    Pilomatrix carcinoma of the lacrimal caruncle: a case report.

    A 45-year-old man presented with a 3-month history of a mass located in the caruncle of his right eye. An incisional biopsy had been performed one month prior by another specialist, and the histopathology report showed basal cell carcinoma. The mass was completely excised with a 2 mm safety margin, and the large conjunctival defect was reconstructed with one sheet of amniotic membrane allograft. A histological diagnosis of pilomatrix carcinoma was established. To prevent recurrence after surgery, we added bevacizumab (25 mg/mL, 1.25 mg/mL per drop) eye drops four times per day for three months. At the one-year follow-up, the patient showed no evidence of local recurrence or distant metastasis after initial excision and remains under close follow-up. Pilomatrix carcinoma should be considered in the differential diagnosis of a caruncular mass

    Gestational trophoblastic neoplasia after miscarriage with dilatation and curettage with normal histological findings

    Gestational trophoblastic neoplasia may develop after a molar, term, ectopic pregnancy, or an abortion. The diagnosis of gestational trophoblastic neoplasia can be made solely based on changes in human chorionic gonadotropin levels without pathologic confirmation. It is important to distinguish molar pregnancy from that disease, as treatment for these entities differs. However, gestational trophoblastic neoplasia developing after a term or ectopic pregnancy, or an abortion may be difficult to diagnose, because there is no tissue confirmation. In such cases, the time between a previous pregnancy event and the current event, and an inconsistency between very high levels of human chorionic gonadotropin and the size of lesions in the uterine cavity may be warning signs of gestational trophoblastic neoplasia. The role of curettage in the treatment of the disease is limited. We present a case of gestational trophoblastic neoplasia that developed after an abortion, serving as a reminder illustration that gestational trophoblastic neoplasia can develop not only after molar pregnancies, but also after other pregnancy events

    Brevibacillus gelatini sp nov., isolated from a hot spring

    Two Gram-stain-positive, moderately thermophilic, endospore-forming, rod-shaped, motile bacteria designated PDF4(T) and PDF10, were isolated from Camkoy hot spring in the provinces of Aydin, Turkey and were characterized in order to determine their phylogenetic position. 16S rRNA gene sequence analysis revealed that the two strains belonged to the genus Brevibacillus. Strain PDF4(T) showed highest 16S rRNA gene sequence similarity to strain PDF10 (99.5 %), Brevibacillus brevis DSM 30(T) (98.9 %), Brevibacillus parabrevis DSM 8376(T) (98.6 %) and Brevibacillus formosus DSM 9885(T) (98.5 %); similarities to other species of the genus Brevibacillus were less than 98.5 %. The predominant fatty acids of strain PDF4(T) were anteiso-C-15 : 0 (60.0 %) and iso-C-15 : 0 (22.3 %). The polar lipids of strain PDF4(T) consisted of diphosphatidylglycerol, phosphatidylglycerol, phosphatidylethanolamine, phosphatidylmonomethylethanolamine, an unknown phospholipid, two unknown lipid, an unknown aminophospholipid and two unknown aminolipids. MK-7 was detected as a sole respiratory quinone, and the cell wall of strain PDF4(T) contained meso-diaminopimelic acid. The DNA G + C content of strain PDF4(T) was 51.7 mol%. DNA-DNA hybridization showed less than 60 % relatedness between strain PDF4(T) and type strains of the most closely related species given above. Based on these data, the two strains are considered to represent a novel species of the genus Brevibacillus, for which the name Brevibacillus gelatini sp. nov. is proposed.     Real-time elastography for differentiation of breast lesions

    Backroung: To investigate the diagnostic performance of the elastography-based strain index ratio in the differential diagnosis of malignant and benign breast lesions. Material/Methods: Seventy-nine breast masses that were classified as BI-RADS category 3, 4, and 5 on B-mode ultrasonography (US) were further prospectively evaluated by real-time sonoelastography (RTE). To obtain an optimal cut-off value of the strain ratio for differentiating between malignant and benign breast lesions, RTE findings were compared with histopathology of core needle biopsy samples or with ultrasound follow-up data of the analyzed masses. Results: Seventy-nine breast lesions [BI-RADS category 3 (n=15), BI-RADS category 4 (n=34), and BI-RADS category 5 (n=30)] were classified as malignant (n=36) or benign (n=43). The mean strain index value was 6.59±3.44 (range 0.6-14) for malignant lesions and 2.79±2.16 (range 0.6-8.7) for benign lesions, respectively (p<0.05). As regards the detection of malignant lesions, US was characterized by sensitivity and specificity of 100% (CI 95%; 88-100) and 90% (CI 95%; 76-97), respectively. When an optimal value of the strain ratio (4.25) was obtained by ROC curve analysis, the sensitivity and specificity for diagnosing malignant lesions were 86% (CI 95%; 70-95) and 76% (CI 95%; 60-87), respectively. Conclusions: RTE can play an important role in the differentiation between malignant and benign breast masses, but it should be used in conjunction with ultrasonography

    Data Model for the Collaboration between Land Administration Systems and Agricultural Land Parcel Identification Systems

    The Common Agricultural Policy (CAP) of the European Union (EU) has dramatically changed after 1992, and from then on forward the CAP focussed on the management of direct income subsidies instead of production-based subsidies. For this purpose, Member States (MS) are expected to establish Integrated Administration and Control System (IACS), including a Land Parcel Identification System (LPIS) as the spatial part of IACS. Different MS have chosen different solutions for their LPIS. Currently, some MS based their IACS/LPIS on data from their Land Administration Systems (LAS), and many others use purpose built special systems for their IACS/LPIS. The issue with different IACS/LPIS is that they do not have standardized structures; rather, each represents a unique design in each MS, both in the case of LAS or special systems. In this study, we aim at designing a core data model for those IACS/LPIS based on LAS. For this purpose, we make use of the ongoing standardization initiatives for LAS (Land Administration Domain Model: LADM) and IACS/LPIS (LPIS Core Model: LCM). The data model we propose in this study implies the collaboration between LADM and LCM and includes some extensions. Some basic issues with the collaboration model are discussed within this study: registration of farmers, land use rights and farming limitations, geometry/topology, temporal data management etc. For further explanation of the model structure, sample instance level diagrams illustrating some typical situations are included.     The utility of ADC measurement techniques for differentiation of low- and high-grade clear cell RCC

    Purpose: To evaluate the diffusion properties of clear cell renal cell carcinoma (ccRCC) on magnetic resonance imaging (MRI) concerning their Fuhrman nuclear grades and sizes, and to compare the diagnostic performance of two ROI placement techniques for apparent diffusion coefficient (ADC) measurement (entire mass vs. only the darkest region of the mass). Material and methods: Fifty-one ccRCC were enrolled in the study and grouped into low-grade ccRCC (Fuhrman grade 1 and 2, n = 37) and high-grade ccRCC (Fuhrman grade 3 and 4, n = 14). Selective ADC (Sel-ADC) measurement was performed by placing a circular ROI that included the darkest region of the tumour on ADC map images. Extensive ADC (Ext-ADC) measurement was performed by drawing an ROI that covered the entire tumour. Results: The Sel-ADC value was lower in high-grade ccRCC (p = 0.019), whereas the Ext-ADC value did not show a statistically significant difference (p = 0.42). Sel-ADC value of a ≤ 1.405 mm2/s has a sensitivity, specificity, positive predictive value, negative predictive value, and accuracy value of 78.6, 72.2, 73.87, 77.13, and 75.4, respectively, to differentiate high-grade from low-grade ccRCC. The size and Fuhrman grade of the ccRCC were inversely correlated with the Sel-ADC value; however, the correlations were weak (r = -0.322, p = 0.021 and r = -0.376, p = 0.006, respectively). There was no difference between ADC values of small (≤ 4 cm) and large (> 4 cm) ccRCCs. Conclusions: The ADC value of the darkest region in solid part of the ccRCC may play a role in predicting the nuclear grade of ccRCC

    Yedi yaşında erkek çocukta servikal bölgede lenf düğümü büyümesi, ateş, sedimentasyon artışı bulguları mevcuttur. Boyutları 3x2.5x1.5 cm olan lenf düğümünün seri kesitlerinde kapsül altında, hilus bölgesinde iğsi hücrelerle birlikte inflamatuar hücreler ve damar yapılarında artış izlendi. ımmunhistokimyasal çalışmada iğsi hücreler, kas spesifik aktin (+), vimentin (+), LCA (-), CD3 (-), CD79(-), FVIII (-), desmin (-) olarak değerlendirildi. Histokimyasal yöntemle Ziehl-Neelsen (-) di. Histopatolojik ve klinik bulgularla birlikte lenf düğümünün inflamatuar psödotümörü (ıPT) tanısı konulmuştur. Lenf nodunun inflamatuar psödotümörü lenfadenopatinin benign nedenleri arasında son zamanlarda tanımlanmıştır. Ayırıcı tanı diğer reaktif durumları, Castleman hastalığını, malign lenfoma (Hodgkin lenfoma, periferikşücreli lenfoma) ve malign fibröz histiositomayı içerir. Olgu seyrek görülmesi nedeniyle literatür eşliğinde sunulmuştur.A 7 year-old boy presented with cervical lymph node enlargement, fever and elevated erythrocyte sedimentation rate. A neck mass measuring 3x2.5x1.5 cms was surgically removed. Histologically, the process showed a proliferation of spindle cells, associated with a mixture of inflammatory cells and small blood vessels involving the connective tissue framework (hilum, trabecula, capsule) of the lymph node. Immunohistochemical analysis showed spindle cells positive for muscle-spesific actin and vimentin, but negative for desmin. Histochemical, Ziehl-Neelsen stain was negative. With these clinical and histopathologic findings a diagnosis of “ inflammatory pseudotumor of lymph nodes” was made. Inflammatory pseudotumor of the lymph nodes is a rare, recently described benign cause of lymphadenopathy. The differential diagnosis includes other reactive processes, Castleman's dissease, malignant lymphoma (Hodgkin's lymphoma, peripheral T cell lymphoma) and malignant fibrous histiocytoma.
