    Prevalencia y pronóstico de la lepra en Pinar del Río

    Introduction: leprosy is a chronic, mildly contagious granulomatous disease caused by the bacillus Mycobacterium leprae, affecting chiefly the skin, the peripheral nerves, the mucosa of the upper respiratory tracts, the eyes and other structures. Objective: to estimate the real prevalence by the calculation of the hidden prevalence and the prognosis of leprosy in Pinar del Río for the year 2013. Material and method: an observational descriptive study of prospective character was carried out; for obtaining the information, compulsory declaration cards and the epidemiological surveys of the leprosy cases were reviewed. Several indicators were calculated: leprosy prevalence rate, hidden prevalence, and the prognosis of the disease by the method of exponential smoothing. Results: in the last years, the prevalence of leprosy in P. Rio has a tendency towards decrease, existing a difference between the known prevalence and the real prevalence, on the increase of the latter to the sum of the hidden prevalence of 5 cases, with a forecast in the disease for 2013 similar to the previous year. Conclusion: the study revealed that although leprosy does not constitute a health problem in the Province, there are gaps in the surveillance system, identified after calculation of the estimate of hidden prevalence representing the number of leprosy cases, not diagnosed in the Province in the last five years, which will have an influence in the operational and epidemiological indicators of the leprosy program.Introducción: la lepra es una enfermedad infecciosa crónica, granulomatosa, transmisible, poco contagiosa, causada por el bacilo Mycobacterium leprae, que afecta principalmente la piel, los nervios periféricos, la mucosa de las vías respiratorias superiores, los ojos y otras estructuras. Objetivo: estimar la prevalencia real a partir del cálculo de la prevalencia oculta y el pronóstico de la lepra en Pinar del Río para el 2013. Material y método: se realizó un estudio observacional descriptivo de carácter prospectivo, para la obtención de la información se revisaron las tarjetas de declaración obligatoria y las encuestas epidemiológicas de los casos de lepra. Se calcularon diferentes indicadores: tasa de prevalencia de lepra, la prevalencia oculta y el pronóstico de la enfermedad por el método de alisamiento exponencial. Resultados: La prevalencia de la lepra en la provincia en los últimos años tiene una tendencia al descenso, existiendo una diferencia entre la prevalencia conocida y la prevalencia real, al incrementarse esta última con la sumatoria de la prevalencia oculta que fue de 5 casos, con un pronóstico para la enfermedad para el 2013, similar al año anterior. Conclusión: el estudios reveló que aunque la lepra no constituye un problema de salud en la provincia, existen brechas en el sistema de vigilancia, que se identifican con el calculo del estimado de la prevalecía oculta que representa el número de casos de lepra, que no se diagnosticaron en la provincia en los últimos cinco años, lo que influirá en los indicadores operacionales y epidemiológicos del programa de lepra

    A comparison of Covid-19 early detection between convolutional neural networks and radiologists

    [EN] Background The role of chest radiography in COVID-19 disease has changed since the beginning of the pandemic from a diagnostic tool when microbiological resources were scarce to a different one focused on detecting and monitoring COVID-19 lung involvement. Using chest radiographs, early detection of the disease is still helpful in resource-poor environments. However, the sensitivity of a chest radiograph for diagnosing COVID-19 is modest, even for expert radiologists. In this paper, the performance of a deep learning algorithm on the first clinical encounter is evaluated and compared with a group of radiologists with different years of experience. Methods The algorithm uses an ensemble of four deep convolutional networks, Ensemble4Covid, trained to detect COVID-19 on frontal chest radiographs. The algorithm was tested using images from the first clinical encounter of positive and negative cases. Its performance was compared with five radiologists on a smaller test subset of patients. The algorithm's performance was also validated using the public dataset COVIDx. Results Compared to the consensus of five radiologists, the Ensemble4Covid model achieved an AUC of 0.85, whereas the radiologists achieved an AUC of 0.71. Compared with other state-of-the-art models, the performance of a single model of our ensemble achieved nonsignificant differences in the public dataset COVIDx. Conclusion The results show that the use of images from the first clinical encounter significantly drops the detection performance of COVID-19. The performance of our Ensemble4Covid under these challenging conditions is considerably higher compared to a consensus of five radiologists. Mitjançant una selecció de projectes, es mostra el rigor acadèmic i la capacitat d’anàlisi i experimentació que tenen els alumnes del Grau de Disseny i dels diferents Màsters i Postgraus d’EINA. Aquesta filosofia, que fomenta el potencial innovador del disseny i l’art, també es recull en la praxis professional que duen a terme els nostres alumni. Tots els projectes publicats són una síntesi de l’aprenentatge enriquidor, obert a diferents sistemes de pensament i llenguatges, amb què EINA treballa des de la seva fundació el 1967. Una metodologia en constant evolució a d’avançar-se a les necessitats i exigències de la nostra societat canviant, per poder donar resposta als nous reptes i fer realitat nous productes, nous serveis i noves experiències. En definitiva, aquesta publicació és el testimoni de la trajectòria de l’escola com a plataforma de cultura i coneixement de generacions de professionals del disseny i l’art que participen activament en el desenvolupament d’una societat més sostenible, ètica, reflexiva i compromesa.Esta publicación es una recopilación del trabajo realizado por los estudiantes y exestudiantes de EINA. All of the published projects are a synthesis of the enriching learning style, open to different systems of thought and language, which EINA has prioritized since its founding in 1967.     A decline in inflammation is associated with less depressive symptoms after a dietary intervention in metabolic syndrome patients: a longitudinal study

    BACKGROUND: Metabolic syndrome (MetS) and depression have become two prevalent diseases worldwide, whose interaction needs further investigation. Dietary treatment for weight loss in patients with MetS may improve depressive manifestations, however, the precise interactive pathways remain uncertain. Therefore, the aim of this study was to examine the effects of a hypocaloric diet designed to reduce MetS features on self-perceived depression and the possible underlying factors. METHODS: Sixty subjects (Age:50 +/- 1 y; BMI:36.1 +/- 0.6 kg/m2) with MetS were selected from the RESMENA study (control and intervention) after they completed the 6-months hypocaloric treatment and rated for depressive symptoms using the Beck Depression Inventory (BDI). Anthropometric and biochemical measurements including leptin, C-reactive protein (CRP) and insulin levels were evaluated. RESULTS: Depressive symptoms decreased during the weight loss intervention, with no differences between both dietary groups (control group -4.2 +/- 0.8 vs RESMENA group -3.2 +/- 0.6, P = 0.490). The number of criteria of the MetS was higher among subjects with more somatic-related depressive symptoms at baseline (B = 1.032, P-trend = 0.017). After six months of dietary treatment, body weight decreased in all subjects (-8.7%; confidence interval (95%CI) = 7.0-9.7) and also self-perceived depression (-37.9%; 95%CI = 2.7-4.9), as well as circulating leptin (-20.1%; 95%CI = 1.8-6.8), CRP (-42.8%; 95%CI = 0.6-3.0) and insulin (-37.7%; 95%CI = 4.1-7.2) concentrations. The decrease in BDI was significantly associated with declines in body fat mass (B = 0.34, 95%CI = 0.11-0.56) and also with the decrease in leptin (B = 0.16, 95%CI = 0.04-0.28) and CRP (B = 0.24, 95%CI = 0.01-0.46) concentrations. CONCLUSIONS: The decrease in depressive manifestations after a weight loss intervention was related with adiposity, CRP and leptin in subjects with MetS

    Spatially restricted drivers and transitional cell populations cooperate with the microenvironment in untreated and chemo-resistant pancreatic cancer

    Pancreatic ductal adenocarcinoma is a lethal disease with limited treatment options and poor survival. We studied 83 spatial samples from 31 patients (11 treatment-naïve and 20 treated) using single-cell/nucleus RNA sequencing, bulk-proteogenomics, spatial transcriptomics and cellular imaging. Subpopulations of tumor cells exhibited signatures of proliferation, KRAS signaling, cell stress and epithelial-to-mesenchymal transition. Mapping mutations and copy number events distinguished tumor populations from normal and transitional cells, including acinar-to-ductal metaplasia and pancreatic intraepithelial neoplasia. Pathology-assisted deconvolution of spatial transcriptomic data identified tumor and transitional subpopulations with distinct histological features. We showed coordinated expression of TIGIT in exhausted and regulatory T cells and Nectin in tumor cells. Chemo-resistant samples contain a threefold enrichment of inflammatory cancer-associated fibroblasts that upregulate metallothioneins. Our study reveals a deeper understanding of the intricate substructure of pancreatic ductal adenocarcinoma tumors that could help improve therapy for patients with this disease

    Este libro es el resultado de buscar y rebuscar, acuciadas en un primer momento por una natural curiosidad hacia el siglo XIX, en el baúl de nuestras abuelas o incluso bisabuelas. Y nos encontramos no sólo con el pasado sino con nuestro pasado, rico, complejo, sorprendente en su infinita variedad de hallazgos. Experimentamos un sentimiento de ternura por esas mujeres y de profunda solidaridad ante el esfuerzo que realizaron, muchas veces en condiciones inhóspitas, incomprendidas por sus contemporáneos, varones y mujeres. No sólo mujeres con nombre propio sino mujeres generalmente anónimas forman una parte esencial de la historia no contada, oculta(da) en el polvo acumulado durante el transcurso de los años. Nuestra tarea, nuestra pasión es desenvolverla, descubrirla, deconstruirla. Un acto compartido entre muchas mujeres y algunos varones. En mayor o menor grado, cada artículo en Mujeres y cultura en la Argentina del siglo XIXcontribuye a este fin y también al de la construcción –la re-construcción- de la historia de mujeres en la Argentina. Hay todavía mucho por revolver en esos arcones. Sabemos que hay nombres escondidos todavía en publicaciones que debemos encontrar. Nombres de mujeres, sus labores, que han sido muy poco investigadas o que sencillamente desconocemos. Se trata de hacer visible una producción ignorada y una tradición desdibujada

    El presente volumen recoge las contribuciones presentadas al II Congreso Internacional sobre Tarteso, Nuevas Fronteras, que tuvo lugar en Mérida entre los días 17 y 19 de noviembre de 2021. Su lectura permite un viaje desde el extremo oriental del Mediterráneo hasta el suroeste de la península ibérica, mostrando las diversas realidades históricas acontecidas en este territorio durante la I Edad del Hierro. El objetivo de esta publicación es mostrar la situación que atravesaba el Mediterráneo durante los años de surgimiento y desarrollo de la cultura tartésica para así comprender mejor la formación y evolución de dicha cultura. El conocimiento de Tarteso ha evolucionado sensiblemente en la última década, desde la celebración y publicación de las actas del I Congreso Internacional, Tarteso. El emporio del metal (Almuzara, 2013).
