    The association of positive psychological factors with work ability one year after myocardial infarction

    Background Positive psychological factors like optimism, resilience and self-efficacy may facilitate successful adjustment after hospitalization for myocardial infarction (MI) and treatment with percutaneous coronary intervention (PCI), including return to work. Objective To examine whether positive psychological factors (optimism, resilience, and self-efficacy) predict work ability one year after PCI for MI. Methods Patients treated with PCI and with paid employment were included and completed questionnaires at 1 and 12 months post PCI discharge. Patients filled out the LOT-R optimism scale, the dispositional resilience scale (DRS-15), and the Cardiac Self-efficacy Scale (CSE) at 1-month, and the work ability index (WAI) at 1-year follow-up. Hierarchical linear regression models were used. Sensitivity analysis was performed for the acuteness of the PCI treatment. Results In total, 323 patients (14% women; mean age 59.5 ± 6.8y; 74% acute PCI) completed both surveys. At 1-year follow-up, resilience (β = 0.152, p = 0.009) and cardiac self-efficacy (β = 0.273, p < 0.001), but not optimism (β = 0.044, p = 0.432), were associated with work ability at 1 year, irrespective of cardiac history, or sex. Age (β = −0.158, p = 0.002) and comorbidity index (β = −0.104, p = 0.044) were significant covariates. Sensitivity analysis revealed that in patients receiving an elective Conclusion Resilience and cardiac self-efficacy were independently associated with work ability 1 year post PCI, whereas optimism was not. Identification and support of patients low in cardiac self-efficacy and resilience may contribute to improved restoration of work ability post PCI

    Migrating medical communications software to a multi-tenant cloud environment

    The rise of cloud computing has paved the way for many new applications. Many of these new cloud applications are also multi-tenant, ensuring multiple end users can make use of the same application instance. While these technologies make it possible to create many new applications, many legacy applications can also benefit from the added flexibility and cost-savings of cloud computing and multi-tenancy. In this paper, we describe the steps required to migrate a. NET-based medical communications application to the Windows Azure public cloud environment, and the steps required to add multi-tenancy to the application. We then discuss the advantages and disadvantages of our migration approach. We found that the migration to the cloud itself requires only a limited amount of changes to the application, but that this also limited the benefits, as individual instances would only be partially used. Adding multi-tenancy requires more changes, but when this is done, it has the potential to greatly reduce the cost of running the application

    BABY BOOM target genes provide diverse entry points into cell proliferation and cell growth pathways

    Ectopic expression of the Brassica napus BABY BOOM (BBM) AP2/ERF transcription factor is sufficient to induce spontaneous cell proliferation leading primarily to somatic embryogenesis, but also to organogenesis and callus formation. We used DNA microarray analysis in combination with a post-translationally regulated BBM:GR protein and cycloheximide to identify target genes that are directly activated by BBM expression in Arabidopsis seedlings. We show that BBM activated the expression of a largely uncharacterized set of genes encoding proteins with potential roles in transcription, cellular signaling, cell wall biosynthesis and targeted protein turnover. A number of the target genes have been shown to be expressed in meristems or to be involved in cell wall modifications associated with dividing/growing cells. One of the BBM target genes encodes an ADF/cofilin protein, ACTIN DEPOLYMERIZING FACTOR9 (ADF9). The consequences of BBM:GR activation on the actin cytoskeleton were followed using the GFP:FIMBRIN ACTIN BINDING DOMAIN2 (GFP:FABD) actin marker. Dexamethasone-mediated BBM:GR activation induced dramatic changes in actin organization resulting in the formation of dense actin networks with high turnover rates, a phenotype that is consistent with cells that are rapidly undergoing cytoplasmic reorganization. Together the data suggest that the BBM transcription factor activates a complex network of developmental pathways associated with cell proliferation and growth

    What could influence workers’ decisions to disclose a mental illness at work?

    Background: Stigma can be a barrier for workers experiencing a mental illness to access accommodations at work. However, work accommodations may be necessary to maintain a worker's ability to work. Therefore, it may be important to develop effective interventions to address workplace stigma. Objective: To determine (1) what proportion of workers would probably disclose their mental health issue to their manager, (2) what are the motivating factors for the decision of whether or not to disclose, and (3) what would potentially change the disclosure decision? Methods: A link to a Web-based questionnaire was sent to a nationally representative sample of 1671 Dutch adults over 18 years of age. The response rate was 74%. We focused on the 892 respondents who indicated they were either employed for pay or looking for employment, not in management positions, and never experienced a mental health issue. This group comprised 73% of the total sample. They were asked if they would disclose their mental health issue to their manager. For what reasons would they disclose/not disclose the issue? And, what could change their decision? Results: We found that almost 75% of workers would disclose to their managers. The perceived relationship with their managers and feelings of responsibility to their workplaces were important contributors to the decision. A large minority of workers would not tell, preferring to deal with their issues alone. In addition, a significant proportion of workers would choose not to disclose fearing negative consequences. Conclusion: Our results indicate that the majority of these Dutch workers would disclose a mental health issue to their managers. The relationship with the manager plays a central role. The advice from a trusted individual and the experiences of colleagues are also significant factors in the disclosure decision

    Effectiveness of a stigma awareness intervention on reemployment of people with mental health issues/mental illness:A cluster randomised controlled trial

    PurposeA barrier for reemployment of people with mental health issues/mental illness (MHI) is workplace stigma and discrimination. In this RCT the effectiveness of a stigma-awareness intervention addressing finding work, retaining work and decisional stress were evaluated.MethodsA cluster RCT was conducted in 8 Dutch municipal practices. Randomisation took place at practice level. Participants were unemployed people with MHI, receiving social benefits. The intervention consisted of a decision aid for workplace disclosure for participants and a 2 × 3 h stigma-awareness training for their employment specialists. Primary outcomes were measured at baseline, 3-, 6- and 12-months. Multilevel analyses, containing random intercepts of participants nested in organizations, were conducted to analyse the effects of the intervention.ResultsParticipants (N = 153) were randomized to an experimental (n = 76) or control group (n = 77). At six months, significantly more participants of the experimental group (51%) had found work compared to the control group (26%). At twelve months, significantly more participants of the experimental group (49%) had retained work compared to the control group (23%). Intention-to-treat analyses showed that randomization to the experimental group was associated with finding (OR(95%CI) = 7.78(1.33–45.53), p = 0.02) and retaining (OR(95%CI) = 12.15(2.81–52.63), p &lt; 0.01) work more often at twelve months. Analyses showed that the experimental and control group did not differ in decisional stress.ConclusionsOur stigma awareness intervention was effective for finding and retaining work. As the percentage of people who found and retained work almost doubled, this suggests that on a societal level, a vast number of unemployed people could be reemployed with a relatively simple intervention.Trial RegistrationThe study was retrospectively registered at the Dutch Trial Register (TRN: NL7798, date: 04-06-2019)

    Psychometric properties of the Internalized Stigma of Mental Illness (ISMI-10) scale in a Dutch sample of employees with mental illness

    BACKGROUND: Internalized stigma can have numerous negative effects on the well-being and employment of people with mental illness. Brief, valid, and reliable measures are needed to get a better understanding of self-stigmatization. The aim of this study is to translate the brief version of the Internalized Stigma of Mental Illness (ISMI-10) scale into a Dutch version and to assess the reliability and validity of this Dutch version in a sample of employees with mental illness. METHODS: The ISMI-10 was translated into Dutch using the forward-backward translation procedure. The sample consisted of 161 employees with mental illness. Internal consistency was evaluated and the retest reliability was tested with 68 respondents. The construct validity was evaluated by testing convergent and divergent validity. RESULTS: The Dutch ISMI-10 showed good internal consistency (α = 0.83) and good test-retest reliability (r = 0.73). The Dutch ISMI-10 demonstrated excellent convergent validity; high correlations were found between the Dutch ISMI-10 and hope (r = -0.54), anxiety and depression (r = 0.59), self-esteem (r = -0.56), and empowerment (r = − 0.59). Acceptable divergent validity was indicated; small correlations were found between the Dutch ISMI-10 and the physical functioning subscale (r = -0.27) and the role limitation due to physical problems subscale (r = -0.21), and medium correlations were found between the Dutch ISMI-10 and the general health subscale (r = -0.36). CONCLUSION: The Dutch ISMI-10 demonstrated adequate psychometric properties for assessing internalized stigma and can be used by researchers in Dutch speaking countries to get a better understanding of self-stigmatization among people with mental illness.
