    Influence of menopausal symptoms on sexual function in middle-aged women

    PURPOSE: To evaluate the influence of climacteric symptoms on the sexual function in middle-aged women. METHODS: A cross-sectional population study was conducted on a sample of 370 middle-aged women, aged 40 to 65 years-old, cared for at the Basic Health Units in Natal, in the state of Rio Grande do Norte, Brazil. We used a questionnaire containing questions on sociodemographic, clinical, and behavioral characteristics. Sexual function was evaluated by the Female Sexual Function Index (FSFI), while the menopause symptoms by the Menopause Rating Scale (MRS). RESULTS: In the studied group, 67% of the women reported risk for sexual dysfunction (FSFI&#8804;26.5). All FSFI domains (desire, arousal, lubrication, orgasm, satisfaction, and pain) were lower in women with risk for sexual dysfunction (p<0.001). The arousal, orgasm, and pain domains were most likely to contribute to lower FSFI scores. All somatovegetative, urogenital, and psychological MRS symptoms were more elevated in women with risk for sexual dysfunction, being significant for all comparisons (p<0.001). Logistic regression analysis revealed that the likelihood of women with risks of sexual dysfunction to present hot flushes, depression, sexual problems, and vaginal dryness was, respectively, 2.1 (95%CI 1.2 - 3.5); 2.4 (95%CI 1.5 - 4.1); 2.3 (95%CI 1.4 - 3.8), and 2.2 (95%CI 1.3 - 3.6) times higher, respectively, compared to those without any risk. CONCLUSION: Climacteric symptoms seem to influence the sexual function in middle-aged women.OBJETIVO: Avaliar a influência dos sintomas climatéricos na função sexual de mulheres de meia-idade. MÉTODOS: Estudo populacional de corte transversal, com amostra de 370 mulheres entre 40 e 65 anos, atendidas nas Unidades Básicas de Saúde da cidade de Natal, no estado do Rio Grande do Norte, Brasil. Aplicou-se um questionário referente s características sociodemográficas, clínicas e comportamentais das mulheres. A função sexual foi avaliada pelo Female Sexual Function Index (FSFI), enquanto os sintomas do climatério pelo Menopause Rating Scale (MRS). RESULTADOS: No grupo estudado, 67% das mulheres apresentaram risco de disfunção sexual (FSFI&#8804;26,5). Todos os domínios do FSFI (desejo, excitação, lubrificação, orgasmo, satisfação e dor) apresentaram escores mais baixos nas mulheres com risco de disfunção sexual (p<0,001). Os domínos excitação, orgasmo e dor foram os que mais contribuíram para os baixos escores do FSFI. Os sintomas somatovegetativos, urogenitais e psicológicos do MRS apresentaram-se mais elevados nas mulheres com risco de disfunção sexual, sendo significativos para todas as comparações (p<0,001). A análise de regressão logística revelou que as chances de mulheres com riscos de disfunção sexual apresentarem fogachos, humor depressivo, problemas sexuais e ressecamento vaginal foram, respectivamente, 2,1 (IC95% 1,2 - 3,5); 2,4 (IC95% 1,5 - 4,1); 2,3 (IC95% 1,4 - 3,8) e 2,2 (IC95% 1,3 - 3,6) vezes maior, quando comparadas quelas sem risco. CONCLUSÃO: Os sintomas climatéricos parecem influenciar a função sexual de mulheres na meia-idade.32933

    Reconstrução de polegar utilizando o segundo pododáctilo: Thumb reconstruction using the second pododactyl

    Introdução: O polegar permite uma grande amplitude de movimentos essenciais para as atividades cotidianas. Portanto, a amputação traumática do primeiro quirodáctilo provoca um significativo comprometimento estético e funcional. Nesses casos, a reconstrução do polegar utilizando dedo do pé constitui-se como opção terapêutica, devendo-se avaliar cada caso individualmente. Objetivos: Descrever e avaliar as técnicas cirúrgicas realizadas em dois pacientes com transferência do segundo pododáctilo para a mão devido à amputação traumática. Caso clínico: Dois pacientes do sexo masculino foram submetidos a cirurgias de reconstrução do polegar com transferência do segundo pododáctilo para a mão numa clínica privada localizada na cidade de Fortaleza – Ceará. O primeiro paciente teve amputação total, inclusive do metacarpo, já o segundo teve amputação de falange média e distal. Em ambos os casos, utilizou-se o segundo pododáctilo para a reconstrução do polegar. No pós-operatório tardio, os pacientes evoluíram estética e funcionalmente bem. Considerações finais:&nbsp; O transplante de dedos do pé para a mão, em casos de trauma, como a amputação do polegar, apesar da sua complexidade e dos riscos de complicações, apresenta resultados estéticos e funcionais favoráveis ao paciente

    Impact of microablative fractional radiofrequency on the vaginal health, microbiota, and cellularity of postmenopausal women

    OBJECTIVES: We aimed to evaluate the effectiveness of microablative fractional radiofrequency (MAFRF) in the non-hormonal treatment of genitourinary syndrome of menopause. METHODS: We examined the cases of 55 postmenopausal women before and after treatment with regard to their vaginal health index (VHI), vaginal microbiota, vaginal pH, and cell maturation. Three applications of MAFRF were performed in the vagina/vaginal introitus. During the treatment, six vaginal smears were obtained and stained with the Papanicolaou stain for determining the degree of cell maturation and with Gram stain for classification of vaginal flora, as per the criteria of Spiegel and Amsel. For vaginal pH determination, pH indicator strips were applied against the vaginal wall. Statistical analysis was performed using SPSS for Windows (version 17.0). Data were reported as mean±standard deviation. The differences were analyzed using the statistical method of generalized estimation equations with autoregressive correlation structure ‘‘1’’ and robust standard errors. RESULTS: The mean age was 59.8±4.2 years, and the mean time of menopause was 15.4±4.5 years. After treatment, there was an increase in the percentage of Lactobacillus spp. (po0.001). Consequently, there was a progressive decrease in vaginal pH during the treatment (po0.001). Regarding cell maturation, there was a decrease in the percentage of parabasal cells (p=0.001) and an increase in the rate of superficial cells (po0.001). Additionally, there was an improvement in the VHI index. The mean VHI values before and after treatment were 13.2±5.6 and 22.5±3.7, respectively (po0.001). CONCLUSION: MAFRF treatment is well tolerated and leads to improvement in the vaginal microenvironment

    Management of Atypical Squamous Cell Cases: A Prospective Study of Women seen at a Private Health Service in Northeastern Brazil

    <div><p>Abstract Objective To assess the management chosen by gynecologists after atypical squamous cells (ASCs) cytology results, and to evaluate the outcomes of these cases in Brazilian women. Methods A prospective observational study evaluated the initial management offered by the gynecologist in the case of 2,458 ASCs cytology results collected between January of 2010 and July of 2016. The outcomes of the cytology, high-risk human papilloma virus (HR-HPV) test and histology were compared in two subgroups: atypical squamous cells of undetermined significance (ASC-US) and atypical squamous cells-cannot exclude high-grade squamous intraepithelial lesion (ASC-H). Results In many cases of ASC-US (36.97%) and ASC-H (40.50%), no clinical actions were taken. Cytology was the most frequent follow-up chosen, including for cases of ASC-H, which goes against the conduct recommended in the national guideline. In women over 30 years of age, the period of time elapsed between an ASC-US result and a new cytology was in 13.03 months, in disagreement with the national guideline recommendations (p< 0.0001). Negative for intraepithelial lesions or malignancy (NILM) cytologic (p = 0.0026) and histologic (p = 0.0017) results in the follow-up were associated with prior ASC-US, while negative results for ASC-H were cytologically (p< 0.0001) and histologically associated with high-grade squamous intraepithelial lesion (HSIL) (p< 0.0001). Two invasive cervical carcinomas (ICCs) were found in the follow-up for ASC-H, and there was a statistically significant association (p = 0.0341). A positive HR-HPV test was associated with ASC-H (p = 0.0075). Conclusion The data suggest that even for a population of Brazilian women assisted at private clinics, the national guidelines recommendations for ASCs results are not followed.</p></div

    Perinatal morbidity and mortality associated with chlamydial infection: a meta-analysis study

    OBJECTIVE: To evaluate the effect of Chlamydia trachomatis infection during pregnancy on perinatal morbidity and mortality. METHODS: Systematic review and meta-analysis in an electronic database and manual, combining high sensitivity specific descriptors seeking to answer the research objective. The articles considered to be of high methodological quality (score above 6 on the Newcastle-Ottawa Scale) were assessed by meta-analysis. RESULTS: Summary estimates of 12 studies were calculated by means of Mantel-Haenszel test with 95% confidence interval. It was observed that Chlamydia infection during pregnancy increased risk of preterm labor (relative risk (RR) = 1.35 [1.11, 1.63]), low birth weight (RR = 1.52 [1.24, 1.87]) and perinatal mortality (RR = 1.84 [1.15, 2.94]). No evidence of increased risk was associated with Chlamydia infection in regard to premature rupture of membranes (RR = 1.13 [0.95, 1.34]), abortion and postpartum endometritis (RR = 1.20 [0.65, 2.20] and 0.89 [0.49, 1.61] respectively). CONCLUSION: The diagnosis and treatment of Chlamydia cervicitis during pregnancy can reduce perinatal morbidity and mortality associated with this infection. However, clinical trials are needed to confirm these findings
