Internet usage, frequency and intensity in old age during the COVID-19 pandemic - a case study for Switzerland
IntroductionThis study examines the digital divide among older adults in Switzerland within the rapidly evolving digital environment. It investigates changes in internet usage among this population, focusing on the proportion of users, frequency, and the intensity of their internet usage during the COVID-19 pandemic.MethodsDrawing on Swiss data from the Survey of Health, Aging, and Retirement (SHARE), conducted in 2021, the study analyzes a sample of 1,205 older adults.ResultsThe findings indicate a growing proportion of internet users over time. It also highlights that gender differences persist but are decreasing. Notably, around 9% of individuals in this study had never used the internet, while recent users exhibited high activity levels, spending an average of approximately two and a half hours online daily. The study identified age, education, employment, living arrangements, and attitudes toward technology as influential factors shaping internet usage among older adults. Importantly, the COVID-19 pandemic did not have a significant impact on internet adoption among this demographic.DiscussionThese findings shed light on the complex dynamics that shape internet usage among older adults and underscore the need to promote digital inclusion and engagement within this population
The Excess Costs of Depression and the Influence of Sociodemographic and Socioeconomic Factors: Results from the German Health Interview and Examination Survey for Adults (DEGS)
Introduction
The aim of this study was to estimate excess costs of depression in Germany and to examine the influence of sociodemographic and socioeconomic determinants.
Methods
Annual excess costs of depression per patient were estimated for the year 2019 by comparing survey data of individuals with and without self-reported medically diagnosed depression, representative for the German population aged 18–79 years. Differences between individuals with depression (n = 223) and without depression (n = 4540) were adjusted using entropy balancing. Excess costs were estimated using generalized linear model regression with a gamma distribution and log-link function. We estimated direct (inpatient, outpatient, medication) and indirect (sick leave, early retirement) excess costs. Subgroup analyses by social determinants were conducted for sex, age, socioeconomic status, first-generation or second-generation migrants, partnership, and social support.
Results
Total annual excess costs of depression amounted to €5047 (95% confidence interval [CI] 3214–6880) per patient. Indirect excess costs amounted to €2835 (1566–4103) and were higher than direct excess costs (€2212 [1083–3341]). Outpatient (€498), inpatient (€1345), early retirement (€1686), and sick leave (€1149) excess costs were statistically significant, while medication (€370) excess costs were not. Regarding social determinants, total excess costs were highest in the younger age groups (€7955 for 18–29-year-olds, €9560 for 30–44-year-olds), whereas total excess costs were lowest for the oldest age group (€2168 for 65+) and first-generation or second-generation migrants (€1820).
Conclusions
Depression was associated with high excess costs that varied by social determinants. Considerable differences between the socioeconomic and sociodemographic subgroups need further clarification as they point to specific treatment barriers as well as varying treatment needs.Peer Reviewe
Off-diagonal long-range order for the free Bose gas via the Feynman--Kac formula
We consider the path-integral representation of the ideal Bose gas under
various boundary conditions. We show that Bose--Einstein condensation occurs at
the famous critical density threshold, by proving that its -particle-reduced
density matrix exhibits off-diagonal long-range order above that threshold, but
not below. Our proofs are based on the well-known Feynman--Kac formula and a
representation in terms of a crucial Poisson point process. Furthermore, in the
condensation regime, we derive a law of large numbers with strong concentration
for the number of particles in short loops. In contrast to the situation for
free boundary conditions, where the entire condensate sits in just one loop,
for all other boundary conditions we obtain the limiting Poisson--Dirichlet
distribution for the collection of the lengths of all long loops. Our proofs
are new and purely probabilistic (apart from a standard eigenvalue expansion),
using elementary tools like Markov's inequality, Poisson point processes,
combinatorial formulas for cardinalities of particular partition sets, and
asymptotics for random walks with Pareto-distributed steps.Comment: 34 page
How FAIR are CMC Corpora?
In recent years, research data management has also become an important topic in the less data-intensive areas of the Social Sciences and Humanities (SSH). Funding agencies as well as research communities demand that empirical data collected and used for scientific research is managed and preserved in a way that research results are reproducible. In order to account for this the FAIR guiding principles for data stewardship have been established as a framework for good data management, aiming at the findability, accessibility, interoperability, and reusability of research data. This article investigates 24 European CMC corpora with regard to their compliance with the FAIR principles and discusses to what extent the deposit of research data in repositories of data preservation initiatives such as CLARIN, Zenodo or Metashare can assist in the provision of FAIR corpora
Evaluation of the comorbidities and their various specifications, as well as the different therapeutic approaches in Meniere's disease
Fragestellung:
Die genaue Ätiologie des Morbus Menière ist bis heute nicht eindeutig geklärt. Fraglich ist ebenso, inwiefern Komorbiditäten diese Erkrankung prägen. Das Ziel dieser Studie ist es das patientenspezifische Erkrankungsbild bei Morbus Menière komplexer zu beschreiben. Außerdem soll ein Verteilungsmuster von Komorbiditäten dargelegt werden, in dem diese untereinander, sowie mit den Faktoren Geschlecht, Altersklassen und Anfallshäufigkeiten verglichen werden. Des Weiteren sollen verschiedene Therapieansätze evaluiert werden.
Methoden:
An dieser Studie nahmen 44 Patienten, die unter Morbus Menière leiden teil. Mit Hilfe eines Fragbogens wurden Daten zu allgemeinmedizinischen und spezifisch das Hör- oder Gleichgewichtsorgan betreffenden Erkrankungen gesammelt. Im weiteren Verlauf wurden bekannte Komorbiditäten identifiziert und untereinander sowie mit den Faktoren Altersklassen, Geschlecht und Anfallshäufigkeiten verglichen. Außerdem wurden die Angaben zu den Therapieansätzen mit der Anfallshäufigkeit verglichen.
Ergebnis:
Es konnte die bekannten Komorbiditäten, nämlich Erkrankungen mit Störungen des Immunsystems (31,8%), psychische Belastungsfaktoren (52,3%), die Hypertonie (31,8%), Schilddrüsenerkrankungen (20,5%) und die Migräne (29,5%) identifiziert werden. Bei der Untersuchung dieser, konnte gezeigt werden, dass Männer mit Morbus Menière gegenüber Frauen mit Morbus Menière signifikant häufiger von einer Hypertonie betroffen sind. Bezüglich der Therapie konnte festgestellt werden, dass Patienten mit einem physiotherapeutischen Therapieansatz signifikant weniger als 5 Anfälle im Schnitt pro Jahr erleiden als Patienten, die nicht mit einem physiotherapeutischen Therapieansatz behandelt wurden. Außerdem ließ sich folgende klinische Tendenz erkennen: Bei Vorliegen einer Erkrankung mit Störungen des Immunsystems, einer Hypertonie oder einer Schilddrüsenerkrankung war der Anteil der Patienten, die mehr als 5 Anfälle pro Jahr erleiden größer als bei Nichtvorliegen dieser Erkrankungen.
Schlussfolgerung:
Es wurden Komorbiditäten identifiziert, welche bei der Ätiologie der Menière Erkrankung eine Rolle zu spielen scheinen. Die Ergebnisse tragen dazu bei, das Verständnis der Erkrankung zu verbessern. Die Physiotherapie stellt eine gute Behandlungsmöglichkeit dar, welche künftig bei der Therapie stärker mit einbezogen werden sollte. Die Resultate bieten einen Anreiz, in Zukunft größere Untersuchungen, zum Beispiel in Form von Multicenterstudien durchzuführen.Introduction:
The exact etiology of Menière's disease has not yet been clearly understood. Furthermore, it is questionable to what extent comorbidities characterize this disease. The aim of the present study is to provide a more comprehensive description of the patient-specific clinical picture of Menière's disease. Moreover, a distribution pattern of comorbidities will be presented, in which these will be compared with each other, as well as with the factors gender, age and frequency of seizures. Furthermore, the different therapeutic approaches will be evaluated.
Methods:
This study included 44 patients suffering from Menière's disease. A questionnaire was used to collect data on general medical conditions and conditions that specifically affect the auditory or vestibular organ. Besides, known comorbidities were identified and compared with each other as well as with the factors age groups, gender and seizure frequencies. In addition, data on the therapeutic approaches were compared with seizure frequencies.
Results:
The most common comorbidities could be identified, which are diseases with disorders of the immune system (31,8%), psychological comorbidities (52,3%), hypertension (31,8%), thyroid diseases (20,5%) and migraine (29,5%). Examining these, it could be shown that men with Menière's disease are significantly more often affected by hypertension than women with Menière's disease. Regarding therapy, the results revealed, that patients treated with a physiotherapeutic approach had significantly fewer than 5 seizures on average per year compared to patients who were not treated with a physiotherapeutic approach. Furthermore, the following clinical tendency could be identified: In the presence of a disease involving disorders of the immune system, hypertension or thyroid disease, the proportion of patients experiencing more than 5 seizures per year was greater than in the absence of these conditions.
Conclusion:
Comorbidities were identified that play a role in the etiology of Menière's disease. The results contribute to a better understanding of the disease. Physiotherapy is a good treatment option, which should be more incorporated into therapy in the future. The results provide an incentive to conduct larger studies in the future, for example in the form of multicenter studies
Absicherung hochautomatisierten Fahrens durch passiven virtuellen Dauerlauftest
Die Entwicklung des hochautomatisierten Fahrens (HAF) schreitet in hoher Geschwindigkeit voran. Verschiedene Fahrzeughersteller haben erste Derivate mit automatisierten Fahrfunktionen, die keine Überwachung durch den Fahrer mehr erfordern, für die kommenden Jahre angekündigt. Es fehlt allerdings noch der Sicherheitsnachweis, dass HAF eine insgesamt positive Risikobilanz gemäß den Anforderungen der vom deutschen Verkehrsministerium eingesetzten Ethik-Kommission hat und somit durch die Einführung dieser Funktion die allgemeine Gefahr von Personenschäden im Straßenverkehr nicht zunimmt.
Die aktuell verfügbaren Ansätze zur Erbringung eines Sicherheitsnachweises für Assistenzsysteme sind für HAF ungeeignet. Die wenigen theoretisch möglichen Methoden wie ein Nachweis durch Dauerlauftest sind mit unrealistisch hohem Kosten- und Zeitaufwand verbunden und somit nicht industriell einsetzbar. In dieser Arbeit wird daher eine neue Absicherungsmethode erarbeitet, die einen Beitrag zum Schließen dieser Lücke leistet.
Bei heutigen Entwicklungen von Fahrerassistenzfunktionen kommen nicht-statistische Absicherungsmethoden zum Einsatz. Die Komplexität der Absicherungsaufgabe wird bei diesen dadurch deutlich reduziert, dass ein wesentlicher Teil des Sicherheitskonzepts auf der permanenten Überwachung der Funktion durch den menschlichen Fahrer beruht, der in kritischen Situationen eingreifen muss. Eine Absicherung ist daher nur noch hinsichtlich der Beherrschbarkeit durch den Fahrer notwendig.
Die Fahrerüberwachung kann beim HAF nicht vorausgesetzt werden, sodass die HAF-Funktion selbst alle auftretenden Situationen bis zu einer Fahrerübernahme beherrschen muss. Zusammen mit der großen Situationsvielfalt und dem Einsatz maschinell trainierter Algorithmen erhöht dies die Komplexität der Absicherungsaufgabe in einem Maße, dass die bisher genutzten Absicherungsverfahren an ihre Grenzen stoßen. Dies motiviert den Ansatz "`passives HAF"\u27 zur statistischen Absicherung des hochautomatisierten Fahrens.
Grundidee dieses Ansatzes ist es, einen Teil der jedes Jahr von menschlichen Fahrern gefahrenen Kilometer zur Absicherung zu nutzen, indem ein Teil der Fahrzeuge zusätzlich mit den für HAF erforderlichen Sensoren und Steuergeräten ausgestattet wird. Diese entsprechen dem für den späteren Kundeneinsatz vorgesehen Umfang der HAF-Funktion bis zu der Stelle, an dem Steuer- und Regeleingriffe in die Aktorik des Fahrzeugs erfolgen.
Statt der Umsetzung der vorgegebenen Stellgrößen durch die Fahrzeugaktorik wird der Ausgang der HAF-Funktion mit den vom menschlichen Fahrer tatsächlich durchgeführten Fahrhandlungen verglichen. Die Funktion läuft somit nur "`passiv"\u27 im Hintergrund mit, sodass aus fehlerhaften Aktorvorgaben kein Sicherheitsrisiko resultierten kann. Hierdurch entsteht ein offener Regelkreis, sodass der weitere Verlauf des Szenarios bei Eingriff der HAF-Funktion zunächst unbekannt ist. Der Regelkreis wird geschlossen, indem immer dann anhand der durch die Sensorik aufgenommenen Daten ein Simulationsszenario generiert wird, wenn die Fahrhandlungen vom HAF und menschlichem Fahrer voneinander abweichen. Anhand der Simulation kann überprüft werden, ob die geplante Handlung des HAF zu einer kritischen Situation geführt hätte. Zur Erhöhung der Validität der Simulation werden individualisierte Fahrermodelle verwendet, deren Entwicklung und Zuordnung Teil der entwickelten Methode ist.
Ziel des Ansatzes ist die Nutzung der im realen Straßenverkehr vorkommenden Szenarien als Ausgangspunkt für die Absicherung des HAF mittels Simulation und anschließender Kritikalitätsanalyse. So kann fehlerhaftes Fahrverhalten erkannt und für eine Verbesserung der Funktion verwendet werden.
Die Ergebnisse der Arbeit zeigen, dass mit Hilfe dieser Methode fehlerhafte Fahrentscheidungen des HAF gefunden werden können, welche bei aktivem Funktionsausgang zu kritischen Situationen geführt hätten. Vorteile der Methode sind, dass eine gefahrlose und vergleichsweise kostengünstige und zeiteffiziente Absicherung möglich ist, die aufgrund der Nutzung realer Verkehrsszenarien eine hohe Validität aufweist

Electronic Structure Shift of Deep Nanoscale Silicon by SiO- vs. SiN-Embedding as Alternative to Impurity Doping
Conventional impurity doping of deep nanoscale silicon (dns-Si) used in ultra
large scale integration (ULSI) faces serious challenges below the 14 nm
technology node. We report on a new fundamental effect in theory and
experiment, namely the electronic structure of dns-Si experiencing energy
offsets of ca. 1 eV as a function of SiO- vs. SiN-embedding with a
few monolayers (MLs). An interface charge transfer (ICT) from dns-Si specific
to the anion type of the dielectric is at the core of this effect and arguably
nested in quantum-chemical properties of oxygen (O) and nitrogen (N) vs. Si. We
investigate the size up to which this energy offset defines the electronic
structure of dns-Si by density functional theory (DFT), considering interface
orientation, embedding layer thickness, and approximants featuring two Si
nanocrystals (NCs); one embedded in SiO and the other in SiN.
Working with synchrotron ultraviolet photoelectron spectroscopy (UPS), we use
SiO- vs. SiN-embedded Si nanowells (NWells) to obtain their energy
of the top valence band states. These results confirm our theoretical findings
and gauge an analytic model for projecting maximum dns-Si sizes for NCs,
nanowires (NWires) and NWells where the energy offset reaches full scale,
yielding to a clear preference for electrons or holes as majority carriers in
dns-Si. Our findings can replace impurity doping for n/p-type dns-Si as used in
ultra-low power electronics and ULSI, eliminating dopant-related issues such as
inelastic carrier scattering, thermal ionization, clustering, out-diffusion and
defect generation. As far as majority carrier preference is concerned, the
elimination of those issues effectively shifts the lower size limit of Si-based
ULSI devices to the crystalization limit of Si of ca. 1.5 nm and enables them
