    Subglacial Water Flow Over an Antarctic Palaeo‐Ice Stream Bed

    The subglacial hydrological system exerts a critical control on the dynamic behavior of the overlying ice because its configuration affects the degree of basal lubrication between the ice and the bed. Yet, this component of the glaciological system is notoriously hard to access and observe, particularly over timescales longer than the satellite era. In Antarctica, abundant evidence for past subglacial water flow over former ice-sheet beds exists around the peripheries of the ice sheet including networks of huge channels carved into bedrock (now submarine) on the Pacific margin of West Antarctica. Here, we combine detailed bathymetric investigations of a channel system in Marguerite Trough, a major palaeo-ice stream bed, with numerical hydrological modeling to explore subglacial water accumulation, routing and potential for erosion over decadal-centennial timescales. Detailed channel morphologies from remotely operated vehicle surveys indicate multiple stages of localized incision, and the occurrence of potholes, some gigantic in scale, suggests incision by turbulent water carrying a significant bedload. Further, the modeling indicates that subglacial water is available during deglaciation and was likely released in episodic drainage events, from subglacial lakes, varying in magnitude over time. Our observations support previous assertions that these huge bedrock channel systems were incised over multiple glacial cycles through episodic subglacial lake drainage events; however, here we present a viable pattern for subglacial drainage at times when the ice sheet existed over the continental shelf and was capable of continuing to erode the bedrock substrate

    Primary health care for Aboriginal Australian women in remote communities after a pregnancy with hyperglycaemia

    Background: Hyperglycaemia in pregnancy contributes to adverse outcomes for women and their children. The postpartum period is an opportune time to support women to reduce cardiometabolic and diabetes risk in subsequent pregnancies. Aims: To identify strengths and gaps in current care for Aboriginal women after a pregnancy complicated by hyperglycaemia. Methods: A retrospective review of the 12 month postpartum care provided by primary health centres in remote Australia in 2013-2014 identified 195 women who experienced hyperglycaemia in pregnancy (gestational diabetes (GDM) (n = 147), type 2 diabetes (T2D) (n = 39), and unclear diabetes status (n = 9)). RESULTS: Only 80 women (54%) with GDM had postpartum glycaemic checks. Of these, 32 women were diagnosed with prediabetes (n = 24) or diabetes (n = 8). Compared to women with GDM, women with T2D were more likely to have their weight measured (75% vs. 52%, p <0.01), and smoking status documented as "discussed" (65% vs. 34%, p < 0.01). Most women (97%) accessed the health centre at least once in the 12 month postpartum period but, during these visits, only 52% of women had service provision, either structured or opportunistic, related to diabetes. Conclusion: High rates of dysglycaemia among women screened for T2D after GDM in the 12 month postpartum period highlight the need for increased screening and early intervention to prevent the development of T2D and its complications.     SARS-CoV-2-specific immune responses and clinical outcomes after COVID-19 vaccination in patients with immune-suppressive disease

    Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) immune responses and infection outcomes were evaluated in 2,686 patients with varying immune-suppressive disease states after administration of two Coronavirus Disease 2019 (COVID-19) vaccines. Overall, 255 of 2,204 (12%) patients failed to develop anti-spike antibodies, with an additional 600 of 2,204 (27%) patients generating low levels (<380 AU ml−1). Vaccine failure rates were highest in ANCA-associated vasculitis on rituximab (21/29, 72%), hemodialysis on immunosuppressive therapy (6/30, 20%) and solid organ transplant recipients (20/81, 25% and 141/458, 31%). SARS-CoV-2-specific T cell responses were detected in 513 of 580 (88%) patients, with lower T cell magnitude or proportion in hemodialysis, allogeneic hematopoietic stem cell transplantation and liver transplant recipients (versus healthy controls). Humoral responses against Omicron (BA.1) were reduced, although cross-reactive T cell responses were sustained in all participants for whom these data were available. BNT162b2 was associated with higher antibody but lower cellular responses compared to ChAdOx1 nCoV-19 vaccination. We report 474 SARS-CoV-2 infection episodes, including 48 individuals with hospitalization or death from COVID-19. Decreased magnitude of both the serological and the T cell response was associated with severe COVID-19. Overall, we identified clinical phenotypes that may benefit from targeted COVID-19 therapeutic strategies

    The Cholecystectomy As A Day Case (CAAD) Score: A Validated Score of Preoperative Predictors of Successful Day-Case Cholecystectomy Using the CholeS Data Set

    Background Day-case surgery is associated with significant patient and cost benefits. However, only 43% of cholecystectomy patients are discharged home the same day. One hypothesis is day-case cholecystectomy rates, defined as patients discharged the same day as their operation, may be improved by better assessment of patients using standard preoperative variables. Methods Data were extracted from a prospectively collected data set of cholecystectomy patients from 166 UK and Irish hospitals (CholeS). Cholecystectomies performed as elective procedures were divided into main (75%) and validation (25%) data sets. Preoperative predictors were identified, and a risk score of failed day case was devised using multivariate logistic regression. Receiver operating curve analysis was used to validate the score in the validation data set. Results Of the 7426 elective cholecystectomies performed, 49% of these were discharged home the same day. Same-day discharge following cholecystectomy was less likely with older patients (OR 0.18, 95% CI 0.15–0.23), higher ASA scores (OR 0.19, 95% CI 0.15–0.23), complicated cholelithiasis (OR 0.38, 95% CI 0.31 to 0.48), male gender (OR 0.66, 95% CI 0.58–0.74), previous acute gallstone-related admissions (OR 0.54, 95% CI 0.48–0.60) and preoperative endoscopic intervention (OR 0.40, 95% CI 0.34–0.47). The CAAD score was developed using these variables. When applied to the validation subgroup, a CAAD score of ≤5 was associated with 80.8% successful day-case cholecystectomy compared with 19.2% associated with a CAAD score >5 (p < 0.001). Conclusions The CAAD score which utilises data readily available from clinic letters and electronic sources can predict same-day discharges following cholecystectomy

    Visual statistical learning in infancy: evidence for a domain general learning mechanism

    The rapidity with which infants come to understand language and events in their surroundings has prompted speculation concerning innate knowledge structures that guide language acquisition and object knowledge. Recently, however, evidence has emerged that by 8 months, infants can extract statistical patterns in auditory input that are based on transitional probabilities defining the sequencing of the input’s components (Science 274 (1996) 1926). This finding suggests powerful learning mechanisms that are functional in infancy, and raises questions about the domain generality of such mechanisms. We habituated 2-, 5-, and 8-month-old infants to sequences of discrete visual stimuli whose ordering followed a statistically predictable pattern. The infants subsequently viewed the familiar pattern alternating with a novel sequence of identical stimulus components, and exhibited significantly greater interest in the novel sequence at all ages. These results provide support for the likelihood of domain general statistical learning in infancy, and imply that mechanisms designed to detect structure inherent in the environment may play an important role in cognitive development

    GLoP : Enabling Massively Parallel Incident Response Through GPU Log Processing

    Large industrial systems that combine services and applications, have become targets for cyber criminals and are challenging from the security, monitoring and auditing perspectives. Security log analysis is a key step for uncovering anomalies, detecting intrusion, and enabling incident response. The constant increase of link speeds, threats and users, produce large volumes of log data and become increasingly difficult to analyse on a Central Processing Unit (CPU). This paper presents a massively parallel Graphics Processing Unit (GPU) Log Processing (GLoP) library and can also be used for Deep Packet Inspection (DPI), using a prefix matching technique, harvesting the full power of off-the-shelf technologies. GLoP implements two different algorithm using different GPU memory and is compared against CPU counterpart implementations. The library can be used for processing nodes with single or multiple GPUs as well as GPU cloud farms. The results show throughput of 20 Gbps and demonstrate that modern GPUs can be utilised to increase the operational speed of large scale log processing scenarios, saving precious time before and after an intrusion has occurred

    Pattern Matching is a computationally intensive task used in many research fields and real world applications. Due to the ever-growing volume of data to be processed, and increasing link speeds, the number of patterns to be matched has risen significantly. In this paper we explore the parallel capabilities of modern General Purpose Graphics Processing Units (GPGPU) applications for high speed pattern matching. A highly compressed failure-less Aho-Corasick algorithm is presented for Intrusion Detection Systems on off-the-shelf hardware. This approach maximises the bandwidth for data transfers between the host and the Graphics Processing Unit (GPU). Experiments are performed on multiple alphabet sizes, demonstrating the capabilities of the library to be used in different research fields, while sustaining an adequate throughput for intrusion detection systems or DNA sequencing. The work also explores the performance impact of adequate prefix matching for alphabet sizes and varying pattern numbers achieving speeds up to 8Gbps and low memory consumption for intrusion detection systems
