    An Empirical Study of Health Consumer Beliefs, Attitude and Intentions toward the Use of Self-Service Kiosks

    There is increasing interest in reaching and empowering patients and health consumers directly through informationtechnology (IT). However, consumer readiness for an increasingly IT-enabled healthcare system has been questioned andthere remains a need for more theory driven research into IT adoption by health consumers. This study has contributed byexamining the influence of selected individual difference variables on health consumer beliefs, attitudes and intentionstoward the use of self-service kiosks in healthcare. A survey of 192 patients in two private healthcare clinics operating inurban centers in South Africa was carried out. Results show that four individual difference variables, namely computeranxiety, self-efficacy, need for interaction, and trust are significant predictors of patient beliefs and attitude. Expected easeof-use was found the strongest predictor of adoption intentions

    A cis-regulatory logic simulator

    <p>Abstract</p> <p>Background</p> <p>A major goal of computational studies of gene regulation is to accurately predict the expression of genes based on the cis-regulatory content of their promoters. The development of computational methods to decode the interactions among cis-regulatory elements has been slow, in part, because it is difficult to know, without extensive experimental validation, whether a particular method identifies the correct cis-regulatory interactions that underlie a given set of expression data. There is an urgent need for test expression data in which the interactions among cis-regulatory sites that produce the data are known. The ability to rapidly generate such data sets would facilitate the development and comparison of computational methods that predict gene expression patterns from promoter sequence.</p> <p>Results</p> <p>We developed a gene expression simulator which generates expression data using user-defined interactions between cis-regulatory sites. The simulator can incorporate additive, cooperative, competitive, and synergistic interactions between regulatory elements. Constraints on the spacing, distance, and orientation of regulatory elements and their interactions may also be defined and Gaussian noise can be added to the expression values. The simulator allows for a data transformation that simulates the sigmoid shape of expression levels from real promoters. We found good agreement between sets of simulated promoters and predicted regulatory modules from real expression data. We present several data sets that may be useful for testing new methodologies for predicting gene expression from promoter sequence.</p> <p>Conclusion</p> <p>We developed a flexible gene expression simulator that rapidly generates large numbers of simulated promoters and their corresponding transcriptional output based on specified interactions between cis-regulatory sites. When appropriate rule sets are used, the data generated by our simulator faithfully reproduces experimentally derived data sets. We anticipate that using simulated gene expression data sets will facilitate the direct comparison of computational strategies to predict gene expression from promoter sequence.     A pilot study exploring the acute effects of aerobic exercise and relaxation on fatigue and executive function in breast cancer survivors

    More than 250,000 new cases of invasive breast cancer, and roughly 63,000 new cases of in situ breast cancer were estimated to be diagnosed in 2017, within the United States (DeSantis, Ma, Goding Sauer, Newman, & Jemal, 2017). Chronic fatigue occurs at an extremely high rate in women diagnosed with breast cancer and can lead to increased sequelae and decreased quality of life. Aerobic exercise has been shown to improve certain aspects of cognitive functioning among breast cancer survivors (F. C. Dimeo, 2001). Additionally, common relaxation therapies such as guided mindfulness training (Deimling, Sterns, Bowman, & Kahana, 2005) have been shown to reduce fatigue, anxiety, and other negative affective states (DeSantis, Ma, Goding Sauer, et al., 2017). Though researchers have explored the separate impacts of aerobic exercise and relaxation methods (i.e., meditation and yoga) (Jacobs, Mehling, Goldberg, & Eppel, 2004; Sadja & Mills, 2013), the existing literature lacks a clear consensus regarding the effectiveness of acute aerobic exercise and relaxation techniques for reducing fatigue and related symptoms in breast cancer survivors. Driving this line of research are desires of survivors for an enjoyable, feasible intervention focused on the negative effects (e.g., fatigue) of chemotherapy treatment. Certain forms of chemotherapy (e.g., anthracycline-based) are more associated with decreased quality of life, including severe fatigue, as well as cognitive and physical functioning deficits. To date, no studies have tested the combined effects of acute aerobic exercise and adjuvant relaxation therapy delivered via technology for producing an enjoyable and feasible intervention to target fatigue and related outcomes within the breast cancer survivorship population. The addition of guided mindfulness-based relaxation after aerobic exercise may increase perceived energy, reduce fatigue, enhance mental focus, and boost one’s overall interpretation of exercise experiences. This, in turn, may facilitate more favorable attitudes towards exercise and personal health and well-being

    Flow Dynamics And Plasma Heating Of Spheromaks In SSX

    We report several new experimental results related to flow dynamics and heating from single dipole-trapped spheromaks and spheromak merging studies at SSX. Single spheromaks (stabilized with a pair of external coils, see Brown, Phys. Plasmas 13 102503 (2006)) and merged FRC-like configurations (see Brown, Phys. Plasmas 13, 056503 (2006)) are trapped in our prolate (R = 0.2 m, L = 0.6 m) copper flux conserver. Local spheromak flow is studied with two Mach probes (r(1) = rho(i) ) calibrated by time-of-flight with a fast set of magnetic probes at the edge of the device. Both Mach probes feature six ion collectors housed in a boron nitride sheath. The larger Mach probe will ultimately be used in the MST reversed field pinch. Line averaged flow is measured by ion Doppler spectroscopy (IDS) at the midplane. The SSX IDS instrument measures with 1 mu s or better time resolution the width and Doppler shift of the C-III impurity (H plasma) 229.7 nm line to determine the temperature and line-averaged flow velocity (see Cothran, RSI 77, 063504 (2006)). We find axial flows up to 100 km/s during formation of the dipole trapped spheromak. Flow returns at the wall to form a large vortex. Recent high-resolution IDS velocity measurements during spheromak merging show bi-directional outflow jets at +/- 40 km/s (nearly the Alfven speed). We also measure T-i \u3e= 80 eV and T-e \u3e= 20 eV during spheromak merging events after all plasma facing surfaces are cleaned with helium glow discharge conditioning. Transient electron heating is inferred from bursts on a four-channel soft x-ray array. The spheromaks are also characterized by a suite of magnetic probe arrays for magnetic structure B(r,t), and interferometry for n(e) . Finally, we are designing a new oblate, trapezoidal flux conserver for FRC studies. Equilibrium and dynamical simulations suggest that a tilt-stable, oblate FRC can be formed by spheromak merging in the new flux conserver

    Functional Imaging of Decision Conflict

    Decision conflict occurs when people feel uncertain as to which option to choose from a set of similarly attractive (or unattractive) options, with many studies demonstrating that this conflict can lead to suboptimal decision making. In this article, we investigate the neurobiological underpinnings of decision conflict, in particular, the involvement of the anterior cingulate cortex (ACC). Previous studies have implicated the ACC in conflict monitoring during perceptual tasks, but there is considerable controversy as to whether the ACC actually indexes conflict related to choice, or merely conflict related to selection of competing motor responses. In a functional magnetic resonance imaging study, we dissociate the decision and response phases of a decision task, and show that the ACC does indeed index conflict at the decision stage. Furthermore, we sho that it does so for a complex decision task, one that requires the integration of beliefs and preferences and not just perceptual judgments

    3D Reconnection And Flow Dynamics In The SSX Experiment

    Several new experimental results are reported from plasma merging studies at the Swarthmore Spheromak Experiment (SSX) with relevance to collisionless three-dimensional magnetic reconnection in laboratory and space plasmas. First, recent high-resolution velocity measurements of impurity ions using ion Doppler spectroscopy (IDS) show bi-directional outflow jets at 40 km/s (nearly the Alfven speed). The SSX IDS instrument measures with 1 mu s or better time resolution the width and Doppler shift of the C-III impurity (H plasma) 229.7 nm line to determine the temperature and line-averaged flow velocity during spheromak merging events. High flow speeds are corroborated using an in situ Mach probe. Second, ion heating to nearly 10(6) K is observed after reconnection events in a low-density kinetic regime. Transient electron heating is inferred from bursts on a 4-channel soft x-ray array as well as vacuum ultraviolet spectroscopy. Third, the out-of-plane magnetic field and the in-plane Lorentz force in a reconnection volume both show a quadrupolar structure at the ion inertial scale (c/omega(pi)). Time resolved vector magnetic field measurements on a 3D lattice B(r, t)) enables this measurement. Earlier work at SSX has shown that fori-nation of three-dimensional structure at the ion inertial scale is temporally and spatially correlated with the observation of superthermal, super-Alfvenic ions accelerated along the X-line normal to the local 2D plane of reconnection. Each of these measurements will be related to and compared with similar observations in a solar or space context

    Who is meeting the strengthening physical activity guidelines by definition: A cross-sectional study of 253 423 English adults?

    The current UK physical activity guidelines recommend that adults aged 19 to 65 years perform activity to strengthen muscle and bone a minimum of twice weekly. The number of adults meeting strengthening activity guidelines is lower than for aerobic activity, but estimates vary between studies partly due to differences in how muscle-strengthening activity is defined. We aimed to provide estimates for strengthening activity prevalence in English adults based on a nationally representative sample of n = 253,423 18-65-year-olds. We attempted to quantify the variation in estimates attributable to differences in the way strengthening activity is defined. Finally, we aim to provide a brief descriptive epidemiology of the factors associated with strengthening activity. Adults met guidelines for aerobic activity if they reported the activity equivalent to &gt;150 min/week moderate-intensity exercise. Respondents met strengthening guidelines if they reported at least two bouts per week of strengthening activity. We defined strengthening activity, first, according to criteria used in the Health Survey for England (HSE). Second, we counted bouts of strengthening activities for which we could find evidence of health-related benefits (Evidence). Third, we included bouts of strengthening activity as defined in current UK physical activity guidelines (Guideline). Two-thirds (67%) of adults met guidelines for aerobic activity (69% of men, 65% of women). Less than one-third (29% of men and 24% of women) met guidelines for the HSE definition of strengthening activity. Under the Evidence definition, 16% of men and 9% of women met strengthening guidelines. Using the most-stringent definition (Guideline) just 7.3% of men and 4.1% of women achieved the recommendations for strengthening activity. We found females and older adults (50–65 years) were less likely to meet guidelines for aerobic, strengthening, and combined aerobic plus strengthening activity. The prevalence of meeting activity guidelines was lower in adults from more deprived areas (compared with the least deprived); Adults with lower academic qualifications (Level 1) were less likely to meet activity guidelines than those educated to Level 4 (Degree Level) or higher. Having a limiting disability was associated with a lower prevalence of meeting activity guidelines. Associations between socio-demographic measures and the prevalence of adults meeting activity guidelines were stronger for strengthening activity than for aerobic 51(or combined aerobic plus strengthening) activity Compared with aerobic activity, fewer adults engage in strengthening activity regardless of how it is defined. The range in estimates for how many adults meet strengthening activity guidelines can be explained by variations in the definition of ‘strengthening’ that are used and the specific sports or activities identified as strengthening exercise. When strengthening activity is included, the proportion of English adults meeting current physical activity guidelines could be as high as 1 in 3 but possibly as low as just 1 in 20. A harmonized definition of strengthening activity, that is aligned with physical activity guidelines, is required to provide realistic and comparable prevalence estimates

    NextGen Flight Deck Data Comm: Auxiliary Synthetic Speech Phase I

    Data Comm—a text-based controller-pilot communication system—is critical to many NextGen improvements. With Data Comm, communication becomes a visual task. Interacting with a visual Data Comm display may yield an unsafe increase in head-down time, particularly for single-pilot operations. This study examined the feasibility of supplementing Data Comm with synthetic speech. To this end, thirty-two pilots flew two experimental scenarios in a Cessna 172 Flight Training Device. In one scenario, ATC communication was with a text-only Data Comm display, in the other, communication was with a text Data Comm display with synthetic speech that read aloud each message (i.e., text+speech). Pilots heard traffic with similar call signs on the party line and received a conditional clearance (in both scenarios); in either scenario, pilots received a clearance that was countermanded by a live controller. Results indicated that relative to the text-only display, the text+speech display aided single-pilot performance by reducing head-down time, and may have prevented participants from acting early on the conditional clearance. Supplementing text Data Comm with speech did not introduce additional complications: participants were neither more likely to erroneously respond to similar call signs, nor to ignore a live ATC countermand

    At risk of being risky: The relationship between "brain age" under emotional states and risk preference.

    Developmental differences regarding decision making are often reported in the absence of emotional stimuli and without context, failing to explain why some individuals are more likely to have a greater inclination toward risk. The current study (N=212; 10-25y) examined the influence of emotional context on underlying functional brain connectivity over development and its impact on risk preference. Using functional imaging data in a neutral brain-state we first identify the "brain age" of a given individual then validate it with an independent measure of cortical thickness. We then show, on average, that "brain age" across the group during the teen years has the propensity to look younger in emotional contexts. Further, we show this phenotype (i.e. a younger brain age in emotional contexts) relates to a group mean difference in risk perception - a pattern exemplified greatest in young-adults (ages 18-21). The results are suggestive of a specified functional brain phenotype that relates to being at "risk to be risky.
