Reactivation of hepatitis B virus with immune-escape mutations after ocrelizumab treatment for multiple sclerosis
Ocrelizumab is an anti-CD20 monoclonal antibody for the treatment of multiple sclerosis (MS) that is closely related to rituximab. We describe a case of hepatitis B virus (HBV) reactivation in an MS patient with resolved HBV infection receiving ocrelizumab. HBV reactivation was monitored with HBV-DNA and HBV surface antigen periodic assessment. Anti-HBV treatment with entecavir was started after HBV-DNA detection. Ocrelizumab can reactivate viral replication in patients with resolved HBV infection. HBV reactivation monitoring seems an effective and safe option for the management of these patients. More studies are needed to assess the optimal management of HBV reactivation in MS patients on ocrelizumab treatment
JC virus-DNA detection is associated with CD8 fffector accumulation in peripheral blood of patients with multiple sclerosis under natalizumab treatment, independently from JC virus serostatus
Although natalizumab (anti-α4 integrin) represents an effective therapy for relapsing remitting multiple sclerosis (RRMS), it is associated with an increased risk of developing progressive multifocal leukoencephalopathy (PML), caused by the polyomavirus JC (JCV). The aim of this study was to explore natalizumab-induced phenotypic changes in peripheral blood T-lymphocytes and their relationship with JCV reactivation. Forty-four patients affected by RRMS were enrolled. Blood and urine samples were classified according to natalizumab infusion number: 0 (N0), 1-12 (N12), 13-24 (N24), 25-36 (N36), and over 36 (N > 36) infusions. JCV-DNA was detected in plasma and urine. T-lymphocyte phenotype was evaluated with flow cytometry. JCV serostatus was assessed. Ten healthy donors (HD), whose ages and sexes matched with the RRMS patients of the N0 group, were enrolled. CD8 effector (CD8 E) percentages were increased in natalizumab treated patients with detectable JCV-DNA in plasma or urine compared to JCV-DNA negative patients (JCV-) (p < 0.01 and p < 0.001, resp.). Patients with CD8 E percentages above 10.4% tended to show detectable JCV-DNA in plasma and/or urine (ROC curve p = 0.001). The CD8 E was increased when JCV-DNA was detectable in plasma or urine, independently from JCV serology, for N12 and N24 groups (p < 0.01). As long as PML can affect RRMS patients under natalizumab treatment with a negative JCV serology, the assessment of CD8 E could help in the evaluation of JCV reactivation

Evidence of neutral transcriptome evolution in plants

environmental and developmental perturbations, it is widely assumed that transcriptome divergence among taxa represents adaptive phenotypic selection. This assumption has been challenged by neutral theories which propose that stochastic
processes drive transcriptome evolution. To test for evidence of neutral transcriptome evolution in plants, we quantified 18 494 gene transcripts in nonsenescent leaves of 14 taxa of Brassicaceae using robust cross-species transcriptomics which includes a two-step physical and in silicobased normalization procedure based on DNA similarity among taxa. Transcriptome divergence correlates positively with evolutionary distance between taxa and with variation in gene expression among samples. Results are similar for pseudogenes and chloroplast genes evolving at different rates. Remarkably, variation in transcript abundance among root-cell samples correlates positively with
transcriptome divergence among root tissues and among taxa.
Because neutral processes affect transcriptome evolution in plants, many differences in gene expression among or within taxa may be nonfunctional, reflecting ancestral
plasticity and founder effects. Appropriate null models are required when comparing transcriptomes in space and time
Pneumatic retinopexy for primary rhegmatogenous retinal detachment: from a clinical trial to the real-life experience
Background: To report real-world outcomes of patients with primary Reghmatogenous Retinal Detachment (RRD) treated with Pneumatic Retinopexy (PnR) according to the indications of the Pneumatic Retinopexy versus Vitrectomy for management of Primary Rhegmatogenous Retinal Detachment Outcomes Randomized Trial (PIVOT) trial. Methods: Multicenter, retrospective study. Patients treated with PnR for RRD between 2021 and 2023 and a follow-up of at least 6 months were included. Single-procedure anatomical success, final anatomical success, complications, causes of failures, best corrected visual acuity (BCVA) after surgery, and the vision-related quality of life using the 25-Item National Eye Institute Visual Function Questionnaire (NEI VFQ-25) were reported. Results: A total of 76 eyes of 76 patients were included. Mean age was 60 ± 8.1 years. Primary anatomic reattachment was achieved by 84.3% of patients and final anatomical reattachment after pars plana vitrectomy was obtained in 100% of patients. BCVA improved from 0.32 (20/40) to 0.04 (20/20) logMar (p < 0.001) at 6 months. The main cause of failure was related to the presence of additional (likely missed) retinal breaks (66.6% of cases). Also, primary PnR failure was more frequent in eyes of patients with older age, macular involvement, worse baseline BCVA, greater extent of the RRD, and increased duration from diagnosis to treatment. Overall, the mean NEI-VFQ 25 composite score was 93.9% ± 6.4 at 6 months. Conclusions: The criteria of the PIVOT trial can be applied to real-world scenarios in the decision-making process for the treatment of primary RRD, with excellent anatomical and functional outcomes

Environmental sustainability of typical agro-food products: A scientifically sound and user friendly approach
The paper introduces an approach, developed in Agriculture & Quality programme, to evaluate the environmental sustainability of Apulian quality agro-food products that is integrated in the regional quality scheme “Quality Products”. It highlights the methodological approach adopted, the sustainability themes identified and the indicators selected. Indicators measurable at the farm/firm level were selected in relation to the following environmental themes: biodiversity, land use and management, energy use and climate change, use of chemical inputs, and responsible management of by-products and waste. A scoring scale was developed for each indicator; going from 0 (unsustainable) to 10 (very sustainable) with 5 corresponding to the sustainability threshold or reference value. The presented approach is both robust and user friendly and is in line with the principle entailing continuous improvement; the key sustainability thresholds will be periodically reviewed and updated. It represents a practical and innovative way to develop an information scheme for typical agri-food products and can be, with some refinement and contextualisation, easily scaled up to other territories
Solar retinopathy: a new setting of red, green, and blue channels
PURPOSE: To introduce a new color imaging technique using improved settings of red, green, and blue channels for improved delineation of retinal damage in patients with solar retinopathy.
METHODS: A retrospective case series of patients with poor vision secondary to solar retinopathy were analyzed. All patients underwent visual acuity, refraction, and dilated fundus examination. A spectral domain–optical coherence tomography of the macula and color fundus imaging using optimized red, green, and blue color setting was performed. Patients were reviewed over a 6-month period. The data were analyzed for statistical significance using an independent t test and a receiver operating characteristic curve.
RESULTS:
In total, 20 eyes of 10 patients were included between 2009 and 2017. The mean age was 24.9 ± 18.1 years. Best corrected visual acuity at first consultation was 0.78 ± 0.11 and after 6 months was 0.83 ± 0.09. Spectral domain–optical coherence tomography demonstrated retinal abnormalities at the myoid zone, ellipsoid zone, and the outer segment of photoreceptors. Receiver operating characteristic curve analysis showed an improving effect (area under the curve = 0.62; 95% confidence interval = 0.42–0.79). The color channels parameters, which improve visualization of the lesions were found to be 67-0.98-255 for the R-guided setting, 19-0.63-121 for the B-guided setting, and 7-1.00-129 for the G-guided setting. The ideal red, green, and blue setting was in 24-0.82-229.
CONCLUSIONS: The use of a new setting of red, green, and blue channels could improve the diagnosis and monitoring of solar retinopathy, hence improving patient care
Different ramp-incremental slopes elicit similar V̇o2max and fatigability profiles in females and males despite differences in peak power output
The aim of this article is to investigate the effects of different ramp-incremental (RI) slopes on fatigability and its recovery in females and males. Ten females and 11 males performed RI tests with distinct slopes, in separated and randomized sessions, 15 (RI15), 30 (RI30), and 45 (RI45) W min-1. Performance fatigability was assessed by femoral nerve electrical stimuli evoked during and after isometric maximal voluntary contraction (IMVC) of knee extensors at baseline and after task failure at min 0.5, 1.5, 2.5, 5, and 10. Maximal oxygen uptake (V O2max) and peak power output (POpeak) were also measured. There were significant and similar declines from pre- to post-RI test in RI15, RI30, and RI45 for IMVC (-23%; -25%; -25%, respectively; P 0.05). There were no RI condition effects, nor time × condition interaction for IMVC, potentiated single twitch and voluntary activation (all P > 0.05). V O2max was not different among RI15, RI30, and RI45 conditions (3.30, 3.29, and 3.26 L min-1, respectively; P = 0.717), but POpeak was (272, 304, and 337 W, respectively; P 0.05 for sex differences). In conclusion, RI tests of different slopes that elicited similar V O2max but different POpeak did not affect the profile of performance fatigability at task failure in females and males
Biological and methodological factors affecting VO2max response variability to endurance training and the influence of exercise intensity prescription

This is an open access article under the terms of the Creative Commons Attribution License. https://creativecommons.org/licenses/by/4.0/Changes in cardiorespiratory fitness (CRF) in response to endurance training (ET) exhibit large variations, possibly due to a multitude of biological and methodological factors. It is acknowledged that ~20% of individuals may not achieve meaningful increases in CRF in response to ET. Genetics, the most potent biological contributor, has been shown to explain ~50% of response variability, whilst age, sex, and baseline CRF appear to explain a smaller proportion. Methodological factors represent the characteristics of the ET itself including the type, volume, and intensity of exercise, as well as the method used to prescribe and control exercise intensity. Notably, methodological factors are modifiable and, upon manipulation, alter response rates to ET, eliciting increases in CRF regardless of an individual’s biological predisposition. Particularly, prescribing exercise intensity relative to a physiological threshold (e.g. ventilatory threshold) is shown to increase CRF response rates compared to when intensity is anchored relative to a maximum physiological value (e.g. maximum heart rate). It is, however, uncertain whether the increased response rates are primarily attributable to reduced response variability, greater mean changes in CRF, or both. Future research is warranted to elucidate whether more homogenous chronic adaptations manifest over time among individuals, as a result of exposure to more homogenous exercise stimuli elicited by threshold-based practices.Peer reviewe
