    Simulation studies for dielectric wakefield programme at CLARA facility

    Short, high charge electron bunches can drive high magnitude electric fields in dielectric lined structures. The interaction of the electron bunch with this field has several applications including high gradient dielectric wakefield acceleration (DWA) and passive beam manipulation. The simulations presented provide a prelude to the commencement of an experimental DWA programme at the CLARA accelerator at Daresbury Laboratory. The key goals of this program are: tunable generation of THz radiation, understanding of the impact of transverse wakes, and design of a dechirper for the CLARA FEL.     How do men in the United Kingdom decide to dispose of banked sperm following cancer treatment?

    Current policy in the UK recommends that men bank sperm prior to cancer treatment, but very few return to use it for reproductive purposes or agree to elective disposal even when their fertility recovers and their families are complete. We assessed the demographic, medical and psychological variables that influence the decision to dispose by contacting men (n = 499) who banked sperm more than five years previously, and asked them to complete questionnaires about their views on sperm banking, fertility and disposal. From 193 responses (38.7% response rate), 19 men (9.8%) requested disposal within four months of completing the questionnaire. Compared with men who wanted their sperm to remain in storage, they were significantly more confident that their fertility had recovered (OR = 1.78, 95% CI = 1.05-3.03, p = 0.034), saw fertility monitoring (semen analysis) as less important (OR = 0.61, 95% CI = 0.39-0.94, p = 0.026), held more positive attitudes to disposal (OR = 5.71, 95% CI = 2.89-11.27, p < 0.001), were more likely to have experienced adverse treatment side-effects (OR = 4.37, CI = 1.61-11.85, p = 0.004) and had less desire for children in the future (OR = 0.41, 95% CI = 0.26-0.64, p < 0.001). Information about men's reasons to dispose of banked sperm may be helpful in devising new strategies to encourage men to engage with sperm banking clinics and make timely decisions about the fate of their samples

    Prehension of a flanked target in individuals with amblyopia.

    yesPurpose: Reduced binocularity is a prominent feature of amblyopia and binocular cues are thought to be important for prehension. We examine prehension in individuals with amblyopia when the target-object was flanked, thus mimicking everyday prehension. Methods: amblyopes (n=20, 36.4±11.7 years; 6 anisometropic, 3 strabismic, 11 mixed) and visually-normal controls (n=20, 27.5±6.3 years) reached forward, grasped and lifted a cylindrical target-object that was flanked with objects on either (lateral) side of the target, or in front and behind it in depth. Only 6 amblyopes (30%) had measurable stereoacuity. Trials were completed in binocular and monocular viewing, using the better eye in amblyopic participants. Results: Compared to visual normals, amblyopes displayed a longer overall movement time (p=0.031), lower average reach velocity (p=0.021), smaller maximum aperture (p=0.007) and longer durations between object contact and lift (p=0.003). Differences between groups were more apparent when the flankers were in front and behind, compared to either side, as evidenced by significant group-by-flanker configuration interactions for reach duration (p<0.001), size and timing of maximum aperture (p≤0.009), end-of-reach to object-contact (p<0.001), and between object contact and lift (p=0.044), suggesting that deficits are greatest when binocular cues are richest. Both groups demonstrated a significant binocular advantage, in that in both groups performance was worse for monocular compared to binocular viewing, but interestingly, amblyopic deficits in binocular viewing largely persisted during monocular viewing with the better eye. Conclusions: These results suggest that amblyopes either display considerable residual binocularity or that they have adapted to make good use of their abnormal binocularity

    Development of the Screening Tool for Everyday Mobility and Symptoms (STEMS) for skeletal dysplasia

    Background: Skeletal dysplasia are genetic disorders of cartilage and bone, characterized by impairments commonly resulting in short stature, altered movement biomechanics, pain, fatigue and reduced functional performance. While current tools quantify functional mobility performance, they have not been standardly used in this population group and do not capture patient-reported symptoms such as pain or fatigue. This study evaluated a new tool, the Screening Tool for Everyday Mobility and Symptoms (STEMS), designed to accurately and objectively assess functional mobility and associated symptomology for individuals with skeletal dysplasia. Methods: Individuals aged 5-75 years with a skeletal dysplasia completed the STEMS, the Functional Mobility Scale (FMS) and Six Minute Walk Test (6MWT). The correlation among the STEMS, use of mobility aides, FMS and 6MWT normalised for leg length was calculated. One-way analysis of variance compared the STEMS symptomatology to normalised 6MWT distance. Results: One hundred and fifty individuals with skeletal dysplasia (76 achondroplasia, 42 osteogenesis imperfecta, 32 other; 74= 18 years) participated. Almost two thirds of the group reported pain and/or fatigue when mobilising at home, at work or school and within the community, but only twenty percent recorded use of a mobility device. The STEMS setting category demonstrated highly significant correlations with the corresponding FMS category (r=-0.983 to -0.0994, all p<0.001), and a low significant correlation with the normalised 6MWT distance (r=-0.323 to -0.394, all p<0.001). A decreased normalised 6MWT distance was recorded for individuals who reported symptoms of pain and/or fatigue when mobilising at home or at work/school (all p <= 0.004). Those who reported pain only when mobilising in the community had a normal 6MWT distance (p=0.43-0.46). Conclusions: The Screening Tool for Everyday Mobility and Symptoms (STEMS) is a useful new tool to identify and record mobility aide use and associated self-reported symptoms across three environmental settings for adults and children with skeletal dysplasia. The STEMS may assist clinicians to monitor individuals for changes in functional mobility and symptoms over time, identify individuals who are functioning poorly compared to peers and need further assessment, and to measure effectiveness of treatment interventions in both clinical and research settings

    Defining critical factors in multi-country studies of assisted reproductive technologies (ART): data from the US and UK health systems

    As the worldwide use of assisted reproductive technologies (ART) continues to grow, there is a critical need to assess the safety of these treatment parameters and the potential adverse health effects of their use in adults and their offspring. While key elements remain similar across nations, geographic variations both in treatments and populations make generalizability challenging. We describe and compare the demographic factors between the USA and the UK related to ART use and discuss implications for research. The USA and the UK share some common elements of ART practice and in how data are collected regarding long-term outcomes. However, the monitoring of ART in these two countries each brings strengths that complement each other’s limitations

    Why don't some men with banked sperm respond to letters about their stored samples?

    Long-term storage of banked sperm, especially when it is not needed, for reproductive purposes, is costly and poses practical problems for sperm banks. For sperm banks to function efficiently, men must understand the implications of unnecessary storage, and make timely decisions about disposal of their own samples. Men who bank sperm prior to cancer treatment are routinely offered follow-up consultations to test their fertility, update consent and, where necessary, expedite referral for Assisted Conception. Yet sperm banks report that men do not respond to letters, suggesting samples are stored needlessly. We conducted semi-structured interviews with six men with a history of not responding to letters, to document reasons for non-response. Interviews were transcribed and analysed using Interpretive Phenomenological Analysis. Men's reasons for not responding are a complex interplay between past, present and future perspectives. In terms of their past, information is important on diagnosis, because men must understand that fertility can change after treatment. Present and future concerns focus on fears of being told fertility has not recovered and being pressured to dispose of banked sperm. The challenge is to devise invitation letters that address men's concerns while offering them tangible benefits and peace of mind

    Magmatic fluids implicated in the formation of propylitic alteration: oxygen, hydrogen, and strontium isotope constraints from the Northparkes porphyry Cu-Au district, New South Wales, Australia

    In porphyry ore deposit models, the propylitic alteration facies is widely interpreted to be caused by convective circulation of meteoric waters. However, recent field-based and geochemical data suggest that magmatic-derived fluids are likely to contribute to development of the propylitic assemblage. In order to test this hypothesis, we determined the oxygen and hydrogen isotope compositions of propylitic mineral separates (epidote, chlorite, and quartz), selected potassic mineral separates (quartz and magnetite), and quartz-hosted fluid inclusions from around the E48 and E26 deposits in the Northparkes porphyry Cu-Au district, New South Wales, Australia. In addition, the strontium isotope composition of epidote was determined to test for the potential contribution of seawater in the Northparkes system given the postulated island-arc setting and submarine character of some country rocks. Oxygen isotope geothermometry calculations indicate potassic alteration occurred between ~600° and 700°C in magmatic/mineralized centers, persisting to ~450°C upon lateral transition into propylitic alteration. Across the propylitic facies, temperature progressively decreased outward to &lt;250°C. These temperature estimates and additional data from chlorite geothermometry were utilized to calculate the oxygen and hydrogen isotope composition of the fluid in equilibrium with the sampled minerals. Results show that propylitic fluids spanned a range of compositions with δ18O between 0.5 and 3.7‰ and δD between –49 and –17‰. Comparison of these results with the modeled compositions of meteoric and/or magmatic fluids during their evolution and isotopic exchange with local country rocks shows that a magmatic fluid component must exist across the propylitic halo during its formation. Strontium isotope data from propylitic epidote provide initial (based on formation at ~450 Ma) 87Sr/86Sr values in the range of 0.704099 to 0.704354, ruling out the presence of seawater as a second fluid in the system. Although we cannot exclude magmatic-meteoric mixing, especially toward the fringes of the system, our results support a model in which magmatic-derived fluid is the primary driver of propylitic alteration as it undergoes cooling and chemical equilibration during outward infiltration into country rocks. This is consistent with chemical mass transfer calculations for Northparkes and published chemical-thermodynamic models that only require a magmatic fluid for the production of propylitic assemblages. In view of this and supporting data from other deposits, we suggest that magmatic fluids are essential drivers of propylitic alteration in porphyry systems

    Effects of Chlamydia trachomatis infection on sperm chromatin condensation and DNA integrity

    The present study was performed to investigate the relation of Chlamydia trachomatis infection to sperm chromatin/DNA integrity in a population of infertile men (male partner of infertile couples) from Iran. Blood, semen and first-void urine samples were obtained from 250 infertile men. Data were analysed with regard to the results of (i) serological analysis for specific antibodies to C. trachomatis in serum; (ii) the presence of C. trachomatis and DNA in first-void urine; and (iii) in the semen sample of the male partner, in addition to sperm analysis, four different tests (aniline blue, chromomycin A3, acridine orange and TUNEL) were used to detect sperm chromatin and DNA abnormalities. The main conclusions of the results were: (i) no evidence of C. trachomatis infection in semen samples was found; (ii) sperm DNA fragmentation and chromatin studies were not correlated with C. trachomatis diagnosis; (iii) the percentage of DNA fragmentation is positively correlated with the percentage of immotile sperm but negatively with semen volume, normal morphology; and (iv) in sperm chromatin evaluations, only the percentage of chromatin protamination was related to male age
