    The new automated daily mortality surveillance system

    The experience reported in an earlier Eurosurveillance issue on a fast method to evaluate the impact of the 2003 heatwave on mortality in Portugal, generated a daily mortality surveillance system (VDM) that has been operating ever since jointly with the Portuguese Heat Health Watch Warning System. This work describes the VDM system and how it evolved to become an automated system operating year-round, and shows briefly its potential using mortality data from January 2006 to June 2009 collected by the system itself. The new system has important advantages such as: rapid information acquisition, completeness (the entire population is included), lightness (very little information is exchanged, date of death, age, sex, place of death registration). It allows rapid detection of impacts (within five days) and allows a quick preliminary quantification of impacts that usually took several years to be done. These characteristics make this system a powerful tool for public health action. The VDM system also represents an example of inter-institutional cooperation, bringing together organisations from two different ministries, Health and Justice, aiming at improving knowledge about the mortality in the population

    Bayesian assessment of an existing bridge - a case study

    This article presents a case study involving the assessment of an existing bridge, starting with simple methods and ending with a probabilistic analysis, the latter emphasising Bayesian methods. When assessing an existing bridge, it is common practice to collect information from the bridge in the form of samples. These samples are in general of small size, raising the question of how the corresponding statistical uncertainty can be taken into account on reliability estimates. The case study illustrates how Bayesian methods are especially suitable to deal with that source of uncertainty. Another strong point of the Bayesian methods is their ability to combine the information contained in the samples collected from the bridge with prior information, if any. This aspect will also be illustrated through the case study

    Diplopia, Convergent Strabismus, and Eye Abduction Palsy in a 12-Year-Old Boy with Autoimmune Thyroiditis

    Pseudotumor cerebri (PTC) is defined by clinical criteria of increased intracranial pressure, elevated intracranial pressure with normal cerebrospinal fluid (CSF) composition, and exclusion of other causes such tumors, vascular abnormalities, or infections. The association of PTC with levothyroxine (LT4) has been reported. A 12-year-old boy has been followed up for autoimmune thyroiditis under LT4. Family history was irrelevant for endocrine or autoimmune diseases. A TSH level of 4.43 μUI/mL (0.39-3.10) motivated a LT4 adjustment from 75 to 88 μg/day. Five weeks later, he developed horizontal diplopia, convergent strabismus with left eye abduction palsy, and papilledema. Laboratorial evaluation revealed elevated free thyroxine level (1.05 ng/dL [0.65-1.01]) and low TSH, without other alterations. Lumbar puncture was performed and CSF opening pressure was 24 cm H2O with normal composition. Blood and CSF cultures were sterile. Brain MRI was normal. LT4 was temporarily discontinued and progressive improvement was observed, with a normal fundoscopy at day 10 and reversion of diplopia one month later. LT4 was restarted at lower dose and gradually titrated. The boy is currently asymptomatic. This case discloses the potential role of LT4 in inducing PTC. Despite its rarity and unclear association, PTC must be seen as a potential complication of LT4, after excluding all other intracranial hypertension causes.info:eu-repo/semantics/publishedVersio

    Extending the deterministic Riemann-Liouville and Caputo operators to the random framework: A mean square approach with applications to solve random fractional differential equations

    [EN] This paper extends both the deterministic fractional Riemann¿Liouville integral and the Caputo fractional derivative to the random framework using the mean square random calculus. Characterizations and sufficient conditions to guarantee the existence of both fractional random operators are given. Assuming mild conditions on the random input parameters (initial condition, forcing term and diffusion coefficient), the solution of the general random fractional linear differential equation, whose fractional order of the derivative is ¿ ¿ [0, 1], is constructed. The approach is based on a mean square chain rule, recently established, together with the random Fröbenius method. Closed formulae to construct reliable approximations for the mean and the covariance of the solution stochastic process are also given.
