    Evaluation of optimizing Monteggia fracture-dislocation care: surgical innovations, radiological insights, and functional rehabilitation in adult patients

    Background: Monteggia fractures, rare in adults, involve proximal ulna fracture and radial head dislocation. Managing these injuries poses challenges, fueling historical debates and driving advancements in internal fixation. Watson Jones' frustration highlights the ongoing pursuit of effective surgical approaches for optimal outcomes and functional limb restoration. his study aims to evaluate Monteggia fracture-dislocation treatment by analyzing radiological outcomes for structural insights and alignment post-surgery. Methods: This prospective observational study, conducted at Swapno general hospital, Mirpur-2, Dhaka, Bangladesh from 1st January 2021 to 31 January 2024, enrolled 30 patients with radiologically confirmed Monteggia fracture-dislocation. Surgical procedures involved creating an interval, anatomical reduction, and fixation, with regular follow-ups assessing outcomes, including range of motion, X-rays, and VAS scores, while statistical analysis utilized SPSS version 23. Results: The highest frequency percentage in the age distribution was observed among individuals aged 41-45, constituting 20% of the total sample, while the lowest frequencies were recorded in the 31-35 and >51 age groups, each representing 10% of the sample. Physical assault emerged as the leading cause of injury, accounting for 40% of cases, followed by road traffic accidents at 36.66% and falls at 23.33%. In terms of final outcomes, the majority of patients (43.33%) achieved a good outcome, while the lowest percentage (10%) resulted in poor outcomes.  Conclusions: In conclusion, addressing Monteggia fracture-dislocation in adults requires navigating inherent complexities. Modern internal fixation methods prove impactful, emphasizing the need for precise classification and stable anatomical reduction

    An Efficient Solution to Travelling Salesman Problem using Genetic Algorithm with Modified Crossover Operator

    The traveling salesman problem (TSP) is a famous NP-hard problem in the area of combinatorial optimization. It is utilized to locate the shortest possible route that visits every city precisely once and comes back to the beginning point from a given set of cities and distance. This paper proposes an efficient and effective solution for solving such a query. A modified crossover method using Minimal Weight Variable, Order Selection Crossover operator, a modified mutation using local optimization and a modified selection method using KMST is proposed. The crossover operator (MWVOSX) chooses a particular order from multiple orders which have the minimum cost and takes the remaining from the other parent in backward and forward order. Then it creates two new offspring. Further, it selects the least weight new offspring from those two offspring. The efficiency of the proposed algorithm is compared to the classical genetic algorithm. Comparisons show that our proposed algorithm provides much efficient results than the existing classical genetic algorithm

By learning the properties of images efficiently, a machine can give better accuracy of classification. Several Hand-Engineered feature coding exists for identifying garments design classes. Recently, Deep Convolutional Neural Networks (CNNs) have shown better performances for different object recognition. Deep CNN uses multiple levels of representation and abstraction that helps a machine to understand the types of data more accurately. In this paper, a CNN model for identifying garments design classes has been proposed. Experimental results on two different datasets show better results than existing two well-known CNN models (AlexNet and VGGNet) and some state-of-the-art Hand-Engineered feature extraction methods

    Internet use and addiction among medical students of Universiti Sultan Zainal Abidin,Malaysia

    The use of Internet has now become indispensable, and the technology has revolutionized the medical education and practice worldwide. Currently, medical students and professionals have an enormous opportunity to keep them always updated with the exponential growth of knowledge because of potential progression of Internet throughout the world that enables them to become a lifelong learner. Internet addiction is a widespread phenomenon among students and academicians at universities in Malaysia. Students use the Internet for recreational purpose and personal and professional development. The Internet has become an integral part of day-to-day life of the university students, including medical students. The aim of the present study was to examine the Internet use and addiction among students of Universiti Sultan Zainal Abidin, Malaysia. Methods: This was a cross-sectional study in which a questionnaire, Internet Addiction Diagnostic Questionnaire, developed by the Center for Internet Addiction, USA, was used. One hundred forty-nine medical students of Universiti Sultan Zainal Abidin participated in this study. Data were analyzed using Statistical Package for the Social Sciences software. Results: The mean scores were 44.9±14.05 and 41.4±13.05 for male and female participants, espectively, which indicated that both the genders were suffering from mild Internet addiction. Conclusion: This study shows almost similar level of Internet usage among medical students irrespective of their socioeconomic background, with no statistically significant (p>0.05) differences, except among the years of study (p=0.007). Overall, from the research data and having worked with this cohort very closely, Universiti Sultan Zainal Abidin medical students can be labeled as wonted and recurring users of the Internet. Nevertheless, it is very difficult to define as Internet addicts or pathological users of the Internet because of small sample size and cross-sectional study

    Ten golden rules for optimal antibiotic use in hospital settings: the WARNING call to action

    Antibiotics are recognized widely for their benefits when used appropriately. However, they are often used inappropriately despite the importance of responsible use within good clinical practice. Effective antibiotic treatment is an essential component of universal healthcare, and it is a global responsibility to ensure appropriate use. Currently, pharmaceutical companies have little incentive to develop new antibiotics due to scientific, regulatory, and financial barriers, further emphasizing the importance of appropriate antibiotic use. To address this issue, the Global Alliance for Infections in Surgery established an international multidisciplinary task force of 295 experts from 115 countries with different backgrounds. The task force developed a position statement called WARNING (Worldwide Antimicrobial Resistance National/International Network Group) aimed at raising awareness of antimicrobial resistance and improving antibiotic prescribing practices worldwide. The statement outlined is 10 axioms, or “golden rules,” for the appropriate use of antibiotics that all healthcare workers should consistently adhere in clinical practice

    The geography and topography of Bangladesh has created many rural and remote areas. The progress of rural electrification in Bangladesh faces specific problems because of the scarcity of power generation. The major electricity grid and a number of isolated local grids cannot economically reach many rural areas. A photovoltaic system is one suitable solution because of its flexibility, low environmental impact and freedom from fuel requirements. Therefore photovoltaic technology is a most feasible option to provide an alternative source of energy in areas where conventional grid electrification is a major issue. Various photovoltaic systems ranging from solar home systems, photovoltaic-pumping systems for drinking water, TV repeaters, public health centers, institutions and office systems have been installed in Bangladesh. More recently PV system are also effectively used in income generating enterprises such as grocery shops, tailoring shops, clinics, restaurants, rice mills, cellular phone services, barber shops, buzzers, and micro utilities which sell electricity to customers in the neighborhood.     An overview of energy profile for Bangladesh: past, present and future uses

    [Abstract]: Bangladesh is a relatively poor country. Although its infrastructure level is relatively low, with few paved roads and poor telephone or electricity access, the potential for growth is quite high and entails a large future increase in energy demand. And although it is the most densely populated agricultural country in the world it also falls among the lowest per capita energy users in the world. However, Bangladesh was recently discovered to contain large reserves of natural gas, a major attraction to foreign investors. Natural gas is also a good source of relatively clean energy for the future, especially in a country where energy demand is growing at a rate of about 10% per year. This survey will first look at the past trends of energy use in Bangladesh, the current energy profile of the country's production, imports, exports and usage of fossil fuels and/or biomass. It will then address the predicted energy increases over the next twenty years, and conclude with an estimation of what the energy mix may look like by the year 202

    Tensile behaviours of activated carbon (AC) coconut shell reinforced recycle polyethylene (PET) hybrid composites

    This research is to develop the natural Activated Carbon (AC) composites prepared from carbon coconut shell reinforced with recycled polyethylene (PET). Carbon coconut shell were selected from in-productive of coconut shell (without endocarp and exorcap) specifically namely as carbon Komeng coconut shell (CKCS) with different weight percentages of AC(2, 4 and 6wt%) and recycled PET(4, 6 and 8wt%) contents. The entire specimens were prepared using SRM (Silicon Rubber Moulds) with dumbbell shape and rectangular shape according to the standard ASTM D2099 and ASTM D256 respectively. The mechanical properties of all samples were investigated to characterise the quality of the samples. The morphological study of reinforced samples was also conducted in this research by using SEM machine. The results showed that tensile stress increases when AC is increased specifically for samples with (PET-2wt%+AC-8 wt%) with maximum tensile stress as 388.37MPa

    Previous research studies investigating the impact of situational variables on project planning effectiveness have not examined how the situational variables work together to influence planning effectiveness. A structural equation model (SEM) has been developed for describing and quantifying the influence of situational factors in project environments and organizational characteristics of performing organizations on project planning effectiveness. The individual effects of directly-observable variables on the project planning process were evaluated using regression analysis. The SEM approach enables the modelling of multiple factors as latent not-directly-measurable variable sets, thus providing a theoretical insight into how individual influence factors work together to determine the effectiveness of project planning efforts. The project environment has a dominant significant influence on the potential effectiveness of project planning efforts.
