    Neonatal intraventricular hemorrhage and hospitalization in childhood

    Contexte: L’hémorragie intraventriculaire néonatale est associée à des séquelles neuro-développementales, mais le risque à long terme d'autres issues est inconnu. L'association entre l'hémorragie intraventriculaire néonatale et le risque de morbidité durant l’enfance a été évaluée. Méthodes: Une cohorte longitudinale de 794,384 bébés nés entre 2006 et 2016 au Québec, Canada a été analysé. Les nouveau-nés ont été suivis jusqu'à une période de 12 ans après leur naissance, pour identifier les hospitalisations subséquentes. Dans les modèles de régression de Cox, ajustés pour les caractéristiques maternelles et néonatales, les « hazard ratios » et intervalles de confiance (IC) à 95% ont été estimés pour l'association entre l'hémorragie intraventriculaire avec l’hospitalisation ultérieure. Résultats: Au total, 1,322 nourrissons (0,2%) ont développé une hémorragie intraventriculaire de grade I à IV. L'incidence de l'hospitalisation était plus élevée chez les bébés présentant une hémorragie intraventriculaire que chez les bébés sans hémorragie (23,8 vs 5,7 par 100 personnes-années). Comparés aux bébés sans hémorragie, les bébés affectés avaient un risque d'hospitalisation 1,56 fois plus élevé (IC à 95% 1,43-1,70). Le risque était 2,81 fois plus élevé pour les grades III/IV (IC à 95% 2,23 à 3,53) comparés à ceux nés sans hémorragie. Les hémorragies intraventriculaires pré-terme était associée à 1,82 fois le risque (IC 95% 1,66-2,00) comparés aux bébés nés termes sans hémorragie. Les hémorragies intraventriculaires à terme étaient associées à 3,19 fois le risque d'hospitalisation (IC 95% 2,55-4,00), comparativement à ceux nés termes sans hémorragie. Les raisons principales des hospitalisations comprenaient les maladies du système nerveux central, ophtalmologiques, musculo-squelettiques et cardiovasculaires. Conclusion: L'hémorragie intraventriculaire, notamment de grades sévères et parmi les bébés à terme, est un déterminant important du futur risque d’hospitalisation durant l’enfance.Background: Neonatal intraventricular hemorrhage is associated with neurodevelopmental sequelae, but the long-term risk of other outcomes is unknown. The association between neonatal intraventricular hemorrhage and the risk of childhood morbidity was assessed. Methods: A longitudinal cohort of 794,384 infants born between 2006 and 2016 in Quebec, Canada was analyzed. Infants were tracked over time to identify later hospitalizations with follow-up extending up to 12 years after birth. In Cox regression models adjusted for maternal and infant characteristics, the hazard ratios and 95% confidence intervals (CI) were estimated for the association of intraventricular hemorrhage with future hospitalization. Results: A total of 1,322 (0.2%) infants developed grade I to IV intraventricular hemorrhage. The incidence of childhood hospitalization was higher in infants with intraventricular hemorrhage than in infants without hemorrhage (23.8 vs. 5.7 per 100 person-years). Compared with no hemorrhage, infants with intraventricular hemorrhage had 1.56 times the risk of hospitalization (95% CI 1.43-1.70). The risk was 2.81 times higher for grade III/IV hemorrhage (95% CI 2.23-3.53) compared to those born without hemorrhage. Preterm intraventricular hemorrhage was associated with 1.82 times the risk (95% CI 1.66-2.00) compared to term infants born without hemorrhage. Intraventricular hemorrhage at term was associated with 3.19 times the risk of hospitalization (95% CI 2.55-4.00) compared to those born term without hemorrhaging. Primary reasons for hospitalizations included central nervous system, ophthalmologic, musculoskeletal, and cardiovascular disorders. Conclusion: Intraventricular hemorrhage, especially of higher grades and in term neonates, is an important determinant of the future risk of child hospitalization

    Severity in the Genomic Age:The Significance of Lived Experience to Understandings of Severity

    This article explores the significance of lived experience to understandings of severity in the genomic age. It draws upon data from structured interviews with 21 people living with monogenic conditions in England. The article argues that while lived experiences are subjective, participants consider the severity of disease by the impact a condition has on a person’s quality of life and mental health; both of these interplays are influenced by social, economic, and environmental factors. The three factors and considerations to the impact of living with disease on mental health are generally absent from current frameworks designed to assess severity for clinical applications of genomic technologies such as preimplantation genetic testing (PGT). This article describes ways in which such factors impact the quality of life and the mental health of people living with genetic conditions. It also indicates what lived experiences, which illustrate the impact of these factors, have to offer policy-makers when they are assessing the concept of severity or seriousness of genetic conditions for applications of existing and potential genomic technologies in the genomic age

    The scope and use of in vitro fertilisation (IVF), a technology which inherently presents gender inequalities, and its platform applications differ across countries according to respective legislation and regulation (Inhorn, 2015). Using the context of human germline genome editing (hGGE) as a framework, this article will explore and discuss whether differences in legislation and regulation across countries force individuals/couples to seek transnational care to fulfil their reproductive desires. This article will primarily focus on regulation and practices in the United Kingdom (UK) and use these as a comparative to regulation and practices in other countries. The primary research upon which this article is based was conducted in the UK between 1st March 2018 – 31st October 2019. The research consisted of a largely qualitative, online public survey with a final data set of 521 respondents, semi-structured interviews with 11 experts/professionals who were/are involved in the scope of hGGE in the UK, and semi-structured/interactive interviews with 21 people affected by a respective range of genetic conditions. The findings reveal that 65.64% of respondents were supportive of people utilising transnational care to achieve their reproductive desires in relation to hGGE and that 76.39% felt they should not be prosecuted if they do

    Currently, the construction projects are advancing gradually around the world as a result of the recent global economic crisis. In order to accommodate public needs within the current economic situation, the Malaysian Government has confined the acquirement of public sector projects to “necessary to meet public need” extends just subsequently narrowing the quantity of domestic projects accessible. A tremendous demand of development worldwide has picked up enthusiasm of Malaysian construction firms to go into international construction projects. Therefore, most major contractors have decided to change their focus by looking into international projects outside Malaysia not exclusively to guarantee the suitability of their businesses but also for long-term survival. Even though, some Malaysian contractors have managed to penetrate successfully into international construction projects, their critical success factors (CSFs) are absolutely obscure. Thus, the aim of this study is to determine the Critical Success Factors (CSFs) of Malaysian Contractors in International construction projects. A general literature review and initial interview was completed and results show on the CSFs of Malaysian contractors in International construction projects. This study is based on interviews with project managers, and the person in charged with CIDB International department. They have established an excellent reputation and worldwide success in the last decade. The CSFs are then exhibited in the form of AHP priority-ranking model, which prioritizes the success factors distinguished in successive way, from the most up to the less critical ones. The establishment of this kind of priority-ranking model enables contractors in Malaysia to not just recognize the key components or factors that should be completely considered and managed yet in addition enable them to get ready appropriate mitigation strategies and contingency plans prior to entering into international construction projects

    Tobacco Usage among Dental Interns in Panchkula, Haryana, India

    BACKGROUND: Tobacco use is one of the major preventable reasons for death. Despite this, its prevalence is increasing among dental students. For the purpose of advocacy of planning tobacco control and interventions, data on prevalence of tobacco usage is needed. AIM: To assess prevalence of tobacco usage among dental interns of district Panchkula, Haryana, India. MATERIAL AND METHODS: A cross-sectional study was conducted from July-September 2019 among dental interns of Panchkula using a self-administered questionnaire. Data based on demographic factors, smoking habits, and associated risk factors was collected. Descriptive analysis for demographic variables and smoking habits were analyzed using the SPSS 24.0 software. RESULT: The prevalence of current tobacco use was 29.54% (22.2% males and 6.8% females). Male interns were significantly more likely than female interns to be current cigarette smokers. On probing the cause of smoking, a majority of participants cited friends and stress as the main cause for smoking. The proportion of regular smokers whose parents smoke was more significant compared to occasional smokers. CONCLUSION: The prevalence of tobacco usage among dental interns is more in males as compared to females so dental interns need to be trained regarding anti-tobacco counselling
