    A Conceptual Map of Structural Racism in Health Care

    Longstanding racial and ethnic disparities in health care experiences contribute to profoundly inequitable health and life outcomes in the United States. Researchers, policymakers, practitioners, advocates, and communities seeking to eradicate these disparities must understand and intervene in their root causes. In this brief, we develop a conceptual map of structural racism in health care that demonstrates the connections between (1) mental models that, in often unnoticed ways, guide how society thinks and acts; (2) inequitable structures, including laws and policies that codify the distribution of and access to resources; and (3) racial and ethnic disparities in health care experiences and outcomes

    Oral Health Status of Institutionalized Orphans and Non-Orphans Aged 6-12 Years in a Central Indian City: A Comparative Study

    INTRODUCTION: Oral Health, like general health, is a basic fundamental right for all, and even more so for the underprivileged. AIM: To evaluate the oral health status of Institutionalized orphans aged 6-12 years and non-orphans in Indore, Madhya PradeshMATERIALS AND METHODS: The present descriptive cross-sectional study was conducted among various registered orphanages in Indore, Madhya Pradesh. Oral Examination was done by two standardized examiners (ADA type III examination) and data was entered using a pre-tested and pre-validated proforma. Data was transferred into MS excel and after application of descriptive statistics, the independent samples t-test and multiple logistic regression were applied (p value significant at ≤ 0.5). RESULTS: Of the total 263 study subjects, there were 132(50.2%) orphans and 131(49.8%) school children aged 6-12 years. Among orphans, males (62.9%) formed the majority; use of toothbrush with toothpaste was observed among 69.7% of orphans, while 6.1% did not use any oral hygiene aid. The most common observed hard lesion was dental caries (31.8%) as compared to 20.6% in school children and its presence revealed a significant association (p=0.02). CONCLUSION: It is recommended that a special emphasis should be made to promote oral health among the children residing in orphanages

    Large adnexal mass: is laparoscopic surgery a safe option?

    Background: Laparoscopic surgery is regarded as the gold standard for management of adnexal tumours due to many advantages. Currently, the exact size of the adnexal tumour contraindicating laparoscopic management has not been clearly defined. Some studies suggest laparotomy for the treatment of adnexal tumours larger than 8 to 10 cm. Risk of malignancy increases with large cysts. Issues with operation of huge adnexal masses are limited surgical field, difficulty in inserting trocars and removing the specimen without rupture. This study aims to evaluate the feasibility and surgical outcomes of laparoscopic surgery for large adnexal masses.Methods: A prospective observational study was conducted in the Department of Obstetrics and Gynaecology, JSS Medical College, Mysuru.Results: 35 patients with large adnexal masses, which were thought to be benign, were subjected to laparoscopic surgery. Mean diameter of masses as per the pre-operative ultrasound measurements was 12.2cm (range: 10.1-20.4cm). The mean operative time was 51.15minutes, estimated blood loss was 85.4ml, duration of postoperative hospital stay was 2.8 days. Serous cystadenoma was the most common histopathological finding. However, one case of borderline tumour was found.Conclusions: The potential risk of malignancy is the most important limiting factor for laparoscopic management of large ovarian cysts. Therefore, it is necessary to establish risk profiles of patients with adnexal masses in order to benefit from minimally invasive surgery wherever possible. The current study supports laparoscopic management of large ovarian cysts as a technically feasible method if proper case selection is made

    This paper describes our first research experience in Smart Surveillance System using PIR Sensor with Raspberry pi, by using open-source software tools such as Python, Raspbian OS and etc.Smart Surveillance System using PIR Sensor with Raspberry piemerged with the development of the Internet of Things. The basic concept of the application is to allow the client to uses wireless technology to provide essential security using Surveillance system. The proposed security system captures information and transmit it via a Wi-Fi to static IP, which is viewed using a web browser from any smart devices. Raspberry pi controls a video camera for surveillance. It streams live video and records the motion detected part in the cloud and/or in the window shared folder for future playback. The cameras automatically initiate recording when motion is sensed and Raspberry pi devices stores it in a secured folder

    Case Report: Integrative naturopathic approach for the management of sequestered lumbar disc herniation with neurological impairments: a case series with two year follow up

    Lumbar Disc Herniation (LDH) is a common condition, and contemporary pain research emphasizes the importance of adopting a comprehensive biopsychosocial perspective in pain treatment for positive clinical outcomes. Integrated Naturopathy and Yoga (INY) is a non-invasive medical system that takes a holistic and patient-centric approach to healing diseases. However, there is limited evidence on the effectiveness of INY, particularly in managing Sequestered LDH. We present two cases of patients experiencing radicular low back pain, lower limb weakness, and neuro-claudication who opted for conservative naturopathic management with INY. Following the INY treatments, both patients reported gradual relief from lower back pain, radicular pain, and neurological deficits. These findings are significant and contribute valuable evidence, suggesting that INY could be a viable therapeutic approach for managing sequestered LDH. This represents the first report on a non-invasive method for resolving sequestered LDH by utilizing INY

    Time Waits for No One! Analysis and Challenges of Temporal Misalignment

    When an NLP model is trained on text data from one time period and tested or deployed on data from another, the resulting temporal misalignment can degrade end-task performance. In this work, we establish a suite of eight diverse tasks across different domains (social media, science papers, news, and reviews) and periods of time (spanning five years or more) to quantify the effects of temporal misalignment. Our study is focused on the ubiquitous setting where a pretrained model is optionally adapted through continued domain-specific pretraining, followed by task-specific finetuning. We establish a suite of tasks across multiple domains to study temporal misalignment in modern NLP systems. We find stronger effects of temporal misalignment on task performance than have been previously reported. We also find that, while temporal adaptation through continued pretraining can help, these gains are small compared to task-specific finetuning on data from the target time period. Our findings motivate continued research to improve temporal robustness of NLP models.Comment: 9 pages, 6 figures, 3 table

    Reliability of Bell’s Test Conducted with Virtual Reality

    Cancellation tasks are commonly used assessment tools to detect unilateral neglect. Bell’s Test, one of the most commonly administered cancellation tasks, requires individuals to quickly and accurately identify “bells” randomly placed in an array of symbols. The reliability of Bells Test conducted with a paper and pencil is well established. A newly developed, commercial software application, allows Bell’s Test to be administered in fully immersive virtual reality environment. PURPOSE: The purpose of this study was to measure the reliability of the Bell’s Test using a virtual reality (VR) system and to establish the level of agreement between the pen-paper and VR administration. METHODS: Fourteen apparently healthy individuals between the age of 24 – 73y volunteered (47.9±20.7y; 166.0±5.2cm; 77.5±16.3kg). Subjects were excluded if they had a history of concussion or had perceptual or visual deficits. Participants completed Bell’s Test a total of four times. The first time, it was completed with a pen-paper (PP). Then, participants completed an initial assessment in VR (VRbase); the same test was administered again in VR one-hour (PostVR1hr) and one-week (PostVR1wk) post baseline. Realization time, total time, and the number of errors committed in the right and left field of view were recorded. Cronbach’s alpha was computed on realization and total time in all VR conditions. Additionally, an ANOVA with repeated measures was used to determine differences in PP, VRbase, PostVR1HR, and PostVR1Wk. The Holms-Sidak method was used to identify pairwise differences. Alpha was set at pa priori. RESULTS: Reliability for realization time for the Bell’s test conducted in VR was acceptable (α=0.79). There was, however, a significant difference between trials (F=6.65; p=0.013). VRbase (29.25±8.11s) was significantly different than PostVR1HR (15.52±4.30; p=0.006). and VRpost1wk (21.24±5.89s; p=0.01). Additionally, PP realization time (15.31±4.25s) was significantly different than PostVR1HR (p=0.02) and PostVR1WK (p=0.03). Reliability for total time for the Bell’s test conducted in VR was good. (α=0.82). There was not a significant difference between the trials (F=4.34; p=0.06) for VRbase(24.1±6.7s), PostVR1HR (45.44±12.6) or VRpost1wk(p=0.06). However, there was a significant difference found in PPtotal time taken and VR1wk with the (p=0.03). The average number of left side omitted bells was 0.50±0.65; 0.07±0.26; 0.07±0.26; 0.14±0.36 for PP, VRbase, Post VR1HR, and PostVR1WK; respectively. The number of bells omitted on the right side was 0.50±0.85, 0.21±0.57, 0.14±0.36, for 0.14±0.36 for PP, VRbase, PostVR1HR, and PostVR1WK; respectively. CONCLUSION: These data indicate that the reliability of the Bell’s Test for the realization time is acceptable, and the total time reliability was good in the virtual reality system. Healthy individuals performing the Bell’s Test in VR show slight improvements one hour after baseline, but there was no difference after one week. More data are needed within different age groups to determine reliability in young and older individuals. Additionally, future studies are required to determine the reliability of Bell’s Test in VR for individuals suffering from neurological injuries or diseases
