    Metaplastic Carcinoma of the Left Breast with Extensive Chondroid Differentiation

    Metaplastic breast carcinoma is very rare neoplasm which contains mixture of carcinomatous (epithelial) and sarcomatous (mesenchymal) elements in variable proportion. Metaplastic carcinoma with chondroid differentiation is even rarer. We report a case of metaplastic carcinoma with extensive chondroid differentiation as there is paucity of information regarding pathological features and clinical outcomes for these rare tumors. Tumor had characteristic definite areas of classic infiltrating duct carcinoma with abundant chondromyxoid matrix, focal areas of chondrosarcoma and cartilagenous metaplasia. Tumour cells were immunoreactive for S-100, ER, and PR. When pathologist encounter breast tumor with chondroid differentiation, careful gross sampling, histopathology and immunoreactivity for mesenchymal and epithelial component are most useful to differentiate metaplastic carcinoma from malignant phylloides tumors and malignant adenomyoepithelioma

    A CLINICAL EVALUATION OF PANCHAGAVYA GHRITA IN BAL APASMARA (EPILEPSY IN PAEDIATRIC AGE GROUP)

    Background: Epilepsy is defined as the primary disorder of the brain due to an abnormal and excessive neuronal activity involving cerebral grey matter, resulting in a seizure.Objective: The objective was to do a clinical evaluation of Panchagavya ghrita in Bal apasmara (epilepsy in paediatric age group).Materials and Methods: It is a randomized open interventional parallel efficacy drug trial. A total number of 60 patients were selected from the age group of 4 years to 16 years of age. All the patients were recently diagnosed with Idiopathic type of epilepsy and were not on any kind of medication, before the start of the study. The selected patients were randomly divided into two groups – The patients in Group I (n = 30) were given Panchagavya ghrita and patients in Group II (n = 30) were kept on Tab. Tegretol for the study. Both the groups were studied for 12 months to see the efficacy of the drug.Results: Group I showed Statistically significant results in Frequency of convulsions, Duration of convulsion and Smritinasha (Amnesia). Where as in Group II highly significant results were observed in Frequency of convulsions and duration of convulsion; but not   significant results were observed in Smritinasha (Amnesia).Conclusion: Panchagavya ghrita has shown promising results in the management of Bal - apasmara (Epilepsy in paediatric age group) without any complications and side-effects

    Association of adverse pregnancy outcome and domestic/intimate partner violence

    Background: Domestic/intimate partner violence is not a mere household issue. It is a global phenomenon that occurs in all the countries and cuts across all strata, races and countries. Apart from being an issue of human/individual rights, it also has major health consequences. The main aim of this study is to stress the need of screening and detecting violence in antenatal women.Methods: This was a questionnaire based observational cross sectional study done at King Edward Memorial Hospital, Mumbai, India over a span of one year. A total of 200 antenatal/postnatal patients seeking healthcare were enrolled after an informed consent with approval from the Institutional Ethics Committee. Percentages of various parameters were analysed. Test of significance was Chi square test and Odd’s ratio (p <0.05).Results: Prevalence of domestic/intimate partner violence in our study was 12.5% and 7% in the group with normal pregnancy outcome and 18% in group with adverse pregnancy outcome. It was found that violence was more prevalent in age group of 21 to 30 years, love marriages, nuclear families, lower educational level of partner, addiction in partner, unplanned pregnancies, lower economic conditions and families supporting dowry and gender bias and allowing freedom of choice and contraception.Conclusions: Need for routine screening for violence in women of reproductive age group with vulnerable subset of pregnant women. Creating awareness/sensitivity amongst healthcare professionals and to train them to identify and help these women

    A COMPARATIVE STUDY OF BHUJANGASANA WITH KATIBASTI OVER ONLY KATIBASTI IN GRUDHRASI (SCIATICA)

    Background: Grudhrasi (Sciatica) is one of the eighty Vatavyadhi mentioned in Ayurvedic Samhitas, which is caused by aggravated Vatadosha. This is characterized by Ruja (pain) in the waist, back, thigh, knee and calf regions along the course of Sciatic nerve. There are many treatment modalities available in Ayurveda and Modern medical science. They have some or the other shortcomings and drawbacks.Objectives: To evaluate the efficacy of Bhujangasana with Katibasti over only Katibasti.Materials and Methods: It is a randomized open interventional parallel efficacy drug trial. A total number of  40 patients were selected from OPD. All the patients were having classical presentation of Grudhrasi and SLR test positive between 300 and 700. The selected were randomly divided in to two groups, namely, Group A (study group n = 20) and Group B (control group n = 20). Group A was given Bhujangasana with Katibasti and Group B was given Katibasti only. The data were collected and observations were made before the treatment, on 8th day, 15th day and on 22nd day of the treatment. The data obtained from the result were subjected for statistical analysis and conclusions were drawn.Results: In Group A, 85% patients were totally relieved of pain and 15% patients were reported to have moderate pain. In Group B, 55% patients reported total relief from pain and 25% had mild pain. In 90% cases SLR became negative in Group A, whereas in Group B 75% patients reported negative result in SLR test.Conclusion: Analysis of overall effect of treatment in the present study reveals that Bhujangasana with Katibasti was statistically significant compared to that of Katibasti

    Data Compression Algorithm for Wireless Sensor Network

    In use of wireless sensor network technology for environmental monitoring, the two main fundamental activities of wireless sensor network is data acquisition and data transmission. However, transmitting or receiving data are power consuming task. Wireless sensor network consists of a set of sensor nodes. The processing capacity varies with each node. Sensors in it have storage capacity which is limited. The main challenge today in this field is to improvise the power and energy management of sensor network. Different techniques have been introduced by various researchers. In order to reduce power consumption during transmission, we introduce data compression by processing information locally.             This paper presents the simulation results for RLE method for data compression. The MATLAB GUI model is employed to simulate the RLE method. The simulation result shows that the RLE method can effectively compress the data with minimum power consumption

    SOLUBILITY AND DISSOLUTION ENHANCEMENT OF PIOGLITAZONE USING SOLID DISPERSION TECHNIQUE

    Objective: To design the study to improve the solubility and hence enhance the dissolution of hydrophobic drug Pioglitazone in order to increase its bioavailability.Methods: Solid dispersion of Pioglitazone using carriers Poloxomer 188 and HPβCD was formulated in different ratios by microwave induced fusion method. In particular, the Microwave technology has been considered in order to prepare an enhanced release dosage form for poorly water soluble drug Pioglitazone. Statistical Analysis: Their physicochemical characteristics and solubility were compared to the corresponding dispersions and marketed drug. Drug and polymer were further characterized by FTIR.Results: The results of FTIR revealed that no chemical interaction between the drug and the polymer exist.Conclusion: All the formulations showed a marked increase in drug release with the increase in the concentration of Poloxomer 188 and HPβCD.Â

    Prediction of Fiber Orientation in Compression Molded Parts of Short-Fiber-Reinforced Thermoplastics

    Short Fiber molded thermoplastics like poly ether ketone ketone (PEKK) have the potential to be used to build reinforcement structures for next generation air crafts. Because of the anisotropic properties of these fibers, it is necessary to know the orientation of fibers in molded parts to determine the strength of parts. The goal of the project is to prepare samples that will be used to determine the spatial distribution of fibers as a function of the parts thickness and for comparison with computer generated models to test for accuracy. Using a full factorial method the effects of four key control parameters, namely compression load, material dimension, incubation time and platen temperature, will be observed on a predetermined mold shape to determine an optimal manufacturing strategy to ensure larger scale production with minimum variability between the parts. Primary investigations have revealed a surprising adverse effect of incubation temperature on the filling of the part. The effect of other parameters has been as expected; increasing pressure and smaller material size promote better parts. However a more detailed study is needed to develop a sound understanding of the role of each parameter in the manufacturing process. The completed work hopes to address these problems and ensure that thermoplastics become a viable alternative for structural reinforcements
