    Relationship between serum total magnesium and serum potassium in emergency surgical patients in a tertiary hospital in Ghana

    Introduction: Potassium and magnesium are the two most abundant intra cellular cations. They play pivotal roles inĀ many essential biological processes. Deficiencies of these electrolytes are of clinical importance in hospitalised patients.Aim: To determine the relationship between serum total magnesium and potassium levels in adult patients requiringĀ an emergency intra-abdominal surgery and the clinical utility of this relationship in the care of patients.Methods: A cross sectional study was conducted over a five month period at the Korle-Bu Teaching Hospital. AllĀ 101 adult patients admitted for emergency intra abdominal surgery, who met the inclusion criteria and gave informedĀ consent, were consecutively recruited and enrolled into the study. Patientsā€™ characteristics preoperative totalĀ serum magnesium, serum potassium and albumin levels were determined. Regression analysis and correlation coefficientsĀ were used to determine the relation between serum magnesium and potassium. Analysis was done usingĀ SPSS version 20.Results: Mean serum total magnesium and serum potassium were 0.66Ā±0.20mmol/L and 3.79Ā±0.65mmol/L respectively.Ā There was a significant but weak positive correlation between serum hypokalaemia and serum magnesiumĀ levels. Pearsonā€™s correlation coefficient (2 tailed) was 0.21, R2= 0.04, p = value 0.038. Analysis generated a regressionĀ model: [Mg] = 0.06[K] + 0.42mmol/L, with a p-value = 0.038.Conclusion: A mathematical relationship was found between serum total magnesium and serum potassium amongĀ adult patients who require emergency intra-abdominal surgery.     Complexities in Subsetting Level 2 Data

    Satellite Level 2 data presents unique challenges for tools and services. From nonlinear spatial geometry to inhomogeneous file data structure to inconsistent temporal variables to complex data variable dimensionality to multiple file formats, there are many difficulties in creating general tools for Level 2 data support. At NASA Goddard Earth Sciences Data and Information Services Center (GES DISC), we are implementing a general Level 2 Subsetting service for Level 2 data to a user-specified spatio-temporal region of interest (ROI).     What is a sentinel node? Re-evaluating the 10% rule for sentinel lymph node biopsy in melanoma

    Introduction Many surgeons use the ā€œ10% ruleā€ to define whether a lymph node is a sentinel node (SLN) when staging malignant melanoma. However, this increases the number of SLN removed and the time and cost of the procedure. We examined the impact of raising this threshold on the accuracy of the procedure. Methods We reviewed the records of 561 patients with melanoma (624 basins) who underwent SLN with technetium Tc99 labeled sulfur colloid using a definition of a SLN as 10% of that of the node with the highest counts per minute (CPM). Results Of the 624 basins, 154 (25%) were positive for metastases. An average of 1.9 nodes per basin were removed (range 1ā€“6). Metastases were found in the hottest node in 137 cases (89% of positive basins, 97% of basins overall). Increasing the threshold above 10% decreased the number of nodes excised and the costs involved, but incrementally raised the number of false negative cases above baseline (a 4% increase for a ā€œ20% rule,ā€ 5% for a ā€œ30% rule,ā€ 6% for a ā€œ40% rule,ā€ and 7% for a ā€œ50% ruleā€). Taking only the hottest node would raise the false negative rate by 11%. Conclusions Although using thresholds higher than 10% for the definition of a SLN will minimize the extent of surgery and decrease the costs associated with the procedure, it will compromise the accuracy of the procedure and is not recommended.     Intense hurricane transports sand onshore:Example from the Pliocene Malbusca section on Santa Maria Island (Azores, Portugal)

    Southern cliffs on Santa Maria Island in the Azores archipelago (North Atlantic Ocean) feature submarine volcanic sequences inter-bedded with Pliocene coralline algal limestone, shelly coquinas, and mixed volcaniclastic-calcarenite sandstone. Within the 20-m sedimentary succession at Malbusca, a singular, 5-m sandstone bed is distinguished by dark and light laminae dominated alternately by heavy minerals and carbonate detritus. Carbonate grain-size varies between that of coarse silt and very fine sand. The basal part shows coarser and more poorly sorted sand in an upward transition to increasingly finer carbonates. Accessible over a lateral space of 34 m, the big bed is shouldered against and overlaps the remnants of a drowned rocky shore with a paleorelief of 4 m that preserves intertidal to shallow subtidal biotas. Extrapolated from the big bed's rock face (1830 m2) and the width of the eroded shelf on which it resides (8 m), calculations yield a projected volume of 14,500 m3. Unique to the island, the big bed is interpreted as a major hurricane deposit that moved sand from an offshore bar in an onshore path. Such an event fits the context of the Pliocene Warm Period, during which global El NiƱo conditions were more intense than today

    BACKGROUND: The aim of this study is to evaluate effectiveness of a new ISUOG (International Society of Ultrasound in Obstetrics and Gynecology) Outreach Teaching and Training Program delivered in Muscat, Oman. METHODS: Quantitative assessments to evaluate knowledge and practical skills were administered before and after an ultrasound course for sonologists attending the ISUOG Outreach Course, which took place in November, 2017, in Oman. Trainees were selected from each region of the country following a national vetting process conducted by the Oman Ministry of Health. Twenty-eight of the participants were included in the analysis. Pre- and post-training practical and theoretical scores were evaluated and compared. RESULTS: Participants achieved statistically significant improvements, on average by 47% (pĀ <ā€‰0.001), in both theoretical knowledge and practical skills. Specifically, the mean score in the theoretical knowledge test significantly increased from 55.6% (Ā± 14.0%) to 81.6% (Ā± 8.2%), while in the practical test, the mean score increased from 44.6% (Ā± 19.5%) to 65.7% (Ā± 23.0%) (pĀ <ā€‰0.001). Performance was improved post-course among 27/28 participants (96.4%) in the theoretical test (range: 14 to 200%) and among 24/28 (85.7%) trainees in the practical skills test (range: 5 to 217%). CONCLUSION: Application of the ISUOG Basic Training Curriculum and Outreach Teaching and Training Course improved the theoretical knowledge and practical skills of local health personnel. Long-term re-evaluation is, however, considered imperative to ascertain and ensure knowledge retention

    Angiotensin-Converting Enzyme Inhibitor Therapy Affects Left Ventricular Mass in Patients With Ejection Fraction >40% After Acute Myocardial Infarction

    AbstractObjectives. We tested the hypothesis that angiotensin-converting enzyme (ACE) inhibitor therapy decreases left ventricular (LV) mass in patients with a left ventricular ejection fraction (LVEF) >40% and no evidence of heart failure after their first acute Q wave myocardial infarction (MI).Background. Recently, ACE inhibitor therapy has been shown to have an early mortality benefit in unselected patients with acute MI, including patients without heart failure and a LVEF >35%. However, the effects on LV mass and volume in this patient population have not been studied.Methods. Thirty-five patients with a LVEF >40% after their first acute Q wave MI were randomized to titrated oral ramipril (n = 20) or conventional therapy (control, n = 15). Magnetic resonance imaging (MRI) performed an average of 7 days and 3 months after MI provided LV volumes and mass from summated serial short-axis slices.Results. Left ventricular end-diastolic volume index did not change in ramipril-treated patients (62 Ā± 16 [SD] to 66 Ā± 17 ml/m2) or in control patients (62 Ā± 16 to 68 Ā± 17 ml/m2), and stroke volume index increased significantly in both groups. However, LV mass index decreased in ramipril-treated patients (82 Ā± 18 to 73 Ā± 19 g/m2, p = 0.0002) but not in the control patients (77 Ā± 15 to 79 Ā± 23 g/m2). Systolic arterial pressure did not change in either group at 3-month follow-up.Conclusions. In patients with a LVEF >40% after acute MI, ramipril decreased LV mass, and blood pressure and LV function were unchanged after 3 months of therapy. Whether the decrease in mass represents a sustained effect that is associated with a decrease in morbid events requires further investigation.(J Am Coll Cardiol 1997;29:49ā€“54)
