    Bundling Economics Under Adverse Selection in the Individual Health Insurance Market

    We examine the impact of bundling strategies on the level of consumer participation and premium rates realized in an individual health insurance market characterized by an adverse selection problem. In this context we show that society may use private insurers to attain universal coverage at equitable premiums under a pure bundling strategy, where insurers offer only a comprehensive policy to the market. This result is strengthened as the number of medical conditions covered in the comprehensive policy increases and as applicant risk aversion increases. When insurance applicants exhibit low levels of risk aversion a mixed bundling strategy (or offering single-disease policies along with the comprehensive policy) improves consumer participation and decreases premium rates when compared to a pure bundling strategy. In this case market performance is improved by increasing policy options offered to applicants. Alternatively, when insurance applicants exhibit moderate levels of risk aversion a mixed bundling strategy reduces consumer participation and increases premium rates when compared to a pure bundling strategy. In this case market performance is improved by reducing policy options offered to applicants. In addition, when insurance applicants exhibit sufficiently high levels of risk aversion the consumer participation and premium rates realized under a pure bundling strategy and mixed bundling strategy converge toward full market participation. Finally, under all levels of risk aversion we show that offering an exclusion policy along with the comprehensive policy decreases consumer participation

    Recent global climate models with sufficient resolution and physics offer a promising approach for simulating real-world tropical cyclone (TC) statistics and their changing relationship with climate. In the first part of this study, we examine the performance of a high-resolution (;40-km horizontal grid) global climate model, the atmospheric component of the Australian Community Climate and Earth System Simulator (ACCESS) based on the Met Office Unified Model (UM8.5) Global Atmosphere (GA6.0). The atmospheric model is forced with observed sea surface temperature, and 20 years of integrations (1990–2009) are analyzed for evaluating the simulated TC statistics compared with observations. The model reproduces the observed climatology, geographical distribution, and interhemispheric asymmetry of global TC formation rates reasonably well. The annual cycle of regional TC formation rates over most basins is also well captured. However, there are some regional biases in the geographical distribution of TC formation rates. To identify the sources of these biases, a suite of model-simulated large-scale climate conditions that critically modulate TC formation rates are further evaluated, including the assessment of a multivariate genesis potential index. Results indicate that the model TC genesis biases correspond well to the inherent biases in the simulated large-scale climatic states, although the relative effects on TC genesis of some variables differs between basins. This highlights the model’s mean-state dependency in simulating accurate TC formation rates

    The sport of cricket has a history of its players suffering from mental health issues. The psychological study of cricket and, in particular, the attendant demands of participating at an elite level has not previously received rigorous academic attention. This study explored ten elite male cricketers’ experiences of keeping a daily reflective diary for one month during the competitive season. The aim was to assess how valuable qualitative diaries are in this field. Participants were interviewed regarding their appraisal of the methodology as a self‐help tool that could assist coping with performance pressures and wider life challenges. Three outcomes were revealed: first, that diary keeping was an effective opportunity to reflect upon the past and enhance one’s self (both as an individual and a performer); second, that diary keeping acted as a form of release that allowed participants to progress; and third, that diary keeping allowed participants to discover personal patterns of success that increased the likeliness of optimum performance

    Seismic evidence for shallow gas-escape features associated with a retreating gas hydrate zone offshore west Svalbard

    Active gas venting occurs on the uppermost continental slope off west Svalbard, close to and upslope from the present-day intersection of the base of methane hydrate stability (BMHS) with the seabed in about 400 m water depth in the inter-fan region between the Kongsfjorden and Isfjorden cross-shelf troughs. From an integrated analysis of high-resolution, two-dimensional, pre-stack migrated seismic reflection profiles and multibeam bathymetric data, we map out a bottom simulating reflector (BSR) in the inter-fan region and analyze the subsurface gas migration and accumulation. Gas seeps mostly occur in the zone from which the BMHS at the seabed has retreated over the recent past (1975–2008) as a consequence of a bottom water temperature rise of 1°C. The overall margin-parallel alignment of the gas seeps is not related to fault-controlled gas migration, as seismic evidence of faults is absent. There is no evidence for a BSR close to the gas flare region in the upper slope but numerous gas pockets exist directly below the predicted BMHS. While the contour following trend of the gas seeps could be a consequence of retreat of the landward limit of the BMHS and gas hydrate dissociation, the scattered distribution of seeps within the probable hydrate dissociation corridor and the occurrence of a cluster of seeps outside the predicted BMHS limit and near the shelf break indicate the role of lithological heterogeneity in focusing gas migration

    Developing Theory to Guide Building Practitioners’ Capacity to Implement Evidence-Based Interventions

    Public health and other community-based practitioners have access to a growing number of evidence-based interventions (EBIs), and yet EBIs continue to be underused. One reason for this underuse is that practitioners often lack the capacity (knowledge, skills, and motivation) to select, adapt, and implement EBIs. Training, technical assistance, and other capacity-building strategies can be effective at increasing EBI adoption and implementation. However, little is known about how to design capacity-building strategies or tailor them to differences in capacity required across varying EBIs and practice contexts. To address this need, we conducted a scoping study of frameworks and theories detailing variations in EBIs or practice contexts and how to tailor capacity-building to address those variations. Using an iterative process, we consolidated constructs and propositions across 24 frameworks and developed a beginning theory to describe salient variations in EBIs (complexity and uncertainty) and practice contexts (decision-making structure, general capacity to innovate, resource and values fit with EBI, and unity vs. polarization of stakeholder support). The theory also includes propositions for tailoring capacity-building strategies to address salient variations. To have wide-reaching and lasting impact, the dissemination of EBIs needs to be coupled with strategies that build practitioners’ capacity to adopt and implement a variety of EBIs across diverse practice contexts

    Association between exposure to environmental tobacco smoke and biomarkers of oxidative stress among patients hospitalised with acute myocardial infarction

    Objective To determine whether exposure to environmental tobacco smoke was associated with oxidative stress among patients hospitalised for acute myocardial infarction.<p></p> Design An existing cohort study of 1,261 patients hospitalised for acute myocardial infarction.<p></p> Setting Nine acute hospitals in Scotland.<p></p> Participants Sixty never smokers who had been exposed to environmental tobacco smoke (admission serum cotinine ≥3.0 ng/mL) were compared with 60 never smokers who had not (admission serum cotinine ≤0.1 ng/mL).<p></p> Intervention None.<p></p> Main outcome measures Three biomarkers of oxidative stress (protein carbonyl, malondialdehyde (MDA) and oxidised low-density lipoprotein (ox-LDL)) were measured on admission blood samples and adjusted for potential confounders.<p></p> Results After adjusting for baseline differences in age, sex and socioeconomic status, exposure to environmental tobacco smoke was associated with serum concentrations of both protein carbonyl (beta coefficient 7.96, 95% CI 0.76, 15.17, p = 0.031) and MDA (beta coefficient 10.57, 95% CI 4.32, 16.81, p = 0.001) but not ox-LDL (beta coefficient 2.14, 95% CI −8.94, 13.21, p = 0.703).<p></p> Conclusions Exposure to environmental tobacco smoke was associated with increased oxidative stress. Further studies are requires to explore the role of oxidative stress in the association between environmental tobacco smoke and myocardial infarction.<p></p&gt
