Inzidenz und klinische Relevanz ventrikulärer Fernfeldsignale bei ICD-Patienten: Vergleich von Elektroden mit ultrakurzem und normalem Tip-Ring Abstand der atrialen Elektrode
Ventrikuläres FFRW Oversensing kann als Störsignal verschiedene ungünstige Folgen haben. Dazu gehören bei Herzschrittmachern unter anderem inadäquater Mode-Switch und bei ICDs eine fehlerhafte Beeinflussung der SVT-VT Diskrimination. Bipolare atriale Sonden mit ultrakurzem Tip-Ring Abstand zeigten im Vergleich zu konventionellen Sonden bei Herzschrittmachern eine Verbesserung des Quotienten Stör-/Nutzsignal und konnten zu einer Abnahme von inadäquaten Mode-Switch Episoden führen. Im Gegensatz dazu wurde das Verhalten dieses Quotienten und die Inzidenz von FFRW sowie dessen Oversensing und die daraus resultierenden klinischen Konsequenten in ICD-Populationen bisher noch nicht ausreichend untersucht und sind Thema der Dissertation.
Datenbasis ist die prospektiv randomisierte DECREASE Studie, die den Effekt einer Erhöhung der Schwelle zur Detektion von Tachyarrhythmien auf die Abgabe von inadäquaten Schocks bei 543 Patienten mit primärprophylaktischer ICD-Indikation im Vergleich zu einer konventionellen Programmierung mit niedriger Detektionsschwelle untersuchte. Im Rahmen der Studie wurde der Einsatz von atrialen Sonden mit einem ultrakurzen Tip-Ring Abstand (1,1 mm) empfohlen, allerdings nur bei ca. 38% der Patienten mit Zwei- oder Dreikammersystemen eingesetzt. Die restlichen Patienten erhielten abgesehen von wenigen Ausnahmen atriale ST-Sonden mit einem handelsüblichen Tip-Ring Abstand von 10 mm.
Die aktuell durchgeführte Arbeit entspricht einer nachträglichen Subanalyse, bei der alle Patienten mit Zweikammer- oder CRT-ICDs, welche kein permanentes Vorhofflimmern hatten und bei denen IEGMs vorlagen, während des Grundrhythmus (Baseline) und während Tachykardieepisoden untersucht wurden. Daraus ergibt sich eine Gesamtzahl von n=61 untersuchten Patienten.
Ziel der Arbeit war es herauszufinden, welchen Effekt der Einsatz von UST-Sonden in Bezug auf FFRW spontan, dessen Oversensing und daraus resultierenden klinischen Konsequenzen im Vergleich zu ST-Sonden hat. Untersuchte Parameter waren die Amplituden von Nahfeldsignal und FFRW spontan, der Quotient FFRW spontan-/Nahfeldsignal und die Inzidenz von FFRW und dessen Oversensing sowie daraus resultierende klinische Auswirkungen.
Als Ergebnis konnte festgestellt werden, dass sowohl die Amplituden von Nahfeldsignalen (ST: 2,92 ± 1,49 mV; UST: 2,33 ± 1,28 mV; p=0,141) als auch von FFRW spontan (ST: 0,34±0,40 mV; UST: 0,04±0,13 mV; p=0,0027) während der Baseline bei UST-Sonden deutlich kleiner als bei ST-Sonden waren. Für den letztgenannten Parameter war der Unterschied dabei signifikant. Auch in Bezug auf den resultierenden Quotienten Stör-/Nutzsignal zeigte sich ein signifikanter Vorteil der UST-Sonden.
Hinsichtlich der Inzidenz von FFRW spontan im Bereich der kumulativen Baseline war diese bei ST-Sonden signifikant höher als bei UST-Sonden (ST: 25/42, UST: 3/19); p=0,002). Während der Tachykardieepisoden war der diesbezügliche Unterschied allerdings knapp nicht signifikant (ST: 12/26, UST: 1/10; p=0,06). Wurden die Häufigkeiten von kumulativer Baseline und Tachykardieepisoden zusammengefasst, ließ sich ebenfalls eine signifikant höhere Inzidenz von FFRW spontan bei ST-Sonden nachweisen (ST: 37/68, UST 4/29; p=0,002). Tatsächliches FFRW Oversensing trat nur bei einem Patienten mit ST-Sonde auf.
Insgesamt führte der Einsatz der UST-Sonden somit sowohl zu einer deutlichen Reduktion der Amplitude und der Inzidenz von FFRW spontan.
Die Tachykardiedetektion durch tatsächliches FFRW Oversensing wurde im meinerseits untersuchten Patientenkollektiv nicht beeinflusst. Allerdings berichteten andere Autoren über durch FFRW Oversensing bedingte Fehlfunktionen im Bereich der SVT-VT Diskrimination oder inadäquaten Mode-Switch bei ICD-Patienten. Somit wäre prinzipiell die Reduktion von FFRW und dessen Oversensing durch Einsatz von UST-Sonden wünschenswert.
Im klinischen Alltag haben diese heutzutage allerdings kaum Relevanz, da sie nicht MRT-fähig sind und unter optimaler Beachtung beziehungsweise Konfiguration diverser weiterer Parameter trotz des handelsüblichen Tip-Ring Abstands moderner atrialer Sonden von 10-12 mm eine ausreichende Reduktion von FFRW Oversensing in der Regel möglich zu sein scheint.
Angesichts der genannten Vorteile in Bezug auf FFRW und dessen Oversensing wäre die häufigere Verwendung von UST-Sonden im klinischen Alltag sowie deren technische Weiterentwicklung trotzdem wünschenswert

Weyl-fermions, Fermi-arcs, and minority-spin carriers in ferromagnetic CoS2
The pyrite compound CoS2 has been intensively studied in the past due to its
itinerant ferromagnetism and potential for half-metallicity, which make it a
promising material for spintronic applications. However, its electronic
structure remains only poorly understood. Here we use complementary bulk- and
surface-sensitive angle-resolved photoelectron spectroscopy and ab-initio
calculations to provide a complete picture of its band structure. We discover
Weyl-cones at the Fermi-level, which presents CoS2 in a new light as a rare
member of the recently discovered class of magnetic topological metals. We
directly observe the topological Fermi-arc surface states that link the
Weyl-nodes, which will influence the performance of CoS2 as a spin-injector by
modifying its spin-polarization at interfaces. Additionally, we are for the
first time able to directly observe a minority-spin bulk electron pocket in the
corner of the Brillouin zone, which proves that CoS2 cannot be a true
half-metal. Beyond settling the longstanding debate about half-metallicity in
CoS2, our results provide a prime example of how the topology of magnetic
materials can affect their use in spintronic applications
Controllable orbital angular momentum monopoles in chiral topological semimetals
The emerging field of orbitronics aims at generating and controlling currents
of electronic orbital angular momentum (OAM) for information processing.
Structurally chiral topological crystals could be particularly suitable
orbitronic materials because they have been predicted to host topological band
degeneracies in reciprocal space that are monopoles of OAM. Around such a
monopole, the OAM is locked isotopically parallel or antiparallel to the
direction of the electron's momentum, which could be used to generate large and
controllable OAM currents. However, OAM monopoles have not yet been directly
observed in chiral crystals, and no handle to control their polarity has been
discovered. Here, we use circular dichroism in angle-resolved photoelectron
spectroscopy (CD-ARPES) to image OAM monopoles in the chiral topological
semimetals PtGa and PdGa. Moreover, we also demonstrate that the polarity of
the monopole can be controlled via the structural handedness of the host
crystal by imaging OAM monopoles and anti-monopoles in the two enantiomers of
PdGa, respectively. For most photon energies used in our study, we observe a
sign change in the CD-ARPES spectrum when comparing positive and negative
momenta along the light direction near the topological degeneracy. This is
consistent with the conventional view that CD-ARPES measures the projection of
the OAM monopole along the photon momentum. For some photon energies, however,
this sign change disappears, which can be understood from our numerical
simulations as the interference of polar atomic OAM contributions, consistent
with the presence of OAM monopoles. Our results highlight the potential of
chiral crystals for orbitronic device applications, and our methodology could
enable the discovery of even more complicated nodal OAM textures that could be
exploited for orbitronics.Comment: 16 pages, 8 figure
Correlation of plasma copeptin and vasopressin concentrations in hypo-, iso-, and hyperosmolar states
Background: Copeptin, the C-terminal moiety of provasopressin, is cosecreted with vasopressin. Copeptin may be a useful parameter to characterize disorders of water homeostasis and can be readily measured in plasma or serum. However, it is unknown to date how circulating copeptin and vasopressin levels correlate at different plasma osmolalites. Objective: To correlate plasma copeptin with plasma osmolality and vasopressin concentrations in healthy subjects during iso-, hypo-, and hyperosmolar states. Methods: Plasma osmolalities, copeptin, and vasopressin levels were measured in 20 volunteers at baseline, after an oral water load, and during and after iv infusion of 3% saline. Correlation coefficients were determined between plasma osmolalites and copeptin and vasopressin concentrations, as well as between vasopressin and copeptin concentrations. Results: Median plasma osmolalities decreased from 290 mOsm/kg (range, 284-302) at baseline to 281 (273-288) mOsm/kg after water load and rose to 301 (298-307) mOsm/kg after hypertonic saline. Median plasma copeptin concentrations decreased from 3.3 (1.1-36.4) pm at baseline to 2.0 (0.9-10.4) pm after water load and increased to 13.6 (3.7-43.3) pm after hypertonic saline. Vasopressin and copeptin concentrations correlated with plasma osmolality (Spearman's rank correlation coefficient 0.49 and 0.77, respectively). There was a close correlation of vasopressin and copeptin concentrations (Spearman's rank correlation coefficient 0.8). Conclusion: Plasma vasopressin and copeptin correlate strongly over a wide range of osmolalities in healthy individuals. Therefore, the measurement of copeptin, which remains stable for several days, is a useful alternative to vasopressin measurements and will likely facilitate the differential diagnosis of disorders of water metabolis
