Thyroid autoimmunity and thyroid nodule elasticity - relation to thyroid nodule biological nature.

Tyreoidální uzly představují velmi častou patologii. S využitím moderních ultrazvukových přístrojů jejich prevalence dosahuje až 68 %. Nejzásadnějším úkolem je včasná detekce tyreoidálního karcinomu, který sice tvoří asi 5-15 % klinicky signifikantních uzlů, ale jeho incidence stále stoupá. Ani s využitím aspirační biopsie tenkou jehlou není vždy možné rozhodnout o biologické povaze uzlu. Velká část těchto pacientů je pak nucena podstoupit diagnostickou operaci štítné žlázy, která je spojena s riziky pro pacienta a zároveň s finančními náklady pro zdravotní systém. V posledních desetiletích je snaha o zlepšení neinvazivní diagnostiky biologické povahy tyreoidálních uzlů. Elasticita tyreoidálních uzlů stanovená pomocí elastografického vyšetření a přítomnost tyreoidální autoimunity patří mezi zkoumané rizikové parametry. S využitím strain elastografie jsme v naší práci prokázali, že tyreoidální karcinomy mají signifikantně sníženou elasticitu v porovnání s benigními tyreoidálními uzly. Elastografie tyreoidálních uzlů může být využita jako vhodný doplněk konvenčního sonografického vyšetření. Kombinace obou vyšetření zvyšovala negativní prediktivní hodnotu v porovnání s konvenčním přístupem. Výsledky naší práce dále naznačují, že pro potřebu semikvantitativní analýzy elastografie tyreoidálních uzlů...Thyroid nodules represent a very common pathology. Using modern high-resolution ultrasound, nodules could be found in up to 68 % of patients. The most important task is the diagnosis of thyroid cancer which represents only about 5-15 % of nodules, however the incidence is still growing. Even with the use of a fine needle aspiration biopsy, it is not always possible to decide on the biological nature of the nodule. A significant proportion of such patients have to undergo thyroid surgery for diagnostic reason. Thyroid surgery is associated with risks to the patient and financial costs to the health-care system. In recent decades, the efforts to improve non-invasive diagnostics of thyroid nodules have been made. The thyroid elastography and thyroid autoimmunity are among the examined risk parameters. Using real-time strain elastography, thyroid carcinomas elasticity has been significantly reduced compared to benign thyroid nodules in our group of patients. The elastography of thyroid nodules can be used as a suitable complement to conventional sonographic examination. In our work, the combination of both methods (conventional ultrasound and elastography) increased the negative predictive value compared to both methods individually. Mezi lidmi a věcmi: Experimentální výzkum vlivu prostředí na prosociální jednání

V této studii bych rád vysvětlil, jakým způsobem je možné vnímat naše okolní žité prostředí jako místo, kde se odehrává proces myšlení. Svůj argument podpořím příkladem experimentální studie testující jeden z dílčích momentů tohoto problému. Od představy přirozeného světa a jeho možností vlivu skrze afordance a přímou percepci postupně vysvětlím zákonitosti kulturně modifikovaného prostředí jako pro náš život uzpůsobeného seskupení materiálních objektů a k nim se vážících celků kulturních konvencí implikujících určité režimy interakcí mezi lidmi a věcmi. Myšlení jako situovaný proces je možné chápat právě jako jednu z takových. Tento moment se stává jasnější, pokud začneme prozkoumávat hranice a přechody mezi lidmi a věcmi a zdůrazňovat právě ty typy zapojení, při nichž se vnější objekty stávají součástí myslícího celku a podílí se na vyšších kognitivních funkcích – pochodech myšlení tradičně považovaných za specificky lidské kompetence. Jako příklad uvedu výzkum vlivu antropomorfních reprezentací na prosociální jednání. V druhé části práce tak předkládám teoretické pozadí experimentu i jeho návaznost na svět náboženských tradic. Snažím se také nabídnout určitá možná vysvětlení povahy vlivu materiálního objektu na naše myšlení

Phyllotactic model linking nano and macro world

Recently, the arrangement of diffraction primitives according to a phyllotactic model was presented. This arrangement was used to benchmarking purposes of the e-beam writer nano patterning. The phyllotactic arrangement has several interesting properties. One of them is related with the coherence between the nano-or microscopic domain of individual optical primitives and the properties of visually perceived images crated by these structures in the macro domain. This paper presents theoretical analysis of the phyllotactic arrangement in the referred context. Different approaches enabling the creation of diffractive optically variable images are proposed. The practical part of the presented work deals with the nano patterning of such structures using two different types of the e-beam pattern generators. One of them is a system with a variable shaped beam of electrons, while the other one is a system with a Gaussian-shaped beam. Cognitive sociology and the study of human cognition : a critical point

I base my paper on review of a leading texts from the field of cognitive sociology with the attempt to compare the implicit notion of cognition with the conceptions elaborated in the field of cognitive science and allied disciplines (e.g. cognitive psychology, cognitive anthropology, cognitive science of religion, cognitive archeology etc.). I will refer mainly to Cerulo (2002a, 2009), DiMaggio (2002), Vaughan (2002), Wakefield (2002) and Zerubavel (1997, 2002, 2003). The exemplar issues will be presented in the course of four steps. First, I problematize the notion of cognition limited merely to habituated behavioral forms related to specific local situations as presented in study by Vaughan (2002). Second, I discuss the excessive focus on local structures of meaning that are conceived as one of the goals of sociology of mind presented by Zerubavel (1997). I point out the problematic position of sociology of mind, since it draws a substantial focus on intersubjectivity defined in contrast to cognitive individualism and universalism. I present this methodological stance in relation to interpretative program of social sciences. Consequently, I show that this type of cognitive theorizing casts vital doubts on results emerging from the field itself as well as on cross-disciplinary relevancy of that investigation. Hunting and noble festivities on the Kinsky's dominion of Chlumec nad Cidlinou

Autor se ve své práci zabývá tématem lovu, reprezentačních sídel a šlechtických slavností. V úvodu své práce shrnuje důvody, proč si toto téma vybral a jak ho bude chtít ve své práci naplnit. Na začátku vlastního textu popisuje historii rodu Kinských. Tato kapitola nás dovede až ke dvěma hlavním generacím, které jsou stěžejní pro autorovu práci. Po seznámení s jejími představiteli se práce dostává k tématu lovu. Zde jsou popisovány jeho různé způsoby i zvyklosti, nejvýznamnější lovecký řád v Čechách a také lovecká sídla. Autor na grafikách od J. E. Ridingera, které se nacházejí na zámku Karlova Koruna v Chlumci nad Cidlinou, přibližuje různé způsoby lovu. V následující kapitole se autor zabývá šlechtickými sídly, jako nástroji k reprezentaci. Tato funkce šlechtických sídel je přiblížena opět na příkladu zámku Karlova Koruna a na příkladu starého chlumeckého zámku. Jsou zde popsány jednotlivé prostory určené k reprezentaci. Na konci této práce autor popisuje dvě nejvýznamnější návštěvy na chlumeckém panství Kinských, a to návštěvu Karla VI. v roce 1723 a návštěvu Marie Terezie o dvacet let později. V závěru je shrnut způsob práce a poznatky nabyté během jejího psaní.In his essay, the author focuses on the theme of hunting, representative mansions and aristocratic festivities. At the opening, he summarises the reasons that led him to choose this theme and describes how he will be dealing with it in his work. At the beginning of the text of the essay itself he gives an account of the Kinsky dynasty´s history. This chapter will bring us as far as the two main generations which are central to the author´s work. After the introduction of the representatives of these two Kinsky generations, the essay comes to the theme of hunting. Described in this part are various ways of hunting, customs, the most prominent hunting order in Bohemia as well as hunting mansions. The author concentrates on the ways of hunting depicted by the prints of J. E. Ridinger that are to be found in the chateau of Karlova Koruna in Chlumec nad Cidlinou. In the following chapter the author deals with the aristocratic mansions as a means of representation. Representative purposes of aristocratic mansions are, again, illustrated by the example of Karlova Koruna and, also in this chapter, by the example of the old chateau of Chlumec. Described here are the particular places designated for representative purposes at the chateau of Karlova Koruna. At the end of this work, the author gives an account of two most important visits of the Kinskys at the manor of Chlumec, namely the visit of Charles VI in 1723 and that of Maria Theresa twenty years later.
