    Intramuscular fat in gluteus maximus for different levels of physical activity

    We aimed to determine if gluteus maximus (GMAX) fat infiltration is associated with different levels of physical activity. Identifying and quantifying differences in the intramuscular fat content of GMAX in subjects with different levels of physical activity can provide a new tool to evaluate hip muscles health. This was a cross-sectional study involving seventy subjects that underwent Dixon MRI of the pelvis. The individuals were divided into four groups by levels of physical activity, from low to high: inactive patients due to hip pain; and low, medium and high physical activity groups of healthy subjects (HS) based on hours of exercise per week. We estimated the GMAX intramuscular fat content for each subject using automated measurements of fat fraction (FF) from Dixon images. The GMAX volume and lean volume were also measured and normalized by lean body mass. The effects of body mass index (BMI) and age were included in the statistical analysis. The patient group had a significantly higher FF than the three groups of HS (median values of 26.2%, 17.8%, 16.7% and 13.7% respectively, p < 0.001). The normalized lean volume was significantly larger in the high activity group compared to all the other groups (p < 0.001, p = 0.002 and p = 0.02). Employing a hierarchical linear regression analysis, we found that hip pain, low physical activity, female gender and high BMI were statistically significant predictors of increased GMAX fat infiltration

    Uncemented femoral stem orientation and position in total hip arthroplasty: A CT study

    In total hip arthroplasty (THA), accurate positioning of components is important for the functionality and long life of the implant. Femoral component version has been underinvestigated when compared with the acetabular cup. Accurate prediction of the femoral version on the preoperative plan is particularly important because a well-fitting uncemented stem will, by definition, press-fit into a version that is dictated by the anatomy of the proximal femur. A better understanding of this has recently become an unmet need because of the increased use of uncemented stems and of preoperative image-based planning. We present the first, three-dimensional (3D) comparison between the planned and achieved orientation and position of the femoral components in THA. We propose a comparison method that uses the 3D models of a, computed tomography-generated (CT-generated), preoperative plan and a postoperative CT to obtain the discrepancy in the six possible degrees of freedom. We ran a prospective study (level 2 evidence) of 30 patients undergoing uncemented THA to quantify the discrepancy between planned and achieved femoral stem orientation and position. The discrepancy was low for femoral stem vertical position and leg length, and varus-valgus and anterior-posterior orientation. The discrepancy was higher for femoral version with a mean (±SD) of -1.5 ± 7.8 deg. Surgeons should be aware of the variability of the eventual position of uncemented stems in THA and acknowledge the risk of achieving a less-than-optimal femoral version, different from the preoperative 3D CT plan

    Reference values for volume, fat content and shape of the hip abductor muscles in healthy individuals from Dixon MRI

    Healthy hip abductor muscles are a good indicator of a healthy hip and an active lifestyle, as they are greatly involved in human daily activities. Fatty infiltration and muscle atrophy are associated with loss of strength, loss of mobility and hip disease. However, these variables have not been widely studied in this muscle group. We aimed to characterize the hip abductor muscles in a group of healthy individuals to establish reference values for volume, intramuscular fat content and shape of this muscle group. To achieve this, we executed a cross-sectional study using Dixon MRI scans of 51 healthy subjects. We used an automated segmentation method to label GMAX, GMED, GMIN and TFL muscles, measured normalized volume (NV) using lean body mass, fat fraction (FF) and lean muscle volume for each subject and computed non-parametric statistics for each variable grouped by sex and age. We measured these variables for each axial slice and created cross-sectional area and FF axial profiles for each muscle. Finally, we generated sex-specific atlases with FF statistical images. We measured median (IQR) NV values of 12.6 (10.8-13.8), 6.3 (5.6-6.7), 1.6 (1.4-1.7) and 0.8 (0.6-1.0) cm3/kg for GMAX, GMED, GMIN and TFL, and median (IQR) FF values of 12.3 (10.1-15.9)%, 9.8 (8.6-11.2)%, 10.0 (9.0-12.0)% and 10.2 (7.8-13.5)% respectively. FF values were significantly higher for females for the four muscles (p < 0.01), but there were no significant differences between the two age groups. When comparing individual muscles, we observed a significantly higher FF in GMAX than in the other muscles. The reported novel reference values and axial profiles for volume and FF of the hip abductors, together with male and female atlases, are tools that could potentially help to quantify and detect early the deteriorating effects of hip disease or sarcopenia

    Automated measurement of fat infiltration in the hip abductors from Dixon magnetic resonance imaging

    PURPOSE: Intramuscular fat infiltration is a dynamic process, in response to exercise and muscle health, which can be quantified by estimating fat fraction (FF) from Dixon MRI. Healthy hip abductor muscles are a good indicator of a healthy hip and an active lifestyle as they have a fundamental role in walking. The automated measurement of the abductors' FF requires the challenging task of segmenting them. We aimed to design, develop and evaluate a multi-atlas based method for automated measurement of fat fraction in the main hip abductor muscles: gluteus maximus (GMAX), gluteus medius (GMED), gluteus minimus (GMIN) and tensor fasciae latae (TFL). METHOD: We collected and manually segmented Dixon MR images of 10 healthy individuals and 7 patients who underwent MRI for hip problems. Twelve of them were selected to build an atlas library used to implement the automated multi-atlas segmentation method. We compared the FF in the hip abductor muscles for the automated and manual segmentations for both healthy and patients groups. Measures of average and spread were reported for FF for both methods. We used the root mean square error (RMSE) to quantify the method accuracy. A linear regression model was used to explain the relationship between FF for automated and manual segmentations. RESULTS: The automated median (IQR) FF was 20.0(16.0-26.4) %, 14.3(10.9-16.5) %, 15.5(13.9-18.6) % and 16.2(13.5-25.6) % for GMAX, GMED, GMIN and TFL respectively, with a FF RMSE of 1.6%, 0.8%, 2.1%, 2.7%. A strong linear correlation (R2 = 0.93, p < .001, m = 0.99) was found between the FF from automated and manual segmentations. The mean FF was higher in patients than in healthy subjects. CONCLUSION: The automated measurement of FF of hip abductor muscles from Dixon MRI had good agreement with FF measurements from manually segmented images. The method was accurate for both healthy and patients groups

    Cadmium, manganese, nickel and lead contents in surface sediments of the lower Ulla River and its estuary (northwest Spain)

    The authors determined the total contents of cadmium, manganese, nickel and lead in 16 surface sediment samples along the last 7 km of the Ulla River and in its estuary. Prior to chemical analysis, the grain-size fractions below 500, 63 and 2 μm were separated. The metal concentrations were analysed in the fraction below 63 μm, and in selected samples from the clay fraction ( 2 μm). The soil samples were digested with a HF + HNO₃ + HCl mixture and the total concentrations of Cd, Mn, Ni and Pb were analysed with the AAS method. The highest contents of metals were found in the riverine sediments, near the town of Padrón, and in the estuary. The enrichment of metals in the sediments was mainly related to the proximity of the industrial sources, and to the fine particles transported and deposited in the sediments. The Ni and Mn contents found are considered very high, at some points reaching values of up to 357 mg kg⁻¹ and 2 110 mg kg⁻¹, respectively. Lead and Cd were mainly found within the range considered as natural background levels in the sediments of the Galician rias, with concentrations of 25-68 mg kg⁻¹ for Pb and 0.01-0.92 mg kg⁻¹ for Cd. The lower part of the Ulla River and its estuary present: l) moderate to high contamination levels of Mn and Ni; 2) moderate Pb pollution levels; and 3) no Cd contamination. The highest concentrations of metals were obtained in the river, near the wastewater discharges, and in the estuary, associated with fine particles.El contenido total de cadmio, manganeso, níquel y plomo se determinó en 16 muestras de sedimentos superficiales a lo largo de los últimos 7 km del río Ulla y en su estuario. Previamente a los análisis químicos, se realizó la separación granulométrica de los sedimentos en las fracciones inferiores a 500, 63 y 2 μm. La concentración de metales se determinó en la fracción inferior a 63 μm y, para algunas muestras, en la fracción arcilla ( 2 μm). La digestión del sedimento se realizó en una mezcla ácida de HF + HNO₃ + HCl y se analizó la concentración total de Cd, Mn, Ni y Pb por espectroscopia de absorción atómica. Los contenidos más altos en metales se encontraron en los sedimentos del río, en las inmediaciones de Padrón y en los del estuario. El enriquecimiento de los sedimentos en metales se ha relacionado con la proximidad a las zonas industriales y con el material fino transportado y depositado en los sedimentos. Se han encontrado contenidos muy altos de Ni y Mn con valores de 357 mg kg⁻¹ y 2 110 mg kg⁻¹, respectivamente. Las concentraciones de Cd y Pb son comparables a los valores establecidos como niveles naturales en los sedimentos de las rías gallegas: 25-68 mg kg⁻¹ para el Pb y 0.01-0.92 mg kg⁻¹ para el Cd. El sedimento de la parte baja del río Ulla y su estuario presentan niveles moderados-altos de contaminación para Mn y Ni, moderados para Pb y nulos para Cd. Las mayores concentraciones de estos metales en el sedimento fueron medidas asociadas con las fracciones finas ( 63 μm): cerca de Padrón, zona de influencia fluvial y, también, aunque con valores más bajos, en la desembocadura del Ulla, zona de influencia marina.Instituto Español de Oceanografí

    The in vivo location of edge-wear in hip arthroplasties : combining pre-revision 3D CT imaging with retrieval analysis

    AIMS: Acetabular edge-loading was a cause of increased wear rates in metal-on-metal hip arthroplasties, ultimately contributing to their failure. Although such wear patterns have been regularly reported in retrieval analyses, this study aimed to determine their in vivo location and investigate their relationship with acetabular component positioning. METHODS: 3D CT imaging was combined with a recently validated method of mapping bearing surface wear in retrieved hip implants. The asymmetrical stabilizing fins of Birmingham hip replacements (BHRs) allowed the co-registration of their acetabular wear maps and their computational models, segmented from CT scans. The in vivo location of edge-wear was measured within a standardized coordinate system, defined using the anterior pelvic plane. RESULTS: Edge-wear was found predominantly along the superior acetabular edge in all cases, while its median location was 8° (interquartile range (IQR) -59° to 25°) within the anterosuperior quadrant. The deepest point of these scars had a median location of 16° (IQR -58° to 26°), which was statistically comparable to their centres (p = 0.496). Edge-wear was in closer proximity to the superior apex of the cups with greater angles of acetabular inclination, while a greater degree of anteversion influenced a more anteriorly centred scar. CONCLUSION: The anterosuperior location of edge-wear was comparable to the degradation patterns observed in acetabular cartilage, supporting previous findings that hip joint forces are directed anteriorly during a greater portion of walking gait. The further application of this novel method could improve the current definition of optimal and safe acetabular component positioning. Cite this article: Bone Joint Res 2021;10(10):639-649

    Simulation of hydrogen permeation through pure iron for trapping and surface phenomena characterisation

    There is a need for numerical models capable of predicting local accumulation of hydrogen near stress concentrators and crack tips to prevent and mitigate hydrogen assisted fracture in steels. The experimental characterisation of trapping parameters in metals, which is required for an accurate simulation of hydrogen transport, is usually performed through the electropermeation test. In order to study grain size influence and grain boundary trapping during permeation, two modelling approaches are explored; a 1D Finite Element model including trap density and binding energy as input parameters and a polycrystalline model based on the assignment of a lower diffusivity and solubility to the grain boundaries. Samples of pure iron after two different heat treatments - 950C for 40 minutes and 1100C for 5 minutes - are tested applying three consecutive rising permeation steps and three decaying steps. Experimental results show that the finer grain microstructure promotes a diffusion delay due to grain boundary trapping. The usual methodology for the determination of trap densities and binding energies is revisited in which the limiting diluted and saturated cases are considered. To this purpose, apparent diffusivities are fitted including also the influence of boundary conditions and comparing results provided by the constant concentration with the constant flux assumption. Grain boundaries are characterised for pure iron with a binding energy between 37.8 and 39.9 kJ/mol and a low trap density but it is numerically demonstrated that saturated or diluted assumptions are not always verified, and a univocal determination of trapping parameters requires a broader range of charging conditions for permeation.     Objective: To determine the usefulness of the clinical, biochemical and thyroid imaging parameters in patients with thyroid-associated ophthalmopathy, in predicting their response to immunosuppressive treatment. Methods: This retrospective study of 16 patients with thyroid ophthalmopathy considered a number of variables including sex, age, thyroid hormone levels, treatment of the thyroid dysfunction, clinical activity score (CAS), severity (NOSPECS) of the ophthalmopathy, signs in standardized A-mode ultrasonography, in CAT and/or NMR, and previously used treatments and their effectiveness. Results: The average age of the patients was 50.81 (S.D: 11.89) years; there were 5 males (31.3%) and 11 females (68.8%). The ophthalmopathy was classified as active in 10/16 patients (62.5%) and inactive in 6/16 (37.5%); and as moderate in 9/16 (56.25%) and severe in 7/16 (43.75%) according to the severity defined in NOSPECS. Ultrasonography was diagnostic in 100% of the cases. The severity decreased significantly (p ≤ 0.05), however the clinical activity decrease did not reach significance (p=0.38) during immunosuppressive therapy. Better results were obtained during treatment of patients with a higher CAS (p=0.04) and in those with more severe ophthalmopathy (p=0.02). There was a tendency for the patients with higher levels of TSI to respond better to the treatment (p=0.06). Conclusions: The CAS is the best parameter to quantify the activity of the disease and predicting the response to treatment. The higher the CAS and the more severe the ophthalmopathy, the better the response to treatment.     The 4G/5G PAI-1 polymorphism influences the endothelial response to IL-1 and the modulatory effect of pravastatin

    BACKGROUND: Increased plasminogen activator inhibitor (PAI-1) levels lead to impaired fibrinolytic function associated with higher cardiovascular risk. PAI-1 expression may be regulated by different inflammatory cytokines such as interleukin-1alpha (IL-1). Several polymorphisms have been described in the PAI-1 gene. AIM: We examined the influence of the 4G/5G polymorphism in the promoter region on IL-1alpha-induced PAI-1 expression by human umbilical vein endothelial cells (HUVEC) in presence or absence of pravastatin. METHODS AND RESULTS: Genotyped HUVEC were incubated with IL-1alpha (500 U mL(-1)) in presence or absence of pravastatin (1-10 microm). PAI-1 expression was analyzed by real time polymerase chain reaction (PCR), and PAI-1 antigen measured in supernatants by ELISA. IL-1alpha increased PAI-1 secretion in a genotype-dependent manner, and higher values were observed for 4G/4G compared with both 4G/5G and 5G/5G cultures (P < 0.05). Preincubation of HUVEC with 10 microm pravastatin significantly reduced IL-1-induced PAI-1 expression in 4G/4G HUVEC compared with untreated cultures (177.5% +/- 24.5% vs. 257.9% +/- 39.0%, P < 0.05). Pravastatin also attenuated the amount of secreted PAI-1 by 4G/4G HUVEC after IL-1 stimulation (5020.6 +/- 165.7 ng mL(-1) vs. 4261.1 +/- 309.8 ng mL(-1), P < 0.05). This effect was prevented by coincubation with mevalonate, indicating a dependence on HMG-CoA reductase inhibition. CONCLUSIONS: The endothelial 4G/5G PAI-1 genotype influences the PAI-1 response to IL-1alpha and the modulatory effect of pravastatin. As increased PAI-1 levels have been linked to cardiovascular disease the observed endothelial modulation by pravastatin may have potential clinical implications
