    Determination of antioxidant activity of polyphenol extract from grape seeds

    The significance of carbohydrates for endurance training has been well established, whereas the role of protein and the adaptive response with endurance training is unclear. Therefore, the aim of this perspective is to discuss the current evidence on the role of dietary protein and the adaptive response with endurance training. On a metabolic level, a single bout of endurance training stimulates the oxidation of several amino acids. Although the amount of amino acids as part of total energy expenditure during exercise is relatively low compared to other substrates (e.g., carbohydrates and fat), it may depress the rates of skeletal muscle protein synthesis, and thereby have a negative effect on training adaptation. A low supply of amino acids relative to that of carbohydrates may also have negative effects on the synthesis of capillaries, synthesis and turn-over of mitochondrial proteins and proteins involved in oxygen transport including hamoglobin and myoglobin. Thus far, the scientific evidence demonstrating the significance of dietary protein is mainly derived from research with resistance exercise training regimes. This is not surprising since the general paradigm states that endurance training has insignificant effects on skeletal muscle growth. This could have resulted in an underappreciation of the role of dietary protein for the endurance athlete. To conclude, evidence of the role of protein on endurance training adaptations and performance remains scarce and is mainly derived from acute exercise studies. Therefore, future human intervention studies must unravel whether dietary protein is truly capable of augmenting endurance training adaptations and ultimately performance

    Extreme mass ratio inspiral rates: dependence on the massive black hole mass

    We study the rate at which stars spiral into a massive black hole (MBH) due to the emission of gravitational waves (GWs), as a function of the mass M of the MBH. In the context of our model, it is shown analytically that the rate approximately depends on the MBH mass as M^{-1/4}. Numerical simulations confirm this result, and show that for all MBH masses, the event rate is highest for stellar black holes, followed by white dwarfs, and lowest for neutron stars. The Laser Interferometer Space Antenna (LISA) is expected to see hundreds of these extreme mass ratio inspirals per year. Since the event rate derived here formally diverges as M->0, the model presented here cannot hold for MBHs of masses that are too low, and we discuss what the limitations of the model are.Comment: Accepted to CQG, special LISA issu

    The role of psycho-education in improving outcome at a general hospital psychiatry clinic in Uganda

    Objective: While psychoeducation has been shown to positively affect outcomes in psychiatric disorders, its utility has been little studied in developing countries. The current study sought to examine the role of psychoeducation at a general psychiatric outpatient clinic in Kampala, Uganda in improving clinic attendance, treatment adherence, and clinical outcomes. Method: A prospective casecontrol study using a quasi-experimental design was conducted in 117 patients suffering various psychiatric disorders. Participants were recruited for two months and then followed for a further three months after recruitment ended. Participants in the intervention group received formalized psychoeducation sessions at each clinic visit in addition to the usual psychiatric evaluation and care. Participants in the control group received the usual clinical care. Measured outcomes were knowledge of mental illness, compliance with medications and follow-up, and Clinical Global Impression (CGI). Results: The groups did not differ with respect to sociodemographiccharacteristics or attendance at scheduled follow-up visits. Both groups significantly improved on the CGI, but with no significant difference between the groups. However, the intervention group was more likely to adhere to medication, and their knowledge of mental illness was significantly higher at follow-up. Conclusion: These data suggest that psychoeducation is a beneficial mental health intervention in a developing country that may increase compliance with medication and result in greater knowledge of mental illness. However, other factors such as distance from a centralized clinic or cost of treatment may impact outcomes, including attendance at scheduled follow-up visits.Keywords: Low and middle income countries; Outpatient treatment; Psychosocial interventions; Transcultural Psychiatry; Africa;  Psychoeducatio

    Simulations of Extreme-Mass-Ratio Inspirals Using Pseudospectral Methods

    Extreme-mass-ratio inspirals (EMRIs), stellar-mass compact objects (SCOs) inspiralling into a massive black hole, are one of the main sources of gravitational waves expected for the Laser Interferometer Space Antenna (LISA). To extract the EMRI signals from the expected LISA data stream, which will also contain the instrumental noise as well as other signals, we need very accurate theoretical templates of the gravitational waves that they produce. In order to construct those templates we need to account for the gravitational backreaction, that is, how the gravitational field of the SCO affects its own trajectory. In general relativity, the backreaction can be described in terms of a local self-force, and the foundations to compute it have been laid recently. Due to its complexity, some parts of the calculation of the self-force have to be performed numerically. Here, we report on an ongoing effort towards the computation of the self-force based on time-domain multi-grid pseudospectral methods.Comment: 6 pages, 4 figures, JPCS latex style.     Purpose: Physical activity (PA) is an important behavior when it comes to preventing or slowing down disablement caused by aging and chronic diseases. It remains unclear whether PA can directly prevent or reduce disability in activities of daily living (ADL). This article presents a meta-analysis of the association between PA and the incidence and progression of basic ADL disability (BADL). Methods: Electronic literature search and cross-referencing of prospective longitudinal studies of PA and BADL in community dwelling older adults (50+) with baseline and follow-up measurements, multivariate analysis and reporting a point estimate for the association. Results: Compared with a low PA, a medium/high PA level reduced the risk of incident BADL disability by 0.51 (95% CI: 0.38, 0.68; p < 001), based on nine longitudinal studies involving 17,000 participants followed up for 3–10 years. This result was independent of age, length of follow-up, study quality, and differences in demographics, health status, functional limitations, and lifestyle. The risk of progression of BADL disability in older adults with a medium/high PA level compared with those with a low PA level was 0.55 (95% CI: 0.42, 0.71; p < 001), based on four studies involving 8500 participants. Discussion: This is the first meta-analysis to show that being physically active prevents and slows down he disablement process in aging or diseased populations, positioning PA as the most effective preventive trategy in preventing and reducing disability, independence and health care cost in aging societies

    Clostridium difficile in a farrowing pen

    Clostridium difficile is an important cause of enteric disease in humans. In pigs Clostridium difficile can cause neonatal enteritis and can be isolated from faeces from diseased and healthy animals. According to recent research, isolates from humans and animals show genetic and phenotypic overlap. In The Netherlands, strains isolated from diseased piglets were indistinguishable from strains isolated from Dutch patients. These strains belonged to ribotype 078. Because pigs can either be clinical hosts and/or may be a possible reservoir more understanding of the epidemiology of Clostridium difficile among pigs is needed. The objectives of this study were to specify whether, how and when newborn piglets get infected by Clostridium difficile for the first time. With this intention, six sows, their farrowing crates and litters (71 piglets) at one farm were sampled around the day of birth of the piglets. Within 48 hours after birth, all sampled 71 piglets at the farm became positive for Clostridium difficile ribotype 078. Moreover, all sows became positive within 113 hours after birth of the piglets and the farrowing crates were intermittently positive during the sampling period. This research shows that the sow, the farrowing crate, the air and the teats of the sow are possible transmission routes of Clostridium difficile ribotype 078.This information might help to advise farmers on taking measures against Clostridium difficile infections in neonatal piglets

    Probing Stellar Dynamics in Galactic Nuclei

    Electromagnetic observations over the last 15 years have yielded a growing appreciation for the importance of supermassive black holes (SMBH) to the evolution of galaxies, and for the intricacies of dynamical interactions in our own Galactic center. Here we show that future low-frequency gravitational wave observations, alone or in combination with electromagnetic data, will open up unique windows to these processes. In particular, gravitational wave detections in the 10^{-5}-10^{-1} Hz range will yield SMBH masses and spins to unprecedented precision and will provide clues to the properties of the otherwise undetectable stellar remnants expected to populate the centers of galaxies.     Coping with pain in the hip or knee in relation to physical disability in community-living elderly people

    __Objective__ To investigate the use of pain coping strategies by community-living older people with pain in the hip or knee and the mediating role of coping with pain in the relationship between the chronicity of pain and physical disability. __Methods__ A group of 157 people with pain 'in the last month' was identified. Coping with pain was assessed with the Pain Coping Inventory, physical disability with the Sickness Impact Profile, and household and sport activities with a validated structured interview method. __Results__ People with chronic pain used relatively more 'resting,' and 'reducing demands' as pain coping strategies. Pain chronicity made a significant contribution to physical disability; however, when corrected for other variables in a regression model, no significant partial correlation was found. __Conclusion__ We conclude that pain coping has a mediating role in the relationship between pain chronicity and physical disability. Less use of 'resting' and a physically active lifestyle are independently associated with less physical disability

    Patient Perceptions of Natural Orifice Translumenal Surgery

    Natural orifice translumenal endoscopic surgery (NOTES) is on the forefront of surgical technique, but existing research has produced mixed results regarding factors associated with interest in the procedure. Our objective was to ascertain patient opinions at a Canadian centre regarding scarless surgery. A survey comprising demographic data (gender, age, body mass index [BMI]), interest in NOTES, impact of increased risk, as well as importance of further research and shorter recovery time was administered to volunteer patients at outpatient general surgery clinics. Nonparametric tests were utilized to examine difference in response by age, sex, BMI, and preexisting scars. Of the 335 participants (57% female, mean age of 54.5 ± 15.9 years, mean BMI of 28.7 ± 6.9), the majority (83%) showed some interest, but this dropped to 38% when additional risk was factored in. Generally, women, those under 50 years of age and those of healthy weight, were more interested than male, older, and/or heavier patients. Most felt that research into NOTES and reduced length of inpatient stay were important (80% and 95%, respectively). Further investigation into objective NOTES outcomes are needed to provide patients adequate data to make an informed choice regarding surgical route

    Variety of Clostridium difficile PCR ribotypes in pigs arriving at the slaughterhouse

    Food products of animal origin might play a role in interspecies transmission of C. difficile. In pigs, Clostridium difficile can cause neonatal enteritis and can be isolated from faeces from both diseased and healthy animals. To determine the prevalence of C. difficile in Dutch pigs arriving at the slaughterhouse a pilot study was conducted at one slaughterhouse in the Netherlands. Rectal faecal samples were taken from fifty slaughtering pigs from ten farms just after the pigs were sedated. These samples were examined using a real time PCR (BD GeneOhmTM Cdiff Assay), in combination with culturing after enrichment. Using real time PCR, none of the faecal samples were found to be positive for C. difficile while after culturing 14 samples (coming from pigs from nine different farms) were found to be positive for C. difficile. The positive samples derived from 9 different farms and encompassed seven different ribotypes
