    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Machine learning is applied in every field of human activity using digital data. In recent years, many papers have been published concerning artificial intelligence use in classification, regression and segmentation purposes in medicine and in ophthalmology, in particular. Artificial intelligence is a subsection of computer science and its principles, and concepts are often incomprehensible or used and interpreted by doctors incorrectly. Diagnostics of ophthalmology patients is associated with a significant amount of medical data that can be used for further software processing. By using of machine learning methods, it’s possible to find out, identify and count almost any pathological signs of diseases by analyzing medical images, clinical and laboratory data. Machine learning includes models and algorithms that mimic the architecture of biological neural networks. The greatest interest in the field is represented by artificial neural networks, in particular, networks based on deep learning due to the ability of the latter to work effectively with complex and multidimensional databases, coupled with the increasing availability of databases and performance of graphics processors. Artificial neural networks have the potential to be used in automated screening, determining the stage of diseases, predicting the therapeutic effect of treatment and the diseases outcome in the analysis of clinical data in patients with diabetic retinopathy, age-related macular degeneration, glaucoma, cataracts, ocular tumors and concomitant pathology. The main characteristics were the size of the training and validation datasets, accuracy, sensitivity, specificity, AUROC (Area Under Receiver Operating Characteristic Curve). A number of studies investigate the comparative characteristics of algorithms. Many of the articles presented in the review have shown the results in accuracy, sensitivity, specificity, AUROC, error values that exceed the corresponding indicators of an average ophthalmologist. Their introduction into routine clinical practice will increase the diagnostic, therapeutic and professional capabilities of a clinicians, which is especially important in the field of ophthalmic oncology, where there is a patient survival matter

    Searching for diagnostic correlations in female patients after ischemic stroke

    Aim. To perform a comparative analysis of clinical and laboratory parameters in groups of young women (pregnant or in early postpartum period) with stroke. Materials and methods. The study included 17 pregnant or in early postpartum period women (group 1) and 17 non-pregnant women (group 2) matched by disease severity with mean age 27.7±14.4 years in acute period of stroke in cerebral hemispheres of medium severity. Results. At the discharge improvement in NIH scale results was observed in women in both groups and there were no significant differences in outcomes. Not statistically significant differences of partial pressure of oxygen in venous blood (VB) from the median cubital vein (MCV) and the jugular vein (JV) in both groups at study period were acquired in comparative time interval. Differences in pH levels in VB from the MCV and the JV were observed during hospital stay. In patients from 2 groups pH levels were more acidic in the JV VB than in the MCV VB. Differences in lactate levels in blood from the MCV and the JV were observed. Lactate levels were lower in the MCV VB than in the JV VB during hospital stay in patients from both groups. In patients from both groups glycemia level in the JV VB was lower than in the MCV VB during the whole study period. At the discharge in patients from both groups a decrease in glucose level in arterial and venous blood from the MCV was observed. Arteriovenous oxygen difference (AVOD) in JV VB in 2 study groups was decreasing over time from admission to discharge. Differences in AVOD of VB from JV and MCV and AVOD in JV VB were higher in both study groups during the follow up period. Conclusion. In the conducted study of neurological status, brain and system metabolism charachteristics in patients with brain infarction in dynamics there were no differences found in 2 grops of women of fertile age.
