    Composição racial e a avaliação da reação intradérmica de Montenegro em portadores da leishmaniose cutâneo-mucosa

    Em um estudo retrospectivo, indivíduos com a forma mucosa da leishmaniose foram pareados (sexo, idade e ocupação) a outros pacientes com a forma cutânea em atividade, não demonstrando-se diferença significante da freqüência nos grupos raciais. Do mesmo modo, o teste intradérmico de Montenegro não se mostrou diferente nos diversos grupos raciais, entre os 96 pares de pacientes estudados. Uma maior enduração foi observada em pacientes com o acometimento mucoso quando comparado aos cutâneos. _______________________________________________________________________________________ ABSTRACTIn a retrospective study patients with mucosal leishmaniasis were paired (as regards age sex and occupation) with patients with active cutaneous disease. No signijicant racial dijference could be detected. In the same way the Montenegro cutaneous reaction was not different in 96pairs ofpatients studied as regards different racial groupings.     Immune and inflammatory responses to Leishmania amazonensis isolated from different clinical forms of human leishmaniasis in CBA mice

    Leishmania amazonensis causes different diseases depending on the host and parasitic virulence factors. In this study, CBA mice were infected with L. amazonensis isolates from patients with localized (Ba125), diffuse cutaneous (Ba276) or visceral leishmaniasis (Ba109). Mice infected with Ba125 and Ba276 progressed rapidly and lesions displayed an infiltrate rich in parasitized macrophages and were necrotic and ulcerated. Ba109 induced smaller lesions and a mixed inflammatory infiltrate without necrosis or ulceration. Ba109 induced an insidious disease with lower parasite load in CBA mice, similar to human disease. Levels of IFN-γ, IL-4 and IL-10 did not differ among the groups. Because all groups were unable to control the infection, expression of IL-4 associated with low production of IFN-γ in the early phase of infection may account for susceptibility, but others factors may contribute to the differences observed in inflammatory responses and infection progression. Evaluation of some parasitic virulence factors revealed that Ba276 exhibits higher ecto-ADPase and 5'-nucleotidase activities compared to the Ba109 and Ba125 strains. Both Ba276 and Ba125 had higher arginase activity in comparison to Ba109. Finally, these data suggest that the differences in enzyme activities among parasites can account for differences in host inflammatory responses and infection progression

Apresenta-se estudo prospectivo de 11 pacientes com LV, com o intuito de verificar as repercussões clínico-laboratoriais da função renal na doença. Realizou-se análises laboratoriais das amostras de sangue e urina, colhidas logo após confirmação diagnóstica, através do encontro de leishmanias no aspirado de médula óssea. Dois (18%) pacientes apresentaram complicações associadas a LV. Cinco (45,4%) apresentaram hematúria macroscópica e em um caso (No.9) manifestações clínicas compatíveis com síndrome nefrítica aguda. Os resultados dos exames de urina mostraram: proteinúria em 10 (90,9%) pacientes, hematúria 7 (63,6%) e leucocitúria em 6 (54,5%) casos. Nove (81,8%) pacientes apresentaram níveis elevados de microalbuminúria caracterizando lesão glomerular. A presença de tubulopatia proximal medida através da proteína ligadora de retinol, foi observada em 5 (45,4%) casos. Concluiu-se que o envolvimento renal se fez presente na maioria dos pacientes, contribuindo para a gravidade da doença.Evaluation of the renal involvement of visceral leishmaniasis (kala-azar) aiming at verifying clinical and laboratorial repercussions of the renal implication on VL. Laboratory analysis was performed of blood and urine samples collected immediately after confirming diagnosis through the finding of Leishmania in bone marrow. Two (18.1%) patient presented complications associated with kala-azar. Five patients (45.4%) presented macroscopic hematuria and one case (No 9) clinical manifestations compatible with an acute nephritis syndrome. Laboratorial data of urine exam showed: proteinuria in 10 (90.9%) patients, hematuria in 7 (63.6%) and leukocyturia in 6 (54.5%) cases. Nine (81.8%) patients presented high levels of microalbuminuria, characterizing glomerular lesion. The presence of proximal tubulopathy, measured through retinol binding protein observed in 5 (45.4%) cases. Conclusion: renal involvement was manifested in most subjects, contributing to the severity of the disease

Salvador, Bahia, BrasilPeripheral blood CD16 (Fc receptor for immunoglobulin G III)-positive monocytes have been shown to expand in different pathological conditions, such as cancer, asthma, sepsis, human immunodeficiency virus infection, and AIDS progression, but data in leishmaniasis are lacking. We found that cutaneous leishmaniasis patients (n = 15) displayed a significant increase in the percentage (3.5 vs. 10.1) as well as mean fluorescent intensity (13.5 vs. 29.2) of ex vivo CD16 expression in monocytes as compared with healthy controls. We observed a significant positive correlation between the percentage of ex vivo CD16+ monocytes and lesion size (P = 0.0052, r = 0.75) or active transforming growth factor-beta plasma levels (P = 0.0017, r = 0.78). In addition, two patients with nonhealing lesions during a 3-year follow-up had high (9.1-19.4%) CD16 levels at diagnosis. Our data suggest a deleterious role for CD16 in human leishmaniasis, as well as its possible use as a marker for disease severity and/or adverse disease outcome

    Diabetes melutus associated with pentamidine isethionate in diffuse cutaneous leishmaniasis

    The authors report a case of a male patient from Bacabal, MA with diffuse cutaneous leishmaniasis (DCL), for at least nine years, with 168 lesions on his body. These were tumour-like nodules with some ulceratmi. He usedpentavalent antimonial (glucantime®) and an association of gamma interferon plus glucantime with improvement of the lesions but relapsed later. Recently, pentamidine isethionate (pentacarinat®) was given a dosage of 4mg/kg/weight/day on alternate days for 20 applications. After 3 months a similar course of 10 application was given 2 times. Later he developed diabetic signs with weight loss of 10kg, polydypsia, polyuria and xerostomia. The lower limbs lesions showed signs of activity. Blood glucose levels normalised and remain like this at moment. Attention is drawn to the fact that pentamidine isethionate should be used as a therapy option with care, obeyng rigorous laboratory controls including a glucose tolerance test

    RESUMEN: El cultivo "in vitro" de Leishmania braziliensis braziliensis constituye un método útil en el trabajo de campo, para el aislamiento primario de ésta subes-pécie de Leishmania. Cultivos en dos medios difásicos de agar sangre (DAB y EVANS) y dos medios líquidos (SCHNEIDER'S y AR-103) realizados en pacientes con lesiones cutáneas de Leishmaniasis Tegumentaria Americana (LTA) demostraron: 1) Similar sensibilidad de los medios DAB y Schneider's cuando utilizamos el procedimiento de aspiración de las muestras con aguja. 2) Rendimiento sensible y reproducible, con el medio DAB, cuando comparado, en repetidas ocasiones, con el medio EVANS. 3) Incremento significativo en el aislamiento primario de Leishmania braziliensis brazilensis mediante la ejecución, en la misma lesión de cada paciente, de tres aspiraciones consecutivas en sitios diferentes de la úlcera activa (50% de positividad, con el medio DAB). _______________________________________________________________________________ SUMMARYIn vitro cultivation of Lbb is a useful method in the field for primary isolation of this leismanial subspecies.
