, and Oceania thus dictating the reality that “we are the same not withstanding one stays here and one there (Bedbedjin Bedbedjen, Bedbedjinma wot Kwe)”. It further explains the importance in these similarities and how Marshallese spirituality predates introduced American Judeo-Christianity despite the latter attempting to marginalize the former. It concludes by stating that Marshallese contributions on the global stage are rooted in that culture of love (IaKwe) which is echoed by the custom(s) revealing the significance of Marshallese validation academically, spiritually, economically, & socially to prevent institutionalized discrimination. This paper ends stating that the agency to know one’s self and how one should fit in the world, is a human right in itself and Marshallese are entitled to this sense of self worth through knowing thy self by thy self where real thinking takes place in one’s own mind as we all live our own lives

    Feasibility investigation of direct laser cutting process of metal foam with high pore density

    To avoid damage to the pore structure of metal foam, a laser cutting process for efficiently and directly cutting metal foam into regular shapes is proposed. After analyzing the proposed laser cutting process, its effects when applied to three different types of metal material (copper, ferroalloy, and nickel) and two levels of pore density, namely 90 and 110 pores per inch (PPI), were investigated. The results show that metal foam with a good surface quality can be obtained without damaging the pore structure by using the proposed laser cutting process. Of the three metal types considered, the highest material removal rate (MRR) and material oxidation rate (MOR) were observed for ferroalloy foam. Of the two pore densities, metal foam of 90 PPI showed a larger material removal rate than metal foam of 110 PPI. The MRR and MOR increased with an increase in the laser output power and decrease in the scanning speed. Using a central composite experimental design method, optimized processing parameters of 26 W laser output power and 475 mm/s scanning speed were adopted to cut the metal foam with a high pore density
