    Racial/Ethnic Disparities in Infectious Disease Hospitalizations in Arizona

    Infectious disease (ID) hospitalizations in Arizona, a diverse population with nearly complete race/ethnicity data, were analyzed using the State Inpatient Database for 2005-2008. ID hospitalizations rates were calculated and compared by ID group, race/ethnicity, age, and sex. During 2005-2008, there were 383,597 ID hospitalizations reported in Arizona, resulting in an age-adjusted rate of 1498.1 per 100,000 persons. A range of racial/ethnic disparities in ID hospitalization rates were noted. Persons of Native American and black race/ethnicity had overall ID hospitalization rates higher than the rate for persons of white race/ethnicity; persons of Asian or Pacific Islander race/ethnicity had a lower rate. The lower respiratory tract infection (LRTI) hospitalization rate was the highest rate of all ID groups, followed by cellulitis and septicemia. Persons of black and Native American race had higher LRTI hospitalization rates than persons of white race. Racial/ethnic disparities persist for ID hospitalizations in Arizona. Persons of Native American and black race/ethnicity experience high age-adjusted rates of ID hospitalization. Prevention efforts should focus on high risk race/ethnicity groups and disease groups

    Detection and Interpretation Of Long-Lived X-Ray Quasi-Periodic Pulsations in the X-Class Solar Flare On 2013 May 14

    Quasi-periodic pulsations (QPP) seen in the time derivative of the GOES soft X-ray light curves are analyzed for the near-limb X3.2 event on 14 May 2013. The pulsations are apparent for a total of at least two hours from the impulsive phase to well into the decay phase, with a total of 163 distinct pulses evident to the naked eye. A wavelet analysis shows that the characteristic time scale of these pulsations increases systematically from ∼\sim25 s at 01:10 UT, the time of the GOES peak, to ∼\sim100 s at 02:00 UT. A second ridge in the wavelet power spectrum, most likely associated with flaring emission from a different active region, shows an increase from ∼\sim40 s at 01:40 UT to ∼\sim100 s at 03:10 UT. We assume that the QPP that produced the first ridge result from vertical kink-mode oscillations of the newly formed loops following magnetic reconnection in the coronal current sheet. This allows us to estimate the magnetic field strength as a function of altitude given the density, loop length, and QPP time scale as functions of time determined from the GOES light curves and RHESSI images. The calculated magnetic field strength of the newly formed loops ranges from about ∼\sim500 G at an altitude of 24 Mm to a low value of ∼\sim10 G at 60 Mm, in general agreement with the expected values at these altitudes. Fast sausage mode oscillations are also discussed and cannot be ruled out as an alternate mechanism for producing the QPP

    SERUM OMENTIN AS A POTENTIAL BIOMARKER FOR COMPLEX ATYPICAL HYPERPLASIA AND ENDOMETRIOID ENDOMETRIAL ADENOCARCINOMA

    Obesity is a significant risk factor for endometrial cancer, the most common gynecologic malignancy in the United States, but much remains unclear about the relationship between obesity-related factors and the development of endometrial cancer. Omentin, a recently discovered adipokine, has been shown to be present in lower levels in patients who are obese and/or insulin resistant. A case-control study was conducted using the serum of 140 women with endometrial cancer and 75 women with endometrial hyperplasia who were matched 1:1 based on body mass index (BMI) and menopausal status to women with no history of endometrial cancer (controls). The concentration of omentin in the patients’ serum was experimentally determined by conducting tests in triplicate on an enzyme-linked immunoadsorbent assay (ELISA) specific to human omentin. The mean serum omentin levels of women with endometrial hyperplasia and endometrial cancer were statistically significantly lower than controls, regardless of BMI. The mechanism that induces this change remains unknown, but these results present exciting promise for omentin’s use as a biomarker and role in understanding the relationship between obesity-related factors and endometrial carcinoma

    Cosmological Avatars of the Landscape II: CMB and LSS Signatures

    This is the second paper in the series that confronts predictions of a model of the landscape with cosmological observations. We show here how the modifications of the Friedmann equation due to the decohering effects of long wavelength modes on the wave function of the Universe defined on the landscape leave unique signatures on the CMB spectra and large scale structure (LSS). We show that the effect of the string corrections is to suppress σ8 and the CMB temperature-temperature (TT) spectrum at large angles, thereby bringing WMAP and SDSS data for σ8 into agreement. We find interesting features imprinted on the matter power spectrum P(k): power is suppressed at large scales indicating the possibility of primordial voids competing with the integrated Sachs-Wolfe effect. Furthermore, power is enhanced at structure and substructure scales, k≃10−2−0hMpc−1. Our smoking gun for discriminating this proposal from others with similar CMB and LSS predictions comes from correlations between cosmic shear and temperature anisotropies, which here indicate a noninflationary channel of contribution to LSS, with unique ringing features of nonlocal entanglement displayed at structure and substructure scales

    Opportunities for Cancer Prevention During Midlife Highlights from a Meeting of Experts

    AbstractThis paper provides highlights from a CDC-hosted meeting on opportunities for cancer prevention during midlife (roughly ages 45–64 years). Positive changes during this phase of life have the potential to prevent cancer incidence later in life, making this phase an opportune time for targeted prevention efforts to facilitate healthy aging and increased longevity. Risk and protective factors discussed during the meeting included exposure to radiation from medical imaging procedures, circadian disruption, chemical exposures, dietary factors, alcohol consumption, obesity, physical activity, diabetes, and the human microbiome. Although many of these factors are well recognized as being related to cancer incidence, others are not as widely recognized or have emerged as growing areas of research.Meeting participants discussed promising strategies for cancer prevention targeting this age group. Just as there are multiple determinants of cancer risk, there are likely multiple solutions. Changes to social and physical environments may facilitate healthy behaviors and minimize harmful exposures. Information shared during the meeting about health disparities in the U.S. highlighted the need to go beyond traditional approaches to cancer prevention to truly reach vulnerable populations. Partnerships are also a key component to prevention efforts; community-based and nonprofit organizations, the healthcare system, research institutions, state health departments, and federal agencies were all noted as important partners in prevention efforts. Coordinated, multi-disciplinary efforts across multiple chronic diseases may provide opportunities for synergistic effects. Further, leveraging key partnerships and existing communication channels can maximize success and facilitate timely translation of research findings into public health practice

    Satellite derived offshore migratory movements of southern right whales (Eubalaena australis) from Australian and New Zealand wintering grounds

In Australasia, winter calving grounds used by southern right whales extend from Western Australia across southern Australia to the New Zealand sub-Antarctic Islands. During the Austral-summer these whales are thought to migrate away from coastal waters to feed, but the location of these feeding grounds is only inferred from historical whaling data. We present new information on the satellite derived offshore migratory movements of six southern right whales from Australasian wintering grounds. Two whales were tagged at the Auckland Islands, New Zealand, and the remaining four at Australian wintering grounds, one at Pirates Bay, Tasmania, and three at Head of Bight, South Australia. The six whales were tracked for an average of 78.5 days (range: 29 to 150) with average individual distance of 38 km per day (range: 20 to 61 km). The length of individually derived tracks ranged from 645–6,381 km. Three likely foraging grounds were identified: south-west Western Australia, the Subtropical Front, and Antarctic waters, with the Subtropical Front appearing to be a feeding ground for both New Zealand and Australian southern right whales. In contrast, the individual tagged in Tasmania, from a sub-population that is not showing evidence of post-whaling recovery, displayed a distinct movement pattern to much higher latitude waters, potentially reflecting a different foraging strategy. Variable population growth rates between wintering grounds in Australasia could reflect fidelity to different quality feeding grounds. Unlike some species of baleen whale populations that show movement along migratory corridors, the new satellite tracking data presented here indicate variability in the migratory pathways taken by southern right whales from Australia and New Zealand, as well as differences in potential Austral summer foraging grounds.Publisher PDFPeer reviewe

    Establishing storm thresholds for the spanish gulf of Cádiz coast

    In this study critical thresholds are defined for storm impacts along the Spanish coast of the Gulf of Cádiz. The thresholds correspond to the minimum wave and tide conditions necessary to produce significant morphological changes on beaches and dunes and/or damage on coastal infrastructure or human occupation. Threshold definition was performed by computing theoretical sea-level variations during storms and comparing them with the topography of the study area and the location of infrastructure at a local level. Specifically, the elevations of the berm, the dune foot and the entrance of existing washovers were selected as threshold parameters. The total sea-level variation generated by a storm event was estimated as the sum of the tidal level, the wind-induced setup, the barometric setup and the wave-associated sea-level variation (wave setup and runup), assuming a minimum interaction between the different processes. These components were calculated on the basis of parameterisations for significant wave height (Hs) obtained for the oceanographic and environmental conditions of the Gulf of Cadiz. For this purpose real data and reanalysis time-series (HIPOCAS project) were used. Validation of the obtained results was performed for a range of coastal settings over the study area. The obtained thresholds for beach morphological changes in spring tide conditions range between a significant wave height of 1.5 m and 3.7 m depending on beach characteristics, while for dune foot erosion are around 3.3 to 3.7 m and for damage to infrastructure around 7.2 m. In case of neap tide conditions these values are increased on average by 50% over the areas with large tidal range. Furthermore, records of real damage in coastal infrastructure caused by storms were collected at a regional level from newspapers and other bibliographic sources and compared with the hydrodynamic conditions that caused the damage. These were extracted from the hindcast database of the HIPOCAS project, including parameters such as storm duration, mean and maximum wave height and wave direction. Results show that the duration of the storm is not critical in determining the occurrence of coastal damage in the regional study area. This way, the threshold would be defined as a duration ≥30 hours, with moderate average wave height (≥3.3 m) and high maximum wave height (≥4.1 m) approaching from the 3rd and 4th quadrants, during mean or spring tide situation. The calculated thresholds constitute snapshots of risk conditions within a certain time framework. Beach and nearshore zones are extremely dynamic, and also the characteristics of occupation on the coast change over time, so critical storm thresholds will change accordingly and therefore will need to be updated

    The collective impact of rare diseases in Western Australia: an estimate using a population-based cohort.

    PURPOSE: It has been argued that rare diseases should be recognized as a public health priority. However, there is a shortage of epidemiological data describing the true burden of rare diseases. This study investigated hospital service use to provide a better understanding of the collective health and economic impacts of rare diseases. METHODS: Novel methodology was developed using a carefully constructed set of diagnostic codes, a selection of rare disease cohorts from hospital administrative data, and advanced data-linkage technologies. Outcomes included health-service use and hospital admission costs. RESULTS: In 2010, cohort members who were alive represented approximately 2.0% of the Western Australian population. The cohort accounted for 4.6% of people discharged from hospital and 9.9% of hospital discharges, and it had a greater average length of stay than the general population. The total cost of hospital discharges for the cohort represented 10.5% of 2010 state inpatient hospital costs. CONCLUSIONS: This population-based cohort study provides strong new evidence of a marked disparity between the proportion of the population with rare diseases and their combined health-system costs. The methodology will inform future rare-disease studies, and the evidence will guide government strategies for managing the service needs of people living with rare diseases.Genet Med advance online publication 22 September 2016Genetics in Medicine (2016); doi:10.1038/gim.2016.143
