    Does Capital Structure Effects Profitability of The Firms (Evidence from Firms Listed at KSE 100 Index)

    Adequate capital structure of any firm can enhance the firm performance. To verify this statement this study has been conducted. To check the impact of capital structure on the performance of firms 8 firms have been selected as a sample. These firms have been chosen from KSE 100 index. Ratio analysis have been applied on the data. Different ratios have been calculated for the purpose of analysis. This analysis have been conducted on seven years data from 2007 to 2013. Regression analysis and correlation analysis have been applied on data to check the relationship between capital structure and profitability. It is concluded that the capital structure have negative effect on the profitability of firms. As the debt ratio increase year by year, the profitability of firms decrease.     Epistaxis: comparison of chemical cautery with topical vasoconstrictor in adult anterior epistaxis

    Background: To compare efficacy of 75% silver nitrate chemical cautery as opposed to topical vasoconstrictor spray (xylometazoline 0.1%) in adult anterior epistaxis.Methods: This randomized controlled trial study was carried out at SMHS Hospital Srinagar from Jan 2019 to Dec 2019. 110 subjects that presented to ENT & HNS emergency with epistaxis and fulfilled the inclusion criteria were selected. Subjects were randomly distributed into two groups. Group-A individuals were treated by cauterization with 75% silver nitrate and Group-B individuals were treated with topical vasoconstrictor spray (xylometazoline 0.1%). All the subjects were reviewed at 1 month and success was determined in terms of control of epistaxis from same side of nose.Results: The mean age of the cohort was 48.5yrs (age range, 17-59). There were total 60 (54.4%) males and 50 (45.4%) females among the cases. Both the groups were comparable as regards the age, sex, duration and frequency of epistaxis. 91.2% cases in Group-A (silver nitrate cautery) and 73.5% cases in Group-B (xylometazoline spray 0.1%) had no further epistaxis at one-month follow-up (p=0.014).Conclusions: Chemical cauterization with silver nitrate is a feasible and safe technique for the treatment of adult anterior epistaxis and is more effective than topical vasoconstrictor spray

    Shifting of Agriculture to Horticulture in Anantnag District: A Geographical Analysis

    The vast area of paddy crop in Anantnag district (Jammu and Kashmir) has been converted for the construction of houses, roads and especially for horticultural purposes. The cultivation of paddy has been an old cultural heritage of Anantnag district as well as for the Kashmir valley as a whole, while the farmers are increasingly finding it less profitable in recent times. The present study used both primary (Survey) and secondary data from different sources with an aim to highlight the issue of diversifying of food crops (Paddy) towards cash crops (Apple) in district Anantnag. The findings from the study reveal that 6908 hectares of agricultural cropped area is shifted towards the horticultural cropped area, as agricultural crops (paddy) have low comparative advantage over horticultural (Apple). Paddy land under surveyed farmers has registered a significant decrease from 330 canals (83.75%) in 2000-01 to 158 canals (40.11%) in 2014-15, therefore showing an absolute change of -172 canals. The primary survey further reveals that 09 (30%) of the respondents says less profit is the main reason behind the shift, followed by irrigation (drought) problems 07 (23.34%) and 14 (46.66%) respondents says both reasons are responsible behind shift of Paddy to Apple cultivation in the region

    Unusual foreign body insertion in rectum and vagina in schizophrenic patient

    Insertion of foreign body in to the body is rare in medical literature. Here we report a case of 21 year old female who presented to emergency with acute abdomen and on detailed evaluation turned out to be schizophrenia. Our patient had history of insertion of ball pen in rectum following command hallucination. The foreign body was later removed through laparotomy. The cause of insertion of foreign body are multiple from command hallucination, suicidal attempt, erotism to somatic delusions. Thus the clinician should always be vigilant while dealing with psychiatric patients as sometimes impacted foreign body can present as acute emergency

    Synchronizing Nitrogen Fertilization and Planting Date to Improve Resource Use Efficiency, Productivity, and Profitability of Upland Rice

    Synchronizing nitrogen (N) fertilization with planting date (PD) could enhance resource use efficiency and profitability of upland rice (Oryza sativa L.) production in Thailand. The objective of the study was to assess upland rice responses to four N fertilization rates (NFRs) and three planting dates. Field experiments were conducted during two growing seasons under four NFRs, no N applied (N0), 30 (N30), 60 (N60), and 90 kg N ha−1 (N90), and NFR were applied at the initiation of tillering and panicle emergence stages. The planting dates selected were early (PD1), intermedium (PD2), and late planting (PD3) between September and December of each season. The NFRs and planting dates had a significant influence on N uptake, N use efficiency (NUE), crop water productivity, yield and yield attributes, and profitability of upland rice production. A linear relationship among NFRs, agronomic traits of upland rice, N uptake, and crop water productivity was observed, and a significant seasonal effect was indicated. Fertilization at N90 under PD2 enhanced yields, yield attributes, and grain yields, as well as crop water productivity by 56 and 105% during the second and first seasons, respectively. Grain N, total N, and straw N were increased by 159, 159, and 160%, and by 90, 114, and 153%, during the first and second seasons, respectively. Enhanced N efficiencies, including agronomic efficiency, recovery efficiency, partial factor productivity, and N harvest index, at varying NFRs were observed under PD2 during both seasons. Highly significant (p < 0.001) and positive associations were observed among agronomic attributes, N uptake, NUE, and crop water productivity of upland rice in correlation assessment. Profitability from grain yields was observed with N fertilization and N90 resulted in maximum profit under all the PDs. However, the highest marginal benefit-cost ratio was observed at N60 under PD2 during both seasons. The results suggest that the NFR of 90 kg N ha−1 and planting at the end of September or start of October would enhance resource use efficiency and productivity, and maximize profitability. Furthermore, long–term field investigations with a range of NFRs and adopting forecasting measures to adjust the planting date for upland rice are recommended

    Quantitative and qualitative changes in human Meissner’s corpuscle at different ages: a light microscopic analysis

    Background:Meissner’s corpuscle is the most complex sensory receptors of the skin; providing information about rapidly fluctuating mechanical forces acting on the hairless skin. Aim of current study was to study density and structural changes in human Meissner’s corpuscles at different agesMethods:Samples were obtained from finger tips of fifteen persons and divided into three groups according to age: group A <15years; group B: 16-45 years and group C: 46-72 years. 5 µm thick sections were prepared, impregnated with silver and observed under compound light microscope. Density of Meissner’s corpuscles (Mcs) was studied by calculating Meissner’s Index (MI).Results:In group A, the MI was 0.86-0.90, which increased to 0.96 in group B and dropped in group C to 0.4 (in the seventh decade). The analysis of variance showed significant difference (P = 0.019) in MIs of the three age groups. The size of the Mcs was largest in group B, followed by group C and smallest in group A. The analysis of variance showed that there was highly significant difference (P = 0.003) between the size of Mcs in all the three age groups. It was observed that morphology of Mcs alter with age. In children these consisted of rudimentary coil of nerve fibers around collection of cells. In adults the intra-corpuscular nerve fibers were compressed into a tight spiral and modifications of nerve fibers, like end bulbs, varicosities and networks, were well developed. In old age Mcs were attenuated consisting of tangled mass of nerve fibers.  Conclusion:It can be concluded that in adults the Mcs are greater in number, larger in size and complicated in structure, compared to young and old people.

    Purpose : The main goal of this study is to evaluate the impact of physical incorporation of polyethylene glycol (PEG) into 5-fluorouracil (5-FU)-loaded polymeric nanoparticles (NPs). METHODS: The 5-FU-loaded NPs were prepared utilizing a simple double emulsion method using polycaprolactone (PCL) and polylactic-co-glycolic acid (PLGA) with or without PEG 6000. The surface charge, particle size, and shape of NPs were evaluated by standard procedures. Both Fourier Transform Infrared Spectroscopy and X-ray diffraction spectra of the 5-FU loaded NPs were compared against the pure 5-FU. The in vitro release profile of 5-FU from the NPs was monitored by the dialysis tubing method. Cell death and apoptosis induction in response to 5-FU NP exposure were measured by MTT and Annexin-V/7-amino-actinomycin D (7-AAD) assays, respectively, in Daoy, HepG2, and HT-29 cancer cell lines. RESULTS: The 5-FU loaded NPs were found to be spherical in shape with size ranging between 176±6.7 and 253.9±8.6 nm. The zeta potential varied between -7.13± 0.13 and -27.06±3.18 mV, and the entrapment efficiency was between 31.96% and 74.09%. The in vitro release of the drug followed a two-phase mode characterized by rapid release in the first 8 hrs followed by a period of slow release up to 72 hrs with composition-based variable extents. Cells exposed to NPs demonstrated a significant cell death which correlated with the ratio of PEG in the formulations in Daoy and HepG2 cells but not in HT-29 cells. Formulations (F1-F3) significantly induced early apoptosis in HT-29 cell lines. CONCLUSION: The physical PEGylation significantly enhanced the entrapment and loading efficiencies of 5-FU into NPs formulated with PLGA and PCL. It also fostered the in vitro cytotoxicity of 5-FU-loaded NPs in both Daoy and HepG2 cells. Induction of early apoptosis was confirmed for some of the formulations

    Application of multiple linear regression and machine learning algorithms to elucidate the association of poor glycemic control and hyperhomocysteinemia with microalbuminuria

    Microalbuminuria is an early biomarker of general vascular dysfunction and a predictor of risk for cardiovascular and renal diseases. It is also considered as a marker of insulin resistance in both diabetic and non-diabetic patients. The rationale of this study was to elucidate threshold values of fasting blood glucose (FBS) and glycosylated hemoglobin (HbA1c) that are associated with microalbuminuria. In the parallel association of microalbuminuria with hyperhomocysteinemia was investigated. Machine learning algorithm and multiple linear regression were applied to study the association of poor glycemic control on microalbuminuria and hyperhomocysteinemia. In non-diabetic subjects with FBS &lt;102 mg/dL and HbA1c &lt;6.3%; and in diabetic subjects with good glycemic control (FBS: 102-118 mg/dL; HbA1c: 6.3-7.0%), urinary microalbumin levels were &lt;40µg/mg creatinine. Poor glycemic control (FBS &gt;172 mg/dL and HbA1c &gt;9.0%) was associated with microalbumin &gt;40µg/mg creatinine. Age, gender, HbA1c and FBS were shown to explain variability in urinary microalbumin to the extent of 54.4% as shown by multiple linear regression model. Analysis of variance (ANOVA) revealed higher levels of FBS (F: 39.77, P &lt;0.0001), HbA1c (F: 64.31, P &lt;0.0001) and total plasma homocysteine (F: 3.69, P =0.04) in microalbuminuria and clinical microalbuminuria groups when compared to subjects with normal microalbumin levels. Diabetic patients with poor glycemic index had a more B12 deficiency. Poor glycemic index and hyperhomocysteinemia were associated with clinical microalbuminuria

    Efficacy and safety of moxifloxacin in hospitalized patients with secondary peritonitis : pooled analysis of four randomized phase III trials

    Background: Secondary peritonitis is an advanced form of complicated intra-abdominal infection (cIAI) requiring hospitalization, surgical source control, and empiric antibiotic therapy against causative aerobic and anaerobic bacteria. Methods: This pooled analysis of four prospective, active-controlled randomized clinical trials compared the efficacy and safety of moxifloxacin with that of comparator antibiotics in patients with confirmed secondary peritonitis. The primary efficacy endpoint was clinical success rate at test-of-cure (TOC) between day 10 and 45 post-therapy in the per-protocol (PP) population. Safety and clinical efficacy were assessed also in the intent-to-treat population (ITT). Bacteriological success was assessed at TOC in the microbiologically-valid population as a secondary efficacy endpoint. Results: Overall clinical success rates at TOC were 85.3% (431 of 505 patients) in the moxifloxacin and 88.4% (459 of 519 patients) in the comparator treatment groups (PP population, point estimate for the difference in success rates: -3.0%; 95% CI -7.06%, 1.05%), respectively. Similar clinical success rates between moxifloxacin and comparators were observed by anatomical site of infection, and ranged from 80.6% to 100% for moxifloxacin and from 71.4% to 96.6% for comparators, respectively. Bacteriologic success rates were similar with moxifloxacin (82.4%) and comparators (86.8%), respectively. The proportion of patients experiencing any treatment-emergent adverse events was slightly higher with moxifloxacin (67.3%) versus comparators (59.8%). Rates of drug-related adverse events (20.9% versus 20.0%) and deaths (4.3% versus 3.4%) were similar in moxifloxacin and comparator groups; none of the deaths were drug-related. Conclusions: The data suggests that once-daily IV (or IV/PO) moxifloxacin has a comparable efficacy and safety profile to antibiotic regimens approved previously in the subgroup of patients with secondary peritonitis of mild-to-moderate severity
