    Estimating the Population Impact of a New Pediatric Influenza Vaccination Program in England Using Social Media Content

    BACKGROUND: The rollout of a new childhood live attenuated influenza vaccine program was launched in England in 2013, which consisted of a national campaign for all 2 and 3 year olds and several pilot locations offering the vaccine to primary school-age children (4-11 years of age) during the influenza season. The 2014/2015 influenza season saw the national program extended to include additional pilot regions, some of which offered the vaccine to secondary school children (11-13 years of age) as well. OBJECTIVE: We utilized social media content to obtain a complementary assessment of the population impact of the programs that were launched in England during the 2013/2014 and 2014/2015 flu seasons. The overall community-wide impact on transmission in pilot areas was estimated for the different age groups that were targeted for vaccination. METHODS: A previously developed statistical framework was applied, which consisted of a nonlinear regression model that was trained to infer influenza-like illness (ILI) rates from Twitter posts originating in pilot (school-age vaccinated) and control (unvaccinated) areas. The control areas were then used to estimate ILI rates in pilot areas, had the intervention not taken place. These predictions were compared with their corresponding Twitter-based ILI estimates. RESULTS: Results suggest a reduction in ILI rates of 14% (1-25%) and 17% (2-30%) across all ages in only the primary school-age vaccine pilot areas during the 2013/2014 and 2014/2015 influenza seasons, respectively. No significant impact was observed in areas where two age cohorts of secondary school children were vaccinated. CONCLUSIONS: These findings corroborate independent assessments from traditional surveillance data, thereby supporting the ongoing rollout of the program to primary school-age children and providing evidence of the value of social media content as an additional syndromic surveillance tool

    The added value of online user-generated content in traditional methods for influenza surveillance

    There has been considerable work in evaluating the efficacy of using online data for health surveillance. Often comparisons with baseline data involve various squared error and correlation metrics. While useful, these overlook a variety of other factors important to public health bodies considering the adoption of such methods. In this paper, a proposed surveillance system that incorporates models based on recent research efforts is evaluated in terms of its added value for influenza surveillance at Public Health England. The system comprises of two supervised learning approaches trained on influenza-like illness (ILI) rates provided by the Royal College of General Practitioners (RCGP) and produces ILI estimates using Twitter posts or Google search queries. RCGP ILI rates for different age groups and laboratory confirmed cases by influenza type are used to evaluate the models with a particular focus on predicting the onset, overall intensity, peak activity and duration of the 2015/16 influenza season. We show that the Twitter-based models perform poorly and hypothesise that this is mostly due to the sparsity of the data available and a limited training period. Conversely, the Google-based model provides accurate estimates with timeliness of approximately one week and has the potential to complement current surveillance systems

    “It’s hard to tell”. The challenges of scoring patients on standardised outcome measures by multidisciplinary teams: a case study of Neurorehabilitation

    Background Interest is increasing in the application of standardised outcome measures in clinical practice. Measures designed for use in research may not be sufficiently precise to be used in monitoring individual patients. However, little is known about how clinicians and in particular, multidisciplinary teams, score patients using these measures. This paper explores the challenges faced by multidisciplinary teams in allocating scores on standardised outcome measures in clinical practice. Methods Qualitative case study of an inpatient neurorehabilitation team who routinely collected standardised outcome measures on their patients. Data were collected using non participant observation, fieldnotes and tape recordings of 16 multidisciplinary team meetings during which the measures were recited and scored. Eleven clinicians from a range of different professions were also interviewed. Data were analysed used grounded theory techniques. Results We identified a number of instances where scoring the patient was 'problematic'. In 'problematic' scoring, the scores were uncertain and subject to revision and adjustment. They sometimes required negotiation to agree on a shared understanding of concepts to be measured and the guidelines for scoring. Several factors gave rise to this problematic scoring. Team members' knowledge about patients' problems changed over time so that initial scores had to be revised or dismissed, creating an impression of deterioration when none had occurred. Patients had complex problems which could not easily be distinguished from each other and patients themselves varied in their ability to perform tasks over time and across different settings. Team members from different professions worked with patients in different ways and had different perspectives on patients' problems. This was particularly an issue in the scoring of concepts such as anxiety, depression, orientation, social integration and cognitive problems. Conclusion From a psychometric perspective these problems would raise questions about the validity, reliability and responsiveness of the scores. However, from a clinical perspective, such characteristics are an inherent part of clinical judgement and reasoning. It is important to highlight the challenges faced by multidisciplinary teams in scoring patients on standardised outcome measures but it would be unwarranted to conclude that such challenges imply that these measures should not be used in clinical practice for decision making about individual patients. However, our findings do raise some concerns about the use of such measures for performance management

    Imaging-guided chest biopsies: techniques and clinical results

    Background This article aims to comprehensively describe indications, contraindications, technical aspects, diagnostic accuracy and complications of percutaneous lung biopsy. Methods Imaging-guided biopsy currently represents one of the predominant methods for obtaining tissue specimens in patients with lung nodules; in many cases treatment protocols are based on histological information; thus, biopsy is frequently performed, when technically feasible, or in case other techniques (such as bronchoscopy with lavage) are inconclusive. Results Although a coaxial system is suitable in any case, two categories of needles can be used: fine-needle aspiration biopsy (FNAB) and core-needle biopsy (CNB), with the latter demonstrated to have a slightly higher overall sensitivity, specificity and accuracy. Conclusion Percutaneous lung biopsy is a safe procedure even though a few complications are possible: pneumothorax, pulmonary haemorrhage and haemoptysis are common complications, while air embolism and seeding are rare, but potentially fatal complications

    Redundancy and the Evolution of Cis-Regulatory Element Multiplicity

    The promoter regions of many genes contain multiple binding sites for the same transcription factor (TF). One possibility is that this multiplicity evolved through transitional forms showing redundant cis-regulation. To evaluate this hypothesis, we must disentangle the relative contributions of different evolutionary mechanisms to the evolution of binding site multiplicity. Here, we attempt to do this using a model of binding site evolution. Our model considers binding sequences and their interactions with TFs explicitly, and allows us to cast the evolution of gene networks into a neutral network framework. We then test some of the model's predictions using data from yeast. Analysis of the model suggested three candidate nonadaptive processes favoring the evolution of cis-regulatory element redundancy and multiplicity: neutral evolution in long promoters, recombination and TF promiscuity. We find that recombination rate is positively associated with binding site multiplicity in yeast. Our model also indicated that weak direct selection for multiplicity (partial redundancy) can play a major role in organisms with large populations. Our data suggest that selection for changes in gene expression level may have contributed to the evolution of multiple binding sites in yeast. We conclude that the evolution of cis-regulatory element redundancy and multiplicity is impacted by many aspects of the biology of an organism: both adaptive and nonadaptive processes, both changes in cis to binding sites and in trans to the TFs that interact with them, both the functional setting of the promoter and the population genetic context of the individuals carrying them

    Evaluation of classical precipitation descriptions for γ′′(Ni3Nb−D022) in Ni-base superalloys

    The growth/coarsening kinetics of γ′′(Ni3Nb−D022) precipitates have been found by numerous researchers to show an apparent correspondence with the classical (Ostwald ripening) equation outlined by Lifshitz, Slyozov and (separately) Wagner for a diffusion controlled regime. Nevertheless, a significant disparity between the actual precipitate size distribution shape and that predicted by LSW is frequently observed in the interpretation of these results, the origin of which is unclear. Analysis of the literature indicates one likely cause for this deviation from LSW for γ′′ precipitates is the “encounter” phenomenon described by Davies et al. (Acta Metall 28(2):179–189, 1980) that is associated with secondary phases comprising a high volume fraction. Consequently, the distributions of both γ′′ precipitates described in the literature (Alloy 718) and measured in this research in Alloy 625 are analysed through employing the Lifshitz–Slyozov-Encounter-Modified (LSEM) formulation (created by Davies et al.). The results of the LSEM analysis show good far better agreement than LSW with experimental distributions after the application of a necessary correction for what is termed in this research as “directional encounter”. Moreover, the activation energy for γ′′ coarsening in Alloy 625 shows conformity with literature data once the effect of heterogeneous (on dislocations) precipitate nucleation at higher temperatures is accounted for

    What are the health benefits of active travel? A systematic review of trials and cohort studies.

    BACKGROUND: Increasing active travel (primarily walking and cycling) has been widely advocated for reducing obesity levels and achieving other population health benefits. However, the strength of evidence underpinning this strategy is unclear. This study aimed to assess the evidence that active travel has significant health benefits. METHODS: The study design was a systematic review of (i) non-randomised and randomised controlled trials, and (ii) prospective observational studies examining either (a) the effects of interventions to promote active travel or (b) the association between active travel and health outcomes. Reports of studies were identified by searching 11 electronic databases, websites, reference lists and papers identified by experts in the field. Prospective observational and intervention studies measuring any health outcome of active travel in the general population were included. Studies of patient groups were excluded. RESULTS: Twenty-four studies from 12 countries were included, of which six were studies conducted with children. Five studies evaluated active travel interventions. Nineteen were prospective cohort studies which did not evaluate the impact of a specific intervention. No studies were identified with obesity as an outcome in adults; one of five prospective cohort studies in children found an association between obesity and active travel. Small positive effects on other health outcomes were found in five intervention studies, but these were all at risk of selection bias. Modest benefits for other health outcomes were identified in five prospective studies. There is suggestive evidence that active travel may have a positive effect on diabetes prevention, which may be an important area for future research. CONCLUSIONS: Active travel may have positive effects on health outcomes, but there is little robust evidence to date of the effectiveness of active transport interventions for reducing obesity. Future evaluations of such interventions should include an assessment of their impacts on obesity and other health outcomes

    Diffractive Dijet Production at sqrt(s)=630 and 1800 GeV at the Fermilab Tevatron

    We report a measurement of the diffractive structure function FjjDF_{jj}^D of the antiproton obtained from a study of dijet events produced in association with a leading antiproton in pˉp\bar pp collisions at s=630\sqrt s=630 GeV at the Fermilab Tevatron. The ratio of FjjDF_{jj}^D at s=630\sqrt s=630 GeV to FjjDF_{jj}^D obtained from a similar measurement at s=1800\sqrt s=1800 GeV is compared with expectations from QCD factorization and with theoretical predictions. We also report a measurement of the ξ\xi (xx-Pomeron) and β\beta (xx of parton in Pomeron) dependence of FjjDF_{jj}^D at s=1800\sqrt s=1800 GeV. In the region 0.035<ξ<0.0950.035<\xi<0.095, t<1|t|<1 GeV2^2 and β<0.5\beta<0.5, FjjD(β,ξ)F_{jj}^D(\beta,\xi) is found to be of the form β1.0±0.1ξ0.9±0.1\beta^{-1.0\pm 0.1} \xi^{-0.9\pm 0.1}, which obeys β\beta-ξ\xi factorization.Comment: LaTeX, 9 pages, Submitted to Phys.
