    Launch Detection Satellite System Engineering Error Analysis

    The article of record as published may be found at http://dx.doi.org/10.2514/2.3357An orbiting detector of infrared energy may be used to detect the rocket plumes generated by ballistic missiles during the powered segment of their trajectory. By measuring the angular directions of arrival of the detections over several observations, the trajectory properties, launch location, and impact area may be estimated using a nonlinear least-squares iteration procedure. Observations from two or more sensors may be combined to form stereoscopic lines of sight, increasing the accuracy of the algorithm. A computer model is presented of an estimation algorithm that determines what parameter, or combination of parameters, will have a significant effect on the error of the tactical parameter estimation. This model generates observation data and then, using the data, produces an estimate of the tactical parameters, i.e., the time, position, and heading at launch and burnout, and an impact time and position. For the expected range of values of each of the error sources, the line-of-sight errors, a combination of focal-plane and attitude errors, had the greatest effect on the estimation of the launch and impact points

    Las condiciones particulares del trabajo de las personas afectan su bienestar y estado de salud (Leitner & Resch, 2005). En este sentido, Parra (2003) señala que el trabajo puede ser fuente de salud, causar daño a la salud o gravar problemas de salud. Como fuente de salud, a través del trabajo se puede acceder a una serie de condiciones laborales que satisfacen no sólo las necesidades económicas básicas, sino también las relacionadas con el bienestar individual y grupal. Puede causar daño a la salud, cuando afecta el estado de bienestar de las personas en forma negativa (accidentes de trabajo, enfermedades profesionales). La implementación del sistema de Gestión de la Seguridad y Salud en el trabajo (SGSST) en las organizaciones es de vital importancia, puesto que su objetivo principal consiste en mejorar las condiciones laborales y el ambiente en el trabajo, además de la salud en el trabajo, que conlleva la promoción del mantenimiento del bienestar físico, mental y social de los empleados. Carbones Montiel es una mediana empresa minera (actividad catalogada en riesgo V), que se encuentra comprometida con la vida y la seguridad de sus colaboradores, donde según la matriz de evaluación del SG-SST, cumple con un 98% de los ítems de evaluación, demostrando su compromiso por realizar una minería bien hecha, cumpliendo los protocolos de seguridad necesarios para desarrollar dicha actividadThe particular conditions of people's work affect their well-being and status of health (Leitner & Resch, 2005).     Discordant antibiotic therapy and length of stay in children hospitalized for urinary tract infection

    BACKGROUND: Urinary tract infections (UTIs) are a common reason for pediatric hospitalizations. OBJECTIVE: To determine the effect of discordant antibiotic therapy (in vitro nonsusceptibility of the uropathogen to initial antibiotic) on clinical outcomes for children hospitalized for UTI. DESIGN/SETTING: Multicenter retrospective cohort study in children aged 3 days to 18 years, hospitalized at 5 children's hospitals with a laboratory‐confirmed UTI. Data were obtained from medical records and the Pediatric Hospital Information System (PHIS) database. PARTICIPANTS: Patients with laboratory‐confirmed UTI. MAIN EXPOSURE: Discordant antibiotic therapy. MEASUREMENTS: Length of stay and fever duration. Covariates included age, sex, insurance, race, vesicoureteral reflux, antibiotic prophylaxis, genitourinary abnormality, and chronic care conditions. RESULTS: The median age of the 216 patients was 2.46 years (interquartile range [IQR]: 0.27, 8.89) and 25% were male. The most common causative organisms were E. coli and Klebsiella species. Discordant therapy occurred in 10% of cases and most commonly in cultures positive for Klebsiella species, Enterobacter species, and mixed organisms. In adjusted analyses, discordant therapy was associated with a 1.8 day (95% confidence interval [CI]: 1.5, 2.1) longer length of stay [LOS], but not with fever duration. CONCLUSIONS: Discordant antibiotic therapy for UTI is common and associated with longer hospitalizations. Further research is needed to understand the clinical factors contributing to the increased LOS and to inform decisions for empiric antibiotic selection in children with UTIs.     Treatment and Outcomes of Children With Febrile Urinary Tract Infection Due to Extended Spectrum Beta-lactamase-producing Bacteria in Europe - TOO CUTE Study

    Background: The prevalence of extended-spectrum beta-lactamase producing Εnterobacteriaceae (ESBL-PE) is increasing globally. ESBL-PE are an important cause of urinary tract infections (UTIs) in children. We aimed to characterize the clinical presentation, treatment and outcomes of childhood UTI caused by ESBL-PE in Europe. Methods: Multicenter retrospective cohort study. Children 0 to 18 years of age with fever, positive urinalysis and positive urine culture for an ESBL-PE uropathogen, seen in a participating hospital from January 2016 to July 2017, were included. Main Outcome Measures: Primary outcome measure: day of defervescence was compared between (1) initial microbiologically effective treatment (IET) versus initial microbiologically ineffective treatment (IIT) and (2) single initial antibiotic treatment versus combined initial antibiotic treatment. Secondary outcome measures: Clinical and microbiological failure of initial treatment. Results: We included 142 children from 14 hospitals in 8 countries. Sixty-one children had IET and 77 IIT. There was no statistical difference in time to defervescence for effective/ineffective groups (P = 0.722) and single/combination therapy groups (P = 0.574). Two out of 59 (3.4%) and 4/66 (6.1%) patients exhibited clinical failure during treatment (P = 0.683) when receiving IET or IIT, respectively. Eight of 51 (15.7%) receiving IET and 6/58 (10.3%) receiving IIT patients (P = 0.568) had recurring symptoms/signs suggestive of a UTI. Recurrence of a UTI occurred 15.5 days (interquartile range, 9.0–19.0) after the end of treatment. Conclusions: Time to defervescence and clinical failure did not differ between IET/IIT groups. Non-carbapenem beta-lactam antibiotics may be used for the empiric treatment of ESBL febrile UTIs, until susceptibility testing results become available
