Valores de referencia de la población diabética para la versión española del SF-12v2
ResumenObjetivoLos cuestionarios de salud percibida son instrumentos que proporcionan información importante para la investigación en resultados de salud. La familia de cuestionarios SF requiere valores de referencia para interpretar el impacto en calidad de vida relacionada con la salud. Estudios previos han proporcionado estos valores para la población general española, pero no para condiciones específicas de enfermedad. El objetivo de este trabajo ha sido obtener, para la versión española del SF-12v2, los valores de referencia de la población diabética de la Región de Murcia.MétodosEstudio transversal con encuesta telefónica a 1.500 personas representativas de los diabéticos tipo 1 y 2, no institucionalizados, de 18 años y más, de la Región de Murcia. Se calcularon medidas de tendencia central, dispersión y percentiles de las ocho dimensiones, e índices resumen (físico y mental), del SF-12v2.ResultadosEl índice de salud mental (ISM) medio (50,5±12,8) fue superior al físico (ISF) (42,5±11,8). En función del sexo, los hombres obtuvieron un ISM (53,6±11,6) y un ISF (44,9±11,2) mejores que las mujeres (ISM: 47,7±13,2; ISF: 40,3±11,9). La dimensión mejor valorada fue la vitalidad (hombres: 57,2±11,1; mujeres: 51,9±12,1), y la peor la salud general (hombres: 39,0±9,7; mujeres: 35,8±10,0). Estratificando por grupos de edad, los hombres seguían presentando mejores puntuaciones que las mujeres.ConclusionesEstos resultados deben ser considerados como valores de referencia de la población diabética de la Región de Murcia de la versión española del SF-12v2, y pueden ser útiles para establecer objetivos terapéuticos, comparar con población general, sana y con otras enfermedades.AbstractObjectivePerceived health status questionnaires provide important information for health outcomes research. Reference measures are required to interpret the health-related quality of life questionnaires belonging to the short form (SF) health survey family. Previous studies have provided these reference measures for the Spanish general population, but not for specific disease conditions. The aim of the present study was to obtain diabetic population-based norms for the Spanish version of the 12-item short form health survey version II (SF-12v2) in the region of Murcia (Spain).MethodsWe performed a cross-sectional telephone survey in 1,500 non-institutionalized patients with type 1 or 2 diabetes, aged 18 years or older and representative of the region of Murcia. The central position, dispersion statistics and percentiles were calculated for each of the eight SF-12v2 scales and summary components (physical and mental).ResultsThe mental component summary (MCS) (mean: 50.5±12.8) was higher than the physical component summary (PCS) (42.5±11.8). By sex, men had more favorable MCS (53.6±11.6) and PCS (44.9±11.2) scores than women (MCS: 47.7±13.2; PCS: 40.3±11.9). The dimension with the highest score was vitality (men: 57.2±11.1; women: 51.9±12.1), while that with the lowest score was general health (men: 39.0±9.7; women: 35.8±10.0). When the groups were stratified by age, scores continued to be higher in men than in women.ConclusionsThe results of the present study should be taken as the diabetic population-based norms for the Spanish version of the SF-12v2 in the region of Murcia. These results may be useful to establish therapeutics targets, as well as for comparisons with the general, healthy and ill populations
La cova novella de na llebrona (Manacor, Mallorca)
[cat] En el present treball es descriu la cova Novella de na Llebrona, una cavitat descoberta en el terme municipal de Manacor durant les tasques de construcció de la nova carretera que voreja Portocristo cap al sud. Es tracta d’una cova de dimensions discretes, ja que tan sols assoleix devers 250 m de desenvolupament. No obstant això, en el seu interior es varen localitzar restes de ceràmica posttalaiòtica i també de fauna prehumana del Quaternari de les Balears (Myotragus balearicus i Hypnomys morpheus). Les tasques espeleològiques efectuades han permès la preservació de la cavitat, sense que es vegi afectada per les obres de la nova carretera.[eng] This paper describes the Cova Novella de Na Llebrona, a cave discovered in the municipality of Manacor during construction works of a new road along the south of Portocristo village. It is a cave of discrete dimensions, since only reaches 250 meters of development. Nevertheless, remains of Posttalaiotic pottery and pre-human fauna from the Quaternary of the Balearic Islands (Myotragus balearicus and Hypnomys morpheus) were found inside the cave. The performed speleological tasks have allowed the preservation of the cave, without being affected by the construction of the new road
Chronic wounds in Murcia area health
Objetivo: Identificar la prevalencia de heridas crónicas
en el Área VI del Servicio Murciano de Salud
e identificar, dentro de ellas, la proporción en base
a las siguientes variables: por presión, vasculares,
diabéticas, traumáticas o no conocidas/otros.
Material y Método: Estudio descriptivo, transversal
y retrospectivo. La muestra estuvo compuesta
por todas las personas mayores de 13
años de edad, que presentaron al menos una
herida crónica, en base a los registros clínicos
del año 2014. La fuente de datos utilizada fue
la historia clínica electrónica del paciente, en el
ámbito de atención primaria de un Área de Salud
de Murcia. Análisis estadístico descriptivo.
Resultados: Se identificaron 639 personas con
alguna herida crónica identificada en su historia
clínica. La media de edad fue de 75,7 años. El
género femenino fue significativamente el más
frecuente (p<0,05). Se obtuvo una prevalencia de
1 individuo con úlcera por cada 225 analizados.
Las proporciones y distribución de úlceras encontradas
fueron: úlcera por presión (44%), úlceras
vasculares (29%), úlceras traumáticas (15%),
diabéticas (11%) y úlceras no conocidas (1%).
Conclusiones: La prevalencia de úlceras en las
zonas estudiadas asciende a un 0,45% lo que va
en sintonía con otros estudios. La úlcera más
frecuente es la úlcera por presión, seguidas por
vasculares, traumáticas, diabéticas y otras.Objective: To identify the prevalence of chronic

Material and Method: Descriptive, cross-sectional
and retrospective study. The sample was
composed of all persons over 13 years of age,
who presented at least a chronic wound, on the
basis of the clinical records of the year 2014. The
data source used was the electronic clinical history
of the patient, in the field of primary care
in a Health Area of Murcia. Descriptive statistical
analysis.
Results: We identified 639 people with some
chronic wound identified in its history. The average
age was 75.7 years. The female gender was
significantly the most frequent (p<0.05). It was
obtained a prevalence of 1 individual with ulcer
by each 225 analyzed. The proportions and distribution
of ulcers found were: pressure ulcer (44%),
vascular ulcers (29%), traumatic ulcers (15%), diabetes

Prevention of ventilator-associated pneumonia: the multimodal approach of the spanish ICU pneumonia zero program.
Objectives:
The “Pneumonia Zero” project is a nationwide multimodal intervention based on the simultaneous implementation of a comprehensive evidence-based bundle measures to prevent ventilator-associated pneumonia in critically ill patients admitted to the ICU.
Design:
Prospective, interventional, and multicenter study.
Setting:
A total of 181 ICUs throughout Spain.
Patients:
All patients admitted for more than 24 hours to the participating ICUs between April 1, 2011, and December 31, 2012.
Intervention:
Ten ventilator-associated pneumonia prevention measures were implemented (seven were mandatory and three highly recommended). The database of the National ICU-Acquired Infections Surveillance Study (Estudio Nacional de Vigilancia de Infecciones Nosocomiales [ENVIN]) was used for data collection. Ventilator-associated pneumonia rate was expressed as incidence density per 1,000 ventilator days. Ventilator-associated pneumonia rates from the incorporation of the ICUs to the project, every 3 months, were compared with data of the ENVIN registry (April–June 2010) as the baseline period. Ventilator-associated pneumonia rates were adjusted by characteristics of the hospital, including size, type (public or private), and teaching (postgraduate) or university-affiliated (undergraduate) status.
Measurements and Main Results:
The 181 participating ICUs accounted for 75% of all ICUs in Spain. In a total of 171,237 ICU admissions, an artificial airway was present on 505,802 days (50.0% of days of stay in the ICU). A total of 3,474 ventilator-associated pneumonia episodes were diagnosed in 3,186 patients. The adjusted ventilator-associated pneumonia incidence density rate decreased from 9.83 (95% CI, 8.42–11.48) per 1,000 ventilator days in the baseline period to 4.34 (95% CI, 3.22–5.84) after 19–21 months of participation.
Conclusions:
Implementation of the bundle measures included in the “Pneumonia Zero” project resulted in a significant reduction of more than 50% of the incidence of ventilator-associated pneumonia in Spanish ICUs. This reduction was sustained 21 months after implementation

Método: Estudio epidemiológico transversal, periodo 2010-2015. Se captaron los posibles casos mediante los códigos 275.1 (CIE-9-MC), E83.0 (CIE-10) y 905 ORPHA en 15 RAER y el Registro de Pacientes de Enfermedades Raras del Instituto de Salud Carlos III. Los diagnósticos fueron validados revisando la documentación clínica. Se calcularon el valor predictivo positivo (VPP) de las fuentes de información,
la prevalencia, la mortalidad y la distribución de las características sociodemográficas. Resultados: El Conjunto Mínimo Básico de Datos (CMBD) fue la fuente de información más utilizada por los RAER (VPP = 39,4%), seguida del Registro de Medicamentos Huérfanos (RMH) (VPP = 81,9%). La Historia Clínica de Atención Primaria (HCAP) obtuvo un VPP del 55,9%. Las combinaciones con mayor VPP fueron las del RMH con el CMBD (VPP = 95,8%) y del RMH con la HCAP (VPP = 92,9%). Se confirmaron 514 casos, el 57,2% eran hombres, cuya edad mediana de diagnóstico fue de 21,3 años. La prevalencia fue de 1,64/100.000 habitantes en 2015 y la mortalidad del 3,0%, siendo ambas superiores en los hombres. Conclusión: Se recomienda la incorporación en los RAER del RMH y de la HCAP, ya que su combinación y la del RMH con el CMBD podrían utilizarse como criterio de validación automática para la enfermedad de Wilson. Is hospital discharge administrative data an appropriate source of information for cancer registries purposes? Some insights from four Spanish registries
<p>Abstract</p> <p>Background</p> <p>The use of hospital discharge administrative data (HDAD) has been recommended for automating, improving, even substituting, population-based cancer registries. The frequency of false positive and false negative cases recommends local validation.</p> <p>Methods</p> <p>The aim of this study was to detect newly diagnosed, false positive and false negative cases of cancer from hospital discharge claims, using four Spanish population-based cancer registries as the gold standard. Prostate cancer was used as a case study.</p> <p>Results</p> <p>A total of 2286 incident cases of prostate cancer registered in 2000 were used for validation. In the most sensitive algorithm (that using five diagnostic codes), estimates for Sensitivity ranged from 14.5% (CI95% 10.3-19.6) to 45.7% (CI95% 41.4-50.1). In the most predictive algorithm (that using five diagnostic and five surgical codes) Positive Predictive Value estimates ranged from 55.9% (CI95% 42.4-68.8) to 74.3% (CI95% 67.0-80.6). The most frequent reason for false positive cases was the number of prevalent cases inadequately considered as newly diagnosed cancers, ranging from 61.1% to 82.3% of false positive cases. The most frequent reason for false negative cases was related to the number of cases not attended in hospital settings. In this case, figures ranged from 34.4% to 69.7% of false negative cases, in the most predictive algorithm.</p> <p>Conclusions</p> <p>HDAD might be a helpful tool for cancer registries to reach their goals. The findings suggest that, for automating cancer registries, algorithms combining diagnoses and procedures are the best option. However, for cancer surveillance purposes, in those cancers like prostate cancer in which care is not only hospital-based, combining inpatient and outpatient information will be required.</p
