Changes of Interleukins in Patients with Nosocomial Pneumonia Against a Background of Traumatic Brain Injury: Role in the Diagnosis and Monitoring of Disease

Introduction. Nosocomial pneumonia (NP) is the second leading frequency and mortality among nosocomial infections. NP is a frequent complication of severe traumatic brain injury (TBI). The difficulty in diagnosis and monitoring of disease NP on the background of TBI is that the usual signs NP "masked" manifestations of the underlying disease.The aim of our study was to improve the diagnosis and assess the effectiveness of the treatment of nosocomial pneumonia with clarithromycin by studying of serum interleukins in patients with nosocomial pneumonia on the background of traumatic brain injury.Materials and methods. We determined levels of TNFα, IL-4, IL-6, IL-8, IL-10. We examined 45 patients with isolated TBI, 44 patients with NP (standard treatment scheme), 49 patients with NP, each therapy with clarithromycin. The control group was 33 relatively healthy people.Conclusions. The study revealed a significant increase in the concentration of interleukin TNFα, IL-6, IL-8, IL-10 in the serum of patients with NP on the background of TBI compared with patients with isolated TBI and healthy individuals. Our study found significantly lower concentrations of interleukin TNFα, IL-6, IL-8, IL-10 in the serum of patients after treatment with the addition of clarithromycin compared with patients with standard therapy

At the present time for confident entrance to the European transport system with its standards, technical, organizational and ecological norms, the transport network of regions and the whole Ukraine requires constant qualitative updating. A number of important actions in this direction are already carried out, at the same time, to maintain an effective utilization of the international transport corridors in new conditions, it is important to reach the ultimate coordination of actions of all participants in this sphere.Complexity and immensity of the problems connected to the improvement of management of transport systems, leads to the necessity of an overall problem solution of the balanced transport development, transport systems and multi-modal transport development as a basic element of domestic potential growth of the state. Maintenance of these processes should be carried out through working out the programs of national transport system development and regional development programs in this direction. Therefore a research problem is to lay out the recommendations and methodical approaches to the problems of realization the above-stated programs and development strategy.In the presented article the basic problems of realization of the assigned tasks are designated, the principles underlying their performance are constructed, the legislative base which represents a legal mechanism of programs realization is defined, and recommendations which will give the chance to provide dynamic balanced development of a transport network of the country are presented. Політичні евфемізми як складова сучасної мови ЗМІ (Political euphemism as a component of modern media languages)

У статті проаналізовано евфемізми, що продукуються в сучасній мові ЗМІ, тобто властиві мас-медійному соціалекту. ЗМІ активно репрезентують вітчизняне й зарубіжне політичне життя, тож політичні евфемізми є важливою складовою сучасної мови ЗМІ. Поняття «політичний евфемізм» межує з поняттям «політкоректний термін». У статті також проаналізовано функції евфемізмів і продуктивні теми, що потребують «камуфлювання». (The article analyzes the euphemism that produced in the modern media language that is characterized in the mass media languages. The mass media represents domestic and foreign political life therefore political euphemisms are an important component of the modern mass media language. Political euphemisms are an important component of the modern media language. The usage of researched components is specified of low limits of legal restrictions of the usage of linguistic means to express direct negative assessment in texts of the political communication; keeping all rules of cultural correctness in modern society; the high efficiency impact on the audience in implicit way of transmitting information. Linguists suggests on a link of euphemism with a political correctness. The term is describing the style of speech, ideas, and politics, behavior that aimed at minimizing the image of racial, religious, cultural and other groups. The concept «political euphemism» borders with the concept of «politically correct term», which refers to the euphemism that in modern condition, become a radical form of indirect communication. It is also analyzed the performance of the theme of euphemisms in the article that requires «camouflage»:It is sensitive topics about age, race, gender, social and property status, health and appearance, physical development, unemployment, etc., which require the use of a secondary nomination units to avoid insulting or abusive relationships for the potential recipient. The creation of euphemistic nominations in the modern mass media happens with politically correct purpose, which associated in language with certain themes that belongs to traditional taboo in different cultures. It is necessity of ethical sense denotata from the sphere of the end of human life, smuggling, corruption and other immoral action of people that causes the active usage of euphemisms, which are the most suitable for communication than the direct nomination. It gives the reason to confirm that euphemisation is the active process of hiding, camouflage of «frighten» vocabulary in modern mass media. The article shows the prospects for the further research that is shown in studying the studious euphemisms in Ukrainian modern mass media.

Евфемізми сучасних україномовних ЗМІ: соціальний аспект

У статті проаналізовано евфемізми, що продукуються і функціонують в українській мові сучасних ЗМІ, тобто характерні для українського мас-медійного соціалекту. Дослідження репрезентує результати впливу соціальних факторів на вживання евфемізмів – мовленнєвих фактів, що за своєю природою підпорядковуються соціальним орієнтирам. Евфемізми інтерпретовано в соціальному аспекті. (This paper deals with the euphemisms that are produced and function in the Ukrainian language of modern media. The aim of using euphemisms is the desire to avoid communication conflicts and failures, camouflage reality; encrypting messages from other parties. The work represents the results of the influence of social factors on the usage of euphemisms. The main causes for the social aspects of euphemisms was revealed: 1) human speech – a means of communication that has a social nature; 2) the man and his inner world is social, public-oriented and community driven; 3) speaker’s assessment can be regarded as rude, tactless; 4) psychological mechanisms of euphemistically replacement are activate the communication 5) the selection of such signs, which can soften harsh words and expressions, to mask, to cover up the essence of the phenomenon; 6) selection of euphemisms depends on the situation of communication. In the article named spheres of social life, which often use euphemisms. The spheres of social life include diplomacy; the repressive actions of the authorities; state, military secrets; the activities of the military, intelligence, police, criminal investigation department and other organs of human power, the activity of which is hidden; the distribution and maintenance; relations between different social groups, the status of such groups; some professions, euphemistically replacement names which raises the prestige of these professions or hide negative impression from their direct equivalent. The present paper addresses essential discourse elements of the English language of science in light of the so-called Anglo-American intellectual style. Emphasised here are: various linguistic devices of compression (writing succinctly, avoiding verbosity and negation etc.), keeping subject and verb close together, and common revision patterns. Key standardized discourse models and academic vocabulary – as well as their Ukrainian equivalents – are also discussed. Special attention is given to Active vs. Passive voice usage, and adequate language framing of the author’s viewpoint in scientific discourse
