    Priapism and clozapine use in a patient with hypochondriacal delusional syndrome

    Priapism is a urological emergency that results in a penile or clitoral engorgement, having several triggers by the use of medications, including psychiatric drugs, such as antipsychotics and antidepressants. The most common priapism presentation is the ischemic type that can result in the fibrosis of corporus cavernosus, with a significant risk of persistent erectile dysfunction. It is believed that, in the case of antipsychotic use, priapism is mediated by an imbalance in alpha-1-adrenergic blocking, with large variation affinity for alpha-1-adrenergic receptors among antipsychotics. This case report contributes to the study of this rare but severe side effect. Priapism can limit both treatment adherence and the reproductive future of our patients. We describe a case report of a patient with a delusional hypochondriac syndrome who had multiple priapism episodes using clozapine 50 mg/daily.     A Avaliação de Impacto Ambiental - AIA, em função de suas escalas de abordagem, se divide em dois instrumentos de apoio à tomada de decisão: o Estudo de Impacto Ambiental - EIA, que considera a viabilidade ambiental de projetos, e a Avaliação Ambiental Estratégica - AAE, instrumento de apoio à tomada de decisão que avalia as conseqüências ambientais de políticas, planos e programas inserindo a variável ambiental no processo de planejamento. Observando etapas, procedimentos e objetivos detalhados, a AAE apresenta estudos baseados em cenários, agrega valores ambientais no processo de decisão, é instrumento para tomada de decisão, mas não executa a decisão, e não se aplica a empreendimentos, uma exclusividade do EIA, como acontece ainda hoje no Brasil

    Os impactos dos efeitos imunomodulatórios das beta-glucanas fúngicas na saúde humana

    A modulação cuidadosa do sistema imunológico é essencial para garantir uma resposta imunológica eficaz, equilibrada e adaptativa, promovendo a saúde geral do organismo. Esta revisão tem por objetivo discutir as funções imunomodulatórias das beta-glucanas fúngicas. As beta-glucanas podem interagir com diversos receptores presentes na superfície das células imunológicas, o que resulta na ativação ou na inibição de células tanto da imunidade inata quanto adaptativa. As consequências afetam processos inflamatórios, hipersensibilidades e autoimunidades. Diversos são os fatores que podem interferir e determinar se a interferência das beta-glucanas no sistema imunológico será positiva ou negativa. Dentre estes fatores, podemos citar o peso molecular, as ramificações estruturais e a solubilidade das beta-glucanas. Ensaios clínicos têm reunido evidências dos efeitos benéficos das beta-glucanas na saúde humana, fortalecendo o sistema imunológico e reduzindo infecções. Os desafios encontrados para que as beta-glucanas tenham seu uso clínico aprovado e incentivado incluem a heterogeneidade das estruturas moleculares das beta-glucanas e a variação na resposta entre indivíduos. Por outro lado, o aprofundamento no estudo destas moléculas poderá ultrapassar tais barreiras e colaborar para o desenvolvimento de estratégias que melhorem sua biodisponibilidade e eficácia

    Proteomic profling of hydatid fuid from pulmonary cystic echinococcosis

    Background: Most cystic echinococcosis cases in Southern Brazil are caused by Echinococcus granulosus and Echinococcus ortleppi. Proteomic studies of helminths have increased our knowledge about the molecular survival strategies that are used by parasites. Here, we surveyed the protein content of the hydatid fluid compartment in E. granulosus and E. ortleppi pulmonary bovine cysts to better describe and compare their molecular arsenal at the host-parasite interface. Methods: Hydatid fluid samples from three isolates of each species were analyzed using mass spectrometry-based proteomics (LC-MS/MS). In silico functional analyses of the identified proteins were performed to examine parasite survival strategies. Results: The identified hydatid fluid protein profiles showed a predominance of parasite proteins compared to host proteins that infiltrate the cysts. We identified 280 parasitic proteins from E. granulosus and 251 from E. ortleppi, including 52 parasitic proteins that were common to all hydatid fluid samples. The in silico functional analysis revealed important molecular functions and processes that are active in pulmonary cystic echinococcosis, such as adhesion, extracellular structures organization, development regulation, signaling transduction, and enzyme activity.Conclusions: The protein profiles described here provide evidence of important mechanisms related to basic cellular processes and functions that act at the host-parasite interface in cystic echinococcosis. The molecular tools used by E. granulosus and E. ortleppi for survival within the host are potential targets for new therapeutic approaches to treat cystic echinococcosis and other larval cestodiases

    Quercetin pentaacetate inhibits in vitro human respiratory syncytial virus adhesion.

No. of bitstreams: 0 Previous issue date: 2020-01-15 Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Human respiratory syncytial virus (hRSV) is one of the main etiological agents of diseases of the lower respiratory tract and is often responsible for the hospitalization of children and the elderly. To date, treatments are only palliative and there is no vaccine available. Natural products show exceptional structural diversity and they have played a vital role in drug research. Several investigations focused on applied structural modification of natural products to improved metabolic stability, solubility and biological actions them. Quercetin is a flavonoid that presents several biological activities, including anti-hRSV role. Some works criticize the pharmacological use of Quercetin because it has low solubility and low specificity. In this sense, we acetylated Quercetin structure and we used in vitro and in silico assays to compare anti-hRSV function between Quercetin (Q0) and its derivative molecule (Q1). Q1 shows lower cytotoxic effect than Q0 on HEp-2 cells. In addition, Q1 was more efficient than Q0 to protect HEp-2 cells infected with different multiplicity of infection (0.1–1 MOI). The virucidal effects of Q0 and Q1 suggest interaction between these molecules and viral particle. Dynamic molecular results suggest that Q0 and Q1 may interact with F-protein on hRSV surface in an important region to adhesion and viral infection. Q1 interaction with F-protein showed ΔG= -14.22 kcal/mol and it was more stable than Q0. Additional, MTT and plate assays confirmed that virucidal Q1 effects occurs during adhesion step of cycle hRSV replication. In conclusion, acetylation improves anti-hRSV Quercetin effects because Quercetin pentaacetate could interact with F-protein with lower binding energy and better stability to block viral adhesion. These results show alternative anti-hRSV strategy and contribute to drug discovery and development.     A ingestão de substâncias cáusticas na infância é um problema de saúde pública e acarreta alta morbidade aos pacientes, que geralmente desenvolvem estenose esofágica. Os tratamentos disponíveis para estenose cáustica são pouco satisfatórios e por isso realizamos um trabalho para divulgação de medidas de prevenção. Trata-se de vídeo explicativo sobre os perigos da ingestão de substâncias cáusticas e como evitar tais acidentes com crianças. O vídeo foi produzido a partir do relato de familiares de pacientes vítimas da ingestão acidental de soda cáustica
