Wytwarzanie wielofunkcyjnych blach techniką ścinania
Incremental shear has been used to vary blank properties along the sheet plane. The feasibility of the method has been investigated to manufacture so called Tailor Sheared Blanks featuring property distribution of the blank without thickness variation. Mechanical properties resulting from evolution of coarse grain microstructure towards ultra fine grain one can be achieved. Biliary complications after orthotopic liver transplantation in adults
Przeszczepianie wątroby stało się obecnie leczeniem z wyboru w przypadku schyłkowej niewydolności
wątroby.
Wraz z ciągłym rozwojem transplantologii i immunologii wyniki przeszczepiania wątroby ulegają stopniowej
poprawie. Obecnie średnie 5-letnie przeżycie po ortotopowym przeszczepieniu wątroby (OLT) biorców
kształtuje się na poziomie 72–77% w zależności od stopnia niewydolności wątroby.
Z procedurą transplantacji wątroby nierozerwalnie wiąże się obecność powikłań, które w 80% występują
w ciągu pierwszych 6 miesięcy po OLT. Do najczęściej spotykanych należą powikłania dotyczące dróg żółciowych.
Najważniejsze i najszerzej opisywane to: wycieki żółci, zwężenia — zarówno w miejscu, jak i poza
miejscem zespolenia dróg żółciowych, złogi w drogach żółciowych, dysfunkcja zwieracza Oddiego oraz ucisk
na drogi żółciowe. Są one groźne, gdyż niejednokrotnie prowadzą do utraty przeszczepu lub zgonu chorego.
Obecnie częstość powikłań żółciowych kształtuje się na poziomie 6–35% wszystkich transplantacji, najczęściej
oscylując wokół 20% przy utrzymującej się od wielu lat stałej tendencji spadkowej. Do istotnych
czynników prognozujących wystąpienie powikłań żółciowych po OLT należą: przedoperacyjne stężenie
bilirubiny w surowicy krwi oraz wskaźnik INR biorcy, zastosowanie drenażu Kehra, liczba przetoczonych
podczas operacji jednostek osocza świeżo mrożonego, a także rodzaj graftu wątrobowego.
Zmienia się sposób postępowania w przypadku podejrzenia powikłań żółciowych. Referencyjną metodą
diagnostyczną stał się magnetyczny rezonans dróg żółciowych (MRCP). W przypadku potwierdzenia rozpoznania
powikłań żółciowych leczenie operacyjne w większości zostało zastąpione technikami endoskopowymi
i przezskórnymi. Przy nieskuteczności powyższego postępowania konieczna staje się retransplantacja
wątroby.
Z powodu ciągle niewystarczającej liczby zmarłych dawców wciąż poszukuje się nowszych metod pozyskiwania
narządów do przeszczepienia. Wprowadzono metodę pozyskiwania graftów z częściowej hepatektomii
od dawców żywych oraz podziału na mniejsze części wątroby pochodzącej od zmarłego dawcy.
Cele niniejszego artykułu to analiza i podsumowanie doniesień dotyczących powikłań żółciowych po
OLT u dorosłych. Problem ten wydaje się szczególnie istotny z powodu coraz większej liczby transplantacji
wątroby w Polsce.Liver transplantation has become at present the treatment of choice in cases of end-stage liver disease.
Along with the significant development of transplantology and immunology, the results of liver transplantation
have been gradually improving. Currently, the 5-year survival rate in recipients following orthotopic
liver transplantation (OLT) is at a level of 72–77%, depending on the stage of liver insufficiency.
The presence of complications is inseparably connected with the procedure of liver transplantation, and
occur in 80% of cases during the first six months after OLT. One of the most common are complications
concerning bile ducts. The most important and widely described are bile leakages, anastomotic and nonmagnetyczanastomotic
strictures, bile concrements, sphincter of Oddi dysfunction and compression of the biliary
tract. Such complications are dangerous as many times they lead to the loss of the graft or even death.
Despite permanent improvements in the surgical technique of performing bile ducts anastomoses and
the growing experience of transplantologists, biliary anastomosis is often called the “Achilles heel” of
liver transplantation.
Nowadays, the prevalence of biliary complications is at a level of 6–35% of all liver transplantations, and
most frequently oscillates around 20%, a figure which has been showing a regular downward trend for
many years. The most commonly-listed risk factors of biliary complications after OLT are: preoperative
serum bilirubin level and INR in recipient, T-tube drainage, the number of fresh frozen plasmas used
during the operation and the type of liver graft.
Management options, in cases of suspicion of biliary complications, have also changed with Magnetic
Resonance Cholangiopancreatography (MRCP) having become a “gold standard” in diagnostics. In the
cases of confirmation of the diagnosis of biliary complications, surgical techniques have mainly been
replaced by endoscopic or percutaneous techniques. In cases of the ineffectiveness of the above-mentioned
treatment, liver re-transplantation becomes necessary.
Because of the continuously insufficient number of deceased donors, more and more new methods of
expanding the number of grafts for transplantation are being discovered. The methods of gaining the
grafts from partial hepatectomy from living donors and division of the liver into smaller parts from
a deceased donor have also been introduced.
The aim of this article is to be a review and analysis of reports concerning biliary complications after
orthotopic liver transplantation in adults. This problem has becomes especially important due to more
and more liver transplantations being performed in Poland
The Use of Three-dimensional Analysis of GPR Data in Evaluation of Operational Safety of Airfield Pavements
AbstractDiagnosis of airfield pavements is an important component of airport managing. Proper diagnosis makes possible to take optimal decisions in terms of ongoing maintenance and repairs, which in the case of airfields is of particular importance, among others, in the context of seasonal changes in the intensity of air traffic. The completeness and accuracy of gathered data is important in the diagnostic activities, what means that the entire airfield road network should be measured and identification of all relevant pavement construction parameters at regular intervals should be done. These capabilities give the Ground Penetrating Radar technique (GPR), which enables the evaluation of the pavement structure in different ways and outlining different aspects of construction.GPR application as a tool supporting the process of state assessment gives a wider and better understanding of the potential damage of pavement. Properly prepared methodology of measurement, configuration and selection of the measurement system creates the possibility of observation of the investigated object, not only in a single plane of a typical radar profiling, but also in the three-dimensional image. Spatial representation obtained on the basis of synchronized profiling allows precise localization of interlayer boundaries in longitudinal and transverse directions. An important advantage of three-dimensional analysis is the ability of imaging data by the use of horizontal cuts (slices). This makes it possible to identify plane direction and depth of the cracks and crevices of concrete slabs covered with layers of asphalt or concrete. An additional element of the GPR data analysis can be quantitative assessment of dowels in concrete slabs, reinforcement of prefabricated elements supporting pavement structure, as well as location of pipes, cables, tie bars and other.The paper presents the series of examples illustrating the use of GPR technique as supporting in the process airfield pavement assessment
First records of Kolibacia squamulata (Gebler, 1830), Cucujus haematodes Erichson, 1845, and Clerus dealbatus (Kraatz, 1879) (Coleoptera: Trogossitidae, Cucujidae, Cleridae) from Kazakhstan
Three saproxylic beetle species: Kolibacia squamulata (Gebler, 1830) (Trogossitidae), Cucujus haematodes Erichson, 1845 (Cucujidae), and Clerus dealbatus (Kraatz, 1879) (Cleridae) are recorded from Kazakhstan for the first time. Notes on their biology, habitat preferences and distribution are also provided. All species were found in the eastern part of the country, in the vicinity of the villages of Putintsevo, Kol’bay and Koktal in 2017
Analiza patogenów i ich lekowrażliwości u pacjentów z zespołem stopy cukrzycowej leczonych chirurgicznie
Wstęp. Cukrzyca jako jedna z najczęstszych chorób cywilizacyjnych wywiera znaczący wpływ nie tylko na poszczególne jednostki, lecz także na całe społeczeństwo. Nieprawidłowy metabolizm cukrów może prowadzić do szeregu powikłań dotykających cały organizm. Z chirurgicznego punktu widzenia leczenie powikłań cukrzycy, poza optymalizacją leczenia hiperglikemii, sprowadza się do leczenia zespołu stopy cukrzycowej. Zespół ten rozwija się na podłożu mikroneuroangiopatii i doprowadza do powstania zmian martwiczych będących doskonałym miejscem rozwoju dla szeregu bakterii. Odpowiednia antybiotykoterapia stanowi jeden z ważniejszych elementów całego postępowania interdyscyplinarnego w leczeniu zespołu stopy cukrzycowej.
Materiał i metody. W niniejszej pracy, w celu określenia optymalnej antybiotykoterapii, oceniano wyniki wykonanych posiewów pobranych ze zmian ropno-martwiczych (ocena dotyczy tylko pierwszego wyhodowanego izolatu od danego pacjenta).
Wyniki. U wszystkich 61 chorych stwierdzano naciek zapalny skóry i tkanek miękkich, któremu w większości towarzyszyły rany stóp/podudzi gojące się po drobnych interwencjach chirurgicznych. U prawie połowy chorych wyhodowano jeden patogen, a u pojedynczego chorego — aż osiem patogenów chorobotwórczych. Najczęściej identyfikowanymi były: Staphylococcus aureus, Enterococcus faecalis, Staphylococcus koagulazonegatywny (inny niż S. Cohni, S. epidermidis, S. werneri, S. haemoliticus).
Wnioski. Zakażenie tkanek miękkich w zespole stopy cukrzycowej jest zwykle powodowane przez kilka rodzajów bakterii, z których najczęściej hodowany to Staphylococcus aureus. Na podstawie uzyskanych wyników wydaje się, że optymalnym początkiem antybiotykoterapii byłoby zastosowanie amoksycyliny z kwasem klawulanowym
Immunohistochemical markers of cancerogenesis in the lung.
Lung cancer is the leading cause of cancer deaths for people of both sexes worldwide. Early diagnosis of precancer lesions may be of crucial significance to lowering lung cancer mortality. The World Health Organization has defined three preneoplastic lesions of the bronchial epithelium: squamous dysplasia and carcinoma in situ, atypical adenomatous hyperplasia and diffuse idiopathic pulmonary neuroendocrine cell hyperplasia. These lesions are believed to progress to squamous cell carcinoma, adenocarcinoma and carcinoid tumors, respectively. Apart from WHO classification, two other lesions such as bronchiolization and bronchiolar columnar cell dysplasia (BCCD) can be observed and thought to be preneoplastic lesions leading to adenocarcinoma. In this review we summarize the data of morphological and cell cycle related proteins changes in both central and peripheral compartments of lung. Many molecular changes, which accompany the multistep process of the development of invasive types of cancer, may be observed thanks to the application of immunohistochemical markers. A deeper knowledge of molecular and genetic changes accompanying pre-cancer states may show new directions of early diagnostics of cancer development

Mixture of Growing Medium with IAA, Biochemical and Chemical Properties in Euphorbia Pulcherrima Cultivation
In the experiment there were used rooting of cuttings of poinsettia Euphorbia pulcherrima 'Prestige Early Red'. This cultivation of plants was carried out in cell trays with the use of commercial peat growing medium ‘Klasmann Steck Medium’, containing four concentrations of indole-3-acetic acid (IAA), (natural concentration and 200, 300 and 400 μg kg-1 additionally). Data indicates that the mixture of substrate with IAA had an impact on chemical compounds and enzymes activity before and after rooting of cuttings of poinsettia. These investigations revealed that higher activity of xanthine oxidase and peroxidase were observed before cultivation of poinsettia. However, different results related to phenol oxidase, urease and nitrate reductase activity were noted
Impact of routine invasive strategy on outcomes in patients with non-ST-segment elevation myocardial infarction during 2005–2014: A report from the Polish Registry of Acute Coronary Syndromes (PL-ACS)
Background: Non-ST-segment elevation myocardial infarction (NSTEMI) has become the most frequently encountered type of myocardial infarction. The patient clinical profile and management has evolved over the past decade. As there is still a scarcity of data on the latest trends in NSTEMI, changes herein were observed and assessed in the treatment and outcomes in Poland between 2005 and 2014.Methods: A total of 197,192 patients with NSTEMI who enrolled in the Polish Registry of Acute Coronary Syndromes (PL-ACS) between 2005 and 2014 were analyzed. In-hospital and 12-month mortality were assessed.Results: Coronary angiography use increased from 35.8% in 2005–2007 to 90.7% in 2012–2014 (p < 0.05), whereas percutaneous coronary intervention increased from 25.7% in 2005–2007 to 63.6% in 2012–2014 (p < 0.05). There was a 50% reduction in in-hospital mortality (from 5.6% in 2005–2007 to 2.8% in 2012–2014; p < 0.05) and a 30% reduction in 1-year mortality (from 19.4% in 2005–2007 to 13.7% in 2012–2014; p < 0.05). A multivariate analysis confirmed an immense impact of invasive strategy on patient prognosis during in-hospital observation with an odds ratio (OR) of 0.31 (95% confidence interval [CI] 0.29–0.33; p < 0.05) as well as during the 12-month observation with an OR of 0.51 (95% CI 0.49–0.52; p < 0.05).Conclusions: Over the past 10 years, an important advance in the management of NSTEMI has taken place in Poland. Routine invasive strategy resulted in a significant decrease in mortality rates in all groups of NSTEMI patients
Wartość oznaczania stężenia tyreoglobuliny w aspiratach z węzłów chłonnych szyi u chorych ze zróżnicowanym rakiem tarczycy
Introduction: Recurrent differentiated thyroid cancer generally
occurs first in the neck. Ultrasound is sensitive in
detecting enlarged cervical lymph nodes but is not specific
enough. Ultrasound-guided fine-needle biopsy increases
the specificity but still may fail to detect a recurrence of the
disease in the cystic metastatic lymph nodes.
The aim of the study was to estimate the value of Tg concentration
in the needle washout after fine-needle aspiration
of suspicious lymph nodes.
Material and methods: The 105 patients studied had presented
one or more enlarged suspicious cervical lymph nodes.
All had undergone total thyroidectomy and 131I ablative
therapy. Serum thyroglobulin (Tg) concentration was
within the 0.15-711.5 ng/ml range (mean 22.24 ng/ml) and
Tg recovery range 94-100%. The positive Tg washout concentration
cut-off value was established as equal to the mean
plus two standard deviations of the Tg washout concentration
of patients with negative cytology.
Results: Lymph node involvement was diagnosed by cytology
in 15 patients and in 28 lymph nodes. Positive Tg washout
concentration was found in 22 patients and in 48
lymph nodes. All the lymph nodes which turned out to have
positive cytology had a positive Tg washout concentration.
All lymph nodes with positive cytology were positive in
pathology. Seven patients and 20 lymph nodes with negative cytology were positive in the Tg washout concentration
test. All but one patients and all but two lymph nodes
with a positive Tg washout concentration had positive pathology.
Conclusions: 1. Ultrasound-guided fine-needle biopsy is
not sensitive enough to detect all metastatic lymph nodes.
2. The Tg washout concentration test is 100% sensitive in
the detection of metastatic lymph nodes. 3. Cytology in ultrasound-
guided fine-needle biopsy is 100% specific. 4. The
Tg washout concentration test carries a risk of false-positive
results. 5. Both methods should be used for early detection
of metastatic lymph nodes in patients with differentiated
thyroid cancer.Wstęp: Przerzuty zróżnicowanego raka tarczycy (DTC,
differentiated thyroid cancer) występują najczęściej w węzłach chłonnych szyi. Badanie ultrasonograficzne (USG)
jest czułą metodą w wykrywaniu powiększonych węzłów
chłonnych szyi, ale nie jest wystarczająco swoiste. Biopsja
aspiracyjna cienkoigłowa celowana (BACC) ma większą
swoistość, ale nie jest wystarczająco czuła.
Celem pracy była ocena wartości diagnostycznej stężenia
tyreoglobuliny (Tg) w aspiratach z węzłów chłonnych szyi
u chorych z DTC.
Materiał i metoda: Badaniem objęto 105 chorych po całkowitej
tyreoidektomii i po leczeniu ablacyjnym 131I. Wszyscy
chorzy wykazywali brak jodochwytności w obrębie szyi, prawidłową scyntygrafię całego ciała [u większości - w warunkach
endogennej stymulacji hormonem tyreotropowym
(TSH, thyroid-stimulating hormone)] oraz powiększone w badaniu
USG węzły chłonne szyi. U wszystkich pacjentów objętych badaniem oznaczono stężenie i odzysk Tg w surowicy,
wykonano BACC (2-3 aspiraty z każdego węzła chłonnego
szyi), a następnie przepłukano igłę 125 µl 0,9-procentowym
NaCl i oznaczono stężenie Tg w popłuczynach,
używając zestawu do oznaczania Tg w surowicy. Za podwyższone stężenie Tg w popłuczynach przyjęto wartość
równą średniej plus 2 odchylenia standardowe dla chorych
z ujemnym wynikiem BACC (bez obecności komórek nowotworowych).
Wszystkich chorych z dodatnim wynikiem
BACC (z obecnością komórek nowotworowych) lub podwyższonym stężeniem Tg w popłuczynach operowano.
Wyniki: Dodatni wynik BACC stwierdzono u 15 chorych
(w 28 węzłach chłonnych), a podwyższone stężenie Tg
w popłuczynach u 22 chorych (w 48 węzłach chłonnych). We wszystkich węzłach chłonnych z dodatnim wynikiem
BACC stwierdzono podwyższone stężenie Tg w popłuczynach.
U 7 chorych (w 20 węzłach chłonnych) z ujemnym
wynikiem BACC wykazano podwyższone stężenie Tg
w popłuczynach. U wszystkich chorych z wyjątkiem 1 osoby
(we wszystkich węzłach chłonnych z wyjątkiem 2 węzłów)
z podwyższonym stężeniem Tg w popłuczynach stwierdzono
wynik histopatologiczny wskazujący na przerzut DTC.
Wnioski: 1. BACC nie jest wystarczająco czuła do wykrywania
obecności przerzutów DTC do węzłów chłonnych
szyi. 2. Oznaczanie stężenia Tg w popłuczynach igły biopsyjnej
jest metodą 100-procentowo czułą. 3. BACC charakteryzuje
się 100-procentową swoistością w wykrywaniu
obecności przerzutów DTC do węzłów chłonnych szyi.
4. Oznaczanie stężenia Tg w popłuczynach igły biopsyjnej
może być fałszywie dodatnie. 5. Obie metody powinny być
stosowane do wczesnego wykrywania przerzutów DTC do
węzłów chłonnych szyi
