    Extraction and purification of phosphorus from the ashes of incinerated biological sewage sludge

    Phosphorus depletion represents a significant problem. Ash of incinerated biological sewage sludge (BSS) contains P, but the presence of heavy metals (e.g., Fe and Al) is the main issue. Based on chemical characterization by SEM-EDS, ED-XRF and ICP-OES techniques, the characteristics and P content of bottom ash (BA) and fly ash (FA) of incinerated BSS were very similar. On BA, P extraction carried out in counter-current with an S:L ratio of 1:10 and H2SO4 0.5 M led to better extraction yields than those of a similar test with H2SO4 1 M and an S:L ratio of 1:5 (93% vs. 86%). Comparing yields with H2SO4 0.5 M (S:L ratio of 1:10), the counter-current method gave better results than those of the crossflow method (93% vs. 83.9%), also improving the performance obtained with HCl in crossflow (93% vs. 89.3%). The results suggest that the purification of the acid extract from heavy metals with pH variation was impractical due to metal precipitation as phosphates. Extraction with H2SO4 and subsequent treatment with isoamyl alcohol represented the best option to extract and purify P, leading to 81% extraction yields of P with low amounts of metals

    Vis-NIR luminescent lanthanide-doped core-shell nanoparticles for imaging and photodynamic therapy

    The preparation of smart Ln:ZrO2@SiO2 nanoplatforms with grafted photosensitizer (Rose Bengal) which couple optical imaging with photo-dynamic therapy (PDT) is presented. A careful control of the lanthanide dopant loading is considered to enhance the photoemission properties of the lanthanide ions (Er, Pr, Yb) inside the ZrO2 crystal structure. The nanosystem with the lowest lanthanide loading maintains the size, phase and morphology of pristine ZrO2 nanoparticles and exhibit the best performances in term of the overall luminescence properties. Upon functionalization with a silica shell to covalently bound Rose Bengal, a theranostic platform is prepared which is very efficient in singlet oxygen generation, as demonstrated by EPR and UV\u2013vis spectroscopy studies. Preliminary cell viability tests show that while both pristine and Ln doped ZrO2 nanoparticles do not exert cytotoxicity, neither upon illumination nor in dark condition, Rose Bengal grafted samples are able to significantly reduce cell viability under light exposure, thus confirming the high potential of these nanoparticles as PDT tools

    Antidepressant discontinuation syndrome: A state-of-the-art clinical review

    Antidepressant drugs are prescribed to patients with depressive, anxiety disorders, and other conditions. Evidence about antidepressant discontinuation syndrome (ADS) and related outcomes is sparse, although potentially burdensome in some patients. The present state-of-the-art review aims to appraise the most current evidence about ADS critically. ADS has been documented for most antidepressant drugs, although most literature focuses on selective serotonin reuptake inhibitors prescribed for depression. While down-titration cannot exclude the chance of ADS, it is nonetheless warranted in the clinical setting, especially for short half-life and sedative compounds such as paroxetine. Integrative management with concurrent pharmacotherapy and psychotherapy may minimize the eventual unpleasant effects arising within the discontinuation process. In addition, patient-tailored interventions and education should be part of the discontinuation strategy. Future research must rely on broadly accepted definitions for ADS and related phenomena such as antidepressant withdrawal and shed further light on the underpinning neurobiology. Discriminating between ADS-related phenomena and relapse of depression is likewise warranted, along with a neuroscience-based nomenclature instead of a class one

    An integrated approach to evaluate PAH exposure

    Introduction - An integrated evaluation of PAH exposure should consider different routes of absorption, reliability and specificity of biomarkers of dose, and individual susceptibility. Material and Methods - The present study investigated the exposure to PAHs in coke oven workers (n=100), asphalt pavers (n=98) and ground construction workers (n=47), by means of environmental exposure and biological monitoring. Results \u2013 Personal exposure to total airborne PAHs [from naphtalene to indeno(1,2,3-cd)pyrene] in coke oven workers (median 140 \ub5g/m3), was much higher than in pavers exposed to bitumen fumes (0.6 \ub5g/m3), or in construction workers (0.4 \ub5g/m3),. Urinary 1-hydroxypyrene (1-HOP) in end-shift samples was significantly higher in cokeoven workers (2.2 \ub5g/l) than in asphalt pavers (0.7 \ub5g/l) and in construction workers (0.4 \ub5g/l). An increasing trend in 1-HOP levels, over the workweek and the workshift was registered. A similar trend was observed for urinary 2-hydroxyfluorene (2-FLE) and 3-hydroxyphenanthrene (3-PHE) as well as for some unmetabolised PAH in urine. In smokers with low exposures an important contribution to biomarkers was given by tobacco smoking. PAH-DNA adducts in peripheral blood lymphocytes were not associated with external exposure. Genetic polymorphisms of metabolic and DNA repairing enzymes (CYP1A1, GSTM1, GSTT1, XPD) did not appear to influence the levels of biomarkers. Total dermal contamination of PAHs in pavers, assessed in a subgroup of workers (n=22), ranged from 23 to 623 \ub5g. Significant correlations between dermal phenanthrene or pyrene and 1-HOP (r = 0.41 and 0.55), suggest that dermal exposure significantly contributes to the internal dose of these compounds. Conclusion \u2013 Our results suggest that urinary biomarkers of PAHs, integrating exposure from all sources and routes, are reliable tools for risk assessment in occupational health

    Extreme rainfall events in karst environments: the case study of September 2014 in the Gargano area (southern Italy)

    In the first week of September 2014, the Gargano Promontory (Apulia, SE Italy) was hit by an extreme rainfall event that caused several landslides, floods and sinkholes. As a consequence of the floods, two people lost their lives and severe socio-economic damages were reported. The highest peaks of rainfall were recorded between September 3rd and 6th at the Cagnano Varano and San Marco in Lamis rain gauges with a maximum daily rainfall (over 230 mm) that is about 30% the mean annual rainfall. The Gargano Promontory is characterized by complex orographic conditions, with the highest elevation of about 1000 m a.s.l. The geological setting consists of different types of carbonate deposits affected by intensive development of karst processes. The morphological and climatic settings of the area, associated with frequent extreme rainfall events can cause various types of geohazards (e.g., landslides, floods, sinkholes). A further element enhancing the natural predisposition of the area to the occurrence of landslides, floods and sinkholes is an intense human activity, characterized by an inappropriate land use and management. In order to obtain consistent and reliable data on the effects produced by the storm, a systematic collection of information through field observations, a critical analysis of newspaper articles and web-news, and a co-operation with the Regional Civil Protection and local geologists started immediately after the event. The information collected has been organized in a database including the location, the occurrence time and the type of geohazard documented with photographs. The September 2014 extreme rainfall event in the Gargano Promontory was also analyzed to validate the forecasts issued by the Italian national early-warning system for rainfall-induced landslides (SANF), developed by the Research Institute for Geo-Hydrological Protection (IRPI) for the Italian national Department for Civil Protection (DPC). SANF compares rainfall measurements and forecasts with empirical rainfall thresholds for the prediction of landslide occurrence.     Multi-institutional application of Failure Mode and Effects Analysis (FMEA) to CyberKnife Stereotactic Body Radiation Therapy (SBRT)

    Background A multidisciplinary and multi-institutional working group applied the Failure Mode and Effects Analysis (FMEA) approach to assess the risks for patients undergoing Stereotactic Body Radiation Therapy (SBRT) treatments for lesions located in spine and liver in two CyberKnife\uae Centres. Methods The various sub-processes characterizing the SBRT treatment were identified to generate the process trees of both the treatment planning and delivery phases. This analysis drove to the identification and subsequent scoring of the potential failure modes, together with their causes and effects, using the risk probability number (RPN) scoring system. Novel solutions aimed to increase patient safety were accordingly considered. Results The process-tree characterising the SBRT treatment planning stage was composed with a total of 48 sub-processes. Similarly, 42 sub-processes were identified in the stage of delivery to liver tumours and 30 in the stage of delivery to spine lesions. All the sub-processes were judged to be potentially prone to one or more failure modes. Nineteen failures (i.e. 5 in treatment planning stage, 5 in the delivery to liver lesions and 9 in the delivery to spine lesions) were considered of high concern in view of the high RPN and/or severity index value. Conclusions The analysis of the potential failures, their causes and effects allowed to improve the safety strategies already adopted in the clinical practice with additional measures for optimizing quality management workflow and increasing patient safety

    Major depressive disorder is a chronic and invalidating psychiatric illness and is associated with a greater risk of suicidal behaviors. In recent decades many data have supported a biological link between depressive states and inflammation. Pro-inflammatory cytokines have been found to rise, first of all TNF-α and IL-6. Suicidal behaviors have been consistently associated with increased levels of IL-6 and decreased levels of IL-2.
