    The Art of the Possible: Relationships in Teaching and Learning

    The purpose of this study is to learn from the journeys of teacher educators the qualities that help to engender and sustain a commitment to an empowering pedagogy. An empowering pedagogy is understood as one that enhances students’ sense of agency and confidence in themselves and their relationships with each other. Professional experiences that challenge or enable the development of such pedagogy are also explored through the stories of pre-service teachers and myself. Two case studies of outstanding teacher educators were undertaken in order to explore how they engender relationships in the classroom and their importance to students’ learning. While there has been a great deal of research into quality teaching and learning in schools, understandings of pedagogy in tertiary settings are constrained by conceptions of learning as an individual cognitive process. This means that the affective dimensions of pedagogy have been little explored. This thesis addresses this gap. Arts-informed narrative inquiry is the methodology adopted. This approach recognises the interconnections between personal and professional development and involves participants in the creative reconstruction of experience. The findings from the case studies are represented in the form of a story that draws on the tradition of magical realism. This style enables a sense of the energies and qualities that help to foster an empowering pedagogy to be experienced. The themes that emerge from participating pre-service teachers’ stories illuminate the importance of the embodied ways in which the teacher’s sense of self and sense of relationship with others comes into play in the classroom. These senses manifest in the embodied responsiveness of the case study teacher educators and the creative ways that they foster enjoyment in learning and enabling relationships in the classroom. Experiences of enabling relationships can engender feelings of equality, self-acceptance, mutual trust, inclusion and enjoyment in learning among students. The shared experience of these qualities helps to create an empowering pedagogy

    Cracking the resistance of non-tuberculosis mycobacterium against existing antibiotics

    Mycobacterium avium (M. avium) is quickly becoming the most prevalent Mycobacterium infection in western countries. M. avium is an opportunistic pulmonary pathogen, infecting patients who often are vulnerable due to existing medical conditions. Antibiotic therapy of M. avium is long (generally 18 months) and often unsuccessful at eliminating the infection, promoting resistance to commonly used antibiotics, such as clarithromycin. M. avium exists as a biofilm in lung infections, adding to the difficulty in eradicating the infection. My research investigated potential M. avium biofilm dispersing agents, as well as antibiotic synergy for improved M. avium eradication. Our findings show that ten clinically isolated M. avium samples were all highly resistant (>16μg/mL) to antibiotics commonly used in the treatment of M. avium infections, both in planktonic and biofilm phenotypes. Some isolates were found to have MICs of >128μg/mL. M. avium isolates differed in biofilm phenotype, and correspondingly, their susceptibility to antibiotics and potential biofilm dispersing agents. No antibiotics tested during this research had significant bactericidal or bacteriostatic action on any M. avium isolates, in planktonic or biofilm phenotype. The addition of 3.6mg/mL aspirin (acetylsalicylic acid) or 3.6mg/mL ibuprofen to planktonic and biofilm cells showed significant increase in M. avium cell death. Previously identified biofilm dispersing agents, such as mannitol, N-acetyl-L-cysteine, active manuka honey, methylglyoxal, Lipitor®, 2,4-dinitrophenol and EDTA, had no significant effect on dispersing M. avium biofilms or increasing their susceptibility to antibiotics. A concentrations of 4mg/mL aspirin combined with 8mg/mL of EDTA had significant effects, sterilizing and completely eradicating in vitro three week old mature M. avium biofilms. Future work should be conducted into the specific pathways aspirin and ibuprofen effect. Furthermore, development of effective agonists/antagonists of those pathways to disperse M. avium biofilms should be researched. Research into improved and novel Mycobacterial antibiotics are needed to develop drugs capable of successfully treating M. avium infections

    Long-term success for people living with HIV: A framework to guide practice

    Objectives: In recent decades, the needs of people living with HIV have evolved as life expectancy has greatly improved. Now, a new definition of long-term success (LTS) is necessary to help address the multifaceted needs of all people living with HIV. Methods: We conducted a two-phase research programme to delineate the range of experiences of people living with HIV. The insights garnered from these research phases were explored in a series of expert-led workshops, which led to the development and refinement of the LTS framework. Results: The insights generated from the research phases identified a series of themes that form a part of LTS. These themes were subsequently incorporated into the LTS framework, which includes five outcome pillars: sustained undetectable viral load, minimal impact of treatment and clinical monitoring, optimized health-related quality of life, lifelong integration of healthcare, and freedom from stigma and discrimination. A series of supporting statements were also developed by the expert panel to help in the achievement of each of the LTS pillars. Conclusions: The LTS framework offers a comprehensive and person-centric approach that, if achieved, could help improve the long-term well-being of people living with HIV and support the LTS vision of 'every person living with HIV being able to live their best life'

    : Over the past few decades, the life expectancy of people living with HIV has markedly improved due to the advances in HIV diagnosis, linkage to care, and treatment. However, with these advances, a new set of challenges has emerged that must be addressed to ensure the long-term well-being of people living with HIV. In this article, as part of a wider journal supplement, we explore the unmet needs and challenges across the HIV continuum of care and re-define what long-term success looks like to support the healthy ageing of all people affected by HIV

    Ground Reaction Forces: The Sine Qua Non of Legged Locomotion

    Legged locomotion results from the feet pressing against the ground to generate ground reaction forces (GRFs) that are responsible for moving the body. By changing limb coordination patterns and muscle forces, the GRFs are adjusted to allow the horse to move in different gaits, speeds, and directions with appropriate balance and self-carriage. This article describes the typical GRF patterns in each gait, the adaptations that produce turning, and the GRF patterns used to unload the painful limb when the horse is lame. The intent is to provide information that is of practical interest and value to equine scientists rather than being a comprehensive review of the topic.     Women write the rights of woman: The sexual politics of the personal pronoun in the 1790s

    This article investigates patterns of personal pronoun usage in four texts written by women about women's rights during the 1790s: Mary Wollstonecraft's A Vindication of the Rights of Woman (1792), Mary Hays' An Appeal to the Men of Great Britain (1798), Mary Robinson's Letter to the Women of England (1799) and Mary Anne Radcliffe's The Female Advocate (1799). I begin by showing that at the time these texts were written there was a widespread assumption that both writers and readers of political pamphlets were, by default, male. As such, I argue, writing to women as a woman was distinctly problematic, not least because these default assumptions meant that even apparently gender-neutral pronouns such as I, we and you were in fact covertly gendered. I use the textual analysis programme WordSmith to identify the personal pronouns in my four texts, and discuss my results both quantitatively and qualitatively. I find that while one of my texts does little to disturb gender expectations through its deployment of personal pronouns, the other three all use personal pronouns that disrupt eighteenth century expectations about default male authorship and readership. Purpose: The aim was to evaluate the effectiveness of computer-assisted input-based intervention for children with speech sound disorders (SSD). Method: The Sound Start Study was a cluster-randomized controlled trial. Seventy-nine early childhood centers were invited to participate, 45 were recruited, and 1,205 parents and educators of 4- and 5-year-old children returned questionnaires. Children whose parents and educators had concerns about speech were assessed (n =275); 132 children who were identified with phonological patternbased errors underwent additional assessment. Children with SSD and no difficulties with receptive language or hearing, typical nonverbal intelligence, and English as their primary language were eligible; 123 were randomized into two groups (intervention n = 65; control n = 58), and 3 withdrew. The intervention group involved Phoneme Factory Sound Sorter software (Wren & Roulstone, 2013) administered by educators over 9 weeks; the control group involved typical classroom practices. Participants were reassessed twice by a speech-language pathologist who was unaware of the initial assessment and intervention conditions.     11β-HSD1 inhibition in men mitigates prednisolone-induced adverse effects in a proof-of-concept randomised double-blind placebo-controlled trial

    Glucocorticoids prescribed to limit inflammation, have significant adverse effects. As 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1) regenerates active glucocorticoid, we investigated whether 11β-HSD1 inhibition with AZD4017 could mitigate adverse glucocorticoid effects without compromising their anti-inflammatory actions. We conducted a proof-of-concept, randomized, double-blind, placebo-controlled study at Research Unit, Churchill Hospital, Oxford, UK (NCT03111810). 32 healthy male volunteers were randomized to AZD4017 or placebo, alongside prednisolone treatment. Although the primary endpoint of the study (change in glucose disposal during a two-step hyperinsulinemic, normoglycemic clamp) wasn’t met, hepatic insulin sensitivity worsened in the placebo-treated but not in the AZD4017-treated group. Protective effects of AZD4017 on markers of lipid metabolism and bone turnover were observed. Night-time blood pressure was higher in the placebo-treated but not in the AZD4017-treated group. Urinary (5aTHF+THF)/THE ratio was lower in the AZD4017-treated but remained the same in the placebo-treated group. Most anti-inflammatory actions of prednisolone persisted with AZD4017 co-treatment. Four adverse events were reported with AZD4017 and no serious adverse events. Here we show that co-administration of AZD4017 with prednisolone in men is a potential strategy to limit adverse glucocorticoid effects

    Changes in natural killer cells and exhausted memory regulatory T Cells with corticosteroid therapy in acute autoimmune hepatitis

    Autoimmune hepatitis (AIH) is an immune-mediated liver disease currently treated by immunosuppressive medications with significant side effects. Thus, novel mechanistic treatments are greatly needed. We performed prospective deep immunophenotyping of blood immune cells in patients with acute AIH before and after corticosteroid therapy. Blood samples from 26 patients with acute AIH (United Kingdom-AIH Consortium) were phenotyped by flow cytometry at baseline and 4 months after starting corticosteroids. Pretreatment liver tissues were stained for forkhead box P3-positive (FOXP3POS) regulatory T cells (Tregs), clusters of differentiation (CD)56POS natural killer (NK) cells, and chemokine (C-X-C motif) ligand 10. Chemokine secretion by cultured primary hepatocyte and biliary epithelial cells was measured by enzyme-linked immunosorbent assay. Functional coculture assays with stimulated NK cells and Tregs were performed. CD161 ligand, lectin-like transcript-1 expression by intrahepatic immune cells was demonstrated with flow cytometry. Frequencies of NKbright cells declined with therapy (P < 0.001) and correlated with levels of alanine aminotransferase (P = 0.023). The Treg:NKbright ratio was lower pretreatment, and Tregs had an activated memory phenotype with high levels of CD39, cytotoxic T lymphocyte antigen 4, and FOXP3 but also high programmed death ligand 1, indicating exhaustion. Coculture experiments suggested the Tregs could not efficiently suppress interferon-γ secretion by NK cells. Both Tregs and NK cells had high expression of liver infiltration and T helper 17 plasticity-associated marker CD161 (P = 0.04). Pretreatment and CD161pos NK cells expressed high levels of perforin and granzyme B, consistent with an activated effector phenotype (P < 0.05). Lectin-like transcript 1, a ligand for CD161, is expressed on intrahepatic B cells, monocytes, and neutrophils. Conclusion: Activated effector NK cells, which correlate with biochemical measurements of hepatitis, and exhausted memory Tregs are increased in the blood of patients with treatment-naive AIH and decline with corticosteroid therapy. Inadequate regulation of NK cells by exhausted FOXP3pos Tregs may play a role in AIH pathogenesis and contribute to liver injury. (Hepatology Communications 2018;2:421-436)
