    The radiological presentation of Rasmussen aneurysm secondary to pulmonary tuberculosis and COVID-19: A case report

    Rasmussen aneurysm is rare diagnosis occurring in patients with long-standing tuberculosis. TB-COVID-19 co-infection in the context of Rasmussen aneurysm is a rare occurrence, yet new cases are emerging. An elderly male was recently diagnosed with TB-COVID-19 co-infection and presented with sudden onset massive hemoptysis. The patient was diagnosed with Rasmussen aneurysm after being evaluated by computed tomography pulmonary angiogram due to a suggestive clinical presentation. Interventional radiologists planned for embolization of pulmonary artery, with an unidentifiable source. It was suspected that the patient\u27s source of bleeding was hampered due to local tamponade effect or thrombosis of the aneurysm. The patient remained stable after 24 hours on computed tomography pulmonary angiogram. Our case emphasizes the importance of Rasmussen aneurysm as a differential diagnosis when presented with a TB-COVID-19 co-infection and sudden onset of hemoptysis symptomatically and radiologically. The timely diagnosis and management are key to improve mortality

    COVID-19 vaccine-associated thrombosis with thrombocytopenia syndrome (TTS): A systematic review and post hoc analysis

    Background: A new clinical syndrome has been recognized following the COVID-19 vaccine, termed thrombosis with thrombocytopenia syndrome (TTS). The following systematic review focuses on extrapolating thrombotic risk factors, clinical manifestations, and outcomes of patients diagnosed with TTS following the COVID-19 vaccine.Methods: We utilized the World Health Organization\u27s criteria for a confirmed and probable case of TTS following COVID-19 vaccination and conducted a systematic review and posthoc analysis using the PRISMA 2020 statement. Data analysis was conducted using SPSS V25 for factors associated with mortality, including age, gender, anti-PF4/heparin antibodies, platelet nadir, D-dimer peak, time to event diagnosis, arterial or venous thrombi.Results: Of the 175 studies identified, a total of 25 studies with 69 patients were included in this systematic review and post hoc analysis. Platelet nadir (P \u3c .001), arterial or venous thrombi (χ2 = 41.911, P = .05), and chronic medical conditions (χ2 = 25.507, P = .041) were statistically associated with death. The ROC curve analysis yielded D-dimer (AUC = .646) and platelet nadir (AUC = .604) as excellent models for death prediction.Conclusion: Adenoviral COVID-19 vaccines have been shown to trigger TTS, however, reports of patients having received mRNA COVID-19 vaccines are also present. Healthcare providers are recommended to maintain a high degree of suspicion among individuals who have received the COVID-19 vaccine within the last 4 weeks

    Checklist for a complete chronic urticaria medical history : an easy tool

    ABSTARCT: Existing guidelines do not offer a quick, efficient alternative to the patient's recollection of relevant clinical features during anamnesis and physical examination for chronic urticaria (CU). This study aimed to identify specific items reflecting the main characteristics of CU that should be included in a comprehensive medical history for patients with CU. We also aimed to clarify possible eliciting factors for CU to support accurate diagnosis of the disease. METHODS: A panel of postgraduate dermatologists conducted a literature search for relevant studies on CU using Medline, the Cochrane database, and PubMed. RESULTS: We identified82 articles from which we drew a collection of items to inform development of an easy-to-use checklist and collection of items that should be included in a correct medical history. The final version of the checklist included42 items across two areas: essential clues for anamnesis and diagnosis of CU; and typical symptoms/parameters or characteristics according to subtype, etiology, and laboratory findings. Items included time of disease onset; duration, shape, size, color, and distribution of wheals; associated angioedema; atopy; and triggering factors. CONCLUSIONS: Our guide provides an easy-to-use tool to support clinicians to focus, orient themselves, and save time in medical consultations for CU, allowing better diagnosis and management of this disease

    Use and perceptions of information and communication technologies among ecuadorian nurses:A cross-sectional study

    Background: Nurses represent a key group for the implementation of Information and Communication Technologies (ICTs), however, few studies have explored the current use of these technologies among healthcare professionals in developing countries. Our study aims to achieve a better understanding of how Ecuadorian nurses perceive the theoretical advantages and limitations of ICTs, as well as to explore the current use of these technologies in the setting of the professional nursing practice. Methods: We conducted an anonymous survey-based cross-sectional study where 191 nurses rated their frequency of use and level of agreement to specific statements on perceptions related to ICTs. For the statistical analyses, adjusted binomial logistic regressions and the chi-squared test for association were applied. Results: In general, 96.3% of nurses reported the use of ICTs to communicate with colleagues, and 80.1% reported the use of ICTs to communicate with patients. More than 70% of participants agreed that ICTs can be useful to promote professional services, help in the search for new job opportunities and/or professional development, foster health promotion, and improve the workflow with colleagues. Meanwhile, 78.6% of nurses had privacy or security concerns about personal and/or patient information, and 60.6% reported not having enough time neither to learn how to use ICTs nor to use them. Conclusion: High use of ICTs was found among Ecuadorian nurses for communicating with both colleagues and patients. Most of the participants had a positive perception of the use of ICTs in the healthcare practice, particularly among younger nurses. Finally, the major reservations perceived were related to privacy and patient confidentiality, and lack of time to learn how to use, or use ICTs
