    Role of ethylene on various ripening pathways and on the development of sensory quality of Charentais cantaloupe melons

    Charentais melons (Cucumis melo L., var cantalupensis Naud.) in which ethylene biosynthesis has been suppressed by an antisense ACC oxidase gene have been used to better understand the role of ethylene in the regulation of the ripening process of climacteric fruit and on the development of sensory qualities. We have shown that a number of biochemical and molecular processes associated with the ripening of climacteric fruit are ethylene-independent. In some cases, such as softening of the flesh, the same pathway comprises both ethylene-dependent and -independent components. The various ethylene-dependent events exhibit differential sensitivity to ethylene. The threshold level for degreening of the rind is 1 ppm, while 2.5 ppm are required to trigger the ethylene-dependent component of the softening process. The saturating level of ethylene for all these events is less than 5 ppm, which is by far lower than the internal ethylene concentrations found in the fruit at the climacteric peak (around 100 ppm). Detachment of the fruit influences the development of respiratory climacteric. Fruit remaining attached to the vine, although producing higher levels of ethylene, exhibit a reduced climacteric rise in respiration as compared to detached fruit. The response of antisense ACO fruit to exogenous ethylene in terms of respiration is higher in detached than in attached fruit. Ethylene-suppressed melons show a severe reduction of aroma volatiles production, particularly in ester production. In the biosynthetic pathway of aliphatic esters, the dehydrogenation of fatty acids and aldehydes appears to be ethylene-dependent. In contrast, alcohol acetylation comprises ethylene-dependent and ethylene-independent components, probably corresponding to differentially regulated alcohol acetyl transferases. In terms of sensory quality, these data show that the extension of shelf-life through the inhibition of ethylene production has some beneficial effects on texture and sugar accumulation but is detrimental for the generation of aroma

    The Carbon-Rich Gas in the Beta Pictoris Circumstellar Disk

    The edge-on disk surrounding the nearby young star Beta Pictoris is the archetype of the "debris disks", which are composed of dust and gas produced by collisions and evaporation of planetesimals, analogues of Solar System comets and asteroids. These disks provide a window on the formation and early evolution of terrestrial planets. Previous observations of Beta Pic concluded that the disk gas has roughly solar abundances of elements [1], but this poses a problem because such gas should be rapidly blown away from the star, contrary to observations of a stable gas disk in Keplerian rotation [1, 2]. Here we report the detection of singly and doubly ionized carbon (CII, CIII) and neutral atomic oxygen (OI) gas in the Beta Pic disk; measurement of these abundant volatile species permits a much more complete gas inventory. Carbon is extremely overabundant relative to every other measured element. This appears to solve the problem of the stable gas disk, since the carbon overabundance should keep the gas disk in Keplerian rotation [3]. New questions arise, however, since the overabundance may indicate the gas is produced from material more carbon-rich than the expected Solar System analogues.Comment: Accepted for publication in Nature.     Gradual emergence of superconductivity in underdoped LSCO

    We present triple-axis neutron scattering studies of low-energy magnetic fluctuations in strongly underdoped La2x_{2-x}Srx_{x}CuO4_{4} with x=0.05x=0.05, 0.060.06 and 0.070.07, providing quantitative evidence for a direct competition between these fluctuations and superconductivity. At dopings x=0.06x=0.06 and x=0.07x=0.07, three-dimensional superconductivity is found, while only a very weak signature of two-dimensional superconductivity residing in the CuO2_2 planes is detectable for x=0.05x=0.05. We find a surprising suppression of the low-energy fluctuations by an external magnetic field at all three dopings. This implies that the response of two-dimensional superconductivity to a magnetic field is similar to that of a bulk superconductor.     Use and limitations of malaria rapid diagnostic testing by community health workers in war-torn Democratic Republic of Congo

    <p>Abstract</p> <p>Background</p> <p>Accurate and practical malaria diagnostics, such as immunochromatographic rapid diagnostic tests (RDTs), have the potential to avert unnecessary treatments and save lives. Volunteer community health workers (CHWs) represent a potentially valuable human resource for expanding this technology to where it is most needed, remote rural communities in sub-Saharan Africa with limited health facilities and personnel. This study reports on a training programme for CHWs to incorporate RDTs into their management strategy for febrile children in the Democratic Republic of Congo, a tropical African setting ravaged by human conflict.</p> <p>Methods</p> <p>Prospective cohort study, satisfaction questionnaire and decision analysis.</p> <p>Results</p> <p>Twelve CHWs were trained to safely and accurately perform and interpret RDTs, then successfully implemented rapid diagnostic testing in their remote community in a cohort of 357 febrile children. CHWs were uniformly positive in evaluating RDTs for their utility and ease of use. However, high malaria prevalence in this cohort (93% by RDTs, 88% by light microscopy) limited the cost-effectiveness of RDTs compared to presumptive treatment of all febrile children, as evidenced by findings from a simplified decision analysis.</p> <p>Conclusions</p> <p>CHWs can safely and effectively use RDTs in their management of febrile children; however, cost-effectiveness of RDTs is limited in zones of high malaria prevalence.</p

It is essential that the different species are correctly identified to ensure their exploitation and sustainable conservation. This article raises examples of the taxonomic confusions found in the scientific literature regarding the edible caterpillars of Africa. This study indicates that these taxonomic confusions follow identification based on the comparison of morphological features using photos taken from previous articles that used the same method of identification. Hence there is a repetition of taxonomic confusions in the online scientific literature. To put an end to these confusions, the authors plead for molecular identification of edible caterpillars, which should be extended to all known and available species in order to constitute a real data bank as a reference for all future studies of edible caterpillars

    Diagnosis of obstructive coronary artery disease using computed tomography angiography in patients with stable chest pain depending on clinical probability and in clinically important subgroups: meta-analysis of individual patient data

    OBJECTIVE: To determine whether coronary computed tomography angiography (CTA) should be performed in patients with any clinical probability of coronary artery disease (CAD), and whether the diagnostic performance differs between subgroups of patients. DESIGN: Prospectively designed meta-analysis of individual patient data from prospective diagnostic accuracy studies. DATA SOURCES: Medline, Embase, and Web of Science for published studies. Unpublished studies were identified via direct contact with participating investigators. ELIGIBILITY CRITERIA FOR SELECTING STUDIES: Prospective diagnostic accuracy studies that compared coronary CTA with coronary angiography as the reference standard, using at least a 50% diameter reduction as a cutoff value for obstructive CAD. All patients needed to have a clinical indication for coronary angiography due to suspected CAD, and both tests had to be performed in all patients. Results had to be provided using 2×2 or 3×2 cross tabulations for the comparison of CTA with coronary angiography. Primary outcomes were the positive and negative predictive values of CTA as a function of clinical pretest probability of obstructive CAD, analysed by a generalised linear mixed model; calculations were performed including and excluding non-diagnostic CTA results. The no-treat/treat threshold model was used to determine the range of appropriate pretest probabilities for CTA. The threshold model was based on obtained post-test probabilities of less than 15% in case of negative CTA and above 50% in case of positive CTA. Sex, angina pectoris type, age, and number of computed tomography detector rows were used as clinical variables to analyse the diagnostic performance in relevant subgroups. RESULTS: Individual patient data from 5332 patients from 65 prospective diagnostic accuracy studies were retrieved. For a pretest probability range of 7-67%, the treat threshold of more than 50% and the no-treat threshold of less than 15% post-test probability were obtained using CTA. At a pretest probability of 7%, the positive predictive value of CTA was 50.9% (95% confidence interval 43.3% to 57.7%) and the negative predictive value of CTA was 97.8% (96.4% to 98.7%); corresponding values at a pretest probability of 67% were 82.7% (78.3% to 86.2%) and 85.0% (80.2% to 88.9%), respectively. The overall sensitivity of CTA was 95.2% (92.6% to 96.9%) and the specificity was 79.2% (74.9% to 82.9%). CTA using more than 64 detector rows was associated with a higher empirical sensitivity than CTA using up to 64 rows (93.4% v 86.5%, P=0.002) and specificity (84.4% v 72.6%, P<0.001). The area under the receiver-operating-characteristic curve for CTA was 0.897 (0.889 to 0.906), and the diagnostic performance of CTA was slightly lower in women than in with men (area under the curve 0.874 (0.858 to 0.890) v 0.907 (0.897 to 0.916), P<0.001). The diagnostic performance of CTA was slightly lower in patients older than 75 (0.864 (0.834 to 0.894), P=0.018 v all other age groups) and was not significantly influenced by angina pectoris type (typical angina 0.895 (0.873 to 0.917), atypical angina 0.898 (0.884 to 0.913), non-anginal chest pain 0.884 (0.870 to 0.899), other chest discomfort 0.915 (0.897 to 0.934)). CONCLUSIONS: In a no-treat/treat threshold model, the diagnosis of obstructive CAD using coronary CTA in patients with stable chest pain was most accurate when the clinical pretest probability was between 7% and 67%. Performance of CTA was not influenced by the angina pectoris type and was slightly higher in men and lower in older patients. SYSTEMATIC REVIEW REGISTRATION: PROSPERO CRD42012002780

    Antibiotics for coughing in general practice: a questionnaire study to quantify and condense the reasons for prescribing

    BACKGROUND: Antibiotics are being overprescribed in ambulant care, especially for respiratory tract infections (RTIs). Gaining insight into the actual reasons for prescribing remains important for the design of effective strategies to optimise antibiotic prescribing. We aimed to determine items of importance for the antibiotic prescribing decision and to make them operational for an intervention trial. METHODS: A postal questionnaire based upon focus group findings was sent to 316 Flemish general practitioners (GPs). On a verbal rating scale the GPs scored to what extent they consider the questionnaire items in decision making in case of suspected RTI in a coughing patient and how strongly the items support or counter antibiotic treatment. Factor analysis was used to condense the data. The relative importance of the yielded operational factors was assessed using Wilcoxon Matched Pairs test. RESULTS: 59.5% completed the study. Response group characteristics (mean age: 42.8 years; 65.9% men) approximated that of all Flemish GPs. Participants considered all the items included in the questionnaire: always the operational factor 'lung auscultation', often 'whether or not there is something unusual happening' – both medical reasons – and to a lesser extent 'non-medical reasons' (P < 0.001). Non-medical as well as medical reasons support antibiotic treatment, but non-medical reasons to a lesser extent (P < 0.001). CONCLUSION: This study quantified, condensed and confirmed the findings of previous focus group research. Practice guidelines and interventions to optimise antibiotic prescribing have to take non-medical reasons into account

    Reduced Cognitive Assessment Scores Among Individuals With Magnetic Resonance Imaging-Detected Vascular Brain Injury

    Background and Purpose- Little is known about the association between covert vascular brain injury and cognitive impairment in middle-aged populations. We investigated if scores on a cognitive screen were lower in individuals with higher cardiovascular risk, and those with covert vascular brain injury. Methods- Seven thousand five hundred forty-seven adults, aged 35 to 69 years, free of cardiovascular disease underwent a cognitive assessment using the Digital Symbol Substitution test and Montreal Cognitive Assessment, and magnetic resonance imaging (MRI) to detect covert vascular brain injury (high white matter hyperintensities, lacunar, and nonlacunar brain infarctions). Cardiovascular risk factors were quantified using the INTERHEART (A Global Study of Risk Factors for Acute Myocardial Infarction) risk score. Multivariable mixed models tested for independent determinants of reduced cognitive scores. The population attributable risk of risk factors and MRI vascular brain injury on low cognitive scores was calculated. Results- The mean age of participants was 58 (SD, 9) years; 55% were women.
