    Mechanisms of an extraordinary East Asian summer monsoon event in July 2011

    Previous studies have demonstrated that the strong East Asian summer monsoon results mainly from the westward intensification of the North Pacific subtropical high (NPSH), or equivalently, the enhancement of the western North Pacific subtropical high. However, during early July in 2011 a strong southerly or southeasterly moist flow gave rise to a large amount of precipitation over southwestern Japan and Korea and anomalous dry conditions over central China because of the extraordinary intensification of the NPSH to the north. The formation of the anomalous high that occurred to the east of central Japan during early July is very rare, and its physical mechanisms are investigated in this study. The regressed circulation anomalies and wave-activity flux vectors for July 2011 and data from the previous 32 years show that two teleconnection patterns due to Rossby wave trains are the most important mechanisms: Rossby wave propagation from the eastern North Atlantic and western Europe to East Asia following the North Eurasian jet and East Asian jet in the upper level, and northward propagation of Rossby waves forced by diabatic heating over the western North Pacific (WNP) region in the lower level. This simultaneous forcing of a significant negative phase of the summertime North Atlantic Oscillation in the higher latitudes and enhanced diabatic heating over the subtropical WNP is found to be the cause of the abnormal development of the anomalous high to the east of central Japan, resulting in extremely wet conditions in Korea and southern Japan and dry conditions in southern China

    MRI Findings of Pericardial Fat Necrosis: Case Report

    Pericardial fat necrosis is an infrequent cause of acute chest pain and this can mimic acute myocardial infarction and acute pericarditis. We describe here a patient with the magnetic resonance imaging (MRI) findings of pericardial fat necrosis and this was correlated with the computed tomography (CT) findings. The MRI findings may be helpful for distinguishing pericardial fat necrosis from other causes of acute chest pain and from the fat-containing tumors in the cardiophrenic space of the anterior mediastinum

    EFFECTIVE DOSE MEASUREMENT FOR CONE BEAM COMPUTED TOMOGRAPHY USING GLASS DOSIMETER

    During image-guided radiation therapy, the patient is exposed to unwanted radiation from imaging devices built into the medical LINAC. In the present study, the effective dose delivered to a patient from a cone beam computed tomography (CBCT) machine was measured. Absorbed doses in specific organs listed in ICRP Publication 103 were measured with glass dosimeters calibrated with kilovolt (kV) X-rays using a whole body physical phantom for typical radiotherapy sites, including the head and neck, chest, and pelvis. The effective dose per scan for the head and neck, chest, and pelvis were 3.37±0.29, 7.36±0.33, and 4.09±0.29 mSv, respectively.     In-Situ Wettability Analysis of Al Coating Influence on Hot-Dip Galvanizing Properties of Advanced High-Strength Steels

    We investigated the influence of steel surface properties on the wettability of zinc (Zn). Our main objective is to address the selective oxidation of solute alloying elements and enhance the wetting behavior of Zn on advanced high strength steel (AHSS) by employing an aluminum (Al) interlayer through the physical vapor deposition technique. The deposition of an Al interlayer resulted in a decrease in contact angle and an increase in spread width as the molten Zn interacted with the Al interlay on the steel substrate. Importantly, the incorporation of an Al interlayer demonstrated a significant improvement in wettability by substantially increasing the work of adhesion compared to the uncoated AHSS substrate

    Ethyl Acetate Fraction of Amomum villosum var. xanthioides Attenuates Hepatic Endoplasmic Reticulum Stress-Induced Non-Alcoholic Steatohepatitis via Improvement of Antioxidant Capacities

    Non-alcoholic fatty liver disease (NAFLD), including non-alcoholic steatohepatitis (NASH), affects 25% of the global population. Despite the prevalence of NAFLD worldwide, effective therapeutics are currently lacking. Amomum villosum var. xanthioides (Wall. ex Baker) T.L.Wu & S.J.Chen (AX) is a medicinal herb traditionally used for treating digestive tract disorders in countries across Asia. We aimed to examine the pharmacological effects of the ethyl acetate fraction of AX (AXEF) against tunicamycin (TM)-induced ER stress in a NASH mouse model using C57/BL6J male mice. Following TM injections (2 mg/kg), the mice were orally administrated AXEF (12.5, 25, or 50 mg/kg), silymarin (50 mg/kg), or distilled water daily for 5 days, and the outcomes for fatty liver, inflammation, and oxidative stress were measured in serum or liver tissue levels. AXEF drastically attenuated hepatic ER stress-induced NASH as indicated by decreases in lipid droplet accumulations, serum liver enzymes, hepatic inflammations, and cell death signals in the hepatic tissue and/or serum levels. Interestingly, AXEF showed potent antioxidant effects by quenching reactive oxidative stress and its final product lipid peroxide in the hepatic tissue, specifically an increase in metallothionein (MT). To confirm the underlying actions of AXEF, we observed that AXEF increases MT1 gene promoter activities in the physiological levels. Collectively, AXEF showed antioxidant properties on TM-induced ER stress in a NASH mice model through the improvement of MTs

    A Case of Adenocarcinoma in situ of the Distal Common Bile Duct Diagnosed by Percutaneous Transhepatic Cholangioscopy

    Extrahepatic cholangiocarcinoma is often clinically challenging to diagnose. Even multidisciplinary approaches which include computed tomography, magnetic resonance imaging, and endoscopic retrograde cholangiography are unsatisfactory in some cases, especially with biliary stricture. Percutaneous transhepatic cholangioscopy (PTCS) with its direct visualization for biopsy appears to be a promising technique for detecting cholangiocarcinoma at an early stage. We report a case of adenocarcinoma in situ of the distal common bile duct (CBD) that was confirmed by PTCS. This case suggests the useful role of PTCS in the differential diagnosis of a distal CBD obstruction, particularly when other diagnostic modalities do not provide definitive information

    Understanding medical students’ empathy based on Enneagram personality types

    Purpose High self-awareness can promote communication and empathy. The Enneagram is a well-known personality tool to enhance self-awareness. We evaluated differences in empathy among medical students using the Enneagram typology. Methods This cross-sectional study included first and second grade students at the Inje University College of Medicine. The Jefferson Scale of Empathy was used to measure empathy and the Korean Enneagram Personality Type Indicator was used for examining personality characteristics. Empathy scores were analyzed according to the Triads, Hornevian group, Harmonic group, and each Enneagram type. Results The Instinctive triad, the Withdrawns, and the Positive outlook group were the most common, and the Feeling triad, the Assertives, and the Emotional realness group were the least common. Students in the Feeling triad and the Dutifuls had higher compassionate care (CC) scores as compared to their counterparts. Type 2 and 6 students showed the two highest empathy and CC scores. The empathy score of type 3 students was the lowest. Type 7 had the lowest CC score but the highest perspective taking score. Conclusion These differences in empathy according to Enneagram personality types can be applied to medical education to maintain and improve medical students’ empathy

    Adjuvant cytokine-induced killer cell immunotherapy for hepatocellular carcinoma: a propensity score-matched analysis of real-world data

    Background Several randomized controlled trials have shown that adjuvant immunotherapy with autologous cytokine-induced killer (CIK) cells prolongs recurrence-free survival (RFS) after curative treatment for hepatocellular carcinoma (HCC). We investigated the efficacy of adjuvant immunotherapy with activated CIK cells in real-world clinical practice. Methods A total of 59 patients who had undergone curative surgical resection or radiofrequency ablation for stage I or II HCC, and subsequently received adjuvant CIK cell immunotherapy at two large-volume centers in Korea were retrospectively included. Propensity score matching with a 1:1 ratio was conducted to avoid possible bias, and 59 pairs of matched control subjects were also generated. The primary endpoint was RFS and the secondary endpoints were overall survival and safety. Results The median follow-up duration was 28.0 months (interquartile range, 22.9–42.3 months). In a univariable analysis, the immunotherapy group showed significantly longer RFS than the control group (hazard ratio [HR], 0.42; 95% CI, 0.22–0.80; log-rank P = 0.006). The median RFS in the control group was 29.8 months, and the immunotherapy group did not reach a median RFS. A multivariable Cox proportional hazard analysis showed that immunotherapy was an independent predictor for HCC recurrence (adjusted HR, 0.38; 95% CI, 0.20–0.73; P = 0.004). The overall incidence of adverse events in the immunotherapy group was 16/59 (27.1%) and no patient experienced a grade 3 or 4 adverse event. Conclusions The adjuvant immunotherapy with autologous CIK cells after curative treatment safely prolonged the RFS of HCC patients in a real-world setting
