    Role of three side support ankle鈥揻oot orthosis in improving the balance in children with spastic diplegic cerebral palsy

    Cerebral palsy (CP) is a heterogeneous group of permanent, non-progressive motor disorders of movement and posture. Ankle鈥揻oot orthoses (AFOs) are frequently prescribed to correct skeletal misalignments in spastic CP. The present study aims to evaluate the effect of the three side support ankle鈥揻oot orthosis on standing balance of the spastic diplegic CP children. Thirty spastic diplegic CP children participated in this study from both sexes. They were divided randomly intotwo age and sex matched groups: (Group I: study group and Group II: control group). The degree of spasticity was evaluated by passive movement for both limbs, while the child was completely relaxed. The Biodex stability system, was used for the assessment of the dynamic postural control of all diplegic children. Also the system measures the subject鈥檚 ability to control the platform鈥檚 angle of tilt. The patient鈥檚 performance is noted as stability index which represents the variance of the platform displacement in degrees. Every patient in the study group was exercised on three side support ankle鈥揻oot orthosis for 30 min, three times weekly, for 6 months, also they received the same therapeutic exercise program which was given to the control group. The results revealed no significant difference as regards the pre-treatment mean values of all stability indices in both the control and the study groups (P< 0.05). However comparison between post-treatment mean values of allstability indices in both groups showed significant improvement in favor of the study group (p<0.05). In conclusion: Uses of the three side support ankle鈥揻oot orthosis in addition to physical exercise program is highly useful in rehabilitation of spastic diplegic cerebral palsy children as theyenabled them to gain more balance control and postural reactions

    Global variation in anastomosis and end colostomy formation following left-sided colorectal resection

    Background End colostomy rates following colorectal resection vary across institutions in high-income settings, being influenced by patient, disease, surgeon and system factors. This study aimed to assess global variation in end colostomy rates after left-sided colorectal resection. Methods This study comprised an analysis of GlobalSurg-1 and -2 international, prospective, observational cohort studies (2014, 2016), including consecutive adult patients undergoing elective or emergency left-sided colorectal resection within discrete 2-week windows. Countries were grouped into high-, middle- and low-income tertiles according to the United Nations Human Development Index (HDI). Factors associated with colostomy formation versus primary anastomosis were explored using a multilevel, multivariable logistic regression model. Results In total, 1635 patients from 242 hospitals in 57 countries undergoing left-sided colorectal resection were included: 113 (6路9 per cent) from low-HDI, 254 (15路5 per cent) from middle-HDI and 1268 (77路6 per cent) from high-HDI countries. There was a higher proportion of patients with perforated disease (57路5, 40路9 and 35路4 per cent; P < 0路001) and subsequent use of end colostomy (52路2, 24路8 and 18路9 per cent; P < 0路001) in low- compared with middle- and high-HDI settings. The association with colostomy use in low-HDI settings persisted (odds ratio (OR) 3路20, 95 per cent c.i. 1路35 to 7路57; P = 0路008) after risk adjustment for malignant disease (OR 2路34, 1路65 to 3路32; P < 0路001), emergency surgery (OR 4路08, 2路73 to 6路10; P < 0路001), time to operation at least 48 h (OR 1路99, 1路28 to 3路09; P = 0路002) and disease perforation (OR 4路00, 2路81 to 5路69; P < 0路001). Conclusion Global differences existed in the proportion of patients receiving end stomas after left-sided colorectal resection based on income, which went beyond case mix alone

    The past 5 years have witnessed an enormous growth in the field of Z-selective olefin metathesis. The development of a new class of cyclometalated ruthenium-based catalysts has extended the utility of olefin metathesis to the synthesis of useful Z-olefin-containing small molecules, polymers, and natural products. This review highlights the recent advances in the area of Z-selective olefin metathesis employing cyclometalated ruthenium alkylidene catalysts, with particular focus on its applications and mechanistic basis. A deeper understanding of structure鈥揳ctivity relationships should aid in the future design of even more active and selective olefin metathesis catalysts

    Lung Infection by Human Bocavirus Induces the Release of Profibrotic Mediator Cytokines In Vivo and In Vitro

    Human Bocavirus subtype 1 (HBoV1) is associated with respiratory diseases and may contribute to chronic lung diseases by persisting in the infected host. Here the question was addressed if HBoV infections could contribute to fibrogenesis processes as suggested by previously published clinical observations. Cytokine profiles induced by HBoV infection in CuFi-8 air-liquid interphase cell cultures and in bronchoalveolar lavage fluid (BALF) of 20 HBoV-positive and 12 HBoV-negative patients were analysed by semi-quantitative Western spot blot analyses. Although lots of cytokines were regulated independently of HBoV status, several cytokines associated with lung fibrosis and tumour development, e.g., EGF, VEGF, TARC (CCL17), TNF-alpha, TNF-beta, TIMP-1, were clearly upregulated in the HBoV-positive cohort. These findings suggest that the development of lung fibrosis might be triggered by HBoV induced cytokine expression
