    A recently excavated skeleton from an Anglo-Saxon burial ground in Kent exhibits a pathological right heel. The disorder is identified as a congenital talo-calcaneal bridge, a genetic anomaly which often results in "peroneal spastic flat foot". This 1,300-year-old specimen is presently the oldest reported for this disorder

    Plantaris excision reduces pain in Mid-portion Achilles tendinopathy even in the absence of plantaris tendinosis

    Background It is becoming increasingly apparent that the plantaris can contribute to symptoms in at least a subset of patients with mid-portion Achilles tendinopathy. However the nature of its involvement remains unclear. Hypothesis / Purpose To determine whether excised plantaris tendons from patients with mid-portion Achilles tendinopathy display tendinopathic changes and whether the presence of such changes affect clinical outcomes. Methods Sixteen plantaris tendons patients with mid-portion Achilles tendinopathy recalcitrant to conservative management underwent histological examination for the presence of tendinopathic changes. All patients had imaging to confirm the presence of the plantaris tendon adherent to or invaginated into the focal area of Achilles tendinosis. Visual analogue scores (VAS) and foot and ankle outcome scores (FAOS) were recorded pre and post-operatively. Results Sixteen patients (mean age 26.2; 18-47 years) underwent surgery with a mean follow-up of 14 months (range 6-20 months). The plantaris tendon was histologically normal in 13/16 cases (81%). Inflammatory changes in the loose peritendinous connective tissue surrounding the plantaris tendon were evident in all cases. There was significant improvement in mean VAS scores (p<0.05) and all domains of the FAOS post-operatively (p<0.05). Conclusions The absence of any tendinopathic changes in the excised plantaris of 13 patients who clinically improved suggests plantaris involvement with Achilles tendinopathy may not yet be fully understood and supports the concept that this may be a compressive or a frictional phenomenon rather than purely tendinopathic. Clinical Relevance There is increasing evidence to support that the excision of plantaris results in improved clinical outcomes in a sub-set of patients with mid-portion Achilles tendinopathy. Prior findings have reported that excised plantaris tendons from these patients display tendinopathic changes. However these findings suggest the role of plantaris in the pathology of these patients could be biomechanical since patients improved even when the excised plantaris was not tendinopathic

    The interaction between gaze and facial expression in the amygdala and extended amygdala is modulated by anxiety

    Behavioral evidence indicates that angry faces are seen as more threatening, and elicit greater anxiety, when directed at the observer, whereas the influence of gaze on the processing of fearful faces is less consistent. Recent research has also found inconsistent effects of expression and gaze direction on the amygdala response to facial signals of threat. However, such studies have failed to consider the important influence of anxiety on the response to signals of threat; an influence that is well established in behavioral research and recent neuroimaging studies. Here, we investigated the way in which individual differences in anxiety would influence the interactive effect of gaze and expression on the response to angry and fearful faces in the human extended amygdala. Participants viewed images of fearful, angry and neutral faces, either displaying an averted or direct gaze. We found that state anxiety predicted an increased response in the dorsal amygdala/substantia innominata (SI) to angry faces when gazing at, relative to away from the observer. By contrast, high state anxious individuals showed an increased amygdala response to fearful faces that was less dependent on gaze. In addition, the relationship between state anxiety and gaze on emotional intensity ratings mirrored the relationship between anxiety and the amygdala/SI response. These results have implications for understanding the functional role of the amygdala and extended amygdala in processing signals of threat, and are consistent with the proposed role of this region in coding the relevance or significance of a stimulus to the observer

    Analysis techniques are introduced that allow for estimation of potential sounding uncertainty due to water mass variability from reconnaissance campaigns in which oceanographic parameters are measured at a high temporal and spatial resolution. The analysis techniques do not require sounding data, thus analyses can be tailored to match any survey system; this allows for pre-analysis campaigns to optimize survey instrumentation and sound speed profiling rates such that a desired survey specification can be maintained. Additionally, the output of the analysis methods can potentially provide a higher fidelity estimation of sounding uncertainty due to water mass variability than uncertainty models in common use

    Uncertainty Wedge Analysis: Quantifying the Impact of Sparse Sound Speed Profiling Regimes on Sounding Uncertainty

    Recent advances in real-time monitoring of uncertainty due to refraction have demonstrated the power of estimating and visualizing uncertainty over the entire potential sounding space. This representation format, referred to as an uncertainty wedge, can be used to help solve difficult survey planning problems regarding the spatio-temporal variability of the watercolumn. Though initially developed to work in-line with underway watercolumn sampling hardware (e.g. moving vessel profilers), uncertainty wedge analysis techniques are extensible to investigate problems associated with low-density watercolumn sampling in which only a few sound speed casts are gathered per day. As uncertainty wedge analysis techniques require no sounding data, the overhead of post-processing soundings is circumvented in the situation when one needs to quickly ascertain the impact of a particular sampling regime. In keeping with the spirit of the underlying real-time monitoring tools, a just in time analysis of sound speed casts can help the field operator assess the effects of watercolumn variability during acquisition and objectively seek a watercolumn sampling regime which would balance the opposing goals of maximizing survey efficiency and maintaining reasonable sounding accuracy. In this work, we investigate the particular problem of estimating the uncertainty that would be associated with a particular low-density sound speed sampling regime. A pre-analysis technique is proposed in which a high-density set of sound speed profiles provides a baseline against which various low-density sampling regimes can be tested, the end goal being to ascertain the penalty in sounding confidence that would be associated with a particular low-density sampling regime. In other words, by knowing too much about the watercolumn, one can objectively quantify the impact of not knowing enough. In addition to the goal-seeking field application outlined earlier, this allows for more confi- dent attribution of uncertainty to soundings, a marked improvement over current approaches to refraction uncertainty estimation

    Pseudo-operational trials of Lumicyano solution and Lumicyano powder for the detection of latent fingermarks on various substrates

    This study presents pseudo-operational trials comparing a one-step fluorescent cyanoacrylate process with a number of other enhancement techniques on a variety of substrates. This one-step process involves a product, 4% Lumicyano, which is a solution consisting of 4% by weight of a powdered dye (Lumicyano powder) dissolved in a cyanoacrylate-based solution (Lumicyano solution). The cyanoacrylate in the Lumicyano solution may be of a higher quality than that used in the two-step products.One hundred items were collected from the place of work for each trial. Trial 1 involved a comparison of 4% Lumicyano with the conventional two-step cyanoacrylate fuming-dye staining for the detection of latent fingermarks on plastic carrier bags. Trial 2 assessed the quality of the Lumicyano solution (with no powdered dye) but used in a two-step process with basic yellow 40 (BY40). Trial 1, using 4% Lumicyano powder and traditional cyanoacrylate → BY40 detected a similar amount of fingermarks (~295); however, sequential BY40 treatment (i.e., after 4% Lumicyano) detected an additional 30% marks. Trial 2 resulted in the detection of 565 marks after Lumicyano solution → BY40 in comparison to 489 marks after traditional cyanoacrylate fuming and BY40 staining. Trials 3 through 5 compared 4% Lumicyano, 1,2-indanedione-zinc, and ninhydrin on junk mail, magazines, and cardboard used for food or cosmetic packaging; the detection rate was low for all techniques and substrates. Trial 6 on cardboard packaging using 4% Lumicyano, black iron-oxide powder suspension, and magnetic powder also provided a low detection rate. Trial 7, using 4% Lumicyano → BY40, solvent black 3, and iron-oxide powder suspensions on cardboard packaging from a fast food chain, indicated that 4% Lumicyano → BY40 might be a suitable alternative to solvent black 3 and iron-oxide powder suspensions for suspected greasy marks
