Opening the black box of under-health people: the case of Spain
ABSTRACT: The most famous modern definition of health was created during a Preamble to the Constitution of the World Health Organization in 1946: "Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity." This definition has not been amended and, since then, many indicators have been proposed to measure health such as Self-Assessed Health (SAH) status. It provides an overall measure of a population's health based on individuals' personal perceptions of their own health. In this paper, we focus our analysis on "under-health" as the fact of having a level that falls behind the health requirements necessary to perform what is considered an "expected life based on Self Assessed Health". For Spain using the European Union Statistics on Income and Living Conditions (EU-SILC), we can confirm there exist under-healthy people by occupation, age group and sex. Additionally, under-healthy workers are most likely to be found among skilled agricultural, fishery workers and elementary occupations
Individual Heterogeneity in the Returns to Schooling: Instrumental Variables Quantile Regression Using Twins Data
Considerable effort has been exercised in estimating mean returns to education while carefully considering biases arising from unmeasured ability and measurement error. Recent work has investigated whether there are variations from the “mean” return to education across the population with mixed results. We use an instrumental variables estimator for quantile regression on a sample of twins to estimate an entire family of returns to education at different quantiles of the conditional distribution of wages while addressing simultaneity and measurement error biases. We test whether there is individual heterogeneity in returns to education and find that: more able individuals obtain more schooling and that higher ability individuals (those further to the right in the conditional distribution of wages) have higher returns to schooling consistent with a non-trivial interaction between schooling and unobserved abilities in the generation of earnings. The estimated returns are never lower than 9 percent and can be as high as 13 percent at the top of the conditional distribution of wages but they vary significantly only along the lower to middle quantiles. Our findings may have meaningful implications for the design of educational policies
Improved healing response in delayed unions of the tibia with low-intensity pulsed ultrasound: results of a randomized sham-controlled trial
<p>Abstract</p> <p>Background</p> <p>We compared the healing response of tibial delayed unions between subjects treated with low-intensity pulsed ultrasound (LIPUS) (n = 51) and subjects treated with a sham device (n = 50). Fracture age was ≥ 4 months in all cases. Study personnel and participants were blinded to random treatment assignment throughout the study.</p> <p>Methods</p> <p>This multi-center randomized sham-controlled trial was undertaken at six hospitals in Germany. Adult patients who had sustained a tibial shaft fracture that subsequently showed inadequate progress toward healing (i.e., delayed union) were enrolled and randomized to receive either LIPUS (Exogen 2000/2000+, Smith & Nephew GmbH, Schenefeld, Germany) or an identical nonoperative sham device. The daily treatment duration was 20 minutes, for a period of 16 weeks. Subjects randomly assigned to active treatment had the ultrasound pressure wave signal set at the following parameters: 1.5 MHz frequency, 1 kHz repetition rate, 200 μs pulse duration, 30 mW/cm<sup>2 </sup>spatial intensity. Progress toward healing was estimated from changes in bone mineral density (BMD) and gap area as determined from computed tomography scans. Intention-to-treat analysis was conducted using a multiple imputation methodology.</p> <p>Results</p> <p>Based on log-transformed data, mean improvement in BMD was 1.34 (90% confidence interval (CI) 1.14 to 1.57) times greater for LIPUS-treated subjects compared to sham (p = 0.002). A mean reduction in bone gap area also favored LIPUS treatment (p = 0.014).</p> <p>Conclusions</p> <p>These findings demonstrate significantly greater progress toward bone healing after LIPUS treatment compared to no LIPUS treatment in subjects with established delayed unions of the tibia.</p
Quantum W-symmetry in AdS_3
It has recently been argued that, classically, massless higher spin theories
in AdS_3 have an enlarged W_N-symmetry as the algebra of asymptotic isometries.
In this note we provide evidence that this symmetry is realised
(perturbatively) in the quantum theory. We perform a one loop computation of
the fluctuations for a massless spin field around a thermal AdS_3
background. The resulting determinants are evaluated using the heat kernel
techniques of arXiv:0911.5085. The answer factorises holomorphically, and the
contributions from the various spin fields organise themselves into vacuum
characters of the W_N symmetry. For the case of the hs(1,1) theory consisting
of an infinite tower of massless higher spin particles, the resulting answer
can be simply expressed in terms of (two copies of) the MacMahon function.Comment: 23 pages; v2: References adde
Treatment compliance and effectiveness of a cognitive behavioural intervention for low back pain : a complier average causal effect approach to the BeST data set
Background:
Group cognitive behavioural intervention (CBI) is effective in reducing low-back pain and disability in comparison to advice in primary care. The aim of this analysis was to investigate the impact of compliance on estimates of treatment effect and to identify factors associated with compliance.
Methods:
In this multicentre trial, 701 adults with troublesome sub-acute or chronic low-back pain were recruited from 56 general practices. Participants were randomised to advice (control n = 233) or advice plus CBI (n = 468). Compliance was specified a priori as attending a minimum of three group sessions and the individual assessment. We estimated the complier average causal effect (CACE) of treatment.
Results:
Comparison of the CACE estimate of the mean treatment difference to the intention-to-treat (ITT) estimate at 12 months showed a greater benefit of CBI amongst participants compliant with treatment on the Roland Morris Questionnaire (CACE: 1.6 points, 95% CI 0.51 to 2.74; ITT: 1.3 points, 95% CI 0.55 to 2.07), the Modified Von Korff disability score (CACE: 12.1 points, 95% CI 6.07 to 18.17; ITT: 8.6 points, 95% CI 4.58 to 12.64) and the Modified von Korff pain score (CACE: 10.4 points, 95% CI 4.64 to 16.10; ITT: 7.0 points, 95% CI 3.26 to 10.74). People who were non-compliant were younger and had higher pain scores at randomisation.
Conclusions:
Treatment compliance is important in the effectiveness of group CBI. Younger people and those with more pain are at greater risk of non-compliance
D-brane Instantons as Gauge Instantons in Orientifolds of Chiral Quiver Theories
Systems of D3-branes at orientifold singularities can receive
non-perturbative D-brane instanton corrections, inducing field theory operators
in the 4d effective theory. In certain non-chiral examples, these systems have
been realized as the infrared endpoint of a Seiberg duality cascade, in which
the D-brane instanton effects arise from strong gauge theory dynamics. We
present the first UV duality cascade completion of chiral D3-brane theories, in
which the D-brane instantons arise from gauge theory dynamics. Chiral examples
are interesting because the instanton fermion zero mode sector is topologically
protected, and therefore lead to more robust setups. As an application of our
results, we provide a UV completion of certain D-brane orientifold systems
recently claimed to produce conformal field theories with conformal invariance

Top Quarks as a Window to String Resonances
We study the discovery potential of string resonances decaying to
final state at the LHC. We point out that top quark pair production is a
promising and an advantageous channel for studying such resonances, due to
their low Standard Model background and unique kinematics. We study the
invariant mass distribution and angular dependence of the top pair production
cross section via exchanges of string resonances. The mass ratios of these
resonances and the unusual angular distribution may help identify their
fundamental properties and distinguish them from other new physics. We find
that string resonances for a string scale below 4 TeV can be detected via the
channel, either from reconstructing the semi-leptonic
decay or recent techniques in identifying highly boosted tops.Comment: 22 pages, 6 figure
