    A microcrystalline test is a precipitation reaction between a drug and a reagent, forming an insoluble drug-reagent complex that is unique to that specific test. These tests are quick, requiring minimal sample preparation and can be non-destructive. Therefore, they can be used as preliminary and confirmatory tests with expertise. Microcrystalline tests are one of the oldest analytical chemistry practices and their use for classic drugs such as cocaine, heroin and amphetamines is well-documented. However, there is very limited research on microcrystalline tests for the novel compounds encountered by law enforcement today.     Cost-effectiveness analysis of the management of distal ureteral stones in children

    Objective To determine the most cost-effective approach to the management of distal ureteral stones in children given the potential for recurrent renal colic during a trial of passage versus potential stent discomfort and complications of ureteroscopy. Methods We developed a decision tree to project costs and clinical outcomes associated with observation, medical explusive therapy (MET), and ureteroscopy for the management of an index patient with a 4mm distal ureteral stone. We determined which strategy would be least costly and offer the most pain-free days within 30 days of diagnosis. We performed a one-way sensitivity analysis on the probability of successful stone passage with MET. We obtained probabilities from the literature and costs from the 2016 Pediatric Health Information System Database. Results Ureteroscopywas the costliest strategy but maximized the number of pain-free days within 30 days of diagnosis (5,282/29pain−freedays).METwaslesscostlythanureteroscopybutalsolesseffective(5,282/29 pain-free days). MET was less costly than ureteroscopybut also less effective (615/21.8 pain-free days). Observation cost more than MET and was also less effective ($2,139/15.5 pain-free days). The one-way sensitivity analysis on the probability of successful stone passage with MET demonstrated that ureteroscopyalways has the highest net monetary benefits value and is therefore the recommended strategy given a fixed WTP. Discussion Using a rigorous decision-science approach, we found that ureteroscopy is the recommended strategy in children with small distal ureteral stones. Although it cost more than MET, it resulted in more pain-free days in the first 30 days following diagnosis given the faster resolution of the stone episode

    Aerosols and Particulates Emitted by Speech and Breath

    Human-to-human transmission of upper respiratory diseases such as COVID-19 is primarily driven by the dispersion of virus-laden droplets that are expelled from the nose and mouth. Aerosolized droplets can concentrate in the air for hours and pose a significant threat to human activities in the aerospace and aeronautics industry, which frequently occur in confined environments. Tracking data has shown that the virus spread quickly in confined spaces, such as airplanes and ships. Updated guidelines for human interaction in confined spaces (i.e., less than 6ft in distance between individuals) is critical as the world tries to return to normal operations. A lack of data exists concerning the extent of exposure to aerosolized particles in ecologically relevant, non-intrusive scenarios. Studies involving exhalation into small boxes or funnels do little to advance the fundamental understanding of exposure risk associated with many activities. This research aims to meet the need by investigating the quantity, concentration, and distribution of particles exhaled while speaking and breathing. This data can be used to verify computational models of the same. To accomplish this, specific vocal exercises and activities will be performed in a clean, room-sized environment to generate the airborne droplets associated with each activity. Images of these particles will be captured using a 1200 frame-per-second optical camera and a high-power laser, then analyzed numerically using computer software. The resulting data sets will be compared and used to draw conclusions about the volume and behavior of aerosolized particles associated with various vocal activities. Initial tests suggest very different results than those which implement intrusive data collection. Future studies using this experimental setup will investigate patterns that are discovered and the effectiveness of risk mitigation

    A method of communicating information from a wireless autonomous device (WAD) to a base station. The WAD has a data element having a predetermined profile having a total number of sequenced possible data element combinations. The method includes receiving at the WAD an RF profile transmitted by the base station that includes a triggering portion having a number of pulses, wherein the number is at least equal to the total number of possible data element combinations. The method further includes keeping a count of received pulses and wirelessly transmitting a piece of data, preferably one bit, to the base station when the count reaches a value equal to the stored data element's particular number in the sequence. Finally, the method includes receiving the piece of data at the base station and using the receipt thereof to determine which of the possible data element combinations the stored data element is
