    Web-Based Emotion Regulation Training for Sexual Health: Randomized Controlled Trial

    Effective emotional regulation (ER) skills are important for sexual function, as they impact emotional awareness and expression during sexual activity, and therefore, satisfaction and distress. Emotion regulation interventions may offer a promising approach to improve sexual health. Web-based emotion regulation may be a therapeutic strategy for men and women with sexual health concerns. Nevertheless, there is a scarcity of intervention trials investigating its effects in this context, much less using the internet. This study aims to investigate the effects of a web-based emotion regulation training program for sexual function in both men and women. The participants were recruited based on their self-reported sexual problems, which for men was defined by a score of <25 on the International Index Erectile Function (IIEF) and for women by a score of <26.55 on the Female Sexual Function Index (FSFI). The final sample included 60 participants who were randomized to either a web-based emotion regulation training for sexual function or to a waitlist control group. The treatment consisted of an 8-week web-based emotion regulation training for sexual function. The participants were assessed at baseline, post intervention, and the 3-month follow-up. Of the 60 participants included, only 6 completed all 3 assessment points (n=5, 20% in the treatment group and n=1, 5% in the waitlist control group) after receiving the intervention. At follow-up, there were no significant differences between groups in any measure. Among the intervention completers, large-to-moderate within-group effect sizes were observed between the assessment points on measures of emotion regulation, depression, lubrication, orgasm, thoughts of sexual failure, and abuse during sexual activity. The adherence rate was very low, limiting the generalizability of the findings. Participants who completed the intervention showed improvements in both sexual function domains and emotion regulation. Nonetheless, due to a high dropout rate, this trial failed to collect sufficient data to allow for any conclusions to be drawn on treatment effects.     Influencia de la edad y la escolaridad en las praxias constructivas del test NEUPSILIN

    Avaliação das praxias construtivas, embora tradicional, tem sido insuficientemente explorada no exame neurocognitivo. O objetivo foi verificar efeito de idade e escolaridade, assim como interação entre os itens de praxias construtivas no Instrumento de Avaliação Neuropsicológica Breve – NEUPSILIN, obtendo-se normas de referência por desempenho. Participaram 613 adultos e idosos entre 19 até 90 anos. Realizou-se quatro tarefas, três de cópia (quadrado, margarida e cubo) e uma produção espontânea (relógio). Conduziu-se MANCOVA para avaliar efeito da idade e escolaridade nos grupos; post hoc Bonferroni para normas nos desenhos. Houve efeito de idade no quadrado e no relógio e efeito de escolaridade no relógio. Ocorreu interação entre idade e escolaridade no cubo e na margarida em que o desempenho aumentou nos grupos de maior escolaridade e diminuíram com aumento da idade. Percebe-se a influência da idade e escolaridade como variáveis relevantes para dados normativos de desempenho em praxias construtivas.Constructive praxis evaluation, although traditional, has been insufficiently explored in the neurocognitive examination. MANCOVA was conducted to evaluate effects of age and schooling in the groups; post hoc Bonferroni for standards in drawings. Age had an effect on the drawings of the square and the clock, schooling had an effect on the drawings of the clock. There was interaction between age and schooling in the cube and daisy drawings, in which performance increased in the groups with higher schooling and decreased with increasing age. We can see the influence of age and schooling as relevant variables for normative performance data in constructive praxis.La evaluación de las praxias constructivas, aunque tradicional, no ha sido suficientemente investigada en el examen neurocognitivo. El objetivo fue verificar la interacción entre edad y escolaridad, así como la interacción entre los ítems de praxias constructivas en el Instrumento de Evaluación Neuropsicológica Breve – NEUPSILIN, obteniéndose normas de referencia por rendimiento. Participaron 613 adultos y ancianos con edades entre 19 y 90 años. Fueron realizadas cuatro tareas, tres copias (cuadrado, margarita y cubo) y una producción espontánea (reloj). Se llevó a cabo MANCOVA para evaluar el efecto de edad y escolaridad en los grupos; post hoc Bonferroni para las normas en los dibujos.     Déficits Primários e Secundários de Funções Executivas Pós-TCE: análise de dissociações

    The present study evaluated the presence of associations and dissociations between impairments in episodic memory and executive functions in patients with traumatic brain injury (TBI), and verified whether these deficits were primary or secondary. Eighty-one patients with TBI were assessed using the Rey Auditory Verbal Learning Test and the Hayling Test. The results suggest that impairments in inhibition speed may contribute to deficits in episodic memory, and that initiation and inhibition abilities may be complementary and the first precedes the second. Our findings highlighted that primary executive impairment following TBI may lead to episodic memory deficits.Este estudo avaliou as associações e dissociações encontradas entre déficits de memória episódica em relação aos de funções executivas e verificou se estes déficits encontrados eram primários ou secundários. Os 81 pacientes pós-Traumatismo Cranioencefálico (TCE) foram avaliados por meio do Teste de Aprendizagem Auditivo-Verbal de Rey e do Teste Hayling. Os resultados sugerem que prejuízo na velocidade de controle inibitório pode contribuir para déficit na memória episódica e que as velocidades de iniciação e inibição parecem ser complementares, mas a primeira precede a segunda. Nossos achados ressaltam que os prejuízos executivos provavelmente sejam primários em nossa amostra de pacientes pós-TCE e que estes prejuízos podem causar déficits na memória episódica

    Brief instrument for direct complex functionality assessment: a new ecological tool

    BackgroundThe Direct Assessment of Functional Status (DAFS) is the only instrument validated in Brazil that assesses functionality directly with the patient. However, this clinical tool takes a long time to be administered. This limits its use in hospitals and outpatient clinics that require brief assessment instruments. Additionally, we need to count with a direct assessment because the number of older adults living alone is increasing and we thus lack reliable informants.ObjectiveThis study aimed to present the development and content validity evidence of a direct complex functionality test for older adults, the Brief Instrument for Direct Functionality Assessment (BIDFA).MethodA total sample of 30 older adults and eight expert judges took part in the study stages. The BIDFA construction stages were: (1) literature review of functionality instruments; (2) development of seven ecological tasks to evaluate the performance of daily complex activities with the older adults; (3) content analysis by eight expert judges; (4) pilot study with 30 older adults; (5) the ecological analysis of items; (6) focus group analysis; and (7) final version of the BIDFA.ResultsThe BIDFA had evidence of content validity with an agreement index of 96.5%. The final version of BIDFA was left with six domains of complex functionality divided into semantic memory and time orientation; shopping skills; executive attention, math and finance skills; organization; planning and procedural memory; and problem-solving. The complex functionality score by BIDFA ranges from 0 to 100 points.ConclusionThe BIDFA was found to have good content validity by the expert judges and by the ecological analysis of the items by the older adults. The new instrument is expected to help assess the functional status of older adults, in an abbreviated context including complex functionality demands, with a wider range of total and subdomain scores

However, it is known that within this continuum between normal and pathological aging there are heterogeneous characteristics that are still little explored that need to be better understood to identify factors of cognitive reserve and demential risk. Thus, the present dissertation is composed of two studies, the first with the objective of verifying if there are subgroups of executive processing in the elderly with MCI and controls; secondarily, to examine whether the latent executive profiles are differentiated by sociodemographic, clinical and mnemonic variables. In this study 120 elderly adults (50 controls and 70 MCI patients) participated in a complete neuropsychological evaluation battery. A latent profile analysis was performed to verify possible subgroups of executive functions (EF), followed by a multivariate ANOVA controlling years of formal education and frequency of reading and writing habits (FRWH). Three executive profiles were found (1) Profile 1 with processing speed, executive attention and visuospatial initiation impairment (23.33% of the sample); (2) Profile 2 with intermediate performance in EF (45.84% of the sample); and (3) Profile 3 with superior performance in working memory, planning and verbal organization (30.83% of the sample). Comparative analysis showed differences in education, economy class, FRWH, global cognition, symptoms of depression, immediate episodic memory, recent memory, visual memory, recognition of new information learned and prospective memory. In all variables, Profile 1 had worse performance and Profile 3 had the best performance. The second study aimed at evaluating cognitive processing evolution (mnemonic, executive, linguistic, praxis and attentional), socio-demographic and clinical characteristics of elderly individuals with MCI and elderly controls, as well as verifying whether the elderly will convert the clinical condition and if there were risk factors for this conversion. In study 2, 65 elderly adults (41 MCI patients and 24 controls) participated in two complete neuropsychological assessments (time 1 and time 2). ANOVA analyzes of repeated measures were performed, controlling years of formal education and FRWH. There were differences in the evaluation one year later in the working memory, visual episodic memory, and visual and spatial cognitive flexibility variables. Eighteen participants (27.69%) converted to the clinical condition. The 11 functionality explained 12.5% of clinical conversion. The results of these two studies suggest that there is heterogeneity in the neurocognitive and executive profile within healthy aging and MCI that need to be monitored longitudinally to identify the continuum of these impairments. Variables such as processing speed, working memory and inhibitory control are essential executive measures for monitoring beyond episodic memory evaluation. Future studies should investigate transversally and longitudinally MCI severity, using factorial analyzes and composite scores of cognitive components, more specifically from executive components. These measures may be quantitative and qualitative measures of better measurement of cognitive impairment extent in elderly patients, allowing more accurate and earlier diagnoses towards the implementation of specific neurocognitive stimulation programs.O envelhecimento seja ele saud?vel ou com Comprometimento Cognitivo Leve (CCL) tende a estar relacionado ? exist?ncia de perfis neurocognitivos e fatores sociodemogr?ficos e cl?nicos que podem ser considerados de risco para convers?o para um transtorno neurocognitivo maior. Contudo, sabe-se que dentro desse cont?nuo entre o envelhecimento normal e patol?gico existem caracter?sticas heterog?neas ainda pouco exploradas que necessitam ser mais bem compreendidas para identifica??o de fatores de reserva cognitiva e de risco demencial. Assim, a presente disserta??o ? composta por dois estudos, sendo o primeiro com o objetivo de verificar se h? subgrupos de processamento executivo em idosos com CCL e controles; secundariamente, examinar se os perfis executivos latentes se diferenciam por vari?veis sociodemogr?ficas, cl?nicas e mnem?nicas. Nesse estudo participaram 120 adultos idosos (50 controles e 70 com CCL) que realizaram uma bateria de avalia??o neuropsicol?gica completa. Foi conduzida uma an?lise de perfis latentes para verificar poss?veis subgrupos de fun??es executivas, seguida de uma ANOVA multivariada controlando escolaridade e frequ?ncia de h?bitos de leitura e de escrita (FHLE). Foram encontrados tr?s perfis executivos (1) Perfil 1 com preju?zos de velocidade de processamento, aten??o executiva e inicia??o visuoespacial (23,33% da amostra); (2) Perfil 2 com desempenho intermedi?rio em FE (45,84% da amostra); e (3) Perfil 3 com desempenho superior em mem?ria de trabalho, planejamento e organiza??o verbal (30,83% da amostra). A an?lise comparativa mostrou diferen?as quanto ? escolaridade, classe econ?mica, FHLE, cogni??o global, sintomas de depress?o, mem?ria epis?dica imediata e recente, mem?ria visual, reconhecimento de novas informa??es aprendidas e mem?ria prospectiva, sendo que em todas as vari?veis o Perfil 1 teve pior desempenho e o Perfil 3 teve o melhor desempenho. J? o segundo estudo, teve como objetivo avaliar a evolu??o do processamento cognitivo (mnem?nico, executivo, lingu?stico, pr?xico e atencional), sociodemogr?fico e cl?nico de idosos com CCL e idosos controles verificando, ainda, se os idosos iriam converter o quadro cl?nico e se existiriam fatores de risco para essa convers?o. No estudo 2 participaram 65 adultos idosos (41 CCL e 24 controles) no qual passaram por duas avalia??es neuropsicol?gicas completas (tempo 1 e tempo 2). Foram realizadas an?lises de ANOVA de medidas repetidas, controlando escolaridade e FHLE. Houve diferen?as na avalia??o um ano depois nas vari?veis de mem?ria de trabalho, mem?ria epis?dica visual, e flexibilidade cognitiva visuoespacial. 9 Dezoito participantes da amostra (27,69%) converteram o quadro cl?nico. A funcionalidade explicou 12,5% da convers?o do quadro cl?nico. Os resultados dos dois estudos sugerem que h? uma heterogeneidade no perfil neurocognitivo e executivo dentro do envelhecimento saud?vel e com CCL que precisam ser acompanhadas longitudinalmente para identificar o cont?nuo desses preju?zos.     Cognitive performance in patients with Mild Cognitive Impairment and Alzheimer's disease with white matter hyperintensities: An exploratory analysis

    ABSTRACT. Background: White matter hyperintensities (WMH) are commonly associated with vascular dementia and poor executive functioning. Notwithstanding, recent findings have associated WMH with Alzheimer's disease as well as other cognitive functions, but there is no consensus. Objective: This study aimed to verify the relationship between WMH and cognitive performance in Mild Cognitive Impairment (MCI) and Alzheimer's disease (AD) patients. The study also sought to identify cognitive and demographic/cultural factors that might explain variability of WMH. Methods: The sample was composed of 40 participants (18 MCI and 22 AD patients) aged ≥ 65 years. Spearman's correlation was performed among cognitive performance (memory, language, visuospatial ability, and executive function) and WMH evaluated by the Fazekas and ARWMC scales. Two stepwise linear regressions were carried out, one with cognitive and the other with demographic/cultural variables as predictors. Results: Only naming showed significant correlation with ARWMC. Fazekas score exhibited significant correlation with all cognitive domains evaluated. Fazekas score was better predicted by episodic visual memory and age. Conclusion: This study found that the most relevant cognitive profile in MCI and AD patients with WMH was related to episodic memory. And, without taking clinical aspects into consideration, age was the best predictor of WMH
