    CULTAN-Düngung und Grundwasserschutz – Kann die Nitratauswaschung durch CULTAN-Düngung reduziert werden?

    Die Standorte der alluvialen Kiese wurden vor ca. 30 Jahren drainiert und werden seither ackerbaulich genutzt. Durch die Trockenlegung der früheren Auenstandorte findet kontinuierlich Mineralisierung statt. Dies führte zu einer negativen Humusbilanz und einem hohen Niveau der N-Auswaschung mit mehr als 100 kg N/(ha a), was ca. 140 mg Nitrat/L im Sickerwasser entspricht. Die Humusbilanz auf den Lössstandorten war hingegen ausgeglichen. Herbstliche N-Überschüsse wurden aufgrund der hohen Wasserspeicherkapazität während des Winters teilweise nicht ausgewaschen und standen der Folgekultur zur Verfügung. Daher lag die N-Auswaschung unter 10 kg N/(ha a). Die Nitratkonzentration im Sickerwasser betrug maximal 40 mg/L. Unter CULTAN-Düngung wurde die Nitratauswaschung unter Körnermais um 22 % reduziert. Daher ist diese Düngestrategie im Untersuchungsgebiet geeignet, die Grundwasserqualität zu verbessern. Häufig werden anhand des Herbst-Nmin-Wertes die N-Verluste mit dem Sickerwasser im Winterhalbjahr abgeschätzt. Im Untersuchungsgebiet wurde in den ehemaligen Auenlagen aufgrund der ständigen Nachmineralisierung deutlich mehr Nitrat ausgewaschen als durch Nmin vorhergesagt. Die Nitratauswaschung in den Lössgebieten war hingegen wasserlimitiert, so dass die mit Nmin abgeschätzte maximal mögliche Auswaschung nur selten erreicht wurde. Die Nmin-Methode ist im Untersuchungsgebiet daher nicht geeignet, die winterliche N-Auswaschung korrekt vorherzusagen

    ARE ROUTINE METHODS GOOD ENOUGH TO STAIN SENILE PLAQUES AND NEUROFIBRILLARY TANGLES IN DIFFERENT BRAIN REGIONS OF DEMENTED PATIENTS?

    Introduction: Numerous clinical cases have been reported showing the clinical picture of dementia but not meeting the neuropathological criteria for Alzheimer’s dementia (AD). Different methods used to stain senile plaques (SPs) and neurofibrillary tangles (NFTs) might account for this discrepancy. Subjects and methods: Here, brains of 11 patients with dementia were examined. Cryosections and paraffin sections from 6 different brain regions (frontal medial, temporal medial and occipital gyrus, hippocampus, superior parietal lobe and cerebellum) of all cases were stained with Bielschowsky, Campbell, Gallyas and Congo red stains each. Results: The study shows that the Bielschowsky silver stain is insufficient for detecting SPs and NFTs, whereas two other methods proved to be more accurate. SPs were found in similar frequency in all brain regions examined (exception: cerebellum). The highest amount was shown with Campbell silver stain in paraffin sections. In Congo red only 25 percent of these SPs were stained, which is probably due to a great number of them not containing any amyloid. NFTs were found almost exclusively in the hippocampus. The highest number was detected with Gallyas silver stain in cryosections. Conclusion: These results may suggest that Campbell stain for SPs and Gallyas stain for NFTs should be the methods routinely used

    CRISTOPH – A cluster-randomised intervention study to optimise the treatment of patients with hypertension in General Practice

    <p>Abstract</p> <p>Background</p> <p>Recent guidelines for the management of hypertension focus on treating patients according to their global cardiovascular risk (CVR), rather than strictly keeping blood pressure, or other risk factors, below set limit values. The objective of this study is to compare the effect of a simple versus a complex educational intervention implementing this new concept among General Practitioners (GPs).</p> <p>Methods/design</p> <p>A prospective longitudinal cluster-randomised intervention trial with 94 German GPs consecutively enroling 40 patients each with known hypertension. All GPs then received a written manual specifically developed to transfer the global concept of CVR into daily General Practice. After cluster-randomisation, half of the GPs additionally received a clinical outreach visit, with a trained peer discussing with them the concept of global CVR referring to example study patients from the respective GP. Main outcome measure is the improvement of calculated CVR six months after intervention in the subgroup of patients with high CVR (but no history of cardiovascular disease), defined as 10-year-mortality ≥ 5% employing the European SCORE formula. Secondary outcome measures include the intervention's effect on single risk factors, and on prescription rates of drugs targeting CVR. All outcome measures are separately studied in the three subgroups of patients with 1. high CVR (defined as above), 2. low CVR (SCORE < 5%), and 3. a history of cardiovascular disease. The influence of age, sex, social status, and the perceived quality of the respective doctor-patient-relation on the effects will be examined.</p> <p>Discussion</p> <p>To our knowledge, no other published intervention study has yet evaluated the impact of educating GPs with the goal to treat patients with hypertension according to their global cardiovascular risk.</p> <p>Trial registration</p> <p>ISRCTN44478543</p

    Decompressive craniectomy plus best medical treatment versus best medical treatment alone for spontaneous severe deep supratentorial intracerebral haemorrhage:a randomised controlled clinical trial

    BACKGROUND: It is unknown whether decompressive craniectomy improves clinical outcome for people with spontaneous severe deep intracerebral haemorrhage. The SWITCH trial aimed to assess whether decompressive craniectomy plus best medical treatment in these patients improves outcome at 6 months compared to best medical treatment alone.METHODS: In this multicentre, randomised, open-label, assessor-blinded trial conducted in 42 stroke centres in Austria, Belgium, Finland, France, Germany, the Netherlands, Spain, Sweden, and Switzerland, adults (18-75 years) with a severe intracerebral haemorrhage involving the basal ganglia or thalamus were randomly assigned to receive either decompressive craniectomy plus best medical treatment or best medical treatment alone. The primary outcome was a score of 5-6 on the modified Rankin Scale (mRS) at 180 days, analysed in the intention-to-treat population. This trial is registered with ClincalTrials.gov, NCT02258919, and is completed.FINDINGS: SWITCH had to be stopped early due to lack of funding. Between Oct 6, 2014, and April 4, 2023, 201 individuals were randomly assigned and 197 gave delayed informed consent (96 decompressive craniectomy plus best medical treatment, 101 best medical treatment). 63 (32%) were women and 134 (68%) men, the median age was 61 years (IQR 51-68), and the median haematoma volume 57 mL (IQR 44-74). 42 (44%) of 95 participants assigned to decompressive craniectomy plus best medical treatment and 55 (58%) assigned to best medical treatment alone had an mRS of 5-6 at 180 days (adjusted risk ratio [aRR] 0·77, 95% CI 0·59 to 1·01, adjusted risk difference [aRD] -13%, 95% CI -26 to 0, p=0·057). In the per-protocol analysis, 36 (47%) of 77 participants in the decompressive craniectomy plus best medical treatment group and 44 (60%) of 73 in the best medical treatment alone group had an mRS of 5-6 (aRR 0·76, 95% CI 0·58 to 1·00, aRD -15%, 95% CI -28 to 0). Severe adverse events occurred in 42 (41%) of 103 participants receiving decompressive craniectomy plus best medical treatment and 41 (44%) of 94 receiving best medical treatment.INTERPRETATION: SWITCH provides weak evidence that decompressive craniectomy plus best medical treatment might be superior to best medical treatment alone in people with severe deep intracerebral haemorrhage. The results do not apply to intracerebral haemorrhage in other locations, and survival is associated with severe disability in both groups.FUNDING: Swiss National Science Foundation, Swiss Heart Foundation, Inselspital Stiftung, and Boehringer Ingelheim.</p

    Replication of Lung Cancer Susceptibility Loci at Chromosomes 15q25, 5p15, and 6p21: A Pooled Analysis From the International Lung Cancer Consortium

    Background Genome-wide association studies have identified three chromosomal regions at 15q25, 5p15, and 6p21 as being associated with the risk of lung cancer. To confirm these associations in independent studies and investigate heterogeneity of these associations within specific subgroups, we conducted a coordinated genotyping study within the International Lung Cancer Consortium based on independent studies that were not included in previous genome-wide association studies. Methods Genotype data for single-nucleotide polymorphisms at chromosomes 15q25 (rs16969968, rs8034191), 5p15 (rs2736100, rs402710), and 6p21 (rs2256543, rs4324798) from 21 case-control studies for 11 645 lung cancer case patients and 14 954 control subjects, of whom 85% were white and 15% were Asian, were pooled. Associations between the variants and the risk of lung cancer were estimated by logistic regression models. All statistical tests were two-sided. Results Associations between 15q25 and the risk of lung cancer were replicated in white ever-smokers (rs16969968: odds ratio [OR] = 1.26, 95% confidence interval [CI] = 1.21 to 1.32, Ptrend = 2 × 10−26), and this association was stronger for those diagnosed at younger ages. There was no association in never-smokers or in Asians between either of the 15q25 variants and the risk of lung cancer. For the chromosome 5p15 region, we confirmed statistically significant associations in whites for both rs2736100 (OR = 1.15, 95% CI = 1.10 to 1.20, Ptrend = 1 × 10−10) and rs402710 (OR = 1.14, 95% CI = 1.09 to 1.19, Ptrend = 5 × 10−8) and identified similar associations in Asians (rs2736100: OR = 1.23, 95% CI = 1.12 to 1.35, Ptrend = 2 × 10−5; rs402710: OR = 1.15, 95% CI = 1.04 to 1.27, Ptrend = .007). The associations between the 5p15 variants and lung cancer differed by histology; odds ratios for rs2736100 were highest in adenocarcinoma and for rs402710 were highest in adenocarcinoma and squamous cell carcinomas. This pattern was observed in both ethnic groups. Neither of the two variants on chromosome 6p21 was associated with the risk of lung cancer. Conclusions In this international genetic association study of lung cancer, previous associations found in white populations were replicated and new associations were identified in Asian populations.     Event reconstruction for KM3NeT/ORCA using convolutional neural networks

    The KM3NeT research infrastructure is currently under construction at two locations in the Mediterranean Sea. The KM3NeT/ORCA water-Cherenkov neutrino detector off the French coast will instrument several megatons of seawater with photosensors. Its main objective is the determination of the neutrino mass ordering. This work aims at demonstrating the general applicability of deep convolutional neural networks to neutrino telescopes, using simulated datasets for the KM3NeT/ORCA detector as an example. To this end, the networks are employed to achieve reconstruction and classification tasks that constitute an alternative to the analysis pipeline presented for KM3NeT/ORCA in the KM3NeT Letter of Intent. They are used to infer event reconstruction estimates for the energy, the direction, and the interaction point of incident neutrinos. The spatial distribution of Cherenkov light generated by charged particles induced in neutrino interactions is classified as shower- or track-like, and the main background processes associated with the detection of atmospheric neutrinos are recognized. Performance comparisons to machine-learning classification and maximum-likelihood reconstruction algorithms previously developed for KM3NeT/ORCA are provided. It is shown that this application of deep convolutional neural networks to simulated datasets for a large-volume neutrino telescope yields competitive reconstruction results and performance improvements with respect to classical approaches
