La base de su recuperación está, en primer lugar, en potenciar la participación del usuario en la toma de decisiones; en segundo lugar, en asegurarse que asume la responsabilidad de su propio proceso, y, finalmente en potenciar sus capacidades.     A Study in a Regional Hospital of a Mid-Sized Spanish City Indicates a Major Increase in Infection/Colonization by Carbapenem-Resistant Bacteria, Coinciding with the COVID-19 Pandemic

    Bacterial resistance to antibiotics has proven difficult to control over the past few decades. The large group of multidrug-resistant bacteria includes carbapenemase-producing bacteria (CPB), for which limited therapeutic options and infection control measures are available. Furthermore, carbapenemases associate with high-risk clones that are defined by the sequence type (ST) to which each bacterium belongs. The objectives of this cross-sectional and retrospective study were to describe the CPB population isolated in a third-level hospital in Southern Spain between 2015 and 2020 and to establish the relationship between the ST and the epidemiological situation defined by the hospital. CPB were microbiologically studied in all rectal and pharyngeal swabs and clinical samples received between January 2015 and December 2020, characterizing isolates using MicroScan and mass spectrometry. Carbapenemases were detected by PCR and Sanger sequencing, and STs were assigned by multilocus sequence typing (MLST). Isolates were genetically related by pulsedfield gel electrophoresis using Xbal, Spel, or Apal enzymes. The episodes in which each CPB was isolated were recorded and classified as involved or non-involved in an outbreak. There were 320 episodes with CPB during the study period: 18 with K. pneumoniae, 14 with Klebisella oxytoca, 9 with Citrobacter freundii, 11 with Escherichia coli, 46 with Enterobacter cloacae, 70 with Acinetobacter baumannii, and 52 with Pseudomonas aeruginosa. The carbapenemase groups detected were OXA, VIM, KPC, and NDM with various subgroups. Synchronous relationships were notified between episodes of K. pneumoniae and outbreaks for ST15, ST258, ST307, and ST45, but not for the other CPB. There was a major increase in infections with CPB over the years, most notably during 2020, coinciding with the COVID-19 pandemic. This study highlights the usefulness of gene sequencing techniques to control the spread of these microorganisms, especially in healthcare centers. These techniques offer faster results, and a reduction in their cost may make their real-time application more feasible. The combination of epidemiological data with real-time molecular sequencing techniques can provide a major advance in the transmission control of these CPB and in the management of infected patients. Real-time sequencing is essential to increase precision and thereby control outbreaks and target infection prevention measures in a more effective manner

    Cambio de actitudes de los alumnos que cursan las asignaturas de educación especial

    En este artículo presentamos un proyecto desarrollado con estudiantes de las licenciaturas de Pedagogía y Psicopedagogía en la asignatura troncal de Educación Especial. El trabajo recoge diferentes estrategias que se han desarrollado en las clases para trabajar el cambio de actitudes en los estudiantes que cursan la materia mencionada. Con ellas hemos pretendido que los estudiantes adquieran una perspectiva informada sobre los contenidos de las asignaturas, fomentar el debate sobre aspectos relativos a la confrontación de ideas y conseguir una construcción activa del propio aprendizaje de los estudiantes. Los resultados han sido organizados atendiendo a categorías inductivas que se han extraído de las reflexiones de los estudiantes y de los informes de los profesores participantes en el proyecto. Las conclusiones, en líneas generales, son positivas y parece que la puesta en práctica de las estrategias mencionadas ayuda a los estudiantes a posicionarse y tomar conciencia de los distintos temas que se trabajan en clase, aunque también existen algunas limitaciones relacionadas con la participación de los estudiantes y el interés que generan los temas tratados en clase, apreciándose diferencias entre los estudiantes

    En este trabajo se realiza un estudio de los distintos materiales empleados en la construcción, tanto originales como añadidos, las distintas intervenciones rea lizadas en el Mihrab de la antigua Mezquita de Almería. El estudio de las mues tras ha permitido determinar la naturaleza de cada una de ellas, así como la exis tencia de gran cantidad de sales (clomros y sulfatos en su mayoría). Asimismo, se ha obtenido información sobre su micromorfología. porosidad, mayor o me nor grado de cristalinidad y estado de alteración. Finalmente se ha comprobado la aplicación, en época reciente, de consolidantes que han sido responsables del desprendimiento superficial

    Trace elements and C and N isotope composition in two mushroom species from a mine-spill contaminated site

    Fungi play a key role in the functioning of soil in terrestrial ecosystems, and in particular in the remediation of degraded soils. The contribution of fungi to carbon and nutrient cycles, along with their capability to mobilise soil trace elements, is well-known. However, the importance of life history strategy for these functions has not yet been thoroughly studied. This study explored the soil-fungi relationship of two wild edible fungi, the ectomycorrhizal Laccaria laccata and the saprotroph Volvopluteus gloiocephalus. Fruiting bodies and surrounding soils in a mine-spill contaminated area were analysed. Isotope analyses revealed Laccaria laccata fruiting bodies were 15N-enriched when compared to Volvopluteus gloiocephalus, likely due to the transfer of 15N-depleted compounds to their host plant. Moreover, Laccaria laccata fruiting bodies δ13C values were closer to host plant values than surrounding soil, while Volvopluteus gloiocephalus matched the δ13C composition to that of the soil. Fungal species presented high bioaccumulation and concentrations of Cd and Cu in their fruiting bodies. Human consumption of these fruiting bodies may represent a toxicological risk due to their elevated Cd concentrations

    32-week premature rupture of membranes caused by oropharyngeal microbiota

    Introduction. Preterm premature rupture of membranes (PPROM) usually has a multifactorial etiology that is often unknown, although the most frequently reported cause is infection by group B Streptococcus. Therefore, the etiology of PPROM, although probably infectious, remains unknown in most cases. This case describes a PPROM caused by infection from oropharyngeal microbiota. Case presentation. We report the case of a 26-yr-old pregnant woman. The gestational age was 32 weeks+5 days. Examinations in the emergency department revealed the release of clear amniotic fluid and a closed multiparous cervix with a length of 22 mm. Endocervical culture evidenced the growth of Staphylococcus aureus, serogroup B Neisseria meningitidis and Haemophilus influenzae. Conclusion. Preventive antibiotic therapy should consider: opportunistic infections by normal genital microbiota, infections due to sexual activity, opportunist microorganisms derived from oral sex, and the hematogenous spread of oral bacteria.S

    Digital physiotherapy intervention in children in a low resource setting in Anantapur (India): Study protocol for a randomized controlled trial

    Introduction: In rural India the scarcity of physiotherapists and inequalities complicate the recovery of traumatized children. This study protocol will explore a digital physiotherapy intervention in children with ankle fracture in a low-resource setting to improve functional independence and quality of life. Methods and analysis: A randomized clinical trial with a mixed quantitative-qualitative design will be carried out. It is a single-blind study, where the evaluator does not know the nature of the intervention. Sixty subjects will be enrolled and randomly divided into two groups: the experimental group (EG) will receive a 4-week digital physiotherapy intervention through an app in a recycled mobile device after hospital discharge; the control group (CG) will receive the physiotherapy standard care recommended for patients discharged from the hospital. Subjects will receive a baseline (T0-pre) assessment of Functional Independence and Quality of Life. At the end of the 4-week intervention (T1-post) a new assessment of the outcome will be performed adding data on adherence, satisfaction (ad hoc questionnaire and TSQ), and barriers of use. Qualitative outcomes will also be explored. The author’s hypothesized that the implementation of a digital physiotherapy intervention is feasible and effective to improve functional independence and quality of life. This study protocol is the first to explore the effect of digital physiotherapy intervention in children’s patients in a low resource setting (Anantapur). Discussion: The successful delivery of the intervention, an optimal adherence records, the absence of significant adverse effects, user satisfaction level and the qualitative analysis of limitations, will demonstrate the effectiveness of these procedure. This study will add more evidence in support the use of digital physiotherapy practice as an effective tool. User particularities, provider’s capacity, technological and cultural limitations, and considerations for vulnerable populations will be taken into account
