    A snapshot of cancer-associated thromboembolic disease in 2018-2019: First data from the TESEO prospective registry

    BACKGROUND: The ever-growing complexity of cancer-associated thrombosis (CAT), with new antineoplastic drugs and anticoagulants, distinctive characteristics, and decisions with low levels of evidence, justifies this registry. METHOD: TESEO is a prospective registry promoted by the Spanish Society of Medical Oncology to which 34 centers contribute cases. It seeks to provide an epidemiological description of CAT in Spain. RESULTS: Participants (N=939) with CAT diagnosed between July 2018 and December 2019 were recruited. Most subjects had advanced colon (21.4%), non-small cell lung (19.2%), and breast (11.1%) cancers, treated with dual-agent chemotherapy (28.4%), monochemotherapy (14.4%), or immune checkpoint inhibitors (3.6%). Half (51%) were unsuspected events, albeit only 57.1% were truly asymptomatic. Pulmonary embolism (PE) was recorded in 571 (58.3%); in 120/571 (21.0%), there was a concurrent deep venous thromboembolism (VTE). Most initially received low molecular weight heparin (89.7%). Suspected and unsuspected VTE had an OS rate of 9.9 (95% CI, 7.3-non-computable) and 14.4 months (95% CI, 12.6-non-computable) (p=0.00038). Six-month survival was 80.9%, 55.9%, and 55.5% for unsuspected PE, unsuspected PE admitted for another reason, and suspected PE, respectively (p<0.0001). The 12-month cumulative incidence of venous rethrombosis was 7.1% (95% CI, 4.7-10.2) in stage IV vs 3.0% (95% CI, 0.9-7.1) in stages I-III. The 12-month cumulative incidence of major/clinically relevant bleeding was 9.6% (95% CI, 6.1-14.0) in the presence of risk factors. CONCLUSION: CAT continues to be a relevant problem in the era of immunotherapy and targeted therapies.     Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure &lt;= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     Vascularização arterial da região do nó sinoatrial em corações suínos: origem, distribuição e quantificação

    O nó sinoatrial, por se encontrar topograficamente instalado como componente inicial do sistema de condução, é responsável pela geração dos impulsos nervosos determinantes da contração cardíaca. Estudos relacionados à morfologia do nó, visando conhecer a origem, trajeto e distribuição dos vasos neste tecido são conhecidos, contudo, no que diz respeito a estes aspectos e aos dados quantitativos da irrigação nodal, no que se refere ao comportamento vascular arterial e a densidade vascular arterial desta região, a literatura é escassa. Com este objetivo foram utilizados 30 corações de suínos SRD, sendo 27 injetados com resina vinílica corada, para análise da origem e trajeto da ANSA (artéria do nó sinoatrial) e 3 corações injetados com solução aquosa de carvão coloidal (tinta nanquim) para proceder à análise estereológica. As artérias atriais originaram-se tanto da artéria coronária direita quanto da esquerda, com predominância da primeira (66,66% e 33,33%, respectivamente). Quando originada da coronária direita, a irrigação ocorreu pelo ramo AADAM (artéria atrial direita cranial medial) em 14 casos e pelos ramos AADAI (artéria atrial direita cranial medial) em 2 casos e AADAL (artéria atrial direita cranial lateral) em 2 casos. Em 9 casos (33,33%) originou-se pela artéria coronária esquerda: quatro pelo ramo AASPL (artéria atrial esquerda caudal lateral), dois pelo ramo AASAI (artéria atrial esquerda cranial intermédia) e três pelo ramo AASAM (artéria atrial esquerda cranial medial). Anastomoses interarteriais, com participação dos vasos responsáveis pela irrigação do território do nó sinoatrial foram observadas na maioria dos casos (25 corações). O Volume do órgão ou Volume Referência (V(ref)) foi de 35,32x10(4)µm³. Para as variáveis estereológicas analisadas, a estimação da densidade de comprimento do vaso (Lv) foi de 766; o comprimento do vaso (L) - mm - foi de 27,06x10(5)µm; a densidade de superfície de área (Sv) foi de 182 e a superfície de área (S) - mm² - foi de 64,3x10(6)µm². A estimação da densidade numérica vascular (Nv(vasc)), quantidade de vasos por unidade de volume (cm³), foi de 2,19 10-5 e o número total de vasos no órgão (N(vasc)), estimado pelo método dissector físico em combinação com a estimativa do número de Euler (Xv), foi de 773,6832 x10-2. A elevada densidade vascular e do número total de vasos na região do nó sinoatrial de suínos sugere a existência de uma complexa e densa rede vascular perinodal, ratificando a importância deste marca-passo pelo seu suprimento sangüíneo

    Do Antiangiogenics Promote Clot Instability? Data from the TESEO Prospective Registry and Caravaggio Clinical Trial.

     Venous thromboembolism (VTE) is a common complication in cancer patients. Much of its morbidity stems from the development of fatal pulmonary embolisms (PE). Little is known about the factors involved in clot stability, with angiogenesis possibly being implicated.  The database is from the TESEO prospective registry that recruits cancer patients with VTE from 41 Spanish hospitals. Independent validation was conducted in a cohort from the Caravaggio trial. The objective is to evaluate the association between exposure to antiangiogenic therapies and the PE/VTE proportion in oncological patients.  In total, 1,536 subjects were evaluated; 58.4% (n = 894) had a PE and 7% (n = 108) received antiangiogenic therapy (bevacizumab in 75%). The PE/VTE proportion among antiangiogenic-treated individuals was 77/108 (71.3%) versus 817/1,428 (57.2%) among those receiving other alternative therapies (p = 0.004). The effect of the antiangiogenics on the PE/VTE proportion held up across all subgroups except for active smokers or those with chronic obstructive pulmonary disease. Exposure to antiangiogenics was associated with increased PEs, odds ratio (OR) 2.27 (95% CI, 1.42-3.63). In the Caravaggio trial, PE was present in 67% of the individuals treated with antiangiogenics, 50% of those who received chemotherapy without antiangiogenic treatment, and 60% without active therapy (p = 0.0016).  Antiangiogenics are associated with increased proportion of PE in oncological patients with VTE. If an effect on clot stability is confirmed, the concept of thrombotic risk in cancer patients should be reconsidered in qualitative terms

    Esta antología del pensamiento crítico brasileño, cuyo recorte temporal abarca desde mediados del siglo pasado hasta el presente, supone un esfuerzo por presentar algunos de los principales ejes y autores/as que organizaron la reflexión sobre la sociedad brasileña desde entonces. No se trata de agotar los temas y las cuestiones que fueron emergiendo a lo largo de este tiempo, tampoco los/las autores/as más importantes o representativos/as. Semejante tarea sería siempre frustrante e incompleta. Más bien, se busca ofrecer un panorama general respecto de cómo el pensamiento crítico fue ganando, desde la década de 1950, una relativa organización e incluso cierta acumulación intelectual en el país. La variedad de períodos, autores/as y perspectivas aquí presentes no borra un rasgo relativamente común al pensamiento crítico brasileño: se trata de una reflexión históricamente orientada, que entiende que el proceso de formación de una sociedad de origen colonial y estructurada a partir de la esclavitud y de la gran propiedad rural genera efectos de larga duración en la dinámica social, haciéndose sentir hasta el presente. Presente duro, con fuertes retrocesos sociales y políticos, que hace más urgente que nunca la tarea de rescatar y actualizar el pensamiento crítico. Como decía Francisco de Oliveira en La tarea de la crítica, la excepción obliga a pensar excepcionalmente. Esperamos que la presente antología contribuya en esa dirección y que, al mismo tiempo, sirva para acercar más Brasil al resto de América Latina y el Caribe.     O artigo aborda o papel desempenhado pela Sociologia na análise dos processos de transformação das sociedades latino-americanas, no acompanhamento do processo de construção do Estado e da Nação, na problematização das questões sociais na América Latina. São analisados seis períodos na Sociologia na América Latina e no Caribe: I) a herança intelectual da Sociologia ; II) a sociologia da cátedra; III) O período da "Sociologia Científica" e a configuração da "Sociologia Crítica"; IV) a crise institucional, a consolidação da "Sociologia Crítica" e a diversificação da sociologia; V) a sociologia do autoritarismo, da democracia e da exclusão; VI) a consolidação institucional e a mundialização da sociologia da América Latina (desde o ano de 2000), podendo-se afirmar que os traços distintivos do saber sociológico no continente foram: o internacionalismo, o hibridismo, a abordagem crítica dos processos e conflitos das sociedades latino-americanas e o compromisso social do sociólogo.<br>The article focuses on the role played by Sociology in the analysis of processes of change in Latin American societies, in the process of construction of Nation and State, in the debate of social issues in Latin America and the Caribbean. Six periods in Sociology in Latin America and the Caribbean are examined: I) sociology's intellectual legacy; II) sociology as a cathedra; III) the period of "Scientific Sociology"; IV) the institutional crisis, the consolidation of "Critical Sociology", and the diversifying of sociology; V) sociology of authoritarianism, democracy and exclusion; VI) institutional consolidation and globalization of Latin American sociology (since 2000). It may be said that the distinctive features of sociological knowledge in the continent were: internationalism, hybridism, the critical approach to processes and conflicts of Latin American societies, and the sociologist social commitment
